Article Burkitt's Lymphoma Causing Acute Pancreatitis in a Child(Elsevier Science Bv, 2013) Akil, Muhammed; Kaya, Avni; Bektas, M. Selcuk; Aktar, Fesih; Akbayram, Sinan; Bilici, Salim; Beyazal, MehmetA 8-year-old boy admitted with abdominal pain, fever and vomiting for the previous 10 days. Sensitivity was detected in the epigastric area. There was not defense and rebond. Aspartate aminotransferase was 106 U/L, alanine aminotransferase 25 U/L, alkaline phosphatase 311 U/L, blood amylase level 748 U/L, blood lipase level 391 U/L. In thoracic CT, soft tissue with smooth contours measuring 32 mmx28 mm was identified in the posterior mediastinum. Bone marrow aspiration biopsy was normal. A mass specimen obtained from the duodenum endoscopic biopsy. This specimen was diffuse staining by leukocyte common antigen, CD10 and CD20. The patient was diagnosed with acute pancreatitis associated with stage 3 duodenal Burkitt's lymphoma. Modified LMB-98 was initiated. Delay in initiation of treatment distinctively increases mortality. Treatment response to anti-tuberculosis drugs is usually observed by regression of symptoms and clearance of ascites. With initiation of treatment, laboratory values including CA-125 levels generally return to normal levels in 3 months. However, there is still no consensus about treatment response during the follow-up period. Serum D-dimer level is used as an inflammation marker in some cases. A case with Tuberculous peritonitis successfully monitorised by serum D-dimer levels is presented.Article A Case of Adem Mimicking Cerebral Adrenoleukodystrophy Based on Supratentorial Mri Findings(Aves, 2014) Beyazal, Mehmet; Unal, Ozkan; Yilmaz, Sanem; Bora, AydinA 9-year-old male admitted for syncope also had the complains of pain and numbness in his legs and frequent falling down. There was a history of upper respiratory tract infection 10 days before. On neurologic examination, paraparesia and fall a sleep were identified. On magnetic resonance imaging, the symetric signal increases were seen in biparieto-occipital white matter intented to corpus callosum at T2-weighted sequences and cytotoxic edema was seen at diffusion-weighted images. Heterogeneous contrast enhancement was seen on these areas. In addition, at the C7-Th5 vertebrae levels, spinal cord had diffuse increased signal intensity and contrast enhancement. Acute disseminated encephalomyelitis was thought based on clinical and radiological findings. Steroid therapy was started. Significant improvement was shown after treatment. On 2-year follow-up, there was no recurrence. In conclusion, it must be kept in mind that acute disseminated encephalomyelitis can rarely present with biparieto-occipital involvement which extends to corpus callosum and can mimic adrenoleukodystrophy. For the differential diagnosis butterfly glioma, tumefactive demyelinating lesions or multiple sclerosis should be considered.Article A Case Report of Acute Acalculous Cholecystitis and Acute Hemorrhagic Cystitis Due To Salmonella Typhi(Hindawi Ltd, 2014) Polat, Hatice Beyazal; Beyazal, Mehmet; Celiker, Fatma BeyazalAcute acalculous cholecystitis and acute hemorrhagic cystitis due to Salmonella Typhi are a rare condition. A 24-year-old female patient was admitted to our clinic with abdominal pain, nausea, fever, headache, urinary burning, and bloody urine. Based on clinical, laboratory, and radiological evaluations, the patient was diagnosed with acute acalculous cholecystitis and acute hemorrhagic cystitis due to Salmonella Typhi. The patient was treated with intravenous ceftriaxone for two weeks. After the treatment, the patient's clinical and laboratory findings improved. Acute acalculous cholecystitis due to Salmonella Typhi concomitant with acute hemorrhagic cystitis is very rare and might be difficult to diagnose. Forty patients who were diagnosed to have acute cholecystitis by ultrasonographic examination were included in this study. The control group consisted of 18 patients without symptoms of gallstones and cholecystitis whose gallbladder walls were thickened due to cirrhotic ascites. Both groups were examined using diffusion weighted imaging, and the mean ADC values were compared using Student's t-test. The diagnoses of the 40 patients were proven by histopathological examination. The mean ADC values of patients diagnosed with cholecystitis (1.68 +/- A 0.36 x 10(-3) mm(2)/s) were significantly lower than the mean ADC values of the control group (2.35 +/- A 0.24 x 10(-3) mm(2)/s) (p < 0.05). Receiver operating characteristics curve analysis based on ADC revealed a cut-off value of 2.04 x 10(-3) mm(2)/s for the diagnosis of cholecystitis, with a sensitivity of 94 % and a specificity of 89.7 %. ADC value quantification may be an efficient method for making a diagnosis of cholecystitis and in differential diagnosis of cholecystitis from the extrinsic benign gallbladder wall thickening that can be seen during the course of cirrhotic ascites.specialization-in-medicine.listelement.badge Efficiency of Endovenous Laser Ablation Method in the Treatment of Lower Extremity Superficial Venous Insufficiency(2012) Beyazal, Mehmet; Avcu, SerhatAmaç: Varriköz venlerle ilişkili olan yüzeyel venöz yetmezlik yaygın bir problemdir ve tedavi edilmezse venöz ülserasyonla sonuçlanabilir. Endovenöz Lazer Ablasyon (EVLA) yetmezlikli venlerin ablasyonu için lazer enerjisinin kullanıldığı, yüzeyel venöz yetmezlik ve varriköz venlerin tedavisinde konvansiyonel cerrahiye alternatif minimal invaziv yeni bir yöntemdir. Bu çalışmanın amacı safen venlerin yetmezliğinde 980 nm EVLA tedavisinin etkinliğini değerlendirmektir.Materyal ve Metod: Safenofemoral yetmezliği olan 40 hastada 38 vena safena magna ve 5 vena safena parva 980 nm diode endovenöz lazer ve tamamlayıcı köpük skleroterapi ile tedavi edildi. 39 hastada tek taraflı iki hastada bilateral EVLA uygulandı. Tedavi edilen venlerin ortalama çapı ve uzunluğu, toplam lazer enerjisi ve dağılan enerji yoğunluğu (Joule/cm) kaydedildi. Venöz hastalık şiddetini tespit etmek için Venöz Klinik Şiddet Skorlaması (VCSS) ve Vizüel Analog Skala (VAS) kullanıldı. İşlem sonrası hastalar 6 ay süreyle klinik olarak ve Doppler US ile takip edildi. Komplikasyonlar kaydedildi.Bulgular: Toplam 40 hastada 43 safen ven tedavi edildi. Hastaların ortalama yaşı 39.9 (21-72) idi ve 22 si kadın idi. Tedavi edilen venlerin ortalama çapları ve uzunlukları sırası ile 4.9 mm (3.5 - 8.5) ve 30.2 cm (16-50) idi. Ortalama toplam lazer enerji miktarı 2545 ± 669.6 J (950 - 3940) idi. Dağılan enerji miktarı 83.6 ± 7.5 J/cm (59.3 - 97.1) idi. Altıncı ay takiplerde tam okluzyon oranı % 95.4 (43/41), rekanalizasyon oranı % 4.6 idi. İşlemden önce ve sonraki VCSS ve VAS skorlarında anlamlı oranda düşme izlendi. Derin ven trombozu gibi majör komplikasyonlar gözlemlenmedi. % 84 oranında 1 hafta süre ile ekimoz görüldü.Sonuç: Endovenöz lazer tedavisi yüzeyel venöz yetmezlikte yüksek hasta memnuniyeti ve düşük komplikasyon oranlarıyla lokal anestezi altında uygulanabilen güvenli bir yöntemdir.Article Herlyn-Werner Syndrome: Merits of Sonographic and Magnetic Resonance Imaging for Accurate Diagnosis and Patient Management in 13 Cases(Elsevier Science inc, 2015) Yavuz, Alpaslan; Bora, Aydin; Kurdoglu, Mertihan; Goya, Cemil; Kurdoglu, Zehra; Beyazal, Mehmet; Akdemir, ZulkufStudy Objective: To review the main sonographic and magnetic resonance imaging manifestations of Herlyn-Werner-Wunderlich syndrome, characterized by uterus didelphys, obstructed hemivagina, and ipsilateral renal agenesis, and to discuss the value of techniques for its diagnosis and treatment. Participants: Thirteen patients with Herlyn-Werner-Wunderlich syndrome referred to our department with acute abdomen were identified for the period from 2009-2012. Design: Retrospective chart review. Setting: Two tertiary academic centers. Interventions: None. Main Outcome Measures: The evaluation of the clinical features, ultrasonography and magnetic resonance imaging findings, and the treatment protocols. Results: Ultrasonographic evaluation of didelphic or double uterus in 10 and bicomuate-bicollis uterus in 3 patients with hematocolpos or hematometrocolpos associated with agenesis of the ipsilateral kidney revealed the diagnosis. In 10 patients, magnetic resonance imaging provided extra information. In 1 patient, a 25-week pregnancy was additionally detected and follow-up was suggested. Seven patients underwent surgical excision of the septum and drainage of the obstructed vagina. Total hysterectomy and hemicolpectomy were performed in 1 patient with 2 children suffering from severe chronic pelvic pain due to pelvic inflammatory disease. "It is characterized by the diffuse proliferation of neoplastic glial cells that involve more than two cerebral lobes. For a definitive diagnosis histopathological examination is required. Advanced magnetic resonance imaging techniques are very useful in the pretreatment diagnosis-staging and post-treatment follow-up. In this report a patient with gliomatozis cerebri was presented with magnetic resonance, diffusion weighted imaging and magnetic resonance spectroscopy findings. However, spinal metastasis is extremely rare. A case with yolk sac tumor (YST) and cauda equina metastasis was presented in a 2-year-old boy. Two months prior to admisssion, he underwent left radical orchiectomy for testicular YST. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed spinal and cauda equina involvement. Althought it has a good response to spesific antibiotherapy, the delay in diagnosis and in treatment may result in necrosis and abscess formation. Ultrasonography is the basic imaging method for screening of scrotal diseases such as BEO. In literature, there are many studies which present sonographic and magnetic resonance imaging (MRI) features of scrotal and testicular lesions. However; there is no any sufficient publication in which the findings of BEO have been defined on MRI examination. Only a few cases of hypoglossal paraganglioma have been reported in the published literature. The localization of hypoglossal paragangliomas close to the carotid artery precludes determination of tumor origin preoperatively. Case Report: A 58-year-old female patient was admitted due to neck pain. During physical examination, a significant mass could not be palpated in the upper left part of the neck, despite sensitivity during palpation. Atrophy and left deviation of the left half of the tongue was observed. MRI of the neck revealed a lesion located superior to the carotid bifurcation between the left internal carotid artery and external carotid artery. There was atrophy in the left half of the tongue. The neck mass displaced the left internal carotid artery anteriorly and medially. The operation was performed with left lateral cervical access. This lesion, which derived from the hypoglossal nerve, was excised. Following histopathological evaluation, the lesion was diagnosed as paraganglioma. Conclusions: Hypoglossal paraganglioma is quite rare and there are no established criteria for preoperative diagnosis. Hypoglossal paraganglioma must be considered to determine treatment options if a lateral neck mass and ipsilateral tongue atrophy are present at the level of the 12th cranial nerve tract.Article Successful Retrieval of the Detached Porous Metallic Tip of a Mechanical Aspiration Catheter During Thrombectomy in a Case With May-Thurner Syndrome: a Case Report(Thieme Medical Publ inc, 2016) Yavuz, Alpaslan; Andic, Cagatay; Gur, Ali Kemal; Goya, Cemil; Bora, Aydin; Beyazal, MehmetModern, minimally invasive techniques used to treat deep venous thrombosis, such as percutaneous mechanical thrombectomy (PMT) and catheter-directed thrombolysis, have gained worldwide acceptance. PMT has the advantage of speed and is also associated with improved outcomes, shortened hospital stays, and low complication rates. The main complications associated with PMT have been primarily due to iatrogenic vascular damage resulting in perforation, embolic occlusion, and arteriovenous fistula formation; to date, there has been no publication in the literature describing complications resulting from device failure. We present an unusual complication of PMT resulting from detachment of the catheter tip during thrombectomy and bailout technique employed.Article Value of Dw-Mri Adc Quantification of Colonic Wall Lesions in Differentiation of Inflammatory Bowel Disease and Colorectal Carcinoma(Springer, 2014) Avcu, Serhat; Bulut, Mehmet Deniz; Yavuz, Alpaslan; Bora, Aydin; Beyazal, MehmetThe purpose of our study was to assess the efficiency of diffusion-weighted magnetic resonance imaging (DW-MRI) and the quantification of apparent diffusion coefficient (ADC) values in differentiating colorectal carcinoma from colonic inflammatory bowel disease (IBD) in cases with isolated colonic wall lesions and uncertain clinical and radiologic diagnostic criteria. The study comprised 58 patients with segmental or focal isolated colonic wall thickening. All lacked satisfactory clinical-radiological findings for etiology determination. The mean ADC values of the thickened colonic walls were retrospectively compared with final histopathologic diagnoses. Receiver operating characteristic (ROC) curve analysis was used to determine the ADC cutoff value for differentiation. Mean ADC value in the colorectal carcinoma group was significantly lower than that in the IBD group: n = 27, 1.02 +/- A 0.26 x 10(-3) mm(2)/s; and n = 31, 1.53 +/- A 0.19 x 10(-3) mm(2)/s, respectively (P < 0.001). Cutoff ADC value for differentiating colorectal carcinoma from IBD was calculated as 1.39 x 10(-3) mm(2)/s, with 83.9 % sensitivity and 85.2 % specificity. ADC measurement of the involved colonic wall segments with DW-MRI has the potential to differentiate isolated colonic IBD from colorectal carcinoma in cases in which clinical-radiologic findings are insufficient for a definitive diagnosis.
