Article Comparison of Plaque and Distal-Wedge Tibial Nail in the Treatment of Extra-Articular Distal Tibia Fractures(Yuzuncu Yil Universitesi Tip Fakultesi, 2017) Adanaş, C.; Uymur, Y.E.; Özkan, S.; Kaya, Ş.Tibial fractures are encountered in various age-groups. Depending on fracture type and pattern, many types of treatment options including plate fixation, intramedullary fixation, circular external fixation are widely used. Depending on implant design, surgical approach and accompanying soft tissue conditions, success of these fractures could be various and be jeopardised. In this study, we wish to evaluate the benefits and side effects between plate and distal-wedge tibial nail fixation. Between 2013 and 2016 (in Van Regional, Education and Research Hospital, Turkey) 38 patients (25 male, 13 female patients: ages range 18 to 61 years, mean age is 41) treated surgically using distal-wedge tibial nail fixation (amount:16) and plate fixation (amount:22). The patients were evaluated by AOFAS score (documentated 1.5 and 6 months postoperatively), postop hospitalisation days, perop fluouruscopy imaging (scopy shooting) times and union time (months). Average follow-up time was 11 months (range: 9 to 28 months). There was a shorter recovery period in the nail group and the hospital stay was shorter. (P <0.001)The AOFAS score was higher at 1.5 months postoperatively and the number of scopies was higher at 1.5 months olds (p <0.001) No significant difference was found in postoperative 6th month AOFAS score between plate and nailing groups. We have found and concluded that DSBLS (Distal Supportive Bolt Locking Screw) screw of this tibial nailing fixation system could provide distal interlocking strong enough especially when fracture is quite close to the distal tibial articular face, using distal wedge tibial nails and components. © 2017, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Effect of Breeder Age and Slaughter Weight on Meat Quality Traits of Broiler Breast and Leg Meats(Verlag Eugen Ulmer, 2014) Yalçin, S.; Güler, H.C.; Yaşa, İ.; İzzetoğlu, G.T.; Özkan, S.The objective of the present study was to evaluate the effect of breeder age (28-wk vs. 48-wk of age breeders) on meat quality traits of breast and leg meats of broilers slaughtered at 1800, 2100 and 2400 g weights. The pH24, colour, thawing and cooking loss, texture, initial protein solubility and myofibril dimensions were measured. Slaughter weight affected pH24, lightness, thawing and cooking loss and protein solubilities of broiler breast and leg meat.; Tenderness of leg meat decreased with slaughter weight. Breast meat from 48-wk breeders lost more water during thawing. A significant breeder age by slaughter weight interaction showed that the increases in initial myofibrillar protein solubility of breast meat and both initial myofibrillar and sarcoplasmic protein solubilities of leg meat with slaughter weight were higher in broilers from 28-wk breeders than those from 48-wk breeders There was negative correlation between breast meat pH24 and L*, b*, and thawing loss whereas leg meat pH24 was positively correlated with a*, negatively correlated with b*, texture, thawing and cooking loss. A negative correlation was found between cooking loss and myofibrillar protein solubilities of breast meat.; The results suggested that breeder age may affect thawing loss of breast meat and total initial protein solubility of both breast and leg meats. Higher myofibrillar protein solubility found for breast and leg meat of broilers from 28-wk breeders may indicate better functional qualities. © Verlag Eugen Ulmer, Stuttgart.Article Evaluation of the Hand Anthropometric Measurement in Adhd Children and the Possible Clinical Significance of the 2d:4d Ratio(Yuzuncu Yil Universitesi Tip Fakultesi, 2017) Buru, E.; Gözil, R.; Bahçelioğlu, M.; Özkan, S.; İşeri, E.Attention deficit/hyperactivity disorder (ADHD) is characterized with decreased storage time of the information related to the lack of attention and behavioral changes such as hyperactivity and anxiety with hand features. Our purpose was to investigate the possibility to use the 2D:4D ratio in clinics as an easy prediagnostic parameter. A total of 540 people (104 ADHD, 436 controls) between 7-17 years old were included in our study. Our study group revealed a statistically meaningful difference between the ADHD and their controls in 2D:4D ratio of their right hand and the 2D:4D and 4D:5D ratios of their left hand. The male ADHD subjects and controls also demonstrated significant difference in their 2D:4D, 2D:5D, 3D:4D, and 3D:5D ratios of the right hand and the 2D:4D and 2D:5D ratios of the left hand. Especially, ADHD boys had a more feminized 2D:4D left hand ratio than the controls. While, there was a significant difference in all finger ratios of the right hand and the 2D:4D and 4D:5D ratios of the left hand between the ADHD girls and their controls. Thus, the more masculine 2D:4D right hand ratio was observed in ADHD girls. Our results pointed out that the ADHD and some other child psychiatric disorders could be early diagnosed related to the anthropologic parameters. All rights reserved.Article Hook Plate Applications in Type 3 Acromioclavicular Dislocations(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Özkan, S.; Adanaş, C.Acromioclavicular (AC) joint dislocations are caused by sports injuries fall from height or traffic accidents. While the treatment of Type 1 and Type 2 in AC dislocations is conservative, the treatment of Type 4, 5 and 6 is surgery. Treatment of type III dislocations is controversial. In this study, we aimed to investigate the functional results of patients who surgical treatment by using clavicle hook plate in Type III AC joint dislocations 21 patients (18 males, 3 females; mean age 34 years) with Type III AC joint dislocation who were admitted to our clinic between 2015-2017 were treated with clavicle hook plate. The surgical results of the patients at the 6th month were evaluated according to the Constant score. Of the total 21 patient included in the study, n=18 were male (%86), and n=3 were female (%14). All patients were treated with clavicular hook plate. The average follow-up after treatment is 11 months. Functional results were obtained according to Constant scoring. There was no infection after surgery. Plaque irritation occurred in 2 patients and plaque was removed due to prolonged pain. 1 patient had recurrence. Although conservative treatment of Type 3 AC dislocations can provide adequate functional results in the long term, we believe that early results of the surgical treatment using clavicle hook plate is a reliable and effective method especially in the young age group. © 2019, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Impact of the First Six Months of the Covid-19 Pandemic on Orthopedic and Hand Trauma Surgery(Yuzuncu Yil Universitesi Tip Fakultesi, 2021) Tokyay, A.; Özkan, S.; Güven, N.; Türközü, T.; Adanaş, C.; Gökalp, M.A.; Koç, M.R.Reduced mobility due to COVID-19 prevention measures caused major changes in the number and types of orthopedic trauma cases. This study aimed to compare the profile of trauma-related general orthopedic surgery and hand surgery cases between the first six months of the COVID-19 pandemic and the corresponding periods of previous years in order to determine strategies for managing orthopedic traumas in unusual circumstances such as pandemics. Patients over 18 years old who underwent orthopedic surgery due to trauma were grouped according to sur gery date as pre-pandemic group 1 (G1, March 16–September 18, 2018), pre-pandemic group 2 (G2, March 18–September 20, 2019), and the pandemic group (G3, March 19–September 21, 2020). Between-group differences with p<0.05 were considered significant. The study included a total of 3522 patients: 1351 in G1, 1427 in G2, and 744 in G3. There was a significant decrease in the number of patients in G3 compared to the other groups (p=0.001), while the number of hand surgeries did not differ significantly between the groups. As a result, the ratio of hand trauma surgeries to general orthopedic traumas was significantly higher in G3 (p <0.05). During the COVID-19 pandemic, there was a decrease in orthopedic trauma surgeries and an increase in hand trauma surgeries. All rights reserved.Article Investigation of Correlations Between Joint Health, Functionality, and Mri Score in Hemophilic Arthropathy of the Elbow Joint: Cross-Sectional Study(Yuzuncu Yil Universitesi Tip Fakultesi, 2023) Tat, A.M.; Özkan, S.; Tat, N.M.; Karaman, K.; Öner, A.F.Hemophilic arthropathy (HA) of the elbow joint should be examined separately from the knee and ankle because of the anatomical and biomechanical differences of the elbow. The aim is to investigate possible correlations between age, range of motion (ROM), muscle strength, upper extremity functions, joint health and radiological findings. Twenty-seven joints of 20 patients aged 11-30 years with findings of HA in elbow were evaluated. International Prophylaxis Study Group Magnetic Resonance Imaging (IPSG MRI) score was used in the radiological evaluation. In physical evaluations, ROM and muscle strength were measured by goniometer and digital dynamometer, respectively. Joint health was evaluated with Hemophilia Joint Health Score-Elbow Point (HJHS-EP) and upper extremity functionality with Quick-Disability of Arm Shoulder and Hand (Q-DASH). The age was strongly correlated with HJHS-EP, loss of extension and pronation ROM and moderately correlated with IPSG MRI score. HJHS-EP showed strong correlation with loss of extension. There were a moderate correlation between Q-DASH and muscle strength of the elbow. The IPSG MRI scores were not significantly correlated with physical examinations. The elbow joint should be evaluated both radiographically and physically and these assessments cannot be alternatives to each other. Even goniometric assessment of elbow extension alone may provide important information about joint health. Functionality can be improved by increasing muscle strength. Since the deterioration in physical and radiographic examinations with age, it is recommended to conservatively or surgically treat of the elbow joint with physiotherapy at early ages. © 2023, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Pre-Operative Mean Corpuscular Volume, Red Cell Distribution Width and Neutrophil-To Ratio in Patients With Enchondroma(Yuzuncu Yil Universitesi Tip Fakultesi, 2017) Özkan, S.; Adanaş, C.; Danışman, F.Enchondromas are benign, solitary bone tumors originating from hyaline cartilage, that are usually located in the upper and lower extremities. They are equally distributed in both genders and in all age groups. 40 -65% of enchondromal cases are found in metacarpal bones. The most common involvement is the proximal phalanx, metacarpal bone, and distal phalanx. Involvement of distal phalanx in patients with enchondroma is rare. Our aim in this study was to investigate the pre -operative neutrophil count, white blood cell count, red cell distribution width, mean corpu scular volume, platelet count, hemoglobin, hematocrit and NLR values, and make a comparison with a control group. The most commonly involved sites are also investigated in the patient group. This retrospective study investigated the preoperative WBC, neutrophil, lymphocyte, RDW, MCV, NLR values in 20 patients and 20 controls, and compared the frequency of the involved sites among patients with enchondroma. WBC, neutrophil count, MCV, RDW were not found statistically significant. NLR ratios were different be tween groups, but not statistically significant. Involvement of the proximal phalanx was statistically significantly higher (p<0.05). This is the first study investigating the pre-operative RDW, MCV, NLR, PLT, Hgb, Hct and WBC in patients with enchondroma. Although there is no statistically significant difference when these results are compared we suggest that they should be studied in larger groups and in clinical tests, so that the malignancy potential may be predicted. © 2017, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Reliability of Youtube Videos on Knee Joint Injection(Yuzuncu Yil Universitesi Tip Fakultesi, 2021) Özkan, S.; Adanaş, C.; Kaplan, Ş.; Şah, V.Intra-articular injection requires a visual learning and the knee is the most commonly used joint for t his application. YouTube videos have become a source of visual learning. However, there is a obscurity whether YouTube videos on medical education provide safe and useful information. Therefore, this study aim to evaluate the reliability of YouTube videos on knee joint injection according to the current approach. The term ʺknee joint injection techniqueʺ was used to search the related YouTube videos. The videos were grouped in terms of the uploader, uploading years, and the number of views. The procedure was evaluated and scored for positioning of the patient, palpation of the landmarks, washing hands and wearing gloves, needle approach and entry, drug injection, removing the needle and placing the sponge and finally bandaging. Of the 61 videos, only 11 (18%) had enough quality. The score of videos uploaded by an institution was significantly higher than others (p=0.006). The score of videos viewed over (n=31) and lower (n=30) than 10000 times was 4.5 (IQR 2.0) and 3.5 (IQR 2.0), respectively (p=0.018). There was no statistical significance between the groups according to uploading years (p=0.694). YouTube videos are not enough qualified enaugh reliable visual learning of knee joint injection. The institutional YouTube videos are more educational sources than others. YouTube videos with higher views count also have higher reliability. © 2021, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Treatment of Osteochondral Lesions of the Talus With Transmalleolar Open Mosaicplasty(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Adanaş, C.; Özkan, S.To evaluate middle-term outcomes of patients that underwent open mosaicplasty due to osteochondral lesions of the talus causing chronic complaints. The retrospective study included 23 patients that were diagnosed with a Grade III, IV, and V osteochondral lesion of the talus according to the Bristol classification system between January 2014 and December 2017. Pain assessment was performed using Visual Analog scale (VAS 1-10) and functional outcomes were assessed using the AOFAS (American Orthopedic Foot & Ankle Society) Ankle-Hindfoot Rating System. Additionally, a questionnaire was filled out by each patient both preoperatively and at 1 year postoperatively. Mean AOFAS score was 55.652 preoperatively and was 88.913 postoperatively and a significant difference was found between the two scores (p<0.001). Similarly, mean VAS score was 7.39 preoperatively and was 2.04 postoperatively and a significant difference was found between the two scores (p<0.001). Open mosaicplasty appears to be a useful technique for the treatment of cystic osteochondral lesions of the talar dome larger than 10 mm in diameter. The technique is also safe and effective and allows successful closure of the cartilaginous loss with hyaline cartilage. © 2019, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.
