Article Anti-Inflammatory Effects of Tretinoin (all-Trans Acid) 0.1% and Adapalene 0.1% in Rats(Wiley, 2005) Akdeniz, N; Çalka, Ö; Özbek, H; Metin, AIn this study, the anti-inflammatory effects of tretinoin (all-trans-retinoic acid) 0.1% cream and adapalene 0.1% gel were compared in rats to determine whether there was a difference between these agents. Thirty-six rats of either sex were divided into six groups (two control groups, and an etodolac, indomethacin, tretinoin and adapalene group) of six animals each. Each group was given different drugs or chemicals. The inhibitory activities of the drugs were determined on carrageenan-induced rat-paw oedema. The inhibition rate (53.48%) in the tretinoin group was found to be higher than adapalene and controls (P < 0.05). Adapalene was found to have an inhibition rate of 10.28%, and when compared with the other groups, was found to have no statistically significant anti-inflammatory activity. They may cause symptoms resulting from neurovascular compression, hemorrhage, and direct mass effect. Design and patients: We present an unusual case of cervical tuberculous spondylodiscitis in a 25-year old man with a cold abscesses involving the retropharyngeal, mediastinal, and retroperitoneal areas bilaterally. The abscess tracked from the neck to the psoas muscles bilaterally. Following the diagnosis the patient received 9 months of antituberculous therapy. Results: MRI showed resolution on medical treatment alone. Conclusions: Even in the presence of massive paravertebral cold abscesses medical treatment alone may well suffice for this common worldwide disorder. MRI is ideal for monitoring regression of massive abscesses in deep anatomical locations.Article Palmoplantar Keratoderma Is Associated With Esophagus Squamous Cell Cancer in Van Region of Turkey: a Case Control Study(Bmc, 2005) Ilhan, M; Erbaydar, T; Akdeniz, N; Arslan, SBackground: Esophagus squamous cell cancer (ESCC) is the most common cancer in women with 20.2% and second in men with 10.7% relative frequency among all cancer cases diagnosed in Van Region in the east of Turkey. Ninety percent of all esophageal cancer cases are ESCC and 20 - 30% of them have family history of esophageal cancer. The most clear defined hereditary predisposition associated with ESCC is palmoplantar keratoderma (PPK). To examine the relationship between ESCC and PPK, we have carried out this case control study. Methods: The case group consisted of 48 subjects who had new diagnosis of ESCC and did not receive any chemo or radiotherapy. The control group consisted of 96 healthy individuals who were visitors of their relatives in the hospital. Two control persons who matched for age, gender, living place (urban/rural) and region were selected for each case. All subjects were evaluated for PPK by dermatologist. Evaluation was graded as none, mild, evident and severe. None and mild subjects were classified as negative for PPK; and others as positive. Relationship between ESCC and PPK was evaluated with odds ratios and confidence intervals for cases with or without family history of ESCC. Results: The PPK frequencies were 92.3% in ESCC cases with family history, 62.5% in ESCC cases without family history, 70.8% in all ESCC cases, and 28.1% in the control group. Odds ratios for cases with or without family history of esophageal cancer, and for the whole case group were found as 30.7 (95% CI = 3.8 - 247.4), 4.3 ( 95% CI = 1.9 - 9.8) and 6.2 ( 95% CI = 2.9 - 13.3) respectively. Conclusion: Presence of PPK lesions represents genetic susceptibility for ESCC. This susceptibility for ESCC is the highest among those who have PPK lesions and a positive family history of esophageal cancer. Pers. (Ranunculaceae) is a wild plant. It grows abundantly in the Van province. The species of Ceratocephalus genus possess irritant properties from the presence of an innocuous glycoside, ranunculin. Here, we document 3 patients who were admitted to our clinic in different times from different districts with a common complaint of a large irritant contact dermatitis, resembling second-degree burn injury after application of a plant poultice. The plant specimens in all patients were identified as C. falcatus and it was assessed worthwhile to report.Article Primary Varicella Infection Associated With Stevens-Johnson Syndrome in a Turkish Child(Japanese dermatolgical Assoc, 2005) Bay, A; Akdeniz, N; Çalka, Ö; Kösem, M; Öner, AF; Dogan, MStevens-Johnson syndrome (SJS) is defined as a severe erythema-multiforme-like eruption of the skin and lesions of the oral, genital and anal mucosa and hemorrhagic crusting on the lips; it is associated with fever, headache, and arthralgia. The disease is a part of a continuum of immunologically mediated mucocutaneous diseases at various grades of severity. SJS is often induced by drugs, but the pathophysiologic mechanism is completely unknown. Patients and their first degree-relatives may have genetic defects in their metabolic pathways that lead to the accumulation of toxic metabolites. However, viral infections are known triggers of this skin disorder. Varicella zoster virus has been only very rarely reported as an etiological agent in SJS, despite its high incidence as a pathogen in childhood. In this study, we report a case of primary varicella infection associated with SJS because of its rare presentation. To the best of our knowledge, our case is the youngest child in the literature.Article Report of a Turkish Child With Sjogren-Larsson Syndrome Associated With Peripheral Nerve Involvement(Japanese dermatolgical Assoc, 2003) Akdeniz, N; Çalka, Ö; Anlar, Ö; Akbayram, S; Çaksen, H; Metin, A; Çinal, ASjogren-Larsson syndrome is a rare hereditary neurocutaneous disorder characterized by ichthyosis, spastic di- or tetra-plegia, and mild to moderate mental retardation. In this article, we present a nine-year-old girl with the classical features of the syndrome associated with peripheral nerve involvement because of its rare presentation. To the best of our knowledge, only three cases of Sjogren-Larsson syndrome with peripheral nerve involvement have been previously reported in the literature. We assume that Sjogren-Larsson syndrome involves extensive disorders of the ectodermal tissues, including the peripheral nerves as well as the skin and the central nervous system.Article Serum Interleukin-2, Interleukin-6, Tumour Necrosis Factor-Alpha and Nitric Oxide Levels in Patients With Behcet's Disease(Acad Medicine Singapore, 2004) Akdeniz, N; Esrefoglu, M; Keles, MS; Karakuzu, A; Atasoy, MIntroduction: Behcet's disease (BD) is a chronic systemic disorder characterised by oral and genital ulcerative lesions, ocular and cutaneous manifestations. Cytokines are the major mediators of immunologic and inflammatory reactions. Nitric oxide is reactive nitrogen intermediate which plays a key role in pathogenesis of many inflammatory and autoimmune skin diseases. The study was conducted to determine serum interleukin-2 (IL-2), interleukin-6 (IL-6), tumour necrosis factor (TNF)-alpha and nitric oxide levels in relation to the pathogenesis of Behcet's disease. Materials and Methods: Serum IL-2, IL-6, and TNF-alpha levels were measured with test kits by enzyme-linked immunosorbent assay (ELISA) method, while serum nitric oxide levels were determined with a test kit by colorimetric method. Serum IL-2, IL-6, TNF-alpha and nitric oxide concentrations in 27 patients with Behcet's disease and in 16 healthy controls were determined by extrapolation from their standard curves. The significance of the mean differences between the 2 groups was assessed by the Mann-Whitney U test. Results: The serum levels of IL-2, IL-6, TNF-alpha, and nitric oxide concentrations in patients with BD were significantly higher than those of the controls (P < 0.001). Conclusion: Our results suggest that elevated levels of IL-2, IL-6, TNF-alpha, and nitric oxide in Behcet's disease appear to be related to the disease.Editorial A Verrucous Swelling in a Child(Blackwell Publishing, 2006) Akdeniz, N; Çalka, Ö; Metin, A; Günes Bilgili, S; Kösem, MArticle Visual Evoked Potential Findings in Behcet's Disease Without Neurological Manifestations(Taylor & Francis Ltd, 2006) Anlar, O; Akdeniz, N; Tombul, T; Calka, O; Bilgili, SGBehcet's disease (BD) is a chronic, recurrent multisystem inflammatory disorder firstly described by Turkish dermatologist Dr. Hulusi Beh et in 1937. The classic triad consists of recurrent oral and genital ulcerations and uveitis. The article presents the value of visual evoked potential findings of a series of 44 patients with BD without neurological manifestations seen at the Medical Hospital in Neurology and Dermatology clinics over the past 8 years. The mean latency value of positive peak P100 in BD patients was significantly delayed compared to that of control subjects ( patients's mean: 105.6 ms in right eye and 107.7 ms in left eye; control subject's mean: 101.4 ms in right eye and 101.7 ms in left eye).
