Article 2008-2012 Yılları Arasında Kliniğimize Başvuran, Sınıkçı Müdahalesi Gören, Hastaların Analizi(2015) Güner, Savaş; Gözen, Abdurrahim; Gökalp, Mehmet Ata; Ünsal, Seyyid Şerif; Dogan, AliAmaç: Bu çalışmadaki amacımız sınıkçılara gittikten sonra acile veya polikliniğimize başvuran hastaların yaş, cinsiyet, başvuru zamanı gibi özellilkleri ile birlikte komplikasyonları, tedavi seçeneklerini ve sınıkçıya başvuru nedenlerini değerlendirmekti. Gereç ve Yöntem: Bu çalışmaya kısa veya uzun süre önce sınıkçı müdahalesi görmüş ve sonrasında polikliniğimize başvurmuş hastalar dahil edildi.Hastalara daha önceden hazırlanmış sınıkçı müdahalesi ile ilgili bir takım soruları içeren form doldurtuldu. Başvuran tüm hastaların sınıkçı müdahalesi sonrası meydana gelen fizik muayene bulguları kaydedildi. Hastaların yaş, cinsiyet, yaşadıkları şehir gibi demografik verileri ve sınıkçıya başvurma sebepleri ile meydana gelen komplikasyonlarla ilgili veriler kaydedildi. Hastaların bu verileri istatistiksel olarak değerlendirildi. Bulgular: Çalışmada yer alan hastaların 85'i (%66 ) erkek, 43'ü (%34) ise kadındı. 18 yaşından küçük hasta sayısı 71(%55), 18 yaşından büyük hasta sayısı ise 57(%45) idi. Hastaların 93'ü (%73) Van merkez ve ilçelerinden, 16'sı (%13) Bitlis ve ilçelerinden, 12'si (%9) Hakkari ve ilçelerinden, 5'i (%3,6) Ağrı ve ilçelerinden, 1'i (%0,7) Şırnak İlçesinden ve 1'i (%0,7) de Muş'tan gelmişti. Hastaların 108'i (%84) alt veya üst ekstremitede kırık, 14'ü (%11) yumuşak doku yaralanması, 1'i (%1) radius başı çıkığı, 1'i (%1) dadı çıkığı, 1'i (%1) Perthes Hastalığı, 1'i(%1) enkondrom ve 1'i (%1) de yabancı cisim batması tanısı aldı Hastalara sınıkçı tarafından en sık uygulanan yöntem rendelenmiş sabun ve yumurta bulamacının ekstremite üzerine bir bezle sarılması şeklindeydi. Sınıkçı müdahalesi sonrası en erken hastaneye başvuru; 2 saat sonra düşme sonrası el bileğindeki ağrı için sınıkçı tarafından bez üzerine yumurta konularak sarılmış radius distal uç kırığı, en geç başvuran hasta ise 20 yıl önce sınıkçı müdahalesi görmüş olan ve malunion gelişmiş olan distal femur kırığıydı. Hastalara en sık konservatif tedavi uygulanırken ikinci sıklıkta cerrahi tedavi uygulandı. Hastalardan 31'i önerilen tedaviyi kabul etmedi, bir hasta da başka bir merkeze sevk edildi. It can be classified as primary and secondary HLH. Primary HLH usually presents in childhood, and is associated with gene mutations. Secondary HLH usually presents in adulthood, and is due to an underlying infection, autoimmune disease or malignancy. Bizde akutlenfoblastik lösemi tanılı hastamızda gelişen 3 kez cerrahiuyguladığımız eş zamanlı hem intravenöz hemde burun içilipozomal amfoterisin B uygulayarak başarılı bir şekilde tedaviettiğimiz mukormikozis olgusunu sunduk.Article Amputations in Eastern Turkey (Van): a Multicenter Epidemiological Study(Turkish Assoc Orthopaedics Traumatology, 2008) Dogan, Ali; Sungur, Ibrahim; Bilgic, Serkan; Uslu, Mustafa; Atik, Bekir; Tan, Onder; Akpinar, FuatObjectives: The indications for amputations vary among countries and different regions depending on many factors. This study was designed to evaluate amputations performed in the province of Van and to determine specific causes of amputations associated with geographical and. cultural characteristics of the region. Methods: A total of 475 amputations were performed in 440 patients (345 males, 95 females; mean age 28.5 years; range 3 months to 85 years) in medical institutions of Van between 1995 and 2005. The patients were evaluated with respect to age and sex, etiology, side and level of amputations, and surgical interventions performed. Results: Amputations were the most common in the 21 to 30-year age group with 109 patients. Pediatric patients below the age of 10 years accounted for 13.4%. Of all the amputations, 218 (45.9%) involved the upper extremity, and 257 (54.1%) involved the lower extremity. Amputations were right-sided in 223, left-sided in 188 patients, and 29 patients underwent multiple amputations. Trauma was the most common cause of amputations (n=177, 40.2%), with leading etiologies as gunshot injuries (n=45), land mine (n=3.3) and hand grenade (n=7) blasts. Other frequent indications apart from traumatic causes were diabetes mellitus (n=86), congenital diseases (n=33), and peripheral vascular disease (n=30). Causes more specific to regional characteristics were tandoor burns (n=7), mistreatment by traditional bonesetters (n=3), and frostbites (n=3). Replantation was performed in 24 patients, of which 15 (62.5%) were successful. Conclusion: Our region features gunshot injuries and land mine blasts as the most common traumatic causes of amputations.Article Assessment of the Neutrophil-Lymphocyte Ratio in Classic Hodgkin Lymphoma Patients(Professional Medical Publications, 2019) Dogan, Ali; Demircioglu, SinanBackground and Objective: Besides known risk scoring systems, studies have recently been conducted in relation to NLR to estimate the prognosis of HL. Some studies found a relationship of NLR with PFS and OS. Our objective was to investigate whether NLR, as an inexpensive and easily accessible test, is a prognostic marker for cHL, as in several previous studies. Methods: The study included 232 patients in the age range of 18 to 88 years who were diagnosed with classic Hodgkin Lymphoma and received ABVD chemotherapy and/or radiotherapy at Van Yuzuncu Yil University Hematology Clinic from 2000-2018. Analyses were conducted on the disease stage, risk scores, treatment responses and relapse statuses at the time of diagnosis based on the patients' NLR values at the time of diagnosis. Results: The mean age of the patients was 39.27 +/- 15.90, while 38.8% were female and 61.2% were male. The NLR value at the time of diagnosis was significantly related to stage (p:0.013), early-stage risk score (p:0.022) and treatment response (p:0.032). The cutoff value of NLR was found as 4.23. The Hb value at the time of diagnosis was significantly related to stage (p:0.00), early-stage risk score (p:0.007), treatment response (p:0.006) and the latest status of patients (p:0.005). Conclusion: High NLR values were found to be significantly related to disease stage, early-stage risk scoring and response to the treatment. These findings need to be supported by prospective studies with larger samples for these data to be used prognostic scores.Article Biomechanical Comparison of Achilles Tenotomy and Achilloplasty Techniques in Young Rats an Experimental Study(Amer Podiatric Med Assoc, 2009) Dogan, Ali; Korkmaz, Mahmut; Cengiz, Nurettin; Kalender, A. Murat; Gokalp, M. AtaBackground: Tendo Achillis lengthening is performed by means of Z-plasty in the classic treatment of clubfoot. In the Ponseti method for treating clubfoot, Achilles tenotomy is performed percutaneously for residual equine deformity. A randomized study was designed to compare tendon healing after tenotomy versus Z-plasty. Methods: Thirty-six Sprague-Dawley rats were divided randomly into two groups. On the first day, while the right tendo Achillis of group 1 rats underwent tenotomy, those of group 2 rats underwent Z-plasty. Nine rats from each group were humanely killed on days 21 and 45 postoperatively. The two groups were compared with each other biomechanically and histologically. The Achilles tendons of eight rats in each group were evaluated biomechanically, and the remaining rat in each group underwent histologic evaluation. Results: Mean +/- SD maximum load at rupture of the treated tendons on days 21 and 45 in the tenotomy group was 26.38 +/- 7.31 N and 47.16 +/- 15.36 N, respectively, and in the Z-plasty group was 27.37 +/- 5.20 N and 45.27 +/- 9.59 N, respectively. The biomechanical evaluation revealed no significant difference in terms of breaking forces between the two groups. The difference between breaking forces on days 21 and 45 was statistically significant for both groups. Conclusions: Tendons in the tenotomy group healed as well as those in the Z-plasty group, and Achilles tenotomy in the rat was similar to Z-plasty for Achilles tendon lengthening. Human correlation may or may not exist, but this study suggests that it should be considered and investigated. For instance, rhabdomyosarcoma, neuroblastoma, medulloblastoma, anaplastic oligodendroglioma, small cell carcinoma, Ewing’s sarcoma and neuroendocrine tumors have been reported to display an acute leukemia-like morphology in bone marrow aspirates after metastasizing to the bone marrow. Rhabdomyosarcoma (RMS), a malignant tumour of mesenchymal origin which can occur at various sites in the body, is one of the most common soft tissue sarcomas in both children and adolescents, but is rare in adults with a prevalence of less than 1 %. Bone marrow metastases associated with this condition may be readily confused with acute leukemia or lymphoma. Diagnostic confirmation requires immunohistochemical and flowcytometric examinations. In patients with positive CD56 and negative CD45, rhabdomyosarcoma should be included in the differential diagnosis. Here, we report an unusual case of RMS confined to the bone marrow in an older adult.Article Comparison of the Effects of Oral Iron Treatment Every Day and Every Other Day in Female Patients With Iron Deficiency Anaemia(Wiley, 2020) Oflas, Nur Duzen; Demircioglu, Sinan; Dogan, Narin Yildirim; Eker, Elife; Kutlucan, Ali; Dogan, Ali; Demir, CengizBackground: It is known that daily divided doses and high doses of iron increase hepcidin and reduce iron absorption. Aim: To compare treatments of iron replacement every other day at low doses, once a day and twice a day in terms of their effectiveness and frequencies of side-effects. Methods: For 1 month, Group I received 270 mg ferrous sulphate twice a day (total elemental iron dose: 160 mg/day), Group II received 270 mg ferrous sulphate once a day (total elemental iron dose: 80 mg/day), and Group III received 270 mg ferrous sulphate every other day (total elemental iron dose: 80 mg/every other day). Intragroup and intergroup statistical analyses were carried out. Results: Haemoglobin (Hb) increased significantly in all three groups (P = 0.00). The increase in Hb levels was similar among the groups (P = 0.09). Ferritin significantly increased in all three groups after the treatment (P = 0.00). The increase in ferritin in Group I was significantly higher than those in Groups II and III (P < 0.05). Gastrointestinal tract (GIS) side-effects were also significantly higher in Group I in comparison to the others (P = 0.001). Conclusions: A low dosage of iron treatment every other day may be used in the place of providing iron once or twice every day with similar effectiveness and lower rates of GIS side-effects.Article Compartment Syndrome Due To Extravasation of Iodixanol Contrast Medium: Case Report(Ortadogu Ad Pres & Publ Co, 2012) Guner, Savas; Ceylan, M. The content of the contrast medium, the osmolarity, the ionic/non-ionic structure and the amount of extravasated medium are important factors in the pathogenesis of extravasation injury. Many extravasations are of small volumes causing minimal swelling and erythema. Extravasations of large volumes are important in the development of compartment syndrome due to the non-ionic contrast medium. Early diagnosis is very important in the contrast medium extravasations and conservative treatment is effective in most of the patients. If compartment syndrome develops after extravasation, emergency fasciotomy should be performed immediately. To the best of our knowledge, compartment syndrome due to extravasation of a non-ionic contrast medium, iodixanol, has not previously been reported in the English literature. In this article, a case of extravasation and compartment syndrome due to the administration of iodixanol for thoracic tomography and its treatment was first reported.Article Correlation Between Red Cell Distribution Width and Grade of Hepatic Steatosis and Liver Enzymes in Patients With Non-Alcoholic Steatohepatitis(2018) Doğan, Narin Yıldırım; Dogan, Ali; Ekinci, Ömer; Karincaoglu, MelihRed cell distribution width (RDW) is a marker of chronic inflammation. Hepatic diseases are often associated with several hematological complications; therefore, high levels of RDW are seen in hepatic diseases. Given the fact that factors which cause non-alcoholic steatohepatitis such as inflammation, oxidative stress, free oxygen radicals, and inflammatory cytokines may have a role in the discrepancy in the erythrocyte volume, we aimed to investigate the relationship between RDW and hepatic enzymes and Grade of hepatic steatosis. A total of 120 patients with non-alcoholic steatohepatitis using abdominal ultrasonography which was performed by a gastroenterology specialist, were included in the study. Biochemical analysis, complete blood count, other blood tests were performed. Spearman’s Rho correlation test was used to analyze the relationship between the parameters and p value was accepted significant if it’s <0.05. As a result of our study, there was no significant relationship between RDW and liver steatosis degree (p: 0.704). Also, significant relationship was not found between RDW and alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), gamma glutamyl transferase (GGT) and lactate dehydrogenase (LDH) ( p values were 0,223, 0,856, 0,318, 0,597, 0,195 consecutively). In our study, we tried to find an answer to if there is a relationship between RDW alterations and inflammation which takes place in the pathogenesis of non-alcoholic steatohepatitis. As a result, we found no significant correlation between the RDW values and the degree of steatosis of liver and also liver enzymes.Article Decreased Bone Mineral Density and Associated Factors in Severe Haemophilia a Patients: a Case-Control Study(Wiley, 2019) Ekinci, Omer; Demircioglu, Sinan; Dogan, Ali; Merter, Mustafa; Yildiz, Saliha; Demir, CengizIntroduction Haemophilia patients may exhibit lower levels of bone mineral density (BMD) than the general population for a variety of reasons. Aim We aimed to investigate decreased BMD in people with severe adult haemophilia A (PWH) living in eastern Turkey, and to evaluate the related potential risk factors. Methods The study included 41 PWH and 40 healthy volunteers. Dual-energy x-ray absorptiometry (DXA) was used to measure the BMD. Blood tests and body mass index (BMI) were recorded. The Functional Independence Score in Hemophilia (FISH) test was used to measure functional ability status. Results There was a significant difference between the PWH and control groups with respect to femoral neck and total hip BMD (in g/cm(2)), but the difference for lumbar spine was not significant (P = .017, P < .001, P = .071, respectively). In PWH, patients under 50 years of age, 19.4% were found to have "lower than expected" BMD levels for their age, while 27.8% showed "low normal" levels. In PWH, osteoporosis was found in 60% of the patients over 50 and osteopenia in 20%. Vitamin D insufficiency and deficiency were present in 63.4% of the PWH, significantly higher than the control group (37.5%; P < .001). Conclusion The results indicated that the decrease in BMD was significantly greater in patients with severe haemophilia A than in the normal healthy population. This reduction was correlated with BMI, vitamin D and low functional ability status. However, in multivariate analysis, none of these was a strong independent risk factor.Article Diagnostic Importance of Bone Marrow Aspiration Evaluation: a Single-Center Study(Professional Medical Publications, 2022) Dogan, Ali; Demircioglu, SinanBackground &Objective: Early diagnosis can be made based on the morphological examination of bone marrow aspiration (BMA) until the bone marrow biopsy (BMB) result is reported. This allows for treatment to be started immediately, especially in hematological malignancies for which urgent treatment is indicated. This study aimed to determine the sensitivity and importance of bone marrow aspiration in the diagnosis of hematological malignancies. Methods: In this study, the data of patients who underwent bone marrow aspiration and bone marrow biopsy in Van Yuzuncu Yil University hospital between 2017 and 2019 were retrospectively analyzed. A total of 500 patients who simultaneously underwent BMA and BMB were included in the study. Data were obtained from electronic medical records. Results: Indication for bone marrow evaluation was abnormalities in complete blood count in 270 (54%) of patients. The diagnosis was made based on the evaluation of BMA in 475 (95%). In 456 (96%) of the 475 patients diagnosed with BMA, the diagnosis was consistent with that of BMB. Agreement of BMB with BMA was 100% in acute and chronic leukemias, while BMA was not sufficient for the diagnosis of lymphoma and solid organ metastasis. Conclusion: Our study showed that the evaluation of BMA was highly sensitive in the diagnosis of hematological malignancies, such as acute leukemias, chronic leukemias, and multiple myeloma.Article Effect of Zofenopril on Regeneration of Sciatic Nerve Crush Injury in a Rat Model(Thieme Medical Publ inc, 2009) Kalender, Ali Murat; Dogan, Ali; Bakan, Vedat; Yildiz, Huseyin; Gokalp, Mehmet Ata; Kalender, MahmutBackground: Zofenopril is an antioxidant agent which has been shown to have beneficial effects in hypertension and heart failure. The aim of this study was to test the effects of Zofenopril on nerve regeneration and scarring in a rat model of peripheral nerve crush injury. Methods: Twenty-one adult Sprague-Dawley rats underwent a surgical procedure involving right sciatic nerve crush injury. 15 mg/kg Zofenopril was administered orally to seven rats in group Z for seven days. Seven rats in group S received saline orally for seven days. Seven rats in the control group C received no drug after crush injury. Fourteenth and 42nd days after injury, functional and electromyography assessments of nerves were performed. Functional recovery was analyzed using a walking track assessment, and quantified using the sciatic functional index (SFI). After these evaluations, all rats were sacrificed and microscopic evaluations were performed. Results: The Sciatic functional Index (SFI) in group Z on 14th day is different significantly from group S and group C (p = 0.037). But on 42nd day there was no difference between groups (p = 0.278). The statistical analyses of electromyelographic (EMG) studies showed that the latency in group Z is significantly different from group S (p = 0.006) and group C (p = 0.045). But on 42nd day there was no difference between groups like SFI (p = 0.147). The amplitude was evaluated better in group Z than others (p < 0.05). In microscopic evaluation, we observed the highest number of nerve regeneration in the group Z and the lowest in the group C. But it was not significant statistically. Conclusion: Our results demonstrate that Zofenopril promotes the regeneration of peripheral nerve injuries in rat models.Article Effects of Thymoquinone (Volatile Oil of Black Cumin) on Rheumatoid Arthritis in Rat Models(Wiley, 2007) Tekeoglu, Ibrahim; Dogan, Ali; Ediz, Levent; Budancamanak, Mustafa; Demirel, AdnanMany studies have been carried out in recent years on the pharmacological effects of Nigella sativa seeds that have uncovered their antiinflammatory and immunological effects. The objective of this study was to explore the andinflammatory effects of thymoquinone on arthritis in rat models. Rats with arthritis induced by Freund's incomplete adjuvant were assigned to five groups: group 1: controls 0.9% NaCl (n = 7); group 2: 2.5 mg/kg thymoquinone (n = 7); group 3: 5 mg/kg thymoquinone (n = 7); group 4: Bacilli Chalmette Guerin (BCG) 6 x 105 CFU (n = 7); group 5: methotrexate 0.3 mg/kg (n = 7). Signs of inflammation on the claw and radiological signs were searched for and TNF-alpha and IL-1beta were measured. The results of the control and other groups were compared. As a result, thymoquinone, confirmed clinically and radiologically, suppressed adjuvant-induced arthritis in rats. We aimed to determine the distribution rates of ABO and Rh blood groupsand the incidence of transfusion-transmitted infections in the healthy donors in our region.Materials and Methods: A total of 108,368 healthy individuals were participated in this study. Incidence of HBsAg, anti-HCV, anti-HIV-1/2 and syphilis along with blood group distribution were evaluated retrospectively.Results: Of the blood donors, 98,901(% 91.24) were male and 9,467 (%8.76) were female. Frequency of O, A, B, and AB blood groupdistribution in the donor population were 31.2%, 44.0%, 16.2%, and 8.6%, respectively. Of the blood donors, 95,039 (87.7%) were Rhpositive and 13,229 (12.3%) were Rh negative. A total of 1,685 donors (1.55%) tested positive for HBsAg, 102 (0.09%) for anti-HCV, 23(0.022%) for anti-HIV-1/2, and 58 (0.05%) tested positive for syphilis.Conclusion: The ABO and Rh blood antigens distribution rates found in our study were similar for male and female and resembledthe overall rates for Turkey. When looking at the annual analysis of the donor group, we observed a significant decrease of HbsAgtest positivity over a seven-year-period (p values for time periods 2008-2009, 2010-2011, 2012-2013, 2014-2015 were 0.016, 0.02,0.001, and 0.003 respectively). We believe that our results will be useful for the creation of a database on blood group distributionand the seroprevalence of infectious agents.Article Giant Clinodactyly and Macrodactyly of the Hand Associated With Carpal Tunnel Syndrome(informa Healthcare, 2014) Dogan, Ali; Kalender, A. Both conditions may also be associated with a lipofibromatous hamartoma of the median nerve. However, the effect of prostaglandin I-2, (PGI(2)) on fracture healing remains unclear. To investigate the effect of PGI(2), a study on fracture healing process in closed tibia fractures was designed. Methods Thirty-six Sprague-Dawley male rats were randomized into two groups. On the first day, their right tibias were fractured by three-point bending technique. The study group (n=18) received a single injection of 10 mu g/kg iloprost for 5 days, while the control group (n=18) received saline solution in the same way. On the 7th, 14th and 28th days following the fracture, six rats were sacrificed and their right legs were harvested in each group. The progression of fracture healing was assessed for each specimen by the scores of radiography (by Lane-Sandhu) and histology (by Huo et al). Results On the 7th day, the radiographic and histologic scores were equal. On the 14th day radiographic total score was 6 and histologic total score was 23 in the iloprost group, whereas radiographic total score was 11 and histologic total score was 33 in the control group. On the 14th day radiographic and histologic scores were significantly decreased in the iloprost group compared to the control group (P < 0.05). On the 28th day radiographic total score was 12 and histologic total score was 37 in the iloprost group, whereas radiographic total score was 15 and histologic total score was 40 in the control group. On the 28th day although there was a decrease in radiographic and histologic scores of the iloprost group acording to control group, it was not statistically significant (P > 0.05). Conclusion Iloprost delays fracture healing in early stage in rats.Article Lenfadenopatinin Nadir Bir Nedeni Olarak Kimura Hastalığı: Olgu Sunumu(2017) Ekinci, Ömer; Dogan, Ali; Demir, CengizKimura Hastalığı (KH); etiyolojisi bilinmeyen, çok nadir görülen, bening karekterli kronik inflamatuvar bir hastalıktır. Klinik olarak genellikle baş-boyun bölgesinde görülür. Çok seyrek olarak da oral mukozada, diğer yüzeyel lenf nodu bölgelerinde ağrısız subkutan nodül veya kitleler şeklinde bölgesel lenf nodlarının tutulumuyla seyredebilir. Tanı laboratuvar, görüntüleme ve patolojik incelemeler ile diğer lenfadenopati ve eozinofili nedenlerinin dışlanması sonucu konulur. Olgumuzda 5-6 aydır giderek artan bölgesel lenfadenopati, yaygın vücut kaşıntı şikayetleri ve eozinofili mevcuttu. Ayırıcı tanısı yapılan ve Kimura Hastalığı tanısı konulan olgumuzu literatür eşliğinde sunmayı amaçladık.
