Evaluation of Maternal Kidneys With 'acoustic Radiation Force Impulse Exitation (arfi)' Ultrasound Elastography in Hypertensive Disease of Pregnancy(2021) Akyol, Filiz; Gül, AbdulazizObjective: To determine the diagnostic efficiency of ultrasound elastography in preeclampsia by comparing renal cortical elasticity between pregnant women diagnosed with severe preeclampsia and the control group. Materials and Methods: 80 pregnant women who applied to Van Yüzüncü Yıl University Medical Faculty Gynecology and Obstetrics Clinic in 2020 were included in the study as two separate groups. 40 healthy pregnant women between the ages of 22-38 and without systemic pathology at 28-40 weeks of gestation were determined as control group(Group A) whereas 40 pregnant women who met the criteria for preeclampsia with severe features according to ACOG criteria were determined as the study group (Group B). The tests for Complete Blood Count, AST, ALT, Albumin, Total Protein, LDH, GGT, Total Biluribin, Direct Bilirubin, Urea, Creatine, uric acid TSH, CRP, Na, K, Cl, Ca, Mg, Protein-spot protein in 24-hour urine and creatinine-spot creatinine in 24-hour urine from all pregnant women who meet the study criteria are studied. Each group member participating in the study also underwent OGTT 75-gram screening, indirect ophthalmoscopy technique and peripheral blood smear evaluation as well as routine abdominal ultrasonography evaluation. As an ultrasound elastographic evaluation, 3 different real-time shear wave elastography (SWE) values were measured from both renal cortex at the upper, middle and lower levels, and the average of these 3 values was accepted as the final SWE value. The groups participating in the study were compared in terms of age, body mass index, gestational week, nulliparity, laboratory test results, renal cortex SWE elastography values. Results: The renal cortex mean SWE were found in control and study groups as 2.380 ±0.43 m/sn and 2.935 ±0.55 m/sn respectively. A statistically significant difference was found between the two groups (p<0.005). The SBP, DBP, OAP, protein in 24 hour urine, protein in spot urine, uric acid, LDH, GFR, creatinine in 24 hour urine, creatinine in spot urine were measured in gruop A and Group B, respectively as follows: 110.75±8.28mmHg –175.87±12.75 mmHg, 69.00±7.77 mmHg –110.87±6.39 mmHg, 82.79±7.75 mmHg –131.92 ±10.4 mmHg, 173.20±53.5 mg/day – 2666.60 ±311.98 mg/day, 11.32±4.73 mg/dl –157.00 ±233.30 mg/dl, 3.26±0.63 mg/dl – 6.61 ±1.07mg/dl, 192.6±22.66 U/L–303.7±12.62 U/L, 144.42 ±9.65 ml/dk – 84.42±6.57 ml/dk, 1167.54±105.10 mg/day –837.27±157.66 mg/day, 75.41 ±21.85 mg/dl –42.43±15.9 mg/dl. . A statistically significant difference was found between the groups (p<0.001 –p<0.03) Conclusion: Our study shows that maternal renal cortical stiffness is increased in preeclamptic pregnants with severe symptoms. In this context, our findings shows that ultrasound elastography is a reliable and clinically applicable diagnostic method that can be used in the diagnosis of severe preeclampsia.
