Article Brucellar Aortitis and Meningoencephalitis in an Adolescent(Lippincott Williams & Wilkins, 2016) Bayhan, Gulsum Iclal; Ece, Ibrahim; Oner, Ahmet FaikBrucellosis can affect any organ system and result in possible complications. We present an adolescent male who had brucellar aortitis and meningitis simultaneously. Brucellar aortic involvement is a rare complication of brucellosis and can occur without concomitant endocarditis. Here, the patient was managed with prolonged antibiotic therapy without any surgery.Article The Clinical Characteristics of Fascioliasis in Pediatric Patients(Turkish Pediatrics Assoc, 2020) Bayhan, Gulsum Iclal; Ozkan, Aysegul Taylan; Beyhan, Yunus EmreFascioliasis is a trematode flatworm infection caused by Fasciola hepatica and F. gigantica. Fasciola hepatica is mesoendemic in Turkey. Six cases of pediatric fascioliasis are presented here. All patients had histories of consumption of various raw vegetables. Four of our patients were at the hepatic phase, and two were at the biliary phase. Except for one patient, all patients had eosinophilia. In three patients, total IgE levels were markedly increased. In these patients, there was an eosinophilic leukomoid reaction accompanied by a high total IgE level. Except for one of our patients, all patients had positive indirect hemagglutination tests. This patient was diagnosed as having fascioliasis with a compatible clinical picture. Except one, all of our patients were completely treated with a single-treatment regimen of triclabendazole; one patient needed re-administration. Only in patient 1, liver enzymes increased after triclabendazole administration because he had taken the medication for longer than prescribed by mistake. The elevation of liver enzymes was not observed in the other patients who received the correct dose. All patients recovered completely with triclabendazole treatment. The patients' dietary habits, eosinophilia, and markedly elevated IgE levels may be significant predictors for physicians to suspect fascioliasis.Article Cutaneous Side Effects of Doxycycline: a Pediatric Case Series(Taylor & Francis Ltd, 2017) Bayhan, Gulsum Iclal; Akbayram, Sinan; Yavuz, Goknur Ozaydin; Oner, Ahmet FayikObjective: Brucellosis is highly endemic in Turkey and doxycycline is commonly used for its treatment. The present study aimed at documenting the cutaneous side effects of doxycycline in pediatric brucellosis patients in Turkey.Materials and methods: Pediatric patients with brucellosis that were treated between February 2014 and January 2016 were analyzed retrospectively, and those that developed doxycycline-related cutaneous side effects were identified. Demographic data, epidemiological history, physical examination findings, laboratory test results, anti-brucellosis treatment regimen, duration of follow up and outcome were recorded.Results: Among the 189 brucellosis patients, 141 treated with doxycycline plus rifampicin. Seven patients (5%) (two female and five male) developed doxycycline-related cutaneous side effects. Mean duration of treatment before the onset of cutaneous side effects was 9.5 weeks. Doxycycline therapy was continued in five of these patients and was changed in two patients. In the patients that continued to receive doxycycline the cutaneous side effects gradually improved.Conclusions: Cutaneous side effects of doxycycline should always be a consideration, especially in regions in which brucellosis is endemic and doxycycline is commonly used to treat it.Article Diastolic Dysfunction in Patients With Brucellosis Despite the Absence of Infective Endocarditis(Cambridge Univ Press, 2020) Ece, Ibrahim; Epcacan, Serdar; Bayhan, Gulsum Iclal; Ture, MehmetBackground: Brucellosis is an important systemic infectious disease, especially in developing countries. Every organ and system of the human body can be affected; however, cardiovascular complications of brucellosis are rare. Aim: To assess cardiac functions in patients with acute brucellosis without overt cardiac involvement and to answer the following question: Is there any cardiac dysfunction despite the absence of endocarditis in these patients? Methods: This cross-sectional study included 67 children with brucellosis and 40 healthy children. We performed a detailed echocardiographic examination in individuals at the beginning of the treatment. Patients with infective endocarditis were excluded from the study. Results: Echocardiography revealed no difference of ejection fraction, mitral and tricuspid annular plane systolic excursion, pulsed-wave Doppler-derived early diastolic peak velocity (E)/late diastolic peak velocity (A) ratios in mitral and tricuspid valves between the two groups. The deceleration time of early mitral inflow was longer in patients with brucellosis. Early diastolic peak velocity of the mitral and tricuspid annuluses obtained by tissue Doppler imaging (Ea) was significantly lower in children with brucellosis. The peak velocity obtained by tissue Doppler imaging during late diastole (Aa), Ea and Ea/Aa ratios in the interventricular septum, left ventricle posterior wall and right ventricle free wall was lower in patients with brucellosis than in the control group. The E/Ea ratio, isovolumic relaxation time, right ventricle and left ventricle myocardial performance indices were higher in patients with brucellosis. Conclusion: Patients with acute brucellosis may have diastolic dysfunction without overt cardiac involvement and infective endocarditis.Article Ebv Associated Acute Cholecystitis in a Child(Gazi Univ, Fac Med, 2014) Bayhan, Gulsum Iclal; Bakkal, Muhammet Furkan; Uner, Cigdem; Tanir, GonulAcute acalculous cholecystitis (AAC) is a rare complication of Epstein Barr virus (EBV) infection, with only a few cases reported among pediatric population. This clinical condition is frequently associated with a favorable outcome and, usually, a surgical intervention is not required. We report a 16-year-old girl who presented with AAC following primary EBV infection. The diagnosis of AAC was documented by clinical and ultrasonographic examination, whereas EBV infection was confirmed serologically. A conservative treatment was performed, with a careful monitoring and serial ultrasonographic examinations, which led to the clinical improvement of the patient. Pediatricians should be aware of the possible association between EBV and AAC, in order to offer the patients an appropriate management strategy.Article Epidemiology of Hepatitis E Virus in Children in the Province of Van, Turkey(Aves, 2016) Bayhan, Gulsum Iclal; Demioren, Kaan; Guducuoglu, HuseyinAim: Hepatitis E virus is an etiological agent of hepatitis which is transmitted enterically and may lead to water-born outbreaks. Although it is mainly transmitted by the fecal-oral route, it is estimated that many cases are associated with zoonotic transmission in developing countries. In this study, we aimed to investigate the seroprevalence of hepatitis E in the childhood age group in the province of Van and to demonstrate the relationship between seroprevalence and demographic properties, residential house/region, water supply used at home, dealing with livestock and history of surgery. Material and Methods: In this study, hepatitis E virus IgG antibody was studied by ELISA method in children aged between 2 months and 18 years between June 2014 and September 2014 in the province of Van. Results: A total of 408 children and adolescents were enrolled in the study. Hepatitis E IgG was found to be positive in 4.2% of the subjects. 179 (43.8%) of the subjects were female and 229 (56.2%) were male. The mean age was 123 months +/- 56.6 months (minimum 2 months, maximum 214 months). When the seropositivity rates were compared by age groups and gender, no difference was found. No correlation was found between hepatitis E seropositivity and the variables of residence, dealing with livestock and water resources. No correlation was found between anti-hepatits E virus seropositivity and parental education level, number of cohabitants and history of surgery. Conclusion: In our study, hepatitis E virus seropositivity was found to be lower compared to the mean seropositivity in Turkey. Hepatitis E infection does not constitute a serious problem in children in the province of Van in accordance with the results reported from different parts of our country. Livestock dealing and usage of well water are not considered risk factors for Hepatitis E infection.Article An Interesting Fistula Tract Presenting With Recurrent Gluteal Abscess: Instructive Case(Hindawi Ltd, 2015) Bayhan, Gulsum Iclal; Metin, Ozge; Ardicli, Burak; Karaman, Ayse; Tanir, GonulA fistula extending fromthe gluteus to penis is an extremely rare entity. In this paper, we have highlighted novel variant of congenital penile to gluteal fistula complicated with gluteal and penoscrotal abscess in a previously healthy boy. A fistulous tract extending from the gluteus to penis has been shown by fistulogram. Bleomycin has been used in fistula tract with successful results in our patient.Article Is the Level of Knowledge on Brucellosis Sufficient in the Highly Endemic Region(Aves Yayincilik, Ibrahim Kara, 2019) Bayhan, Gulsum Iclal; Epcacan, Zerrin Karakus; Ertugrul, Yunus; Sarihan, Muhammed Hamdi; Ersari, Server Sena; Makal, Gulan; Sahin, DilekObjective: Brucellosis is the most common zoonotic disease in the world. In this study, we aimed to investigate the awareness of brucellosis in Van province, the city where brucellosis is highly endemic. Material and Methods: This survey was conducted in the city center of Van from July 2014 to March 2015. Results: In our study, 987 people were included; 555 (56.2%) were females and 432 (44.8%) were males. Mean age of the participants was 26.8 +/- 8.3 years. 830 (84.1%) of the participants had previously heard of brucellosis. The rates of correct answers to the questions about brucellosis transmission were compared between the groups that were distinguished by educational level. Correct response rates were found higher in the group with a high school degree or higher to all questions about food-related brucellosis transmission except the question of brucellosis transmission by cheese produced from raw milk. The rate of being informed about the fact that brucellosis can be transmitted by raw milk cheese was 78.7%, but the level of knowledge on other transmission routes was low among all participants. 296 (30%) of the participants had been engaged in animal husbandry in the past and 155 (15.7%) were still involved. The level of knowledge on brucellosis transmission during animal husbandry processes, brucellosis signs in animals and the procedures to be performed for protection against brucellosis was very low. Conclusion: The results of our study showed that the level of knowledge on brucellosis is low in Van province. In order to protect public health and also prevent loss of income in the livestock sector, it is necessary to raise societal awareness about brucellosis.Article Miliary Tuberculosis Disease Complicated by Pott's Abscess in an Infant: Seven Year Follow-Up(Medknow Publications & Media Pvt Ltd, 2015) Bayhan, Gulsum Iclal; Tanir, Gonul; Aydin, Zeynep Gokce Gayretli; Yildiz, Yasemin TasciA 20-month-old boy presented with 1-year history of persistent fever, cough, and progressive abdominal distention. Abdominal ultrasonography showed hepatomegaly and multiple calcifications in the liver and spleen. Thoracic computed tomography showed multiple mediastinal lymph nodes and consolidation in both lungs. Additionally, there was a 2-cm thick retroperitoneal soft tissue mass destroying the T7-8 and L1-L2 vertebral bodies. The patient was preliminarily diagnosed with miliary tuberculosis (TB) and Pott's disease, and began administering anti-TB treatment consisting of isoniazid, rifampin, ethambutol, and pyrazinamide. Acid-resistant bacilli analysis and mycobacterial culture of the biopsy specimen of Pott's abscess were positive. Mycobacterial culture and PCR of gastric aspirate were also positive. The patient's condition progressively improved with anti-TB treatment and he received 12 months of antiTB therapy. At the end of the treatment all of the patient's symptoms were relieved and he was well except for kyphosis. Miliary TB complicated by Pott's abscess is a very rare presentation of childhood TB. The presented case shows that when Pott's abscess is diagnosed and surgically corrected without delay, patients can recover without squeal.Article A Pediatric Case of Fascioliasis With Eosinophilic Pneumonia(Turkish J Pediatrics, 2016) Bayhan, Gulsum Iclal; Batur, Abdulsamet; Taylan-Ozkan, Aysegul; Demiroren, Kaan; Beyhan, Yunus EmreFasciolia spp. are common trematode infestations worldwide. Fasciolia spp. may lead to hepatic diseases in the acute phase and may cause biliary diseases in the chronic phase. In addition, Fasciolia spp. may rarely cause extrahepatic signs and symptoms. The clinical manifestations of fascioliasis are divided into three groups: typical, atypical, and ectopic. Eosinophilic pneumonia is an atypical presentation of acute fascioliasis and it has been reported very rarely. Herein, we report a boy with marked blood eosinophilia and eosinophilic pneumonia who was diagnosed with fascioliasis by serologic tests and abdominal USG. The patient recovered completely following triclabendazole treatment.Article Pediatric Pleural Tuberculosis(Wolters Kluwer Medknow Publications, 2018) Bayhan, Gulsum Iclal; Sayir, Fuat; Tanir, Gonul; Tuncer, OguzBackground: Pleural tuberculosis (TB) diagnosis is sometimes controversial because the microbiologic confirmation ratio is very low in pleural fluid. There are few pediatric pleural TB case series in the literature. Methods: We retrospectively evaluated our TB cases below 18 years of age and extracted pleural TB cases. Results: Seven cases with pleural TB were identified. About 42.9% of the patients had isolated pleural TB whereas 57.10% of the patients had accompanying pulmonary TB. Lymphocytic pleural effusion and increased adenosine deaminase (ADA) (>40 U/L) level are found in 85.7% of the patients. Six patients had uncomplicated effusion (transudate) according to Light's criteria and one had complicated effusion (exudate). Lung decortication was needed in three patients. All patients were given 6 months anti-TB medication and recovered completely. Conclusion: In the lymphocyte-predominant pleural effusion, an increased ADA level highly supported TB disease. The complicated effusion (exudate) in pleural TB is not rule; uncomplicated effusion (transudate) could be seen.Article Pediatric Tuberculosis in Turkey: a Review of 8-Years Period in a Tertiary Care Hospital(Turkish J Pediatrics, 2015) Bayhan, Gulsum Iclal; Tanir, Gonul; Metin, Ozge; Simsek, Hulya; Aydin-Teke, Turkan; Oz, Fatma Nur; Aydin, Zeynep Gokce GayretliThe purpose of this study was to evaluate the clinical, radiological, microbiological characteristics, side effects of anti-TB drugs and treatment outcome of childhood definite or probable TB. The medical records of all childhood TB patients were investigated in the department of pediatric infectious disease of a tertiary care hospital between January 2005 and December 2012. Patients who followed-up until the anti-TB treatment was completed were included in the study. One hundred forty four pediatric cases with active TB were included in the study (27 definite, 117 probable). Twenty-four (16.7 %) patients were asymptomatic at admission. Pulmonary TB was the most common type. The most common localizations for extrapulmonary TB were superficial lymphadenitis. An index case was detected in 39.6% of the patients. In conclusion, the absence of constitutional symptoms does not exclude TB. Household contact screening plays an important role in the diagnosis of childhood TB especially in asymptomatic patients.Article Pulmonary Infections Due To Brucellosis in Childhood(Turkish Assoc Tuberculosis & Thorax, 2020) Bayhan, Gulsum Iclal; Batur, Abdussamet; Ece, IbrahimIntroduction: Brucellosis is widely distributed zoonotic infection. Brucellosis is a multisystemic disease but pulmonary infection due to brucellosis is very rarely reported. Materials and Methods: We retrospectively evaluated our pediatric brucellosis cases between February 2014 and December 2015. The brucellosis patients with and without pulmonary infection were compared with each other. Results: We detected pulmonary infection in 18 of the 98 brucellosis patients. There was no statistical significant difference between the brucellosis patients with and without a pulmonary infection as regards age, gender, and animal husbandry history. The laboratory findings including the Brucella Coombs agglutination titers were also similar. 1 he most common chest X-ray findings were interstitial infiltration and Mar lymphadenopathy. All of our cases with pulmonary infection recovered with the usual anti-brucella treatment. Conclusion: Brucellosis may cause a pulmonary infection more often than thought. Voriconazole is considered the primary antifungal agent for invasive aspergillosis. We report a case of 16-year-old girl who developed visual disturbance and visual and auditory hallucinations after intravenous voriconazole treatment for invasive pulmonary aspergillosis. Due to the visual hallucinations and visual disturbance began acutely and shortly after the initiation of voriconazole, and no other cause could be determined, the symptoms were considered to be the side effects of voriconazole. Bu ça-lışmada brusellozun yüksek endemisite gösterdiği Van ilinde bruselloz farkındalığını araştırmak amaçlanmıştır.Gereç ve Yöntemler: Bu anket çalışması Temmuz 2014-Mart 2015 tarihleri arasında Van ili şehir merkezinde yapıldı.Bulgular:Çalışmamıza987kişidahiledildi;555(%56.2)’ikadın,432(%44.8)’si erkekti. Katılımcıların yaş ortalaması 26.8 ± 8.3 yaş idi. Katılımcı-ların 830 (%84.1)’u daha önce bruselloz hastalığını duyduğunu ifade etti. Katılımcıların öğrenim düzeyleri ile bruselloz bulaşı hakkındaki sorulara verdikleri doğru cevap oranları incelendiğinde, brusellozun çiğ sütten ya-pılmış peynir yemekle bulaştığını bilme oranı ortaokul ve daha az öğrenim almış olanlarda daha yüksekken, diğer tüm sorulara doğru cevap verme oranları lise ve daha üstü öğrenim almış olanlarda daha yüksek bulundu. Katılımcıların çiğ sütten yapılmış peynir ile bruselloz bulaşını bilme oranı %78.7 idi, ancak diğer bulaş yolları hakkında bilgi düzeyi düşük bulundu. Katılımcıların 296 (%30)’sı geçmişte hayvancılıkla uğraşmıştı, 155 (%15.7)’i halen uğraşmaktaydı. Hayvancılık işlemleri sırasında bulaş için riskli durum-lar, brusellozun hayvanlardaki belirtileri ve korunma için yapılacak işlemler konusunda bilgi düzeyi çok düşük bulundu.Sonuç: Çalışmamızın sonuçları Van ilinde bruselloz hakkında bilgi se-viyesinin düşük olduğunu; hem toplum sağlığını korumak hem de hayvancılık sektöründe gelir kayıplarının önüne geçmek için bruselloz hakkında toplumun bilgi seviyesini artırmaya yönelik girişimlere ihtiyaç olduğunu ortaya koymuştur.
