Article Analgesic and Hepatotoxic Effects of Ononis Spinosa(Wiley, 2006) Yilmaz, Betul Sever; Ozbek, Hanefi; Citoglu, Gulcin Saltan; Ugras, Serdar; Bayram, Irfan; Erdogan, EnderThe present study investigated the analgesic and hepatoprotective activities of a water extract of Ononis spinosa L. (OS) in mice. Analgesic activity was based on the pain thresholds measured with the tail-flick test before administration at 30, 90 and 150 min. The results were analysed with one-way variance analysis. The extract of Ononis spinosa showed analgesic activity equivalent to aspirin at 30 and 90 min and even higher than aspirin with the 50 mg/kg dose. At a dose of 100 mg/kg OS showed an analgesic effect equivalent to aspirin at all time points. The hepatoprotective influence of OS on carbon tetrachloride (CCl4)-induced acute liver toxicity was also studied. The extract had no significant effect on the increased levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT) and bilirubin in CCI4 treated animals (p > 0.05). Thus, the results reveal that the extract of OS had no hepatoprotective effect on CCI4-induced acute liver toxicity. Angiokeratoma of Fordyce is one of five types in the group of the angiokeratomas, which occurs on the scrotum, penis or vulva. It is usually observed in young adults or elderly men. A 6-year-old boy presented to the dermatology department because of papular and erythematous lesions on his scrotum and penis. These lesions were found at birth and were asymptomatic. There was a history of occasional bleeding on trauma from the lesions. Histological evaluation of a skin biopsy specimen showed hyperkeratosis and acanthosis of the epidermis and multiple dilated thin-walled vessels in the papillary dermis. Based on the clinical, histopathological and dermoscopic findings, the patient was diagnosed with Fordyce angiokeratoma. Herein, we report a case of angiokeratomas of Fordyce, which is very rare in childhood and the dermoscopic findings.Article A Case of Sarcoidosis With Pleural Involvement(Wiley, 2018) Sunnetcioglu, Aysel; Sertogullarindan, Bunyamin; Batur, Abdussamet; Bayram, IrfanSarcoidosis is a chronic, multisystem inflammatory disorder of unknown etiology that is characterized by noncaseating granulomas. Although lung involvement is common in sarcoidosis, pleural involvement is rare. Pleural involvement may manifest as a pleural effusion, pneumothorax, pleural thickening and nodules, hydropneumothorax, hemothorax, or chylothorax. Here, we describe a case of sarcoidosis with pleural nodular thickening.Article Clinic Experience in Discoid Lupus Erythematosus: a Retrospective Study of 132 Cases(Termedia Publishing House Ltd, 2019) Yavuz, Goknur Ozaydin; Yavuz, Ibrahim Halit; Bayram, Irfan; Aktar, Rojda; Bilgili, Serap GunesIntroduction: Lupus erythematosus (LE) is an autoimmune disease characterized by a broad range of cutaneous manifestations. Discoid LE (DLE) is the most common chronic manifestation of LE. Literature reviews show that there are a limited number of large-series studies investigating DLE. Additionally, there is still no consensus on the etiological factors of DLE such as sun exposure and smoking. Aim: To evaluate the clinical and demographic characteristics of patients with DLE. Material and methods: The study included patients who were hospitalized in the inpatient and outpatient clinics at the Dermatology Department. Age, gender, treatment method, history of smoking, antinuclear antibody positivity, progression to systemic lupus erythematosus (SLE), photosensitivity, and laboratory findings were recorded for each patient. Results: The study included 132 patients comprising 67 (50.8%) men and 65 (49.2%) women. A family history was found in 3.8%, SLE was detected in 5.3%, and photosensitivity was revealed in 50.0% of the patients. ANA positivity was found in 23.7%, a history of smoking was revealed in 61.4%, and chronic sun exposure was detected in 42.4% of the patients. Conclusions: Discoid LE, though identified long ago, remains unelucidated and there are very few studies in the literature reporting on DLE. The results indicated that smoking and chronic UV exposure are important risk factors for DLE. Moreover, although ANA positivity was high in our patients, the rate of progression to SLE was remarkably low. In this article, we present a case who was operated due to conventional osteosarcoma of the rib in the light of the literature review. A 17-year-old female patient had been operated due to a rib tumor 54 months ago. There was no local recurrence or metastatic disease. Siderosis is a disease characterized by the accumulation of iron in various tissues. Brownish-gray discoloration of the skin can be seen as a side-effect on the injection area after the parenteral iron treatment. Here, we present cutaneous siderosis case developed after multiple intramuscular iron injection on the gluteal region for iron-deficiency anemia. Development of cutaneous siderosis after intramuscular iron injection rarely has been reported in the literature before.Article Diffused Erythema Nodosum Leprosum Reaction With Necrotic Ulcers: Case Report(Galenos Yayincilik, 2018) Ozkol, Hatice Uce; Calka, Omer; Bayram, IrfanErythema nodosum leprosum (ENL) is characterized by inflammatory nodules. It is an immune reaction that develops in patients receiving lepromatous leprosy treatment. In extreme severe cases, sometimes these nodules may ulcerate. In this article, we present a case of ENL with diffuse necrotic ulcer arising in a male patient using triple therapy (dapsone, clofazimine and rifampicin) because of lepromatous leprosy. A 39-year-old male patient had papules and nodules commonly found in the upper extremity and body, lividly erythematous, ranging size changes from 2-3 mm to 2 cm, some of them opened and ulcerated. Skin biopsy of the patient was evaluated as ENL. Most commonly it is inherited as an autosomal recessive trait. Squamous cell carcinoma is the most common type of skin cancer found in patient with epidermodysplasia verruciformis. Human papilloma virus 5, 8, and 47 are found in more than 90% of epidermodysplasia verruciformis skin cancers. Treatment for epidermodysplasia verruciformis consists largely of preventive measures. Photoprotection remains essential for management. In this report, two sister case of epidermodisplasia verruciformis with plane warts, pityriasis versicolor-like lesions, and squamous cell carcinomas on sun-exposed areas of skin was presented for it is a rarely encountered disease and associated with early development of malignancy. In this study, the effect of topical tretinoin and oral acitretin on wound healing process was investigated in full-thickness skin lesions on rat model. Method: A circular full-thickness wound was created by 6 mm punch biopsy on the back side of 32 male rats. The rats were seperated into four equal. The first group was assigned as a control group and was observed with no treatment. The second group was administered dexpanthenol once a day over wound area. In the third group, 0.1% tretinoin cream was applied daily. In the fourth group, 2.5 mg/kg/day acitretin was given daily. Additionally, the biopsies were taken from wounds for the histopathologic examination. Results: The complete recovery time was remarkably longer in the tretionin group than the control group and the dexpantenol group (p < 0.05). The average complete recovery time was significantly longer in the acitretin group than the control and dexpanthenol groups whereas it was statistically shorter in the acitrein group than the tretionin group (p < 0.05). Conclusion: Based on our findings, topical tretinoin and oral acitretin can delay secondary wound healing, epithelization and angiogenesis.Article Endobronchial Plasmacytoma in Patient With Multiple Myeloma(Wiley, 2017) Sunnetcioglu, Aysel; Ekin, Selami; Bayram, Irfan; Ekinci, Omer; Bugday, Inci BahadirEndobronchial plasmacytoma is a rare manifestation of extramedullary plasmacytoma. A 49-year-old woman with a history of multiple myeloma consulted to our pulmonary service with progressive dyspnoea and cough and abnormal chest X-ray. A lesion measuring 6 x 5 cm in size existed in anterior baseline of the right lung's lower lobe in thoracic computed tomography in addition to right bronchial narrowing and atelectasis distal in lesions. Diagnostic bronchoscopy was performed previously for the obstruction and biopsy was taken from the lesion in the right middle lobe bronchus. Endobronchial biopsies showed extensive tumour infiltration with plasmocytoid cells. Immunohistochemistry was positive CD138 and Lamda. In order to determine the region's adult nephropathy profile, consecutive renal biopsies performed between January 1997 and December 2003 were reviewed. There were 129 cases (M 63, F 66, mean age 32.2 +/- 15.6), of which 115 had adequate biopsies. Lupus nephritis formed the largest diagnostic entity (23.3%). When lupus cases are excluded, the most common form of glomerulonephritis is membranoproliferative form (27.6%). While high lupus nephritis incidence may be due to our routine biopsy protocol for these patients, relatively higher incidence of MPGN may represent higher prevalences of different infections in the region.Article Evaluation and Treatment of Antrochoanal Polyps(B C decker inc, 2006) Yuca, Koksal; Bayram, Irfan; Kiroglu, A. Faruk; Etlik, Omer; Cankaya, Hakan; Sakin, Feyat; Kiris, MuzafferObjectives: Antrochoanal polyp (ACP) is a benign maxillary sinus polyp that originates from the mucosa of the maxillary sinus, passes through a sinus ostium, and extends into the choana. The common presentation of ACP is unilateral nasal obstruction. The radiographic findings and differential diagnosis of ACPs are discussed by comparing them with data in the literature. Method: This study included 19 (14 male, 5 female; median age 24.5 years, range 8-75 years) surgically treated patients with ACPs diagnosed by clinical examination, nasal endoscopy, and computed tomography. Results: Nasal obstruction was found in all cases. Endoscopic sinus surgery was preferred for removal of the nasal part of ACPs in 13 cases. Only in one case, polypectomy combined with Caldwell-Luc operation and septoplasty was performed. The observed complications were as follows: minor hemorrhage in three cases, mild cheek swelling with pain in two patients, and infraorbital hypoesthesia in one case. Histopathologic examination of ACPs revealed loose mucoid stroma and mucous glands, which were covered by respiratory epithelium. Conclusion: Endoscopic sinus surgery may be indicated in patients with ACPs because the function and capacity of the maxillary antrum are preserved. The greater portion of the antral part of polyp can be removed while leaving the healthy antral mucosa intact.Article Evaluation of 62 Bullous Pemphigoid Patients(2019) Yavuz, Goknur Ozaydın; Tatar, Kübra; Bilgili, Serap Gunes; Yavuz, İbrahim Halil; Bayram, IrfanBullous pemphigoid (BP) is an acquired autoimmune disease often manifesting with subepidermal bullae. Autoantibodies against hemidesmosomes constitute the main cause of BP. In this study, we aimed to evaluate the clinical and histopathological characteristics of BP patients and to discuss them in light of the studies conducted in Turkey and around the world. The retrospective study included 62 patients that were diagnosed with BP in our clinic between 2005 and 2017. Diagnosis of BP was established based on clinical, histopathological, and DIF microscopy findings. Age, gender, duration of disease, presence of pruritus, peripheral eosinophilia, significant histopathological findings, history of smoking, and family history were recorded for each patient. Patients under 18 years of age were excluded from the study.The 62 patients comprised 24 (38.7%) men and 38 (61.3%) women with a median age of 72.0 years. The male-to-female ratio was 1.5. The median age at disease onset was 67.0 (range, 50-75) years and the median duration of disease was 1.0 (0.5-3.0) years. Of the 62 patients, 35 (56.5%) had a history of smoking and mucosal involvement was found in 27 (43.5%) patients. BP is a disease of the elderly and more common in women than in men. Oral mucosa should be an integral part of the physical examination in BP patients. Pruritus is a significant clinical symptom and eosinophilia is an important laboratory marker in the diagnosis of BP, particularly in patients with difficult diagnosis.Article Expression of Cd44s in Advanced Stage Esophageal Squamous Cell Carcinomas and Other Clinicopathological Prognostic Factors(Akad Doktorlar Yayinevi, 2009) Izmirli, Mustafa; Bayram, Irfan; Senol, Serkan; Ilhan, MahmutCD44s is an adhesion molecule which is a member of the cell adhesion molecules family hyaladherins. CD44s has some effects including tumor-endothel interaction, cell motility and migration, cell adhesion and tumor invasion, tumor progression, and metastasing. In this study, we aimed to evaluate the CD44s expression and some other prognostic factors in patients with esophageal squamous cell carcinoma. Between 1999 and 2004, pathological specimens of 35 patients were examined by the Yuzuncu Yil University (YYU) Medical Faculty, Pathology Department and other clinical and laboratory findings were collected from Oncology Department patient files. CD44s staining was positive in 32 patients and negative in 3. The intensity of stained CD44s was positive in 30 and negative in 5 patients. Thirteen patients were well-differentiated, 18 were mid-differentiated, and 4 were poorly differentiated. Inflamatuary reactions were observed in 23 cases. The median survival was 5.3 months and the one year, two year and five year survival rates were 34.2%, 8.6% and 2.9% respectively. Treatment modality, clinical stage and tumour size at the diagnosis time was significant at univariate analysis and only treatment modality was significant in multivariate analysis. Very high CD44s expression was observed in sq cell osephageal cancer patients. CD44s may be an important marker in prognosis. To investigate the expression of laminin receptor 1 (LR1), a non-integrin-type laminin receptor, in gestational trophoblastic diseases and normal first-trimester placenta, since it may play a role in controlling trophoblast invasion in normal and molar pregnancies. Methods. Paraffin sections from 24 gestational age controlled normal first-trimester placentas, 47 partial moles, 56 complete moles, 3 invasive moles, 4 gestational choriocarcinomas, and 1 placental-site trophoblastic tumor were studied immunohistochemically for expression of LR1. Results. In complete and partial moles, decidual cells showed significantly stronger LR1 protein staining compared to the normal placenta (p<0.01). When compared to the partial moles, weak staining in less than 33% of decidual cells was also more prominent in the normal placenta (p<0.05). Complete and partial moles, invasive moles, choriocarcinomas, and placental-site tumors did not differ from each other with respect to staining intensity. Strong immunostaining for LR1 in decidual cells, cytotrophoblasts, syncytiotrophoblasts, and extracellular matrix cells of partial and complete moles was not significantly correlated with the development of persistent postmolar gestational trophoblastic tumors. Conclusions. LR1 may be important in the pathogenesis of gestational trophoblastic diseases. The increased expression of LR1 in decidual cells of partial and complete moles may not influence the development of persistent gestational trophoblastic tumor. Since they are seen rarely, multicentric studies should be planned to study LR1 expression in invasive moles and gestational trophoblastic neoplasms. (C) 2009 Elsevier Inc. Among 548 patients, 347(63.3%) were male. The median age was 57 years. Disease-free survival (DFS) and overall survival (OS) were significantly shorter in patients with GIM than those in patients without GIM (log rank, P = 0.003 and log rank P = 0.003, respectively). Multivariate analysis showed that presence of GIM (HR, 2.1) was found to be an independent factor of worse DFS. In our study, stage pIII patients with GIM had significantly shorter DFS and OS than those without GIM (log rank p = 0.008 and log rank p = 0.001, respectively). However, in subgroup analysis of patients with GIM, there was no significant DFS and OS difference between patients with stage pI and pII disease (log rank p = 0.999, log rank p = 0.184 vs. log rank p = 0.409, log rank p = 0.281, respectively).Article Hepatoprotective and Hypoglycemic Activity of Viburnum Lantana L(Turkish Pharmacists Assoc, 2006) Sever Yilmaz, Betul; Saltan Citolu, Gulcin; Altun, M. Levent; Ozbek, Hanefi; Bayram, Irfan; Cengiz, Nurettin; Altinyay, CigdemIn the present study we investigated the hepatoprotective effect of the water extract of Viburnum lantana L. (VL) leaves on carbon tetrachloride (CC14)-induced hepatotoxicity in rats, hypoglycemic activity and lethal doses of the sante extract in mice. Biochemical parameters of hepatic damage such as serum aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and bilirubin concentrations were determined. CCI4 (0.8 mL/kg i.p. for 7 days) treatment increased the serum AST, ALT, ALP and bilirubin levels significantly as compared to controls. Treatment of animals with silibinin (50 mg/kg) + CC14 (0.8 mL/kg i.p.) or VL (100 mg/kg, i.p.) + CC14 (0.8 mL/kg i.p.) for 7 days significantly ameliorated the levels of AST, ALT and ALP elevated by the CC14 treatment alone. The results of biochemical tests were also confirmed by histopathological examination. The livers of the group treated with VL showed less balooning degeneration and apoptosis. Necrosis had not been observed in VL group. These results suggest that, liver damage occured in VL-treated group is less than the damage occured in silibinin-treated group. To evaluate of hypoglicemic activity of VL, glibenclamide was used as the reference agent. But, VL has not hypoglycemic activity in healthy and diabetic mice.Article The Hepatoprotective Effects of Dihydromyrcenol and Geranyl Formate in an Experimental Model of Acute Hepatic Injury Induced by the Use of Carbon Tetrachloride(Aves, 2011) Bayram, Irfan; Erten, Remzi; Bayram, Yasemin; Bulut, Gulay; Ozbek, HanefiBackground/aims: We aimed to investigate the hepatoprotective effects of dihydromyrcenol and geranyl formate extracted from the Vitis vinifera L. plant in a rat model of acute hepatic injury induced by carbon tetrachloride. Methods: The study was performed on 54 Sprague-Dawley male rats. The animals were divided into 9 study groups with 6 rats in each. At the end of the 7-day study period, the animals were sacrificed. The effects of dihydromyrcenol and geranyl formate on hepatic injury were evaluated based on the comparisons of the changes in the weight, serum levels of alanine aminotransferase and aspartate aminotransferase, and histopathological changes in the liver. Results: Dihydromyrcenol significantly reduced the carbon tetrachloride-associated ballooning degeneration and apoptotic cell counts; this reduction was moderate with low doses of geranyl formate, while no reductions were observed with high doses of geranyl formate. The changes in the alanine aminotransferase and aspartate aminotransferase levels were in accordance with these findings. Conclusions: It can be concluded that in an experimental model of acute hepatic injury induced by carbon tetrachloride, dihydromyrcenol presents a hepatoprotective effect, while geranyl formate presents partial hepatoprotective effects at low doses and no hepatoprotective effects at high doses.
