Article Clinical and Laboratory Features of Cd5-Negative Chronic Lymphocytic Leukemia(int Scientific Literature, inc, 2017) Demir, Cengiz; Kara, Erdal; Ekinci, Omer; Ebinc, SenarBackground: Chronic lymphocytic leukemia (CLL) usually expresses CD5 antigen. However, 7-20% of patients are CD5 negative. We report here a series of 19 CD5-negative B-CLL cases. Material/Methods: We reviewed 19 consecutive CD5-negative B-CLL cases seen in our medical center from 2009 to 2015 and compared them with 105 CD5-positive B-CLL cases. The two groups were compared in terms of clinical parameters, laboratory parameters, and survival characteristics. Results: Lymphadenopathy was present in 31.5% of the CD5-negative group and 51.4% of the CD5-positive group (p=0.029). Splenomegaly was present in 42.1% of the CD5-negative group and 16.1% of the CD5-positive group (p=0.029). There was no difference between the groups in terms of Binet A, B, and C stages (p=0.118, p=0.051, and p=0.882, respectively). The median thrombocyte count was 144x10(9)/L and 160x10(9)/L in the CD5- negative and CD5- positive groups, respectively (p=0.044).There was no difference between the two groups in terms of median neutrophil count (p=0.169). The mean lymphocyte count was 43.2 +/- 4.0x10(9)/L and 36.7 +/- 3.2x10(9)/L in the CD5-negative and CD5-positive groups, respectively (p=0.001). There was no difference between the groups in terms of autoimmune hemolytic anemia and autoimmune thrombocytopenia. In five-year follow-up, 84.2% of CD5-negative patients and 90.5% of CD5-positive patients were alive (p=0.393). Conclusions: We found more isolated splenomegaly, less lymphadenopathy, a higher lymphocyte count, and a lower thrombocyte count in the CD5-negative group. There was no difference between the groups in terms of clinical stage, autoimmune phenomena, hemoglobin and neutrophil count, and survival.Article Clinical Overlap of Multiple Sclerosis and Autoimmune Hepatitis: Three Cases(Coll Physicians & Surgeons Pakistan, 2016) Sayin, Refah; Gokgul, Alper; Ebinc, Senar; Dulger, Ahmet Cumhur; Tombul, TemelMultiple sclerosis (MS) is an autoimmune, inflammatory disease characterized by demyelination and axonal degeneration in the central nervous system. MS is the second major cause of disability following trauma, and is mostly seen between the ages of 20 - 40 years and in women. Autoimmune hepatitis (AH) is a chronic disease characterized by hypergammaglobulinemia, high levels of transaminases, presence of antibodies, and histologically by the necroinflammatory process with interface hepatitis. In AH, the etiological agent of the disease and the cause of liver injury remain unknown. MS may be associated with AH, autoimmune thyroiditis, and type 1 diabetes mellitus (DM). In literature, 8 cases with overlap of MS and AH have been reported. In this report, we present 3 cases which were detected with overlap of MS and AH, and are very rare condition in literature.Article Comparison of First-Line Chemotherapeutics and Validation of the Eortc Prognostic Index in Malignant Pleural Mesothelioma: Retrospective Single-Centre Experience(Coll Physicians & Surgeons Pakistan, 2024) Sezgin, Yasin; Karhan, Ogur; Ileri, Serdar; Ebinc, Senar; Tunc, Sezai; Urakci, ZuhatObjective: To evaluate the efficiency of pemetrexed cisplatin in comparison with gemcitabine cisplatin and to validate the EORTC (European Organisation for Research and Treatment of Cancer) prognostic score in combination chemotherapy treatment for malignant pleural mesothelioma. Study Design: An observational study. Place and Duration of the Study: Department of Oncology, Dicle University Hospital, Diyarbakir, Turkiye, from October 2000 to Methodology: Malignant pleural mesothelioma (MPM) patients with EORTC score 0- were recruited. Factors affecting the prognosis of the disease and the effectiveness of first-line treatment were retrospectively analysed. EORTC prognostic score was calculated with a cut-off and survival analyses were used by the Kaplan-Meier method. Log-rank and univariable Cox regression tests were used to search for prognostic factors' impact on survival. Results: Patients who received gemcitabine cisplatin treatment had a median progression-free survival (PFS) of 9 months, while those who received pemetrexed cisplatin therapy had a median PFS of 7 months. Median overall survival (OS) was 17 months in the gemcitabine cisplatin group and 18 months in the pemetrexed cisplatin group (p = 0.051). When the low-risk group was compared with the high-risk group, the median OS was found to be statistically significant (p = 0.009). Conclusion: The EORTC prognostic score, which is used for prognostic prediction in the period when pemetrexed is not utilised in the treatment of MPM, accurately predicts prognosis subsequent to the administration of pemetrexed in treatment. In the context of first-line treatment, cisplatin in combination with gemcitabine and cisplatin in combination with pemetrexed demonstrated comparable efficacy with respect to both overall survival and progression-free survival.Article Efficacy of Immunotherapy Versus Chemotherapy in Advanced Pleural Mesothelioma: a Turkish Oncology Group (Tog) Study(Mdpi, 2025) Kalkan, Ziya; Ebinc, Senar; Arcagok, Murat; Bilici, Ahmet; Yildiz, Ozcan; Kilickap, Saadettin; Kaplan, Muhammet AliBackground and Objectives: This study aimed to evaluate the effectiveness of immunotherapy compared to chemotherapy across different treatment lines in patients with pleural mesothelioma. It also sought to identify factors influencing the success of immunotherapy, such as histological subtype, PD-L1 expression, type of asbestos exposure, and metastatic status. Materials and Methods: A retrospective analysis was conducted with 60 patients diagnosed with pleural mesothelioma. Data on age, gender, histological subtype, and asbestos exposure were collected for all patients and PD-L1 expression was assessed in a subset of patients. Patients received either chemotherapy or immunotherapy as first-, second-, and third-line treatments, and progression-free survival (PFS) and treatment responses were evaluated. Results: Among the 60 patients, 35 (58.3%) were male and the median age was 59 years. The majority (71.7%) had epithelioid histology and 28.3% had distant metastases. Asbestos exposure was documented in 65% of the cases. PD-L1 expression of >= 1% was found in 13 of 17 patients tested. First-line treatments included immunotherapy for 11 patients and chemotherapy for the others, with immunotherapy achieving median PFS of 9 months versus 6 months for chemotherapy, although the difference was not statistically significant. In third-line treatments, immunotherapy significantly outperformed chemotherapy with median PFS of 6 months compared to 3 months (p = 0.048). Absence of metastasis and prior asbestos exposure in an endemic region were linked to better immunotherapy outcomes. Conclusion: Immunotherapy shows increased efficacy in later treatment lines for pleural mesothelioma, especially for patients without metastases or with prior endemic asbestos exposure. A case of sewing needle ingestion which was removed from the body through defecation in a long time after passing through the small intestine without any complication is presented in this study.Article Hepatotoxicity Associated With the Ingestion of Papaver Rhoease(Pakistan Medical Assoc, 2014) Gonullu, Hayriye; Karadas, Sevdegul; Dulger, Ahmet Cumhur; Ebinc, SenarLiver injury appears to be associated with the use of herbal products and wild plants. This paper presents what is to our knowledge the first case report in the world of acute liver toxicity caused by Papaver Rhoease. A 17-year-old woman from eastern Turkey ingested Papaver Rhoease which is known as "poppy flower" among the local inhabitants. Her liver function tests were initially 1015 fold higher than normal. She was hospitalised in intensive care unit and on day 2 of hospitalisation, the results of her liver function tests showed about a hundred-fold increase compared to the normal level. Her general status deteriorated and she was transferred to another medical centre for liver transplantation.Article Higher Seroprevalence of Hepatitis B Virus Antigen in Patients With Cystic Hydatid Disease Than in Patients Referred To Internal Medicine Clinics in Turkey(Korean Soc Parasitology, Seoul Natl Univ Coll Medi, 2014) Gultepe, Bilge; Dulger, Ahmet Cumhur; Gultepe, Ilhami; Karadas, Sevdegul; Ebinc, Senar; Esen, RamazanTurkey remains an intermediate area for prevalence of hepatitis B virus (HBV) surface antigenemia. The sheepraising areas of Turkey also pose a high risk for cystic hydatid disease (CHD). Both HBV infection and CHD are major public health issues particularly in eastern parts of Turkey; however, there is no data regarding HBV infection in patients who have had CHD. The aims of this study were to evaluate the association between HBV infection and CHD and suggest ways to reduce HBV infection which is still widespread in Turkey. A retrospective study was conducted with 94 adult patients with active CHD referred to the hepatology department, Yuzuncuyil University School of Medicine from December 2010 to December 2012. All subjects came from rural areas of the region and underwent ultrasonography of abdomen which detected CHD of the liver. All the patients were serologically positive for Echinococcus granulosus. The control group consisted of 500 patients (300 men and 200 women) referred to the internal medicine clinics for other reasons. The patients with CHD and in the control group were tested for the existence of HBs antigen according to the standard procedures. The seroprevalence of HBs antigen was significantly higher in patients with active CHD than those in the control group (12.7% vs 5.2%; P=0.0017). Our data indicate that there is significant association between HBV infection and CHD. All patients with CHD should be screened for HBV infection.Article Life-Threatening Hypophosphatemia And/Or Phosphate Depletion in a Patient With Acute Lymphoblastic Leukemia: a Rare Case Report(W B Saunders Co-elsevier inc, 2014) Soyoral, Yasemin; Aslan, Mehmet; Ebinc, Senar; Dirik, Yaren; Demir, CengizAcute severe hypophosphatemia can be life threatening and is associated with mortality and impaired cardiac and respiratory function. Several conditions including decreased absorption or increased urinary phosphate excretion, shifts from the extracellular to intracellular compartments, and phosphate consumption by rapidly proliferating cells are known to induce moderate to severe acute hypophosphatemia. Although hypophosphatemia and/or phosphate depletion in patients with acute or chronic myeloid leukemia have been reported in the literature, hypophosphatemia due to acute lymphoblastic leukemia (ALL) is very rare. We report a case of history of ALL complicated by life-threatening hypophosphatemia manifesting as generalized muscle weakness, fatigue, acute shortness of breath, and difficulty in standing up and walking for 3 days. Serum inorganic phosphate levels were consistently low (0.06 mmol/L). The patient was hospitalized and thought to have a relapsed ALL. Anticancer agents and oral phosphate (660 mg twice daily) were administered. On the second day of treatment, the patient began to improve, and the patient gradually fully recovered within 5 days. We suggested that this hypophosphatemia was induced by a shift of phosphorus into leukemic cells that rapidly replicated in the tissues and excessive cellular phosphate consumption by rapidly proliferating cells. Serum phosphate levels should always be monitored, especially in suspected life-threatening manifestation in relapsed ALL.Article Presence of Paroxysmal Nocturnal Hemoglobinuria in Patients With Idiopathic Portal Vein Thrombosis: a Single-Center Study(Tubitak Scientific & Technological Research Council Turkey, 2020) Demir, Cengiz; Ebinc, Senar; Ekinci, OmerBackground/aim: Paroxysmal nocturnal hemoglobinuria (PNH) is a very rare clonal hematopoietic stem cell disease characterized by chronic hemolytic anemia and thrombosis. We report data from a study of the occurrence of PNH among patients with idiopathic portal vein thrombosis (PVT). Materials and methods: Patients who were followed up with the diagnosis of idiopathic PVT were enrolled into this study. Those with laboratory and/or imaging evidence of any local or systemic factor that could lead to PVT were excluded. PNH clone was examined in all patients using established FLAER methodology. Results: A total of 112 patients (42 males and 70 females), none of them had a markedly PNH clone, but 4 patients (3.6%) with confirmed tests two times had small PNH clones (size between 3.02% and 4.62%). The median ages of PNH clone (-) and PNH clone (+) patients were 42 (range; 24-59) vs 39 (range; 36-42) years, respectively. The median hemoglobin concentration, platelet count and leukocyte count were lower in the PNH clone (+) group than the PNH clone (-) group. Anemia, thrombocytopenia, and leukopenia were detected in all PNH clone (+) patients. In addition, the PNH clone positivity size in monocytes was higher than erythrocytes in all of 4 patients. Conclusions: PNH should be considered during differential diagnosis among patients with idiopathic PVT. Small PNH clones can be detected in PVT patients that we cannot clearly determine its relationship with PVT. We need furthermore studies to explore the potential role of this finding.Article Pulmonary Edema and Myocarditis Developing Due To Scorpion Stings(derman Medical Publ, 2015) Karadas, Sevdegul; Gonullu, Hayriye; Ebinc, Senar; Kurt, Nurettin; Golcuk, YalinAlthough most of the scorpion stings are harmless, deadly species of scorpions may cause multiorgan failure, neurotoxicity, cardiotoxicity, and pulmonary edema. The cases should be observed in the emergency department against the possibility of development of systemic effects. Fatal complications, in particular such as pulmonary edema, and myocarditis should be considered. In this study, a case of myocarditis and pulmonary edema was detected on the patient who had applied to the emergency department due to a scorpion sting is presented.Article Type 2 Diabetes Mellitus and Functional Hypoparathyroidism(Aves, 2014) Atmaca, Murat; Acar, Ismail; Gonultas, Engin; Seven, Ismet; Ucler, Rifki; Ebinc, Senar; Candan, ZehraPurpose: The present study aims to investigate the effect of blood sugar regulation and vitamin D levels on calcium metabolism and parathormone levels in patients with type 2 diabetes mellitus. Material and Method: We included 132 patients with type 2 diabetes mellitus who presented to our outpatient clinic for regular check up between August 2013 and October 2013. Fasting blood glucose, HbA1c, calcium, phosphorus, magnesium, albumin, creatinine, parathormone, 25-Hydroxy vitamin D [25(OH) D], spot urinary calcium and creatinine levels were studied for each patient. Results: Vitamin D levels were below 30 ng/mL in 96.9% (n = 128) and below 20 ng/mL in 78.7% (n = 102) of the patients included in the study. Patients with impaired blood sugar regulation (HbA1c > 10%) had lower levels of PTH, albumin and 25(OH) D levels and higher phosphorus levels compared to patients with HbA1c levels below 10% (p = 0.018, p = 0.043, p = 0.002, p = 0.01, respectively). The rates of functional hypoparathyroidism (parathormone < 65 ng/mL) in patients with vitamin D levels below 30 ng/mL and 20 ng/mL were 63.2% and 59.6%, respectively. Among the diabetic patients with vitamin D levels < 30 ng/mL, magnesium levels were significantly lower in those with functional hypoparathyroidism (parathormone < 65 ng/mL) compared to those with secondary hyperparathyroidism (p = 0.015). Comparative statistical analysis of patients with HbA1c levels above and below 10% demonstrated higher proportion of patients with functional hypoparathyroidism in the group with impaired blood sugar regulation (p = 0.035 for patients with vitamin levels below 30 ng/mL, and p = 0.031 for patients with vitamin levels below 20 ng/mL). Discussion: Impaired blood sugar regulation leads to functional hypoparathyroidism with secondary hypomagnesemia in type 2 diabetes mellitus, as was previously described for subjects with type 1 diabetes mellitus.
