Article Carotid Intima-Media Thickness and Lipid Profile in Children Patients With Hypothyroidism(Yuzuncu Yil Universitesi Tip Fakultesi, 2022) Çetin, M.; Eren, A.; Bitkin, E.Ç.; Yürektürk, E.; Goya, C.; Karaman, K.Hypothyroidism is a condition where there is a decrease or absence of thyroid hormone production as a result of a disorder occurring anywhere in the hypothalamo-pituitary-thyroid axis. There are very few studies in the literature reporting on the effect of hypothyroidism on lipid metabolism and carotid intima-media thickness (CIMT) in children. The present study was designed to determine whether hypothyroidism is associated with an increase in the CIMT and lipid levels in patients under thyroid hormone replacement therapy. The study included 30 children (under thyroid hormone replacement therapy) diagnosed with hypothyroidi and a control group comprising 30 healthy, euthyroid children. All the patients were examined for clinical characteristics, and the serum lipid levels and the CIMT were measured. Mean age was 8.47 ± 5.22 years in the patient group and 8.82 ± 4.91 years in the control group. Patient group TSH value was significantly higher than control group (9.95 ± 18.43, 1.93 ± 1.04, p = 0.021, respectively). The t otal cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and TG values were similar in both group (p = 0.065, p = 0.429, p = 0.219, and 0.391, respectively). CIMT values of the patient and control groups did not differ significantly ( 0.40 ± 0.08 and 0.39 ± 0.07, p = 0.920, respectively) In this study, contrary to the rare studies in the children's age group in the literature, no significant changes in lipid pr ofile and CIMT were detected in patients with hypothyroidism. In spite of prominent falls in the incidence, Acute Rheumatic Fever remains (ARF) an important cause of morbidity and mortality associated with acquired heart disease in underdeveloped territories. Acute poststreptococcal glomerulonephritis (APSGN) still remains a frequent form of glomerulonephritis in third-world countries, particularly in areas where the disease occurs in epidemics. Both diseases may be seen simultaneously in the same patient, but both diseases have different epidemiological, immunological and bacteriological features, however, this is a rare condition. All rights reserved.Article Effects of Dietary Vitamin E Levels on Egg Production Traits, Fertility and Hatching Rate in Laying Partridges(2008) Şengül, T.; Çetin, M.; Söğüt, B.; Yurtseven, S.To determine the effects of different levels of supplemental vitamin E on egg production traits, fertility and hatching rate of laying partridges (Alectoris chukar), total 180 partridges were assigned at random to 4 different treatment groups. The treatment lasted 147 d (March-August) in a window closed and natural ventilated environment. Basal diet was supplemented with vitamin E 30, 45, 60 and 75 mg kg1 in groups 1 to 4, respectively. Supplemental vitamin E doses improved feed intake, fertility rate and hatching rate. © GSP, India.Article Evaluation of the Causes of Early and Late Thrombocytopenia in the Newborn(Ortadog u Reklam Tanitim Yayincilik Turizm Egitim Insaat Sanayi ve Ticaret A.S., 2018) Karaman, K.; Demir, N.; Çetin, M.; Geylan, H.; Değer, İ.; Başaranoğlu, M.; Öner, A.F.Objective: Thrombocytopenia is the most common hematological disorder that is encountered in the neonatal intensive care unit (NICU). The incidence of thrombocytopenia in neonates may varies greatly depending upon admitted patients population of unit. This study was performed on neonates admitted to Yuzuncu Yil University of Dursun Odabaş Medical Center of NICU, the incidence and causes of thrombocytopenia. Material and Methods: This retrospective study was conducted on the medical records of all neonates hospitalized at our hospital between May 2012 and March 2015 and those with thrombocytopenia were included in the study. The laboratory results of these newborns were retrospectively screened from the computer program and files, and included in the study were those with a thrombocytopenia level of less than 150.000/μL. Results: About 2780 patients were admitted to our neonatal intensive care unit during the study period. Of the 2780 neonates, 117 (4.2%) had thrombocytopenia (60.7% early onset and 39.3% late onset). Seventy-two of them (61.5%) were preterm. Intrauterine growth restriction, maternal hypertension, respiratory distress syndrome and sepsis were more common causes. Severe thrombocytopenia (<50.000/µL rate was found in 27% of neonates. In addition, although the rate of thrombocytopenia in asfixia was reported in the literature as 12-14%, this rate was 6.8% in our cases. There was no relation between occurrence of thrombocytopenia and gender. Conclusion: This study reveals that the causes of neonatal thrombocytopenia may show variations. In our study, intrauterine growth restriction, maternal hypertension, respiratory distress syndrome, and sepsis seem to be important risk factors for thrombocytopenia in neonates. Copyright © 2018 by Türkiye Klinikleri.Article The Influence of Loud Crying on Pulmonary Gradients Obtained by Transthoracic Echocardiography in Children With Pulmonary Valve Stenosis(Yuzuncu Yil Universitesi Tip Fakultesi, 2020) Çetin, M.Pulmonary valve stenosis (PS) is the obstruction of blood outflow from the right ventricle of the heart at the level of the pulmonary valve. Crying is associated with an increase in venous return and it has been known to have some acute effects on cardiovascular system, the data about the alterations in echocardiographic findings during crying is limited. To explore the influences of loud crying in evaluation of transvalvular gradient obtained by Doppler echocardiography in children with pulmonary valve stenosis (PS). Our study group included a total of 48 children, aged between 2 and 43 months who were diagnosed with pulmonary valve stenosis based on the clinical signs and laboratory (Echocardiography) findings in our hospital. We compared the pulmonary gradients obtained by echocardiography while the children were crying and while they were calm. Totally 48 patients (23 male, 25 female) diagnosed with PS in our hospital were included in the study. The mean age of the patients was 19.27 ± 11.14 months (range: 2-43 months). During crying, both maximum and mean pulmonary gradients were statistically significantly higher compared with the results obtained while the patients were calm (p < 0.001, p = 0.001, respectively). In conclusion, significant increases in both maximum and mean pulmonary gradients were determined during loud crying. Since these parameters are the main determinants of management of patients with PS, this condition should be kept in mind and if possible these measurements should be obtained while the children are in a calm state. All rights reserved.Article Ovarian Artery Injury as a Peritoneal Dialysis Complication: Case Report(2009) Melek, M.; Edirne, Y.; Beger, B.; Çetin, M.Patients treated with peritoneal dialysis are prone to many complications like peritonitis, catheter exit-site infection, catheter disfunction, insufficient clearance and ultra-filtration. Although hemoperitoneum is described as a benign complication, one should be cautious against it because of its life-threatening severe form seen in approximately 20 % of patients. There are several studies reporting intra-peritoneal hemorrhage during the procedure but a specific localization of the hemorrhage is not mentioned. We present here a case of 2 months old female infant who died due to intra-abdominal hemorrhage caused by ovarian artery injury on the 4th day after peritoneal dialysis was performed. Catheters made of hard material can cause hemorrhage. Fixation of these catheters is vital.Article A Quadricuspid Aortic Valve That Was Misdiagnosed as Rheumatic Carditis: A 13-Year Case(Ortadog u Reklam Tanitim Yayincilik Turizm Egitim Insaat Sanayi ve Ticaret A.S., 2021) Çetin, M.; Başaranoğlu, M.; Karaman, S.; Doğan, E.Quadricuspid aortic valve anomaly is a very rare congenital heart anomaly. Its incidence is 0.01-0.04 percent. This anomaly is characterized by the presence of four aortic valves of the same or different sizes. This may often lead to progressive aortic regurgitation or rarely aortic valve stenosis. In aortic valve evaluation with transthoracic echocardiographic, if a careful examination is not performed, the diagnosis of quadricuspid aortic valve may be overlooked or may be misdiagnosed as insidious acute rheumatic carditis. Here, we presented a 13-year-old patient with quadricuspid aortic valve who was admitted to another center with chest pain, and was mistakenly diagnosed with insidious rheumatic carditis due to the aortic regurgitation detected on his transthoracic echocardiographic examination. We aimed to draw attention to this rare congenital heart valve disease. This defect is seen in all age groups and is usually diagnosed incidentally. The aneurysm can be asymptomatic or present with symptoms related to supraventricular arrhythmias or thromboembolic phenomenon. We present the case report of a 74-year-old male with right atrial aneurysm which was incidentally detected during echocardiographic evaluation. Copyright © 2017 by Türkiye Klinikleri.Article A Study on Determining Protein Level in Diet of Partridge (Alectoris Chukar) During Growing Period (0-4 Wk)(2006) Ôengül, T.; Çetin, M.; Söüt, B.; Gürbüz, Y.This study was conducted to determine growth performance of partridge (A. chukar) fed by diets include different level of CP through 0-4 weeks of age. First, second, third and fourth groups had 28, 26, 24 and 22 % CP in diet, respectively. At the end of 4 weeks of growth period, live weights of group 1, 2, 3 and 4 were 125,8, 128.6, 124.6 and 120.4 g for males, 113.4, 111.3, 110.3 and 105.2 g for females, respectively. Protein levels in diet had significant effect on live weight at 1st and 3rd week of age only. However, sex had significant (P<0.001) effects on live weight of the birds through growth period. Different CP level in diet did not affected weekly and cumulative feed consumption significantly. Through 0-4 week of age, cumulative feed consumption of the groups was 194.7, 190.8, 195.6 and 185.5 g for group 1st, 2nd, 3rd and 4th, respectively. Differences among the groups in terms of FCR were significant (P<0.05) at 1st week of age only, and birds fed by diet includes 26 % CP had the best FCR at this week. FCR of the 1st, 2nd, 3rd and 4th treatment groups were calculated as 1.85, 1.81, 1.83 and 1.89, respectively. As a result, it could be said that 28, 26, 24 and 22 % CP level in diet did not affect growth performance of partridges through 0-4 weeks of age when taking account live weight, feed consumption and FCR of the partridge. © Asian Network for Scientific Information, 2006.Article Two Cases With Familial Hemophagocytic Lymphohistiocytosis(Ortadog u Reklam Tanitim Yayincilik Turizm Egitim Insaat Sanayi ve Ticaret A.S., 2018) Geylan, H.; Karaman, K.; Çetin, M.Hemophagocytic lymphohistiocytosis (HLH) is a fatal condition of severe hyperin-flammation that results from an uncontrolled proliferation of activated lymphocytes and histiocytes secreting high amounts of inflammatory cytokines. The perforin gene mutation was the first genetic deffect to be described in association with HLH. HLH is a distinct clinical entity characterized by fever, pancytopenia, splenomegaly and hemophagocytosis in bone marrow, liver or lymph nodes. We present phenotypical and labaratory description of two patients with familial HLH. The diagnosis of HLH was made according to the guidelines of the Histiocyte Society. Fever was present in two patients with hemophagocytic lymphohistiocytosis. The other most common symptoms were malaise and weight loss. Hepatosplenomegaly was present in two patients. Hemophagocytosis was documented in bone marrow examinations of two cases. Thrombocytopenia, neutropenia, hypertriglyceridemia and hyperferritinemia were found in two patients. Genetic analysis of the patients were performed, with the coding region of the perforin gene. Our cases had perforin gene muatation. HLH is a rare but mortal disease when not treated on time. Awareness of clinical symptoms and diagnostic criteria of HLH, initiation of early immunosuppressive treatment; life-saving as well as allowing for stem cell transplantation. © 2018 by Türkiye Klinikleri.
