Article The Acute Anomaly of Intestinal Non-Rotation(1996) Dilek, O.N.Intestinal non-rotation is a rare fetal disorder of the intestine. It is seen very rarely in adults and may present acutely as bowel obstruction. Surgical intervention is frequently delayed in these patients due to atypical symptoms. The classical surgical treatment for intestinal non-rotation is the Ladd procedure which consists of lysis of duodenal adhesions and placement of the cecum into the left upper quadrant. In this article, we present a 62 year-old man who had volvulus of the right colon caused by intestinal non-rotation. The patient underwent a right hemicolectomy and end-to-end ileotransversostomy. The postoperative period was uneventful, and the patient is asymptomatic at the 9th month of follow-up.Article Gasless Vs Gaseous Laparoscopy in the Treatment of Hepatic Hydatid Disease(Springer New York, 1999) Berberoǧlu, M.; Taner, S.; Dilek, O.N.; Demir, A.; Sari, S.Background: Despite the reduced rate of occurrence, the hydatidosis of the liver is still taking an important place in surgical practice in Asia Minor and the Middle East. Traditional techniques for performing liver cyst surgery seem to be comparatively traumatic. In this clinical study, we present our experience with laparoscopic treatment of hydatid cyst of the liver and discuss the validity of the gasless technique as a solution to carbon dioxide (CO2) ensufflation problems. Methods: All patients were prepared by administrating albendazole for 21 days preoperatively. Surgery was performed on 87 patients under general anesthesia. Working space was obtained in 51 operations by using an abdominal wall lifting device, Laparolift(TM) (Origin Med Systems, Menlo Park, California, USA) (group 1). In 36 patients, the abdominal cavity was insufflated with CO2 gas (group 2). In all cases, hydatid cysts were identified, and gauses soaked in germicide solution were placed around them. The cysts were punctured and aspirated. Then germisid solution was injected into the cysts. The cysts walls were opened, and germinative membranes were evacuated. Results: The median operation time was 50.49 ± 10.9 min (range, 30-75 min) in group 1 and 70.8 ± 16 min (ranges 40-120 min) in group 2. The difference in the operative times of the two groups was significant (p < 0.01). There was no significant difference between the minor complications of the two groups. There were no deaths and no major complications or conversions to open surgery in any of the groups. There were no recurrences during follow-up time. Conclusions: The use of gasless technique for the laparoscopic treatment of liver cyst is a safe, time-saving, and promising procedure that can be applied in selected cases.Article Prophylactic Appendectomy : Is It Worth To Be Done(ARSMB-KVBMG, 2001) Dilek, O.N.; Güler, O.; Güler, A.A.; Demirtas, I.; Altindis, M.; Dilek, F.H.; Özgören, E.There are still many controversial issues in prophylactic appendectomy. In this retrospective study, we have appraised the reasons and results of prophylactic appendectomies performed between January 1997 and August 1999. Sixty four prophylactic appendectomy cases were included in this study and all resected specimens were submitted to histopathological analysis. In 23 patients with incisional hernias, a prophylactic appendectomy was performed as, in the future, dense adhesions may lead to difficult appendectomy. Additional appendectomy had no negative effect on perioperative complications, compared with the patients without appendectomies. In conclusion, in the presence of optimal conditions, prophylactic appendectomy can be performed safely.Article Protection of Intestinal Anastomoses in Septic Environment With Peritoneal Graft and Polyglycolic Acid Mesh : an Experimental Study(ARSMB-KVBMG, 1996) Dilek, O.N.; Bakir, B.; Dilek, F.H.; Demirel, H.; Yiǧit, M.F.We carried out an experimental study in dogs to evaluate the outcome of large bowel anastomosis with 6 stitches (Group C, n : 6) in a septic environment with protection by a polyglycolic acid (PGA) mesh (Group M, n : 12) or peritoneal graft (Group P, n : 12). Thirty dogs were used to compare the techniques. Two dogs in each group were re-operated after 3, 5, 7, 14, 28 and 90 days. They were evaluated for adhesion formation, lumen diameter (anastomotic index), clinical features, histologic appearance and quality of healing at the anastomotic sites. All dogs in group P and group M survived, whereas 2 dogs in the control group died of anastomotic leakage and 3 dogs were re-operated for anastomotic leakage and peritonitis. In group M, one dog was reoperated because of the anastomotic leakage and two dogs were reoperated because of the anastomotic stenosis. Also, 4 anastomoses showed evidence of moderate stenosis. In group P, three anastomoses were graded as minimal stenosis. Histopathologic evaluation showed more complete epithelization, less inflammation, and less adhesion in group P than group M. We could not find any study in the literature that described and compared both techniques. We report here the results of such a study.Article The Role of Power Doppler Sonography in the Evaluation of Superficial Soft Tissue Abscesses(1998) Arslan, H.; Sakarya, M.E.; Bozkurt, M.; Ünal, Ö.; Dilek, O.N.; Harman, M.Objective: To evaluate the efficacy of power Doppler ultrasonography in depicting increased vasculature and hyperemia around the superficial soft tissue abscess. Materials and methods: 21 patients with soft tissue abscess were evaluated with gray scale imaging, color Doppler sonography, power Doppler sonography and computed tomography. In each case attempts were made using power Doppler sonography to demonstrate any areas of increased vascularity around the lesion. The results were compared with computed tomographic findings. Results: Peripheral hyperemia and increased vasculature were demonstrated with power Doppler sonography in 19 of 21 patients with soft tissue abscess. The hyperemic area demonstrated around the wall of the abscess by power Doppler sonography was similar to the enhanced area shown by computed tomography performed after contrast administration. Conclusion: Power Doppler sonography shows increased vasculature and hyperemia in the wall of abscesses. Therefore, power Doppler sonography can be used to assist with the diagnosis of superficial soft tissue abscess.Article Trichobezoars as a Cause of Gastrointestinal Obstructions: the Rapunzel Syndrome(1996) Şeker, B.; Dilek, O.N.; Karaayvaz, M.We report a trichobezoar extending from the stomach through the entire length of the small intestine to the caecum. A case history of a 6-year-old girl with trichobezoar is described. She is the ninth patient with Rapunzel syndrome in the literature. The epidemiology, clinical features, complications, treatment and literature are briefly reviewed.
