Article The Efficacies of Medicinal Leech Therapy and Platelet Rich Plasma Injection Added To Muscle Stretching Exercises in the Treatment of Myofascial Pain Syndrome(Yuzuncu Yil Universitesi Tip Fakultesi, 2021) Ediz, L.; Çağaç, A.; Delen, V.Myofascial pain syndrome is among the most common cause of physical disability and severe chronic muscle pain and affect millions of people worldwide. So far muscle stretching exercises and some therapeutic interventions have been recommended with various effects. Sixty-six patients with myofascial pain syndrome due to trigger points in trapezius muscle were included in this study. Patients were divided randomly into 2 groups of 33 patients: group 1; medicinal leech therapy group, group 2; platelet rich plasma injection group. To the first group, 4 medicinal leech were applied to the trigger points 2 times in a 4 week interval . To the second group, a total of 2 ml platelet rich plasma was administered to each trigger point 2 times in a 4 week interval. All patients had also taught and given trapezius muscle stretching exercises for ten minutes as a daily diary schedule for 12 weeks. At the end of the study there was a significant decrease in visual analogue scale scores in both groups, while a significant increase in pain threshold values in medicinal leech therapy group (p<0.05). Significant differences were found between groups in short form-36 pain, general health and vitality scores in favor of leech therapy group (p <0.05). Although both methods were found effective in the treatment of myofascial pain syndrome, the clinical efficacy of medicinal leech therapy was greater than platelet rich plasma. Leech therapy seems to be an eff ective and reliable treatment option when added to muscle stretching exercises in myofascial pain syndrome. © 2021, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Investıgation of the Effect of Multiple Sclerosis on Peripheral Nerves(Yuzuncu Yil Universitesi Tip Fakultesi, 2021) Gökpinar, G.; Çağaç, A.; Milanlioğlu, A.Multiple sclerosis is an autoimmune central nervous system disease characterized by inflammation, demyelination and axon damage. The aim of this study was to determine peripheral nerve involvement and related factors in patients wi th multiple sclerosis. In our study, 35 (70.0%) of the patients were suffering from Relapsing Remitting Multiple Sclerosis, and 15 (30.0%) of them had Secondary Progressive Multiple Sclerosis. The mean age of the symptom onset of the disease in patients was 30.44 ± 1.06 years, the mean score of the Multiple Sclerosis Disability Scale (EDDS) was 2.97 ± 2.58 points Polyneuropatia was detected in 12 (24.0%) patients. Polyneuropathy was detected in 3 patients (6.0%) in the contr ol group. In our study, peripheral nerve involvement was higher in male patients than female patients. There was a very strong positive correlation between the duration of disease development and the Extended Multiple Sclerosis Disability Scale scores. In the study, it was determined that multiple sclerosis was more commonly seen in women in accordance with the literature, and EDDS scores increased as the disease duration increased. © 2021, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Relationship Between Vitamin D Deficiency and Chronic Tension-Type Headache(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Çağaç, A.Vitamin D Deficiency is a possible factor involved in the etiology of chronic tension-type headache (CTTH). Vitamin D is directly associated with lifestyle and eating habits and affects a large proportion of the general population. The aim of this study was to investigate the role of vitamin D deficiency in CTTH. Patients aged 18 years or older that presented to our Neurology polyclinic with CTTH between December 1, 2017 and March 31, 2018 were reviewed retrospectively. Patients were assigned into three groups based on their Vitamin D levels: (I) deficiency state: <10 ng/mL, (II) insufficiency state: 10-25 ng/mL, and (III) optimal state: 25-70 ng/mL. The groups were compared with regard to age, gender, and Vitamin D levels. The study included a total of 782 patients comprising 32% men and 68% women with a mean age of 34 years. Based on their serum Vitamin D levels, 482 (61.6%) patients were classified into Group I (<10 ng/mL), 211 (27.0%) patients into Group II (10-25 ng/mL), and 89 (11.4%) patients into Group III (>25 ng/mL) and a significant difference was found among the three groups with regard to serum Vitamin D levels (p<0.01). Vitamin D deficiency is an increasingly major health problem due to modern lifestyle.
