Article Acute Intestinal Anisakiasis: Ct Findings(Univ Catholique Louvain-ucl, 2012) Ozcan, H. N.; Avcu, S.; Pauwels, W.; Mortele, K. J.; De Backer, A. J.Small bowel anisakiasis is a relatively uncommon disease that results from consumption of raw or insufficiently pickled, salted, smoked, or cooked wild marine fish infected with Anisakis larvae. We report a case of intestinal anisakiasis in a 63-year-old woman presenting with acute onset of abdominal complaints one day after ingestion of raw wild-caught herring from the Northsea. Computed tomography (CT) scanning demonstrated thickening of the distal small bowel wall, mucosa with hyperenhancement, mural stratification, fluid accumulation within dilated small-bowel loops and hyperemia of mesenteric vessels. In patients with a recent history of eating raw marine fish presenting with acute onset of abdominal complaints and CT features of acute small bowel inflammation the possibility of anisakiasis should be considered in the differential diagnosis of acute abdominal syndromes. (Acta gastroenterol belg, 2012, 75, 364-365).Article Acute Mountain Sickness Occurring With Corpus Callosum, Globus Pallidus and Cerebellar Peduncle Involvement: Mri Findings(2010) Avcu, S.; Arslan, H.; Göktaş, U.; Bulut, M.D.; Ünal, Ö.Acute mountain sickness (AMS) is a pathological reaction, occurring as a result of failure to adapt to altitude. The main symptoms reported are headache, nausea, vomiting and fatigue. However, high altitude cerebral and pulmonary edema, occurring in AMS, cause potentially fatal consequences. In our 31-year-old case, who is the group leader and activity organizer among nine mountaineers, who climbed Mt. Aĝri{dotless} for winter climbing, nausea, vomiting, unconsciousness, lack of appetite and severe headache have occurred in connection with high altitude at 4200 m. Brain MRI of the patient revealed wide-spread hyperintense lesions at subcortical white matter, corpus callosum, globus pallidi, and bilateral cerebellar peduncles on T2 and FLAIR series. The patient was referred to anesthesia-intensive care unit of our hospital. Since the appearances are demonstrative, MRI findings are presented. © 2010 Elsevier Ireland Ltd.Article Anesthesia Mumps After the Cesarean Section in Pregnant Woman(TIP ARASTIRMALARI DERNEGI, 2011) Kati, I.; Kurdoǧlu, Z.; Göktaş, U.; Aytekin, O.Ç.; Avcu, S.Anesthesia mumps is characterized by acute transient swelling of the parotid gland association with general anesthesia. We presented a 25 years old pregnant woman with anesthesia mumps after 24 hours Cesarean section, and discussed treatment of anesthesia mumps and differential diagnosis. 25 years old woman underwent to operation for Cesarean section. Her Medical history had no chronic disease, and no allergies. Her mental and psychological status, neurological, other systematical examination findings and laboratory values were normal. General anesthesia was administered. Twenty four hours after the extubation, a large swelling of right more than left parotid region expanding down to the mandibular angle was noted. In conclusion we suggest that especialy obstetrical cases should be performed gently intubation and extubation and used medications should be attention. In additioal, anesthesia mumps of patients may fully recover within a few days without any treatment or with treatment.Article Aryepiglottic Fold and Subcutaneous Metastases From Breast Carcinoma(Assoc Royal Soc Scientifiques Medicales Belges, 2009) Avcu, S.; Izmirli, M.; Ozcan, H. Nursun; Sengul, I.; Lemmerling, M.We report the CT findings in a 77-year-old woman with left-sided aryepiglottic fold metastasis from breast cancer diagnosed 7 years previously. The lesion obliterated the left piriform sinus, had a lobulated contour and showed mild and heterogeneous enhancement on a post-contrast CT examination. Biopsy of the lesion revealed metastasis from poorly differentiated adenocarcinoma, compatible with metastasis of the breast carcinoma. Before detection of the laryngeal lesion, a second primary tumor in the right ovary was diagnosed. After right oopherectomy, histopathology revealed clear cell adenocarcinoma. The patient also had multiple subcutaneous metastases and bilateral cervical lymphadenopathies. To the best of our knowledge, our case has the property to be the first case in the literature showing the association of these pathologies all together.Article Bilateral Giant Open Lip Schizencephaly With Associated Cerebral Anomalies: a Case Report(2009) Avcu, S.; Özen, Ö.; Ünal, Ö.A nine-month old boy was brought to our hospital with a complaint of growth retardation. On cerebral magnetic resonance imaging examination, giant clefts resulting in the connection of lateral ventricles with subarachnoidal spaces were detected in both cerebral hemispheres, and interpreted as bilateral giant open-lip schizencephaly. Associated anomalies were noted on cerebral magnetic resonance imaging examination. Bilateral thalami were located inferiorly. The caudate nuclei were observed but their configuration was disrupted. The other parts of the basal ganglia including globus pallidi and putamina were absent. Documentation of a preceding illness is not required to make this diagnosis. This report examines the case of a 9-month-old male patient presenting with the features of an acute leukodystrophy following innoculation with the mixed vaccine Pentaxim (Sanofi Pasteur, LyonFrance) while suffering from a lower respiratory tract infection, and who was eventually diagnosed as ADEM. F.A case of unilateral mydriasis secondary to septoplasty under local anaesthesia. Septoplasty is a procedure performed to correct deformities of the nasal septum. Serious complications associated with septoplasty include orbital complications. Here, we present a 26-year-old male patient who developed unilateral mydriasis while undergoing septoplasty under local anaesthesia. The mydriasis resolved spontaneously in an hour without any sequel. N.; Izmirli, M.; Lemmerling, M.Article Diffusion-Weighted Mri in the Diagnosis of Intracranial Hematomas(2013) Şanli, D.; Ünal, O.; Bora, A.; Beyazal, M.; Yavuz, A.; Avcu, S.To determinate the diagnostic value of diffusion-weighted MR imaging (DWI) in intracerebral hematomas, epidural hematomas, subdural hematomas, and subarachnoid hemorrhage, and to assess the contribution of diffusion signal characteristics in the differentiation of hematoma stages. In this prospective study, consecutive 67 patients (range: 3-89 years), 35 (18 men 17 women) with intracerebral hematoma, 18 (10 men 8 women) with subdural hematoma, 2 (1 man 1 woman) with epidural hematoma, and 12 (5 men 7 women) with subarachnoid hemorrhage on conventional MRI sequences constituted the case group and were evaluated with DWI. Intracerebral, subdural and epidural hematomas detected on conventional MR sequences were also shown on DWI. But none of the cases with subarachnoid hemorrhage was demonstrated on DWI. All intracranial hematomas detected on DWI except from chronic stage subdural hematomas showed similar signal intensity characteristics on DWI and T2WI, but were hypointense on ADC maps. All intracerebral, subdural and epidural hematomas seen on conventional MR sequences could be detected by DWI. On the contrary, DWI was not an effective imaging method for detecting subarachnoid hemorrhages. Using DWI alone for detecting the stages of hemorrhages can provide only additional information in subdural and intracerebral hematomas, and it is unable to give definite results, and the lesions should be evaluated by the correlation with other conventional MRI sequences.Article Endovenous Laser Ablation Treatment With 980 Nm Diode Laser for Saphenous Vein Insufficiency: 6 Months Follow Up Results(Assoc Royal Soc Scientifiques Medicales Belges, 2014) Beyazal, M.; Avcu, S.; Ozen, O.; Yavuz, A.; Bora, A.; Unal, O.Objective: Superficial venous insufficiency is a common problem associated with varicose veins which, if untreated, may progress to venous ulceration. Endovenous laser ablation (EVLA) is a new, minimally invasive method for management of superficial venous insufficiency and varicose veins. The aim of this study was to demonstrate the effectiveness of 980 nm EVLA for treatment of symptomatic saphenous venous insufficiency and to present its early outcomes. Methods: Thirty-eight great saphenous veins and 5 small saphenous veins in 40 patients with saphenofemoral reflux were treated with 980 nm diode endovenous laser equipment. The diameter and length of the vein treated, total laser energy and energy density (Joules/cm) delivered were recorded. To determine the severity of the venous disease Venous Clinical Severity Score (VCSS) and Visual Analogue Scale (VAS) were used. Patients were followed up for 6 months after the procedure. Complications were recorded prospectively. Results: Forty-three saphenous veins in 40 patients were treated. The mean age of the patients was 39.9 (range 2172) years. The mean diameter and length of the veins were 4.9 mm (range, 3.5 to 8.5 mm) and 30.2 cm (range, 16 to 50 cm), respectively. At 6 months follow-up, total occlusion rate was 95.4% (41/43), and recanalization rate was 4.6%. Significant decrease was observed for VCSS and VAS scores after the procedure. No major complication was detected. Conclusion: EVLA treatment for superficial venous insufficiency is safe and can be carried out under local anaesthesia in an outpatient setting with good patient satisfaction and low complication rates.Article Gangrenous Meckel's Diverticulum in a Strangulated Umbilical Hernia in a 42-Year Woman: a Case Report(2010) Sengul, I.; Sengul, D.; Avcu, S.; Parlak, O.Introduction: Meckel's diverticulum affects 1 - 3 % of general population and is known as the most common anomaly of gastrointestinal tract. However, its estimated lifetime complication rate is approximately 4 %. Intestinal obstruction is most common complication of Meckel's diverticulum in adult population. Case presentation: In the present study, we reported a 42-year-old female patient with a gangrenous Meckel's diverticulum in a strangulated umbilical hernia sac treated by dissection of diverticulomesenteric bands and diverticulectomy. In 36 months follow-up, there was neither any complication nor recurrence of hernia. Conclusion: This case represents a gangrenous Meckel's diverticulum in a strangulated umbilical hernia sac diagnosed in case of emergency. Although it is a very rare phenomenon, we should be vigilant for this entity especially in case of emergency. © 2010 Sengul et al.Article Giant Aneurysmal Bone Cyst of the Rib(TIP ARASTIRMALARI DERNEGI, 2012) Avcu, S.; Toprak, N.; Bulut, M.D.; Ünal, O.Aneurysmal bone cyst (ABC), especially in the ribs, is a rare disease, considering to be a benign lesion which occurs mostly in children and young adults. This case, however, concerns a 46-year-old female patient, with complaints of pain and palpable mass in the right thoracic wall, who was diagnosed to have ABC originating from the rib demonstrated by radiological examinations, aspiration biopsy, and histopathological verification. F.A giant paediatric mandibular aneurysmal bone cyst and reconstruction with bilateral iliac bone graft. Aneurysmal bone cyst (ABC) is an unusual, non-neoplastic, expansile lesion of the bone. ABC is not a true cyst and it is characterised by the replacement of bone by fibro-osseous tissue containing blood-filled sinusoidal or cavernous spaces. The expanding mass often disrupts the normal bony architecture, with erosion of the cortex. Surgical treatment consists of complete excision or curettage. In this report, we present a 5-year-old girl with a huge aneurysmal bone cyst in the corpus of the mandible, which was reconstructed with bone graft taken from bilateral anterior iliac crests.Article Impact of Contrast Agents on Dose Algorithms of Planning Systems(Ijrr-iranian Journal Radiation Res, 2016) Izmirli, M.; Cakir, T.; Avcu, S.; Nart, M.Background: Use of contrast agents (CAs) during CT simulation for treatment planing system leads to changes in electron density. In this study we aimed to investigate the effect on calculated dose of various concentrations of CAs on treatment planing systems in different dose calculation algorithms. Materials and Methods: Contrast agent (0.769 mg/ml Iopromid) - water mixtures at concentrations of 0%, 1%, 2%, 5% and 10% in total volume of 500 ml for each were made by using five identical balloons. Calculations were performed by Cobalt-60 and 10 MV linear accelerator devices in CMS XIO treatment planning system. The prescribed dose of 100cGy was given to the center of balloon that is isocenter of SAD technique at 10cm from the surface. The doses at maximum dose depth (dmax) and at 5 cm were calculated according to the separate algorithms by either making or not making a correction for CA, and the results were recorded. Results: In all algorithms, as contrast ratio increases, the dose values at dmax and 5 cm-depth increase accordingly. When the doses at dmax and 5 cm-depth were compared for Linac and Co-60 in all algorithms, it has been shown that the dmax value of Co-60 was higher and the difference was greater in parallel with increasing contrast ratio in comparing with Linac. D.Objective: To investigate the regional metabolite abnormalities and changes after treatment in patients with OCD with autogenous and reactive obsessions. Method: We assessed right anterior cingulate cortex (ACC) and amygdala-hippocampal region (Am + Hpp) N-acetyl-aspartate (NAA), choline (Cho) and creatine (Cr) concentrations and NAA/Cr and Cho/Cr ratios using single-voxel proton magnetic resonance spectroscopy in 15 patients with autogenous obsessions (OCD-A), 15 patients with reactive obsessions (OCD-R) and 15 healthy controls (HC). Measurements were repeated after 16 weeks of fluoxetine treatment. Results: Baseline ACC NAA/Cr ratios of both OCD groups were significantly lower than HC. OCD-A group had significantly lower baseline NAA/Cr ratios in the Am + Hpp than other groups. These differences were more likely to be explained by higher Cr levels in ACC. We found no significant differences and changes for Cho levels and Cho/Cr ratios between groups and within groups. Significant increase in NAA/Cr ratios of OCD-A group found in the Am + Hpp was more likely to be explained by increased NAA levels. No significant changes were found in ACC NAA/Cr ratios. Conclusion: While disturbed energy metabolism in ACC might reflect a common pathology in patients with OCD regardless of symptom dimension, alterations in mesiotemporal lobe are more likely for autogenous obsessions.Article Large Chondrosarcoma of Skull Base: an Unusual Intracranial Tumor in a Young Male(2007) Avcu, S.; Turan, A.; Ünal, Ö.; Bora, A.A 25-year-old male presented with headache. He was noted to be neurologically intact on physical examination. He had normal laboratory findings. CT and MR imaging showed a large right temporal mass. Radiological diagnosis was cartilaginous tumor arising from the skull base. The patient underwent surgery at another hospital. The histopathologic features were supporting the radiologic diagnosis being evaluated as mixoid type chondrosarcoma. Although intracranial cartillaginous tumors are rare, the diagnosis can be possible with careful radiological analysis.Article Misplacement of Nasogastric Tube in Two Different Cases: Case Report(2013) Palabiyik, O.; Goktas, U.; Isik, Y.; Kati, I.; Sozen, D.; Avcu, S.Although the insertion of nasogastric tube has been described as an easy and simple procedure, it may cause severe complications and death. It is therefore important to verify correct nasogastric tube placement. Placement of a nasogastric tube is confirmed using several methods, including auscultation over the epigastric area, aspiration of gastric content from the tube, measuring pH of gastric content and radiological methods. We present the inadvertent insertion of nasogastric tube into the airways, that resulted in respiratory distress, in two different patients to serve as a reminder that malpositioning may cause severe complications.
