Article Chronic Intussusception in Childhood: Report of Four Cases(2001) Bakan, V.; Köseoǧlu, B.; Önem, Ö.; Bilici, S.; Demirtaş, I.Intussusception is a frequent cause of bowel obstruction in infants and toddlers. Correct diagnosis in majority of the patient with acute intussusception is made by typical presentation and symptoms. However, atypical presentation and chronic symptoms may cause delay in diagnosis of chronic intussusception. Four cases of chronic intussusception are reported. There was no leading point in patients and they had mobile colon or poor fixation of colon. A typical presentation and diagnostic delay in chronic intussusception are argued.Article Comparison of Bupivacaine With Bupivacaine Plus Fentanyl Administered Via Thoracic Epidural Catheter for Pain After Thoracotomy in Children(2004) Demirel, C.B.; Kati, I.; Köseoǧlu, B.; Bakan, V.; Hüseyinoǧlu, Ü.A.; Silay, E.In this study, we aimed to compare the analgesic and side effects of bupivacaine versus bupivacaine and fentanyl combination administered via epidural cathater. Sixty children, aged 3-12 years, ASA I or II physical status, undergoing thoracic surgery were studied. All patients were monitorized routinely. Anaesthesia induced with sevoflurane or thiopenton, atracurium and fentanyl, and meintained with O2 + air, sevoflurane, atracurium and fentanyl. At the end of the operations, epidural catheters were placed below two dermatomes of the surgical incision. In the First Group, 0.3 mL kg-1 bupivacaine 0.125 % was administered as a bolus through the catheter, and infusion in a dose of 0.3 mL kg-1 h-1 added. In the Second Group, 2 mg mL-1 fentanyl and bupivacaine 0.125% was administered as a bolus in a dose of 0.3 mL kg-1 and infusion in a dose of 0.3 mL kg-1 h-1 added. Pain scores were decreased lower than four points in all patients except two patients in the first group, and three patients in the second group. In the second group, sedation scores were higher and pupil's diameters were smaller than the first group. In conclusion, thoracal epidural analgesia seems to be favorable method for pain relief after thoracotomy. However, we must be careful for sedation if opioids will be added to local anesthetics.Article The Importance of Serum Malondialdehyde and Ascorbic Acid in Empyema(2003) Noyan, T.; Balaharoǧlu, R.; Bakan, V.Although, there were various studies investigating importance of oxidant and antioxidant systems in pleural diseases, the role of free radicals in these diseases have not yet been clarified. In this study, it was aimed to investigate diagnostic importance of malondialdehyd (MDA) an end product of lipid peroxidation and ascorbic acid (AA), uric acid (UA) and total bilurubin (TB) as an endogenous antioxidants in children with empyema. For this reason, serum MDA, AA, UA and TB levels were obtained in 17 patients with empyema and 22 healthy children as control group. While the serum MDA levels were higher in patient group as compared to control group (p<0.05), AA, UA and TB levels were not significantly different between patient and control group (p>0.05). There results suggest that while an increased MDA levels could be developed as a result of increased oxidative damage, AA, UA and TB levels as an antioxidant could not have diagnostic value in patients with empyema.Article Sinus Histiocytosis With Massive Lymphadenopathy (rosai-dorfman's Disease): a Case Report(2003) Bakan, V.; Köseoǧlu, B.; Demirtaş, I.; Uǧraş, S.; Öner, A.F.Rosai-Dorfman disease or Sinus Histiocytosis with Massive Lymphadenopathy (SHML) is an idiopathic, benign, self-limited disorder characterized by painless cervical lymphadenopathy, fever, and weight loss. The disease most commonly affects children and young adults. A 3-year-old child with Rosai-Dorfman Disease was presented. The diagnosis was made by a cervical lymph node biopsy. The disease have regressed spontaneously and 3 years follow-up was uneventful.
