Article Analysis of Sinonasal, Pharyngeal and Allergy-Related Risk Factors for Chronic Suppurative Otitis Media(Carbone Editore, 2013) Bozkus, Ferhat; Bozan, Nazim; Iynen, Ismail; Sakin, Yunus Feyyat; Kiris, MuzafferAim: To evaluate potential sinonasal, pharyngeal and allergy-related risk factors that may be associated with the risk of chronic otitis media development. Methods: After a detailed anamnesis, serum eosinophilic cationic protein levels, total and allergen-specific IgE levels and nasal smear samples were assessed for allergy-related findings. Paranasal sinus computed tomography was performed for rhinologic abnormalities. Results: The present study showed more frequently in patients suffering from chronic otitis media, several abnormalities in different allergic parameters such as eosinophil levels in the smear, serum eosinophilic cationic protein, total and specific serum IgE levels, which might cause nasal stuffiness, as hypertrophy of adenoid tissue, and presence of imaging findings consistent with obstruction of osteomeatal unit. Conclusion: This study demonstrates that the role of sinonasal abnormalities and allergic rhinitis in the pathogenesis of chronic otitis media is still prevalent. We have concluded that although medical history, and physical examination are considered mandatory procedures during the initial evaluation of the patients with chronic otitis media, radiological, endoscopic, and other auxiliary diagnostic tools should be used in the objective assessment of the patients, and lesions of the nasal cavity, and the nasopharynx must be always taken into consideration in the differential diagnosis.Article Ataksi Telenjiektazili Hastada Primer Submandibular Bez Lenfoması: Olgu Sunumu(2004) Kıroğlu, A. The pharyngeal bursa (PB) is a recess lined with respiratory epitheliumalong the posterior wall of the nasopharynx between the longus capitis muscles. If the opening through which the bursa drains into the nasopharynx becomes obstructed, a Thornwaldt's cyst (TC) might develop. Small-sized cysts are mostly asymptomatic, whereas large-sized cysts may lead to various nasal and otologic symptoms depending on their localization. In this report, we present a 71-year-old case with a huge TC who presented to our clinic with the complaints of snoring, nasal obstruction, periodic halitosis, postnasal drip, headache, and hearing loss in the left ear and underwent total excision of the cyst through transnasal endoscopy. The diagnosis and treatment characteristics of the case are presented with the review of the literature.Article Parotis Pleomorfik Adenomlarının Cerrahi Tedavisi(2005) Bozkus, Ferhat; Çankaya, Hakan; Bayram, İrfan; Kıroğlu, A. Faruk; Yuca, KöksalAmaç: Pleomorfik adenom en yaygın olarak parotis bezinden kaynaklanan yavaş büyüyen benign bir tükrük bezi tümörüdür. Bu tümörün etyolojisi bilinmemektedir. Fakat malign dejenerasyonla ilişkili olması cerrahi endikasyonlarından birini oluşturmaktadır. Yüksek tümör, rekürrensi ve komplikasyonları nedeniyle parotis pleomorfik adenomunun uygun cerrahi tedavisi önemlidir. Tedavide geçerli ve en yaygın uygulama süperfisyal parotidektomi yoluyla bu tümörlerin eksizyonu ve operasyon sonrası takibidir. Yöntem: Çalışmamızda Yüzüncü Yıl Üniversitesi Tıp Fakültesi KBB Kliniğinde parotis pleomorfik adenoma tanısı ile öpere edilen 11 hastanın dosya kayıtları retrospektif olarak incelendi. Klinik bulgular, preoperatif araştırmalar, operasyon yöntemi, hiştopatolojik tanı ve postoperatif komplikasyonlar kaydedilerek literatür eşliğinde tartışıldı. Bulgular: Çalışmaya alınan 11 hastanın dokuzu kadın ve ikisi erkekti (yaş aralığı 13-51, ortalama yaş 33.3±13,4). Tüm vakalarda tümör parotis bezinin yüzeyel lobunda bulunmaktaydı ve ince iğne aspirasyon biyopsisi parotis pleomorfik adenomunu destekliyordu. Yedi hastada (% 63.6) sağ taraf ve 4 hastada (%36.4) sol taraf tutulmuştu. Tüm vakalara süperfisyal parotidektomi uygulandı. Ortalama takip süresi 1.2 yıldı. Hiç bir vakada kalıcı fasial sinir disfonksiyonu ve rekürrens gözlenmedi. Sonuç: Parotis bezinin yüzeyel lobunda yer alan pleomorfik adenomlarda, fasial sinir korunarak süperfisyal parotidektomi yoluyla tümörün intakt kapsülle birlikte eksizyonu uygun tedaviyi sağlar ve genellikle rekürrensin önlenmesi mümkün olabilir.Article The Preauricular Sinus / Cyst: a Case Report(2016) Bozkus, Ferhat; Bozan, Nazim; Gözen, Ahmet; Sakin, Yunus FeyyatPreauriküler sinüs embriyonik gelişimde kulağı oluşturan birinci ve ikinci brankial yarığın birleşmesi sırasında meydana gelen bozukluk onucu oluşan konjenital bir anomalidir. Genellikle semptomsuz olup, nadiren tekrarlayan rekürren şişlikle beraber akıntı, üzerinde inflamasyon meydana gelen ve enfeksiyonlarla bir arada olabilmektedir. Sağ kulak önünde 3 yıldır tekrarlayan rekürrent şişlik ve yara enfeksiyonu bulunan 18 yaşındaki erkek hastada preauriküler sinüs saptandı ve literatür eşliğinde vaka tartışıldı. Özellikle preauriküler bölgenin iyileşmeyen ve tedavilere cevap vermeyen tekrarlayan rekürren şişlikle beraber akıntı, üzerinde inflamasyon meydana gelen deri enfeksiyonu ve yaralarının emininde preauriküler sinüsün de olabileceği unutulmamalıdır. Fistül traktusunun yetersiz rezeke edildiği vakalarda geç dönemde nüks meydana gelebilmektedir. Preauriküler sinüs/kist preauriküler bölge lezyonlarında ayırıcı tanıda düşünülmelidir.Editorial Reattachment of a Partially Amputated Ear Without Microsurgery(Pakistan Medical Assoc, 2016) Bozan, Nazim; Sakin, Yunus Feyyat; Bozkus, Ferhat; Ciftci, Mehmet AliTraumatic ear amputations are relatively rare. Whenever possible, ear re-implantation should be attempted; however the choice of the surgical procedure must be judicious. In the current report, a case of complete non-microsurgical salvage of a partially amputated ear treated by the pocket technique described by Mladick was presented. The surgical technique is described in detail by serial photographs, along with the postoperative management and outcome of the patients. The revascularisation of the severed part was successful. Morphological result was very good when the ear was freed from the pocket. We recommend the Mladick's procedure for reimplantation of fragments less than 1/2 of the auricle with favourable tissue condition.Article Suppurative Cervical Tuberculous Lymphadenitis Mimicking a Metastatic Neck Mass(Lippincott Williams & Wilkins, 2016) Bozan, Nazim; Sakin, Yunus Feyyat; Parlak, Mehmet; Bozkus, FerhatTuberculosis is an important public's health problem in developing countries. Although tuberculosis is commonly found in lungs, it could also be found in lymph nodes, mouth, tonsils, tounge, nose, epiglottis, larynx, and pharynx of head-neck region. The most common form of extrapulmonary tuberculosis placed outside the lungs is tuberculous lymphadenitis. Tuberculosis lymphadenitis is the most commonly seen form of the extrapulmoner tuberculosis and usually invades the lymph nodules of the cervical region. An adult patient with tuberculous lymphadenitis has been presented within the context of the literature reviewed. A 42-year-old woman, who has admitted to our clinic with a fistulized mass in the right side of her neck, has been diagnosed and treatment has been performed. No other problems were found in the systemic examination of the patient. Since there is no specific finding of pulmonary tuberculosis, detailed investigations are required in patients admitted with the sypmtomps of neck mass.Article A Thyroglossal Cyst With Double Duct Seen in an Adult Patient(Lippincott Williams & Wilkins, 2016) Bozan, Nazim; Sakin, Yunus Feyyat; Bozkus, Ferhat; Kanmaz, AlperThyroglossal duct cysts (TDCs) are one of the most common congenital cervical masses that develop in children. Double TDCs are reported in very few patients in the literature. However, the authors did not come across any thyroglossal cyst with double duct in the English literature. A 35-year female patient consulted to authors' clinic with complaints of a swelling localized on the left side of the neck and recurrent purulent discharge. A Sistrunk operation was planned and during the operation, 2 fistula tracts, both terminated in the hyoid bone, were exposed. Usually, only one fistulous tract opens onto the skin; however, in the authors' patient, the authors encountered TDC with double ducti. To authors' knowledge, no similar patients were reported in the literature.
