Article The Dynamics of Prehospital/Hospital Care and Modes of Transport During Civil Conflict and Terrorist Incidents(W B Saunders Co Ltd, 2017) Celik, S.; Dursun, R.; Aycan, A.; Gonullu, H.; Adanas, C.; Eryilmaz, M.; Guloglu, C.Objective: Prehospital and hospital care during incidents of mass violence and civil conflict involve a number of aspects that distinguish it from care during times of peace. We aimed to analyze the dynamics and outcomes of prehospital and hospital care during ongoing conflicts. Study design: Multicentric prospective observational study. Method: Patients enrolled in the study, which was conducted in Turkey, were all injured in armed conflict and taken to level 1 trauma centers. On admittance, patients were requested to complete a semistructured questionnaire containing questions on patient demographics, transport type, weapons used, injury severity score (ISS), and other incident-related factors. We analyzed patient outcomes (mortality, morbidity, complications, and length of hospital stay) and transfers of patients between hospitals. The present study evaluated the cases of 390 victims enrolled over a 9-month period and followed up for 6 months. Results: The majority of patients were transported by ambulances (n = 334, 85.6%); other transport modes were helicopters (n = 32, 8.2%) and private vehicles (n = 24, 6.2%). Nearly half of patients (48.7%) did not benefit by changing hospitals. During transport to hospitals, 4.1% of the vehicles in the study were involved in accidents. Using multiple regression analysis, only ISS (odds ratio [OR]: 1.098, 95% confidence interval [CI]: 1.044-1.156) and the Glasgow Coma Scale (OR: 0.744, 95% CI: 0.639-0.866) were found to affect mortality. In Receiver-operator characteristic analysis, a cutoff value of 22.5 for ISS had a sensitivity of 100% and a specificity of 89.6% for mortality. Conclusions: Despite lower ISS values, patient outcomes were worse in terror incidents/civil conflicts. Transport modes did not significantly affect outcomes, whereas hospital transport was found to be inefficiently used. (C) 2017 The Royal Society for Public Health. Published by Elsevier Ltd. All rights reserved.Article Functional and Radiological Out Comes of Patients With Full-Thickness Rotator Cuff Tears Repaired by Arthroscopic Knotted-Suture Bridge Double Row Method(Yuzuncu Yil Universitesi Tip Fakultesi, 2024) Akgun, E.; Tepedelenlioglu, H.E.; Kurdal, D.P.; Adanas, C.; Akpolat, A.O.Introduction: To evaluate the functional and radiological results of double-row arthroscopic rotator cuff repairs performed by the Knotted Suture Bridging method. Materials and Methods: The study included 36 patients. Before surgery, tear type, tear size, and muscle atrophy were examined by ultrasound (USG) and magnetic resonance imaging (MRI) in all patients. In the clinical evaluation, the preoperative and posto perative 2nd year results of the patients were compared by Visual Analogue Scale (VAS), scoring the University of California, Los Angeles (UCLA) and the shoulder index scoring of the American Shoulder and Elbow Surgeons (ASES). Sugaya MRI recovery analysis was used to evalu ate the structural healing of the tendon. Results: A significant decrease was found in the patients' VAS scores in the postoperative period. While the preoperative UCLA (0-35) score was 19.1, the median value in the second postoperative year was 29.7. (p<0.001). The median preoperative ASES index (0-100) was 46.7 before and 85 after surgery. (p<0.001) Both scores show a significant improvement in patients in the second postoperativ e year. MRI imaging showed good rotator cuff healing in 29 patients (Sugaya type 1-2), Sugaya type 3 in 2 patients, type 4 lesions in 2 patients, and type 5 in 3 patients. Conclusion: Double-row arthroscopic rotator cuff repairs performed by the Knotted Suture Bridging method in medium and large rotator cuff tears are highly effective in the tendon's functional and adhesive healing. © 2024, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Our Medium-Term Functional and Radiological Outcomes in Revision Total Knee Arthroplasty(Yuzuncu Yil Universitesi Tip Fakultesi, 2018) Ozdemir, H.; Kaya, S.; Adanas, C.; Tabak, A.Y.To investigate the medium-term outcomes of the patients who underwent revision total knee arthroplasty, and to come to a conclusion regarding the efficacy of the method by comparing these with literature data. 34 knees of 33 patients who underwent revision knee arthroplasty operation between January 2008 and January 2012 and were sufficiently followed-up were included into this study. Single-stage revision knee arthroplasty was performed for 25 knees for aseptic reasons, and two-stage revision knee arthroplasty was performed for 9 knees for septic reasons. For all patients, alpha, beta, gamma, sigma and total valgus angles were measured in pre-and post-operative anteroposterior (AP) X-rays and lateral X-rays based on American Knee Society radiological assessment form. For all patients, pre-and post-operative American Knee Society knee score and functional score was assessed. As laboratory work-up, all patients’ pre-and post-operative erythrocyte sedimentation rate, C-reactive protein, white blood cells values were compared. The patients’ mean knee score and functional score was detected to be 34.7 (4-57) and 33.5 (5-55), respectively, and to be 78.2 (62-90) and 73.8 (50-90) after revision. In pre-operative assessments performed based on Knee Society radiographic assessment form, mean alpha angle was found to be 95.3 (77-103), mean beta angle to be 84.8 (71-100), mean gamma angle to be 5.8 (0-30), mean sigma angle to be 83.3 (49-97), and mean total valgus angle to be 0.1 (-13-12). In post-operative assessments, mean alpha angle was found to be 94.7 (90-99), mean beta angle to be 88.4 (84-92), mean gamma angle to be 3.7 (0-10), mean sigma angle to be 88.1 (85-92), and mean total valgus angle to be 3.1 (-1-8). We think that when revision knee arthroplasty operation is performed for proper indication and according to the surgical technique, it is a highly effective and preferable method in decreasing the pain severity and improving the life quality of the patients. © 2018, Yuzuncu Yil Universitesi Tip Fakultesi.
