Article Çocukluk Çağı Rüptüre Akciğer Kist Hidatikleri(2021) Göksu, Mehmet; Avci, Veli; Ayengin, KemalAmaç: Akciğer kist hidatiğinin en ciddi komplikasyonlarından biri kistin rüptüre olmasıdır. Rüptürüsonrasında asfiksi, anafilaktik şok, akut solunum yetmezliği ve masif hemoptizi meydana gelebilir.Bu çalışmada kliniğimizde takip ve tedavisi yapılan rüptür sonrası akciğer kist hidatik vakalarınısunmayı amaçladık.Yöntem: Çalışmaya Ocak 2012-Ocak 2021 tarihleri arasında kliniğimizde rüptür sonrası akciğerkist hidatik nedeni ile ameliyatı yapılan 0-18 yaş arası hastalar dahil edildi. Hastaların başvuruyaşı, cinsiyeti, şikayeti, kistin lokalizasyonu ve uygulanan cerrahi teknik değerlendirildi. Bulgular: Çalışmaya dahil edilen 60 akciğer kist hidatik vakasının 23 (%38)’ünde rüptür teşhisikonuldu. Rüptür hastaların 12’si (%52) kız, 11’i (%48) erkekti. Ortama yaş 10.2 (5-17) yıldı. En sıkbaşvuru nedeni öksürük ve ateşti. Lokalizasyon olarak en sık sağ lob akciğer tutulumu vardı.Cerrahi işlem esnasında altı (%26) hastaya kapitonaj uygulanırken, 17(%74) hastaya uygulanma dı. Lobektomi yalnızca bir hastada yapıldı. Sonuç: Akciğer kist hidatiğinde rüptür en korkulan komplikasyondur. Son yıllarda tüm dünyada çocuklukçağında gittikçe artan oranlarda görülmektedir. Bu çalışmadaamaç kliniğimizde cerrahi işlem uygulanan tiroid hastalarındatiroid nodül hacminin, çapının ve multisentrisitenin maligniteüzerine etkisini araştırmaktır.Gereç ve Yöntem: 2014-2019 yılları arasında troid nodülünedeni ile total tiroidektomi ve/veya lobektomi uygulanan 1-17 yaş arası toplam 20 çocuk hasta değerlendirildi. Hastalarınyaş, cinsiyet, nodül çapı, hacmi ve multisentrisitenin maligniteüzerine etkisi açısından geriye dönük incelendi.Bulgular: Çalışmadaki hastaların 15’i kız, 5’i erkekti.Hastaneye başvuru yaşı ortalama 14 yıl, nodül çapı malignlerde3 cm, benignlerde 2 cm iken; nodül hacmi hem malign hem debenignlerde 7,8 cm3 bulundu. Cerrahi işlem sonrası patolojisonuçlarına göre multisentrisite dokuz hastada mevcuttu. Budokuz hastanın yedisi malign, ikisi benign karekterdeydi.Sonuç: Çalışmada artmış nodül çapı, hacmi vemultisentrisitenin malignite riskini arttırmadığı görüldü. This study investigated the suppressive effect of curcumin and caffeic acid phenethyl ester (CAPE) on oxidative stress, apoptosis, and whether MMPs mediate doxorubicin (DOX)-induced testicular injury. Male rats were randomly divided into eight groups (n = 8 per group). The groups were as follows: sham, dimethyl sulphoxide (100 mu L), DOX (3 mg/kg), CAPE (2.68 mg/kg), curcumin (30 mg/kg), DOX+CAPE (3 mg/kg DOX and 2.68 mg/kg CAPE), DOX+curcumin (3 mg/kg DOX and 30 mg/kg curcumin) and DOX+CAPE+curcumin (3 mg/kg DOX, 2.68 mg/kg CAPE and 30 mg/kg curcumin). Injections were administered daily for 21 days. The oxidative stress, MMPs, proinflammatory cytokines and apoptotic markers in the DOX group were higher than the sham group (p < .05); these measures were lower in the groups treated with CAPE and curcumin together with DOX compared with the DOX group (p < .05). The results showed that MMPs mediated DOX-induced testicular injury, but CAPE and especially curcumin suppressed testis injury and cell apoptosis by suppressing DOX-induced increases in MMPs, oxidative stress and proinflammatory cytokines. However, curcumin exhibited more pronounced effects than CAPE in terms of all studied parameters.Article The Effect of Laparoscopic and Open Surgery on Oxidative Dna Damage and Il-37 in Children With Acute Appendicitis(Springer London Ltd, 2021) Ayengin, Kemal; Alp, Hamit Hakan; Avci, Veli; Huyut, ZubeyirBackground Although laparoscopic appendectomy (LA) is considered to be minimally invasive compared with open appendectomy (OA), there are few studies showing this in terms of cytokines and oxidative stress markers. Aim LA and OA techniques have been compared in terms of different conditions such as operation time and complication rate. Here, we compared how oxidative DNA damage and some cytokines levels change in relation to LA and OA. Method This study was performed in children diagnosed with acute appendicitis in our hospital. The study was conducted on 30 children with acute appendicitis in each group. Oxidative DNA damage and malondialdehyde levels were determined by high-performance liquid chromatography, and interleukin-1 beta, interleukin-37, ischaemia-modified albumin and total thiol levels were determined by enzyme-linked immunosorbent assays. Results There was a dramatic decrease in ischaemia-modified albumin (IMA) levels after LA. However, there were no statistically significant differences in pre- and postoperative IMA levels in the OA group. Oxidative DNA damage and malondialdehyde levels were low significantly according to preoperative levels after the LA and OA. Total thiol levels were high in children who underwent LA while they were low after OA. In addition, postoperative interleukin-1 beta levels were low in both groups. Furthermore, IL-37 levels postoperatively were low in the LA group, while there was no significant change in the OA group. Conclusion LA had a more positive effect on oxidative DNA damage, IL-1 beta and IL-37 than the OA. In addition, surgical stress was reduced with LA.Article The Effects of Coq10 Supplement on Matrix Metalloproteinases, Oxidative Dna Damage and Pro-Inflammatory Cytokines in Testicular Ischaemia/Reperfusion Injury in Rats(Wiley, 2021) Ayengin, Kemal; Alp, Hamit Hakan; Huyut, Zubeyir; Yildirim, Serkan; Altindag, Fikret; Avci, VeliWe aimed to study the effect of coenzyme Q10 on pro-inflammatory cytokine, matrix metalloproteinase, oxidative DNA damage, caspase 3 and caspase 8 in ischaemia/reperfusion injury led to by testicular torsion/detorsion. Our research is a controlled experimental animal research using rats. This study was conducted with fifty-six adult male Albino Wistar rats. Interleucine-1 beta, 2, 6, 10, tumour necrosis factor-alpha, matrix metalloproteinase-2, 3, 9, 13, tissue inhibitor matrix metalloproteinase-1, 2, malondialdehyde and leucocyte 8-hydroxy-2-deoxy guanosine/10(6) deoxyguanosine was detected in serum and tissue samples. In addition, immunohistochemical analysis of caspase 2 and caspase 8 was performed. In testicular I/R injury, especially 24 hr after detorsion, oxidative damage pro-inflammatory cytokines and matrix metalloproteinases were increased. At the coenzyme Q10 group, a meaningful decrease was observed in these parameters. In addition, a decrease in the expression of caspase3 and caspase 8 was viewed in coenzyme Q10-treated groups. The coenzyme Q10 has beneficial effects on oxidative damage, pro-inflammatory cytokine levels, remodelling of extracellular matrix and apoptosis in testicular I/R injury.Article Fekaloid Umblikal Drenajı Olan Patent Omfalomezenterik Kanal Tanılı Bir Yenidoğan(2023) Aycan, Nur; Yürektürk, Eyyüp; Ateş, Ali; Ayengin, Kemal; Toplar, Emel Nadya; Karaman, Serap; Tuncer, OğuzYenidoğan döneminde umblikal anomaliler oldukça sık görülmektedir. Göbek patolojili olgularda embriolojik gelişim basamaklarını ve anatomiyi bilmek doğru tanı ve tedavi için esastır. Bu anomalilerin nadir bir tipi patent omfalomezenterik kanal olup hayatın ilk gününde tanı koyulabilir. Yenidoğanın geçici taşipnesi nedeniyle ünitemize yatırılan miadında bir erkek bebeğin umbilikustan sarı-yeşil renkli, köpüklü fekaloid drenajı olduğu görüldü. Göbek kordonunda vişne renkli mukoza ile uyumlu doku görüntüsü mevcuttu. Tetkikleri sonucu patent omfalomezenterik kanal tanısı koyularak opere edildi. Yenidoğan döneminde nadir görüldüğü için bu konuda veriler de kısıtlıdır. Bu çalışmada yenidoğan döneminde tanı alıp, cerrahi işlemi gerçekleştirilen Konjenital Morgagni Hernisi vakalarımızla ilgili deneyimlerimizi literatür eşliğinde sunmayı hedefledik. Gereç ve Yöntem: 2008-2017 yılları arasında yenidoğan döneminde Konjenital Morgagni Hernisi nedeni ile tedavi edilen hastalar; demografik bilgileri, semptom ve bulgular, tanı yöntemleri, eşlik eden ek konjenital anomaliler, cerrahi bulguları, komplikasyonlar, hastanede yatış süresi ve morbidite-mortalite açısından geriye dönük olarak değerlendirildi.Bulgular: Çalışmaya 9’u erkek, 5’i kız 14 hasta dahil edildi. Ortalama doğum ağırlığı 2460±820 gram bulundu. Solunum sıkıntısı (%43) en sık gözlemlenen semptomdu. Tanıda en sık (%64) direkt grafiler kullanırken, beş hastanın tanısı intrauterin dönemde konuldu. Hastalara ortalama 5,3 günlük iken cerrahi işlem gerçekleştirildi. Yara yeri enfeksiyonu cerrahi sonrası en sık gelişen komp-likasyondu. Hastalığa eşlik eden en sık konjenital anomali Down sendromu (%21) idi. Hastanede ortalama yatış süresi 13±4 gün bulundu. İki hasta kardiyopulmoner yetmezlik nedeni ile kaybedildi. Sonuç: Çalışmamızda Konjenital Morgagni Hernisi tanılı hastaların çoğunun solunum sıkıntısı çektiğini, en sık eşlik eden ek kon-jenital anomalinin Down sendromu olduğunu, intaruterin dönemde tanının tedavi süreci açısından önemli rol aldığını ve cerrahi sonuçların yüz güldürücü olduğunu gözlemledik.Article Oxidative Dna Damage and Nox4 Levels in Children With Undescended Testes(Georg Thieme verlag Kg, 2019) Avci, Veli; Ayengin, Kemal; Alp, Hamit HakanBackground Undescended testis (UDT) is a common urological disorder. Patients with UDT have a risk of malignancy and infertility. The development of these conditions may be due to oxidative stress mediated by reactive oxygen species. The aim of this study was to investigate the relationship between these parameters by detecting oxidative DNA damage (8-hydroxy 2 deoxyguanosine/10 (6) deoxyguanosine), ischemia-modified albumin (IMA), malondialdehyde (MDA), and nicotinamide adenine dinucleotide phosphate oxidase 4 (NOX4) levels in children with UDT and healthy control group. Materials and Methods The blood samples were obtained from 30 patients with UDT and 40 healthy male subjects. The levels of oxidative DNA damage were detected by high-pressure liquid chromatography method. We used commercially available kits that use enzyme-linked immunosorbent assay method to measure IMA, MDA, and NOX4 levels. Results The levels of MDA, IMA, NOX4, and oxidative DNA damage in children with UDT were statistically significantly higher than control group. In addition, we found that the levels of NOX4, IMA, and oxidative DNA damage after 12 months of age was significantly higher than before 12 months of age. Conclusion We identified increased lipid peroxidation, oxidative DNA damage, IMA, and NOX4 levels in children with UDT. Delay in the treatment of UDT may cause oxidative damage. That is why, according to us the antioxidant treatment may be beneficial in children with UDT.Article Oxidative Stress Enzyme Nox1 Is a New and Important Biomarker for Childhood Appendicitis(Springer india, 2023) Avci, Veli; Ayengin, Kemal; Huyut, Zubeyir; Huyut, Mehmet Tahir; Soysal, Lokman; Bilici, SalimDelayed appendicitis diagnosis may result in perforation and an increased risk of mortality. This study aimed to assess the diagnostic value of ischemia-modified albumin, nicotinamide adenine dinucleotide phosphate oxidase 1, 2, and 4 levels in the diagnosis of non-complicated and complicated appendicitis. The study included 60 pediatric patients who presented to our clinic with a complaint of abdominal pain and underwent surgery with a diagnosis of appendicitis between November 2020 and December 2021 and also included 30 controls. Cases were divided into three groups: (i) non-complicated appendicitis (n = 30), (ii) complicated appendicitis (n = 30), and (iii) control (n = 30). The nicotinamide adenine dinucleotide phosphate oxidase 1 and 4 and ischemia-modified albumin levels were higher in non-complicated and complicated appendicitis compared to the control (p < 0.001). In addition, considering the odds ratio values, the most effective biomarkers in the diagnosis were nicotinamide adenine dinucleotide phosphate oxidase 1 and 2, and procalcitonin, while the most effective biomarkers in the prognosis were nicotinamide adenine dinucleotide phosphate oxidase 1 and 2, and neotrophil/lymphocyte ratios. The data suggested that since the most successful biomarker nicotinamide adenine dinucleotide phosphate oxidase 1, with a value of 0.98- area under the curve, is the most successful biomarker in both diagnosis and prognosis of the disease, it can be used as an important biomarker in childhood appendicitis.Article Thiol-Disulphide Homeostasis in Ovarian Torsion-Detorsion: an Experimental Rat Model(Turkiye Klinikleri, 2020) Avci, Veli; Huyut, Zubeyir; Altinda, Fikret; Ayengin, Kemal; Alp, Hamit HakanObjective: Ovarian torsion is one of the reasons of acute abdomen requiring immediate surgical intervention. There is not any reliable method for its diagnosis. Delay in treatment may result in loss of ovary. In this study, the purpose is to evaluate efficiency of thiol-disulphide homeostasis in early diagnosis of ovarian torsion in experimental rat model and in its follow-up care with biochemical and histopathological findings. Material and Methods: In the study, 24 Albino Wistar female rats were divided into three groups. The first group was the sham group. The second group was the torsion group. The last group was the torsion-detorsion group. Histopathological evaluation was carried out in ovaries in all groups, and values of thiol/disulphide homeostasis were measured in serum samples biochemically. Results: Values of native thiol and total thiol were lower in the torsion-detorsion group compared to the other groups. However, there was no significant difference between of native thiol and total thiol levels in the sham and torsion groups. Interestingly, the index of native thiol/total thiol was relatively low in the torsion and torsion-detorsion groups compared to the sham group. However, this decrease was not significant. Conclusion: These results suggest that native thiol/total thiol values may not be useful for early diagnosis of ovarian torsion but it may be used for follow-up care after detorsionArticle Why Is the Rate of Perforated Appendicitis Higher in Girls in Eastern Turkey, Unlike the Literature(Turkish Pediatrics Assoc, 2019) Avci, Veli; Ayengin, KemalAim: Appendicitis is the most common cause of surgical abdominal pain in children. The incidence of appendicitis is higher in boys than girls. Delay in diagnosis may result in perforation of the appendix. Aim of this study is, to examine the terms of gender distribution of perforated appendicitis cases in eastern Turkey. Material and Methods: Patients who underwent appendectomy in our clinic between the years of 2017-2018 were classified according to histopathology results as acute appendicitis and perforated appendicitis. Two groups was evaluated in term of the age, gender, and length of hospital stay. Results: Of the patients included in the study, 168 were male and 99 were female. The incidence of perforated appendicitis was higher in girls (%59), (p=0,001) and acute appendicitis was higher in boys (%63), (p=0,001). The mean age was 11.44 +/- 3.50 years in acute appendicitis and 9.21 +/- 4.26 years in perforated appendicitis. hi perforated appendicitis, the mean length of hospital stay (6,73 day) was longer than acute appendicitis (2,59 day) (p=0,001). In addition, the mean length of hospital stay (8,13 day) for girls with perforated appendicitis was statistically significantly longer than boys (6,03 day), (p=0,001). Conclusion: While diagnosing appendicitis in the east of Turkey; living environment of the child, education level of the family, socioeconomic status and cultural structure of the society must be considered. It should not be forgotten that the rate of perforated appendicitis in girls is higher than boys in this region.
