Article Amlodipine-Induced Gingival Hyperplasia in Chronic Renal Failure: a Case Report(Makerere Univ, Fac Med, 2012) Aldemir, N. M.; Begenik, H.; Emre, H.; Erdur, F. M.; Soyoral, Y.Amlodipine is a dihydropyridine calcium channel blocker that is used in the management of both hypertension and angina. Amlodipine induced side effects are headache, dizziness, edema, flushing, palpitations, and rarely gingival hyperplasia. The exact reason of amlodipine-induced gingival hyperplasia is not known. We presented a case with chronic renal failure (CRF) that developed gingival hyperplasia due to amlodipine use, which improved after ceasing the drug.Article The Association Between Serum Dickkopf-1 Levels and Esophageal Squamous Cell Carcinoma(Sage Publications Ltd, 2014) Begenik, H.; Kemik, A. S.; Emre, H.; Dulger, A. C.; Demirkiran, I.; Ebinc, S.; Kemik, O.Serum Dickkopf-1 (DKK-1) is elevated in many malignancies and is an important indicator of malignant potential. However, its significance in esophageal squamous cell cancer (ESCC) has not yet been clarified. We hypothesized a role for DKK-1 in patients with ESCC. The study consisted of 90 ESCC patients and 85 healthy controls. After diagnosis, the level of DKK-1 was measured in the serum samples by enzyme-linked immunosorbent assay and the levels of DKK-1 were much higher in the ESCC patients than in the healthy control group (p < 0.0001). For serum DKK-1, the sensitivity and specificity of the assay were 70 and 80%, respectively. The preoperative serum DKK-1 level was elevated in the ESCC patients. Although serum DKK-1 is not a specific biomarker for ESCC, it might be a useful marker for the diagnosis and treatment of ESCC.Article Can Renal Infarction Occur After Renal Cyst Aspiration? The two major causes of RI are thromboemboli originhating from a thrombus in the heart or aorta, and in-situ thrombosis of a renal artery. A.; Duz, R.; Demirel, K. C.; Begenik, H.Introduction: Impaired diastolic flow is characterized by decreased left ventricular (LV) filling diastole, abnormal LV distensibility, or delayed relaxation. B-Type natriuretic peptide (BNP) is an indicator of various cardiovascular diseases and body volume status. The aim of this study was to determine whether the lowering of dialysate sodium (Na) levels is effective on LV systolic and diastolic parameters and BNP in the maintenance of hemodialysis patients. Materials and Methods: The study included 49 chronic hemodialysis patients. Left atrium (LA) diameter and LV ejection fraction, LV systolic and diastolic diameter, deceleration time (DT), pulmonary artery pressure (PAP), inferior vena cava diameter (IVCD), early diastolic transmitral flow (E) and late diastolic transmitral flow (A) velocities, E/A ratio, isovolumic relaxation time, peak early diastolic velocity (E), late diastolic velocity (A) of tissue Doppler mitral annulus, and flow propagation velocity of mitral inflow (V-p) were measured before and 6 months after hemodialysis with low Na dialysate. Results: Six months after low Na hemodialysis, a decrease was observed in echocardiographic parameters such as PAP and IVCD (p < 0.05, p < 0.001, and p < 0.001, respectively). However, a significant difference was not observed in LA diameter. In LV diastolic measurement of E and A waves, E/A ratio, DT, V-p, septal E and A, and lateral E and A exhibited significant improvement by low Na HD. BNP level was significantly reduced (p < 0.001). Conclusions: Lowered dialysate Na concentration improves PAP, IVCD, and LV diastolic properties assessed by mitral inflow filling, tissue Doppler velocity, and mitral inflow velocity propagation.Article Human Neutrophil Peptides 1, 2 and 3 (Hnp 1-3): Elevated Serum Levels in Colorectal Cancer and Novel Marker of Lymphatic and Hepatic Metastasis(Sage Publications Ltd, 2013) Kemik, O.; Kemik, A. S.; Sumer, A.; Begenik, H.; Purisa, S.; Tuzun, S.Objective. Biological levels of localized colon tumors are needed for the diagnosis and prevention of colon cancer. Numerous biomarker studies are produced currently through research. We investigated some peptides with the aim of identifying biomarkers for colorectal cancer and lymphatic and hepatic metastasis. Materials and methods. A total of 100 patients with histologically proven colorectal cancer and 60 normal volunteers were included in this study. Preoperative serum was collected for analysis. Human neutrophil peptides levels (HNP) were assayed by solid phase RIA (Radio Immun Assay) based on monoclonal antibodies. Results. The HNP-1, -2 and -3-also known as alpha-defensin-1, -2 and -3-are present in elevated concentrations in the plasma from colorectal cancer patients. Conclusion. Preoperative high plasma HNP 1-3 levels are associated with colorectal cancer. In some cases peritoneal fluid is unremarkable. In these instances, one must evaluate for other causes of gastrointestinal pathology or a myriad of other causes. We report an unusual case of acute myeloid leukemia who presented with peritonitis in a continuous ambulatory peritoneal dialysis patient.Article Prevalence of Helicobacter Pylori in Patients With Brucellosis(BMJ Publishing Group, 2012) Esen, R.; Dulger, A.C.; Begenik, H.; Demirtas, L.; Ebinc, S.; Aytemiz, E.; Aslan, M.Background: Helicobacter pylori infection affects approximately 40 million individuals in Turkey. Our prefecture is an endemic area for H. pylori infection and brucellosis. However, there are no data on the H. pylori prevalence among patients with brucellosis. Objectives: The primary aim of the current study was to investigate the link between H. pylori infection and brucellosis. The secondary aim was to determine the seroprevalence of H. pylori infection in healthy individuals. Methods: Between May 2010 and May 2011, serum was collected from patients with Brucella infection (n = 111) and healthy individuals (n = 120) and analyzed using an enzyme-linked immunosorbent assay kit. Brucella infection was defined as a positive Wright test or blood culture positivity for Brucella in conjunction with disease symptoms and findings. Results: Among the 111 patients with brucellosis, 69% (n = 77) had a seropositivity result for H. pylori IgG. The healthy control subjects had an 87% (n = 104) seropositivity for H. pylori IgG. The serum H. pylori IgG antibody seropositivity was significantly lower in the patients with Brucella infection compared with the control subjects (P G 0.001). Conclusion: Our results show that the serum prevalence of H. pylori among the patients infected with brucellosis is significantly lower than that in the control population. A negative correlation between H. pylori infection and brucellosis infection might be hypothesized. M.; Emre, H.; Sumer, A.; Kotan, C.Background: Acute-phase response proteins (APRPs), cytokines, and hormones have been claimed to be an independent, important factor of cancers. We suggest that in gastrointestinal system cancers, changes in APRP, cytokines, and hormones are associated. Methods: C-reactive protein (CRP), albumin, interleukin I alpha (IL-I alpha), IL-I beta, IL-6, IL-8, IL-10, tumor necrosis factor alpha (TNF-alpha), midkine, vascular endothelial growth factor-A(VEGF-A), VEGF-C, VEGF receptor I (VEGFR I), leptin, adiponectin, and ghrelin serum levels are studied in 148 gastrointestinal system cancer types and 40 healthy controls. Results: We found statistically significant differences and correlations between groups. We found significantly higher serum CRP, IL-I alpha, IL-1 beta, IL-6, IL-8, IL- 10, TNF-alpha, VEGF-A, VEGF-C, VEGFRI, and leptin concentrations in patients with esophageal, gastric, pancreas, colon, and rectum cancers than controls (p < 0.001, p < 0.0001). But, we found lower levels of the serum albumin, midkine, adiponectin, and ghrelin in patients with esophageal, gastric, pancreas, colon, and rectum cancers compared to control subjects (p < 0.05, p < 0.001). Conclusions: Cachexia in gastrointestinal system cancer types is associated with changes in APRP, cytokines, and hormone concentrations. This may be reflected between the outcomes in malignancies and the biomarkers.Article Relationship Between Clinicopathologic Variables and Serum and Tissue Levels of Dickkopf-1 in Patients With Rectal Cancer(BMJ Publishing Group, 2011) Kemik, O.; Kemik, A.S.; Sumer, A.; Begenik, H.; Purisa, S.; Tuzun, S.; Kotan, C.The major biomarker for rectal cancer is the pathologic development of the tumor. In our study, we identified Dikkopf-1 (DKK1) as a novel biomarker and a therapeutic target for rectal cancer. To emphasize the biological and clinicopathologic significance, we performed tumor tissue and serum analysis of 150 rectal cancer samples with enzymelinked immunosorbent assay. Serum DKK-1 levels are found significantly higher in controls, in poor differentiation, and depth of invasion (in pT3 and pT4), present lymph node metastasis, and TNM stage (in pT3 and pT4) according to good differentiation, depth of invasion (in pT1 and pT2), absent lymph node metastasis, and TNM stage (in pT1 and pT2; P < 0.001 and P < 0.0001, respectively). Tissue DKK-1 levels are found in patients with rectal cancer than in control tissues (P < 0.0001). Dikkopf-1 correlated significantly with depth of invasion (P = 0.009), lymph node metastasis (P = 0.028), venous involvement (P = 0.019), and advanced pTNM stage (P = 0.001). There was no correlation between DKK-1 and age or sex (P > 0.05). This marker is also a potential candidate for development of rectal cancer cells and cancer progression.
