Article Association Between Endothelial Nitric Oxide Synthase Polymorphisms T786c and G894t and Ischaemic Stroke(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Cilingir, V.; Donder, A.; Milanlioğlu, A.; Yilgör, A.; Tombul, T.Endothelial nitric oxide synthase (eNOS) gene polymorphisms are suspected to increase the risk of ischaemic stroke (IS). eNOS-synthesized NO regulates vascular tone and inhibits the progression of atherosclero sis. The present study aimed to determine the association between eNOS polymorphisms G894T and T786C and IS. Sixty acute IS patients (32 male, 28 female) were included and classified in accordance with the Trial of Org 10172 in Acute Stroke Treatment (TOAST) classification. Genotypes of patients with eNOS G894T and eNOS T786C polymorphisms were determined through polymerase chain reaction. Significant differences were observed in the distribution of eNOS T786C polymorphism among IS subgroups. The eNOS T786C polymorphism heterozygote (TC) and homozygote (CC) genotypes more frequent in patients in the large artery atherosclerosis (LAA) subgroup. Considering a dominant model of inheritance for eNOS T786C polymorphism, the risk of IS was higher for the LAA subgroup than for other IS subgroups. Among potential haplotypes, the eNOS 786C+ eNOS 894G haplotype was associated with an increased risk of LAA; however, this finding was not statistically significant. eNOS gene polymorphisms are suspected to increase the risk of ischaemic stroke. N.; Tombul, T.Article Evaluation of Psychiatric Status and Quality of Life in Alzheimer's Patients and Carers(Yuzuncu Yil Universitesi Tip Fakultesi, 2023) Görken, G.; Cilingir, V.Apart from the importance of caregiver support in Alzheimer's Disease (AD), whose frequency is increasing and the destruction process is 8 to 10 years, psychological and physical depressions are inevitable in the caregivers who provide support. In our study, we compared the effect of quality of life and depressive symptoms of the patient and caregiver with sociodemographic characteristics and their relationship with the degree of illness. To work; 60 Alzheimer's patients currently being cared for at home and their caregivers were included. Some test were administered to the patients. The Beck Depression Scale and the World Health Organization's Quality of Life Scale-Short form were filled out by Alzheimer's caregivers. In our study, we found that most of the caregivers were women, married, and most cared for by their brides. We found that the quality of life and mental state of the caregiver was more affected by sociodemographic characteristics and closeness to the patient, rather than the degree of illness of the patient, because among the caregivers, the spouses and then the children were the ones showing the most depression symptoms. In the light of these findings, we determined the necessity of providing a more professional caregiver mechanism other than relatives in Alzheimer's patients and its importance for the mental health of patients, caregivers and their families. © 2023, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Psychotic Attacks Due To Toxic Neurobrucellosis in Two Adolescent Patients(Yerkure Tanitim ve Yayincilik, 2014) Ozdemir, O.; Baran, A.I.; Cilingir, V.; Bulut, M.D.; Ozdemir, P.G.; Yilmaz, E.Brucellosis is a multisystem disease which can present with a broad spectrum of clinical manifestations and complications and affect the central nervous system directly or indirectly. Immunopathologic mechanisms like T-cell mediated cytotoxicity and microglia activation are suggested to play a role in neurobrucellosis. The diagnosis of toxic neurobrucellosis is confirmed by isolation of Brucella organism from blood cultures and/or positive Coombs Wright test and the Standard agglutination test (SAT) in serum when there are no cerebrospinal fluid (CSF) findings. The magnetic resonance imaging (MRI) of brain in patients with neurobrusellosis may show different findings mimicking such neurological diseases as inflammation, white matter changes and vascular involvements and other infectious and inflammatory conditions. Different clinical manifestations of neurobrucellosis have been described including meningitis, meningoencephalitis, myelitis, and psychiatric disorders. It has been indicated in case studies that neurobrucellosis may lead to psychotic disorders. In this study, we present two adolescents who presented with psychotic symptoms due to toxic neurobrucellosis.
