Article Contribution of Sonoelastography in the Differentiation of Benign and Malignant Breast Masses: a Comparative Analysis on Sonographic Birads Classification(Yuzuncu Yil Universitesi Tip Fakultesi, 2017) Arslan, H.; Akdemir, Z.; Yavuz, A.; İslamoglu, N.; Çelik, S.; Özgökçe, M.; Tolunay, H.E.In this study, we aimed to investigate the effectivity of ultrasonographic elastography in the differentiation of benign and malignant lesions detected on B-mode ultrasonography. The retrospective study included 133 consecutive patients who underwent B-mode USG followed by elastography between January 2013 and December 2015. The lesions were scored by using B-mode USG with breast imaging-report and data systems (BIRADS) and then sonoelastographic color-coded maps were obtained by elastography in a single session. Sonoelastographic scoring was achieved with a 5-point scoring system. Elasticity scores and strain ratio (SR) values were compared with histopathological findings. The 133 patients included 132 (99.2%) women and 1 (0.8%) man. Mean age was 40.45±14.8 years. The lesions were mostly localized in the left breast (n=70, 52.3% vs. n=63, 47.7%). Depending on the pathological findings, 94 (71.2%) lesions were classified as benign and 39 (28.8%) as malignant. Mean SR value was 4.419±1.43, with 5.5 in malignant lesions and 4.3 in benign lesions. At the cut-off value of 4.95 for SR, the sensitivity and specificity of USG in elastographic examination were 81.6% and 78.7%, respectively. A significant correlation was found between the SR value and the BIRADS score calculated on USG (rho=0.86; two-tailed; p<0.001). Similarly, a significant correlation was found between the SR value and the elasticity score (rho=0.88; two-tailed; p<0.001). In conclusion, the use of sonoelastographic examination following the USG examination with B-mode ultrasonography can be a complementary diagnostic method that increases the specificity of USG. All rights reserved.Article Effects of Urea, Molasses and Urea Plus Molasses Supplementation To Sorghum Silage on the Silage Quality, in Vitro Organic Matter Digestibility and Metabolic Energy Contents(Asian Network for Scientific Information, 2007) Güney, M.; Demirel, M.; Çelik, S.; Bakici, Y.; Levendoǧlu, T.Silage type sorghum harvested at milk stage were ensiled for 70 days with no additive or differing levels of Urea (U), Molasses (M) and Urea+Molasses (U+M) to determine silage quality, in vitro organic matter digestibilities (IVOMD) and metabolic energy contents (ME). According to physical properties, all silages are either excellent or satisfactory. While addition of urea decreased organic matter digestibilities and metabolic energy contents of the silages, it was not negatively affect fermentation quality. It has been noted that it is possible to produce high-quality silages with all of the additives and 0.5U%+5M% addition into silages produced the best quality silages in terms of fermentation quality criteria, organic matter digestibility and metabolic energy contents. © 2007 Asian Network for Scientific Information.Article Evaluation of Fermentation Qualities and Digestibilities of Silages Made From Sorghum and Sunflower Alone and the Mixtures of Sorghum-Sunflower(Asian Network for Scientific Information, 2006) Demirel, M.; Bolat, D.; Çelik, S.; Bakici, Y.; Tekeli, A.The objectives of this study were to evaluate fermentation qualities and digestibilities of silages prepared om sorghum (S) and sunflower (SF) alone and the mixtures of sorghum and sunflower at differing rates 75% sorghum+25% sunflower (75S25SF), 50% sorghum+50% sunflower (50S50SF) and 2% 5sorghum+75 sunflower (25S75SF)] ensiled in 120 L plastic barrel for 90 days. pH values of sorghum and the mixtures were significantly lower than that of sunflower silage (p<0.05). Concentrations of lactic, acetic, propionic and butyric acids were significantly greater in sunflower silage compared with sorghum silage (p<0.05). Silage acid concentration tended to decrease with increasing levels of sorghum in the mixtures. While concentrations of DM, OM and NDF were lower, concentrations of CP and EE were higher in sunflower than sorghum silage (p<0.05). Sunflower had greater DM, CP and EE but lower ADF and NDF digestibilities compared with sorghum silage (p<0.05). As percentage of sunflower increased, DM, CP and EE digestibilities increased but ADF and NDF digestibilities decreased in the mixtures. It has been concluded that better quality silages could be obtained by mixing sorghum and sunflower at 50% ratio. © 2006 Asian Network for Scientific Information.Article Factors Affecting Mortality in Emergency Surgery in Cases of Complicated Colorectal Cancer(Medical Association of Zenica-Doboj Canton, 2016) Kızıltan, R.; Yılmaz, Ö.; Aras, A.; Çelik, S.; Kotan, Ç.Aim To evaluate retrospectively demographic, clinical and histopathological variables effective on mortality in patients who had undergone emergency surgery due to complicated colorectal cancer. Methods A total of 39 patients underwent urgent surgical interventions due to complicated colorectal cancer at the Department of General Surgery, Dursun Odabaş Medical Center, between January 2010 and January 2015. Thirty three of these were included in the study. Six patients were excluded because complete medical records had been missing. Medical records of the 33 cases were retrospectively reviewed. Results There were 14 (42.5%) male and 19 (57.5%) female patients. Mean age was 60 years (range: 32- 83 years); 14 (42.5%) patients were less than 60 years old, while 19 (57.5%) were 60 years old or older. Operations were performed due to perforation (39.3%) and obstruction (60.6%) in 13 and 20 patients, respectively. Tumor localization was in the right and transverse colon in nine (21.2%) and in the left colon in 24 cases (72.7%). Eleven (33.3%) patients underwent resection and anastomosis, 13 (39.3%) resection and ostomy, and nine (27.2%) patients underwent ostomy alone without any resection. Postoperative mortality occurred in nine cases (27.2%). Conclusions High mortality should be expected in females older than 60 years with a left sided colon tumor or with another synchronous tumor and in perforated tumors. Unnecessary major resections should be avoided and primary pathology should be in the focus of treatment in order to decrease the mortality and morbidity rates. © 2016, Medical Association of Zenica-Doboj Canton. All rights reserved.Article Roadside Bombs and Routes To Life: the Experience of Turkey(Yuzuncu Yil Universitesi Tip Fakultesi, 2018) Çelik, S.Injuries caused by roadside improvised explosive devices (IEDs) have recently been on the increase in Turkey. The aim of the present study is to evaluate the types of injuries sustained by victims of IEDs, and the pre -hospital/inter-hospital handling of patients transported to a Level 1 trauma center following roadside bombings. Data were obtained from the registry of an earlier multi-centric prospective study designed to evaluate injuries caused by high kinetic energy weapons. Here, we retrospectively analyze injuries resulting from roadside bombs. All patients in the registry from August 2015 to May 2016 who were victims of roadside explosions were included. Patient demographics, injury severity scores (ISS), wound types, and transfusions need s were recorded and outcomes (length of hospital stay and mortalities) were evaluated. Of 390 patients injured by high kinetic energy weapons, 51 were wounded by roadside bombs (an estimated hospital incidence of 13; 1%). Fourteen separate incidents were observed (an average of 3.6 patients per incident). Of these patients, 8 were civilian and 43 were security personnel. The mean injury rate to multiple body regions was 71.8% and the mortality rate was 15.7%. Pneumothorax and pulmonary contusion were the le ading pathologies in terms of lethality. Vehicular accidents were frequently observed as causes of tertiary blast injuries. Twelve patients were transported to hospital who did not have physical injuries Our experience has shown that a roadside bomb not only causes an explosion, but also possibly a vehicular accident, burns, and, severe psychological trauma as well. © 2018, Yuzuncu Yil Universitesi Tip Fakultesi.
