Article Comparison of Upper and Lower Extremity Functions in Primary and Secondary Progressive Multiple Sclerosis Patients(Turkiye Klinikleri, 2019) Atilla, N.; Çilingir, V.; Milanlioğlu, A.; Yilgör, A.Objective: The aim of this study was to find out whether upper extremity is relatively preserved and to what extent it is affected by Primary Progressive Multiple Sclerosis (PPMS) through the evaluation and comparison of upper extremity fine motor skills and walking ability in PPMS and Secodary Progressive Multiple Sclerosis (SPMS) patients with Expanded Disability Status Scale (EDSS) score of 4 and above. Material and Methods: A total of 65 progressive Multiple Sclerosis (MS) patients admitted to the Neurology Outpatient Clinic of Yüzüncü Yıl University Faculty of Medicine between January and December 2017 with EDSS scores ranging from 4.0 to 8.5 were included in our study. Expanded Disability Status Scale (EDSS), 9-Hole Peg Test (9-HPT) and Timed 25-Foot Walk (T25FW) Test were applied to the patients. Results: 9-Hole Peg Test right hand score of PPMS patients with EDSS score between 5.1 and 6.0 was 32.89±7.42 sec, whereas in SPMS patients within the same score range, it was 56.15±54.57 sec (p>0.05). 9-Hole Peg Test left hand score was found as 39.64±16.71 sec in PPMS patients and 47.51±27.32 sec (p>0.05) in SPMS patients. 9-Hole Peg Test mean score was 36.25±11.83 sec in PPMS patients and 51.83±38.18 sec in SPMS patients (p>0.05). The T25FW test was calculated only for 39 patients who were able to perform all tests. In 14 patients with PPMS, the first-stage T25FW test score was 35.62±22.2 sec, the second-stage score was 36.21±24.73 sec, and the mean T25FW test score was 35.92±22.81 sec. In 25 of SPMS patients, the first-stage T25FW test result was 36,12±32,84 sec, the second-stage T25FW test result was 32,77±26,11 sec, and the mean T25FW test result was 34,4±28,75 sec. Conclusion: In PPMS and SPMS patients with EDSS score of 5.1-6.0, the mean EDSS scores were observed to be closer to each other, and in PPMS patients in this group, the upper extremity function was relatively better, although not statistically significant. No significant difference was found between the groups in terms of lower extremity function. © 2019 by Türkiye Klinikleri.Article Dyke-Davidoff Syndrome With Cerebral Hypometabolism and Unique Crossed Cerebellar Diaschisis in 18f-Fdg Pet/Ct(2015) Demir, Y.; Sürücü, E.; Çilingir, V.; Bulut, M.D.; Tombul, T.A 23-year-old man with Dyke-Davidoff-Masson syndrome (DDMS) was admitted to the hospital with increasing frequency of epileptic seizures. Physical examination revealed mental retardation, left facial asymmetry, and left-sided spastic hemiparesis. Dysdiadochokinesia on the left upper limb was detected, and there was no dysmetria. MRI confirmed the well-known radiological features of DDMS. PET/CT demonstrated cerebral and contralateral cerebellar hypometabolism. We present DDMS with crossed cerebellar diaschisis, which was demonstrated by PET/CT.Article Ischemic Stroke in a Young Man With Mthfr A1298c and Ace I/D Polymorphism(Turkish Society of Cerebrovascular Diseases, 2014) Çilingir, V.; Milanlioǧlu, A.; Tombul, T.Ischemic stroke is a leading cause of death and disability worldwide. Mutations in several candidate genes involving angiotensin-converting enzyme (ACE) and methylenetetrahydrofolate reductase (MTHFR) gene have been found to be associated with ischemic stroke. This report describes the case of a 29 year-old man who presented with sudden onset left hemiparesis and hemihipoestesia. Magnetic resonance imaging investigations showed acute ischemic infarct in the right parietal region. Mutation analysis revealed homozygout MTHFR A1298C and ACE I/D polymorphisms and laboratory invastigations showed mild hyperhomocysteinemia. No other risk factor was detected for ischemic stroke etiology.Article Levels of Leukocyte Oxidative Dna Damage (8-Ohdg), Serum Coenzyme Q10 and Lipid Peroxidation in the Formation Attacks of Patients With Multiple Sclerosis(Suleyman Demirel University, 2017) Cokluk, E.; Milanlıoğlu, A.; Huyut, Z.; Çilingir, V.; Alp, H.H.; Aydın, M.N.; Balahoroğlu, R.Multiple sclerosis (MS) is a demyelinating disease of the nervous system. Evidence about oxidative stress plays an important role in the pathogenesis of MS is increasing day by day. In our study, we aimed to investigate the effect of oxidative DNA damage and oxidative stress in the pathogenesis of MS disease. Blood samples were obtained from during an attack (Group 1), between attacks (Group 2) of MS patients (20 male and 10 female) and 30 healthy volunteers (Group 3). Malondialdehyde (MDA) levels as indicator of oxidized lipids were detected using flourescence dedector with high pressure liquid chromatograph (HPLC). DNA was extracted from leukocytes of control and patients with MS and then we measured 8-hydroxy-2' -deoxyguanosine (8-OHdG) and deoxyguanosin (dG) by using HPLC method with electrochemical and UV detector, respectively. Measurement of oxidized coenzyme Q10 (CoQ10) and reduced CoQ (CoQ10H) was performed by using UV detector with HPLC method. Serum MDA level of group 1 was significantly higher than those in group 2 and group 3 (p< 0.001). 8-OHdG/106 dG ratio of group 1 was significantly higher than those in group 2 and group 3 (p< 0.001). CoQ10/CoQ10H rates of group 1 were significantly increased compared with group 2 and group 3 (p<0.001). In conclusion, we observed that oxidative DNA damage, lipid and mitochondria oxidative damage were high in blood of patients with MS. It seems that oxidative stress acts a play role the pathogenesis of MS patients as well as induces attacks. © 2017 Suleyman Demirel University. All rights reserved.
