Article The Effects of Blood Glucose Regulation and Treatment Regime on Mean Platelet Volume in Type 2 Diabetic Patients(Wolters Kluwer Medknow Publications, 2021) Alay, M.; Atmaca, M.; Ucler, R.; Aslan, M.; Seven, I; Dirik, Y.; Sonmez, M. G.Background: Mean platelet volume (MPV) is associated with cardiovascular morbidity and mortality in type 2 diabetic patients. However, the effects of blood glucose regulation and treatment regime on MPV has not been adequately studied in type 2 diabetic patients. Aims: We studied the effects of blood glucose regulation and treatment regimen on mean platelet volume in Type 2 diabetic patients. Subjects and Methods: A total of 232 diabetic patients who were admitted to the hospital in short intervals of 3 months in the last 2 years were included in the study. When the second admission HbA(1c) was greater than the first admission HbA(1c), they were classified as being in the deteriorated blood glucose regulation group, otherwise they were classified in the improved blood glucose regulation group. Also, the deteriorated and improved blood glucose regulation groups were classified based on therapy modalities as the sulfonylurea + metformin group and the insulin + metformin group. Paired t-test was used for comparison of the groups. Results: Of the 232 patients, 98 (42.2%) were male and 134 (57.8%) were female. There were 126 (55.2%) patients using sulfonylurea + metformin, while 106 (44.8%) patients were using insulin + metformin. MPV levels were significantly increased in patients with deteriorating glucose regulation (p = 0.003). This increase in MPV was only seen in the oral hypoglycemic treatment group (p = 0.003). Conclusions: Our results suggested a close relationship between poor glycemic control and increased platelet activity in type 2 diabetic patients with oral antidiabetic therapy when compared to the insulin and metformin therapy modality.Article Gastrıc İntestınal Metaplasıa And Covıd 19 İnfectıon(Yuzuncu Yil Universitesi Tip Fakultesi, 2020) Aydın, M.; Dülger, A.C.; Özer, S.; Dirik, Y.; Keskin, S.; Demir, C.Gastric intestinal metaplasia (GIM) is defined as immigration of enteric or colonic mucosa within gastric lining.Gastric intestinal metaplasia (GIM) is a premalign condition seen in gastric lining. It is prevalent in Asia and may lead to gastric carcinoma at a rate of 1% annually. COVID 19, which has been a global issue since late 2019, causes mostly respiratory symptoms; however, some patients may present with gastrointestinal symptoms including diarrhea, vomiting (5%) and abdominal pain (3.8%).We tried in this text to handle any possible association between COVID 19 infection and GIM. In this retrospective study we included 39 COVID 19 patients and 180 age-matched control subjects to compare rate of intestinal metaplasia. All statistical analyses were performed with SAS software (SAS Institute, Cary, N.C.). The clinical and radiological characteristics of patients were compared using Student's t-test. Intestinal metaplasia was found in 11 (28%) of 39 patients while in 7 (3,9%) of 181 controls, which meant a statistical significance (p<0,05). Of the 181 subjects, atrophy was found in 39 (21%) whereas it was noted in 9 (23%) of 39 patients, which yielded no statistical significance (p>0,05). All rights reserved.Article The Role of Paroxysmal Nocturnal Hemoglobinuria in Idiopathic Habitual Abortion(Yuzuncu Yil Universitesi Tip Fakultesi, 2017) Dirik, Y.; Ekinci, Ö.; Kara, O.; Dirik, D.; Doğan, A.; Demir, C.Paroxysmal nocturnal hemoglobinuria (PNH) which is an acquired clonal hematopoietic stem cell disease, usually presents with intravascular hemolysis, thrombosis and bone marrow failure. Hereditary and acquired thrombophilia are known to play significant role in the etiology of patients with habitual abortion (HA). PNH, which is a cause of acquired thrombophilia, may have a role in the etiology of HA. In the present study, we investigated the presence of PNH clone among the patients with a history of HA. 150 patients were enrolled in the study group, diagnosed with habitual abortus of unknown etiology and 150 healthy women with no history of habitual abortus as a control group. The age range for both groups was 18–55 years. The PNH clone was screened by the FLAER (fluorescein-labeled proaerolysin) method. The PNH clone was positive in five (3.3%) patients in the study group. Four of the PNH clone positive patie nts were found to have a very low clone positivity level (0.05%, 0.24%, 0.12%, 0.21%), while one had a high level (30%). PNH clone positivity results in the study group indicate that PNH should be investigated in cases of idiopathic HA, as one patient required treatment. © 2017, Yuzuncu Yil Universitesi Tip Fakultesi. We tried in this study to assess efficacy of plain x-ray and endoscopic decompression in diagnosis and treatment, respectively. A total of 34 patients diagnosed with colonic volvulus were retrospectively analyzed and success rates of endoscopic decompression were noted. Descriptive statistics for continuous variables were expressed as Mean±Standard Deviation and Median (Min-Max), and it is expressed as number and percentage for categorical variables. Correlations between some parameters in colonic volvulus patients were examined. A total of 34 patients, 76% (n = 26) male and 24% (n = 8) female, were included in the study. All patients except those with mucosal necrosis underwent successful colonoscopic detorsion (n=29; 85%). Plain x-ray alone was sufficient for diagnosis in 94%. Colonoscopic detorsion, by gaining time for elective surgery, is an effective treatment modality and computed tomography (CT) is not necessary for diagnosis in colonic volvulus. © 2020, Yuzuncu Yil Universitesi Tip Fakultesi.
