Most of PTA has no symptoms and are noticed incidentally. We report a case of PTA compressing the right side pituitary gland, in a patient with hormonal disorder. In this report, we emphasize importance of MR imaging and MR angiography on demonstrating the relationship between the pituitary gland and PTA. © 2004 Elsevier Ireland Ltd. All rights reserved.Article Helicobacter Pylori Infection in Gastric Carcinoma in the Van Region of Turkey(1999) Turkdogan, M.K.; Alici, S.; Ilhan, M.; Dilek, H.; Akman, E.; Ayakta, H.; Karakok, M.The prevalence of Helicobacter pylori (H. pylori) infection was assessed in endoscopic biopsies of 39 patients with gastric cancer (23 male, 16 female). Biopsies were taken both from the cancerous and peripheal sites of the cancer (Ca) tissues and evaluated with Giemsa staining method. In a control group of 59 patients (34 male, 26 female), endoscopic biopsies were evaluated by the same method. H. pylori was positive in 56.4 % (22/39) of gastric Ca patients at the peripheral site biopsies while it was only 25.6 % positive (10/39) in the cancer tissues. H. pylori positivity was 36.6 % (22/60) in the control group. Thus, the prevalance of H. pylori infection was evaluated as significantly high (p<0.05) in the peripheal site biopsies of gastric Ca patients. The cumulation of H. pylori was 47.6 % in the cardia, 55.5 % in the corpus and 66.6 % in the antrum. In conclusion, gastric Ca is significantly related to H. pylori infection in the Van region. Public health education, the recognition of transmission routes and successful eradication therapy are the cornerstones of future prevention of gastric carcinogenesis.Article Inflammatory Pseudotumor of the Lung Associated With Anemia: a Case Report and Review of the Literature(2000) Yalcinkaya,; Demirtas, I.; Ayakta, H.; Akpolat, N.; Dilek, I.; Dilek, D.H.Inflammatory pseudotumor of the lung is a benign tumor which is rarely seen. A mass in the left lung was observed in the chest roentgenogram of an 8 year-old boy while investigating for anemia. Preoperative investigations could not render the exact diagnosis. Pneumonectomy was required in exploratris thoracotomy. Histopathologic diagnosis was fibrohistiocytic type of inflammatory pseudotumor. Four months after the operation, anemia was cured without any further treatment.Article Influence of the Refinement of Surgical Technique and Surgeon's Experience on the Rate of Complications After Total Thyroidectomy for Benign Thyroid Disease(ARSMB-KVBMG, 2003) Kotan, C.; Kösem, M.; Algün, E.; Ayakta, H.; Sönmez, R.; Söylemez, O.Background: In recent years, many surgeons dealing with endocrine surgery have increasingly performed total thyroidectomy for benign thyroid disease. However, total excision of the thyroid in the treatment of benign lesions has been surrounded by even more controversy than its role in cancer treatment. The complication rate appears to be higher when the operation is done by inexperienced surgeons who have no special skills in endocrine surgery using proper techniques. The aim of this study is to determine whether surgeons experience and the refinement of surgical techniques are associated with postoperative recurrent laryngeal nerve (RLN) palsy or hypocalcemia after total thyroidectomy for benign thyroid disease. Methods: A total of 68 consecutive patients who underwent total thyroidectomy for benign thyroid disease were reviewed. Twenty-six of these were from between January 1998 and June 1999 (first period) and 42 from between June 1999 and September 2000 (second period). Patients were divided into two subgroups according to different periods and different surgical techniques to identify the RLNs and the parathyroid glands. RLNs function was evaluated pre- and post-operatively by an otolaryngologist, and serum calcium levels were measured at the postoperative follow-up. Results: During the first period of the study, transient hypocalcaemia was determined in 8 (31%) patients. Hypocalcaemia was clinically symptomatic in 5 (19%) patients. Transient RLN palsy developed in 4 (15%) patients. Unilateral permanent RLN palsy due to operative injury was observed in 1 (4%) patient. During the second period, we noted transient hypocalcemia in 11 (26%) patients and symptomatic hypocalcemia in 6 (4%) patients. Serum calcium levels returned to normal within 4 weeks after operation in all patients. Neither transient nor permanent RLN palsy was observed during this period. Conclusions: Complications of total thyroidectomy can be minimized with increasing experience and the refinement of surgical technique.Article Multidetector Computed Tomography in the Diagnosis of Parathyroid Adenomas: Contribution of Contrast Enhancement Pattern To Diagnosis(2004) Harman, M.; Algün, E.; Ayakta, H.; Sakarya, M.E.; Etlik, O.PURPOSE: Contrast enhancement patterns of parathyroid adenomas in the arterial and venous phases were evaluated with multidetector computed tomography and specificity of the method to characterize the lesions was investigated. MATERIALS AND METHODS: Arterial and venous enhancement patterns of parathyroid adenomas were evaluated retrospectively with multidetector computed tomography in 12 patients. All adenomas were removed surgically and the diagnosis was confirmed histopathologically. An area from the mandibular angle to the aortic arch was scanned with 1.5 mm section thickness in the arterial phase (20 seconds delay time) and venous phase (70 seconds delay time) after bolus injection of 50 ml of iodinated contrast media. Arterial and venous contrast enhancement of parathyroid adenomas was evaluated in the axial and coronal reformat images. RESULTS: Ten (83.3%) of 12 parathyroid adenomas showed a heterogeneous enhancement with a hypodense small central area in the arterial phase and a homogeneous enhancement in the venous phase. However, two small lesions, four and five mm in diameter respectively, showed homogeneous contrast enhancement in the arterial phase unlike the larger lesions which displayed a hypodense center in this phase. CONCLUSION: Dynamic scanning with multidetector computed tomography after contrast administration may be helpful in characterizing parathyroid adenomas.Article Polyserositis as a Rare Component of Polyglandular Autoimmune Syndrome Type Ii(Medicom Group, 2001) Algün, E.; Erkoç, R.; Ckotan,; Güier, N.; Sahn, L.; Ayakta, H.; Aksoy, H.Polyglandular autoimmune (PGA) syndromes (types I and II) may affect various endocrine and non-endocrine organs in the body. In the commoner PGA type II, primary adrenal insufficiency, autoimmune thyroid disease and type I diabetes mellitus are the most frequent manifestations. Serositis with pericardial or pleural involvement is not a well known component of the disease. Here, we report a 21-year-old man who first presented with a pleuropericardial effusion and Graves' disease, and who then developed type I diabetes mellitus.Article Post-Traumatic Omental Herniation Into Pleural Space: Radiologic Features(Elsevier BV, 2003) Harman, M.; Arslan, H.; Ayakta, H.; Odabasi, H.A 43-year-old male with history of trauma was admitted to our clinic for the further evaluation of a blunted left costophrenic angle on chest roentgenograms. Computed tomographic (CT) scans demonstrated a crescent, fat-dense, extra-pleural mass on the left lateral hemi-thorax. Vertical funicular densities consistent with blood vessels were identified in this extra-pleural lesion on enhanced CT scans. T1-weighted axial and coronal magnetic resonance images (MRI) disclosed that the omentum ran continuously from the abdominal cavity into thoracic cavity a long left lateral hemi-thorax through a defect in the diaphragm. Surgical exploration revealed the laceration of left diaphragm and left parietal pleura. The omentum was herniated into the left pleural space through small diaphragmatic defect. We report X-ray, CT and MRI features of post-traumatic omental herniation into the pleural space that, to our knowledge it has not yet been reported so far.Article Relationship Between Leptin Levels and Body Indexes in Patients With Haematologic Malignancy(2001) Mete, R.; Dilek, I.; Algün, E.; Erkoç, R.; Ayakta, H.; Topal, C.; Aksoy, H.Leptin is a recently found hormone regulating body weight. In human obesity, this weight-regulating hormone level is in a positive correlation with FMI (fat mass index) and BMI (body mass index). In this study, we aimed to investigate the relation between serum leptin levels and BMI, PF (percentage fat), LMI (lean mass index), FMI and some other parameters of patients with haematologic malignant diseases. Fourty-four patients with haematologic malignant diseases and 25 healthy control group were taken into the study. In the comparison, there were no significant between the PF and FMI values of both groups, while the mean BMI and LMI values of the control group were significantly higher than that of the patient group. There was a positive correlation between leptin levels and BMI and FMI among parameters studied in our control group, whereas we couldn't demonstrate any such correlation in patient group. We estimate that the alteration may be due to disturbances in the feed back mechanism developing in patient with haematologic malignancy.
