Article The Association Between Serum Dickkopf-1 Levels and Esophageal Squamous Cell Carcinoma(Sage Publications Ltd, 2014) Begenik, H.; Kemik, A. S.; Emre, H.; Dulger, A. C.; Demirkiran, I.; Ebinc, S.; Kemik, O.Serum Dickkopf-1 (DKK-1) is elevated in many malignancies and is an important indicator of malignant potential. However, its significance in esophageal squamous cell cancer (ESCC) has not yet been clarified. We hypothesized a role for DKK-1 in patients with ESCC. The study consisted of 90 ESCC patients and 85 healthy controls. After diagnosis, the level of DKK-1 was measured in the serum samples by enzyme-linked immunosorbent assay and the levels of DKK-1 were much higher in the ESCC patients than in the healthy control group (p < 0.0001). For serum DKK-1, the sensitivity and specificity of the assay were 70 and 80%, respectively. The preoperative serum DKK-1 level was elevated in the ESCC patients. Although serum DKK-1 is not a specific biomarker for ESCC, it might be a useful marker for the diagnosis and treatment of ESCC.Article Effect of Helicobacter Pylori Infection on the First-Line Treatment Outcomes in Patients With Immune Thrombocytopenic Purpura(Verduci Editore s.r.l, 2022) Dogan, A.; Ekinci, O.; Ebinc, S.OBJECTIVE: Helicobacter pylori (H. pylori) eradication therapy is known to increase the platelet count, but in immune thrombocytopenic purpura (ITP), the effect of H. pylori infection on the response to treatment is not clear. This study aims to determine whether the response to the first-line treatment is affected by the states of H. pylori-positivity and -negativity in ITP patients. PATIENTS AND METHODS: Adult newly diagnosed or chronic ITP patients who had not received eradication therapy for H. pylori infection were included. Characteristics of the patients, presence and severity of bleeding, initial platelet count, administered treatments, and treatment response rates were inspected. RESULTS: Of 119 total patients, 66 (55.5%) were female, 32 (26.9%) were H. pylori-positive, 87 (73.1%) were H. pylori-negative. H. pylori-positive and H. pylori- negative groups were not significantly different in terms of age (p=0.127), gender (p=0.078), diagnosis status (p=0.094) and the distribution of bleeding symptoms (p=0.712). The most common treatment was standard-dose steroid in both groups (62.5% vs. 68.9%, p=0.524). Rates of complete response, partial response, no response were comparable for the two groups (respectively, 75% vs. 73.6%, and 18.8% vs. 19.5%, and 6.2% vs. 6.9%), and there was no significant difference between the groups (p=0.283). CONCLUSIONS: It can be stated, according to the present study, that in ITP patients in whom treatment is indicated, the response to the firstline treatment without the administration of H. pylori eradication therapy is similar between H. pylori-positive and H. pylori-negative patients. © 2022 Verduci Editore s.r.l. We report a case of extrapulmonary and intrapulmonary TB in end stage renal disease (ESRD). A 58- year-old female with ESRD was admitted to emergency with the complaints of intermittent fever, weakness, impairment of consciousness for last three days. Because of mental confusion she was entubated, transferred to the intensive care unit. Thorax CT showed widespreadcentryasinarnodules in both lungs. The empirically anti-TB therapy was initiated. There was in size of 4x5 cm palpable lymphadenopaty on left supraclavicular region, excisional biopsy revealed caseating granulomatous lymphadenitis. Lumbar puncture was performed and analysis was consistent with TB meningitis. The patient was diagnosed as miliary TB, TB meningitis and TB lymphadenitis and died on the tenth day on admission. Our prefecture is an endemic area for H. pylori infection and brucellosis. However, there are no data on the H. pylori prevalence among patients with brucellosis. Objectives: The primary aim of the current study was to investigate the link between H. pylori infection and brucellosis. The secondary aim was to determine the seroprevalence of H. pylori infection in healthy individuals. Methods: Between May 2010 and May 2011, serum was collected from patients with Brucella infection (n = 111) and healthy individuals (n = 120) and analyzed using an enzyme-linked immunosorbent assay kit. Brucella infection was defined as a positive Wright test or blood culture positivity for Brucella in conjunction with disease symptoms and findings. Results: Among the 111 patients with brucellosis, 69% (n = 77) had a seropositivity result for H. pylori IgG. The healthy control subjects had an 87% (n = 104) seropositivity for H. pylori IgG. The serum H. pylori IgG antibody seropositivity was significantly lower in the patients with Brucella infection compared with the control subjects (P G 0.001). Conclusion: Our results show that the serum prevalence of H. pylori among the patients infected with brucellosis is significantly lower than that in the control population. A negative correlation between H. pylori infection and brucellosis infection might be hypothesized. Copyright © 2012 by The American Federation for Medical Research.
