Article Anesthesia Management in Laparoscopic Sleeve Gastrectomy Cases(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Tekeli, A.E.; Eker, E.; Bartın, M.K.; Öner, M.Ö.Obesity is a rapidly growing health condition worldwide and negatively affects the quality and duration of life. Anesthesia for morbidly obese patients is considered to be a real challenge to the anesthetist. In this study we aimed to evaluate difficult tracheal intubation and anesthesia management for obese patients. This study included 60 patients who had undergone laparascopic sleeve gastrectomy (LSG) between July 2012 and July 2017.We recorded gender, age, and body mass index (BMI) at the time of LSG, intraoperative incident, comorbidities, length of intensive care unit (ICU) admission. The relation of weight with difficult intubation was statistically significant, but it was not correlated with height and bod y mass index. Weight and BMI were found to have a statistically significant relationship with neck extension limitation. Difficult cases of intubation were seen mostly by 28% of patients. In this retrospective study, it was determined that laparoscopic sleeve gastrectomy and intubation difficulty was independent of body mass index. More extensive studies should be done to demonstrate the specific airway management and anesthesia of laparoscopic sleeve gastrectomy. © 2019, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Augmented Hunger Games Search Algorithm Using Logarithmic Spiral Opposition-Based Learning for Function Optimization and Controller Design(King Saud University, 2024) Izci, D.; Ekinci, S.; Eker, E.; Kayri, M.This paper explains the construction of a novel augmented hunger games search algorithm using a logarithmic spiral opposition-based learning technique. The proposed algorithm (LsOBL-HGS) is used as an efficient tool for both function optimization and controller design. To assess the performance of the algorithm for function optimization, benchmark functions from the CEC2017 test suite were employed and comparisons were made with available and good performing algorithms. In terms of controller design, the proposed LsOBL-HGS algorithm was utilized to design a FOPID controlled magnetic ball suspension system. Comparative assessments were also performed for FOPID controller design, as well using other state-of-the-art methods reported for the magnetic ball suspension system. The results showed that the proposed LsOBL-HGS algorithm has good capability for FOPID controller design employed in a magnetic ball suspension system as it provided an improvement of more than 13% in terms of the transient response-related parameters and more than 34% in terms of bandwidth compared to the best-reported approach used for comparisons. © 2022 Karabuk UniversityArticle The Effects of Perioperative Autotraiisfusion on Postoperative Blood Transfusion in Cardiopulmonary Bypass Patients(Turkish Anaesthesiology and Intensive Care Society, 2017) Gür, A.K.; Eker, E.; Yargi, M.; Kaplan, M.Objective: It is known that cardiopulmonary bypass has negative effects on patient blood values during cardiac surgeries. In our study, we aimed to investigate the effects of acute normovolemic hemodilution in patients who underwent coronary artery bypass surgery. Material and Method: In our study we included a total number of 40 patients who underwent elective coronary arter)' bypass grafting using a cardiopulmonary bypass in Siyami Ersek Chest and Cardiovascular Surgery Training and Research Hospital. Male, and female patients whose preoperative hematocrit values ivere ≥36% and, ≥ 40% were included in our study. Results: Group 1 (n=20) consisted of 18 male and 2 female patients with a mean age of 54.7±6 years who received autotraiisfusion. Control group (Group 2: n=20), consisted of 17 male, and 3 female patients with a mean age of 58.1±9 years. Hematocrit values were found to be lower in the autotraiisfusion group during the cardiopulmonary bypass procedure than those of the control group, and higher at the end of the operation and at the discharge from the hospital (p<0.05). Conclusion: In our study, if there were no additional comorbidities in patients who will undergo open heart surgery, blood drawn for autotraiisfusion during the perioperative period reduced the need for allogeneic blood during the postoperative period. Based on all these factors, we think that autotraiisfusion in cardiac surgery can be performed safely because of its positive effects on hemodynamics, metabolic and functional outcomes.Article A Novel Modified Opposition-Based Hunger Games Search Algorithm To Design Fractional Order Proportional-Integral Controller for Magnetic Ball Suspension System(John Wiley and Sons Inc, 2022) Izci, D.; Ekinci, S.; Eker, E.; Kayri, M.This study focuses on construction of novel enhanced metaheuristic algorithm using a modified opposition-based learning technique and the hunger games search algorithm. The proposed modified opposition-based hunger games search (mOBL-HGS) algorithm is aimed to be used as an efficient tool to tune a fractional order proportional-integral-derivative (FOPID) controller in order to control a magnetic ball suspension system with greater flexibility. The challenging benchmark functions from CEC2017 test suite are used to confirm the greater performance of the proposed mOBL-HGS algorithm. The proposed mOBL-HGS algorithm is also utilized to reach the optimum values of a FOPID controller employed in a magnetic ball suspension system in order to demonstrate its capability in terms of a complex real-world engineering problem. The latter case is confirmed through comparative evaluations of statistical analysis, convergence profile, transient response, frequency response, disturbance rejection, and robustness. The demonstrated results confirm the greater ability of the proposed approach to control a magnetic ball suspension system. © 2022 John Wiley & Sons Ltd.Article Relationship Between Total Atrial Conduction Time and Epicardial Adipose Tissue in Patients With Subclinical Hypothyroidism(Yuzuncu Yil Universitesi Tip Fakultesi, 2025) Ceylan, Y.; Eker, E.Subclinical hypothyroidism (SCH) is characterized by elevated thyroid-stimulating hormone (TSH) levels with normal free thyroxine (fT4) levels. It is associated with increased cardiovascular risk, necessitating regular monitoring of cardiovascular parameters. This study investigated the relationship between total atrial conduction time (TACT) and epicardial adipose tissue (EAT) in patients diagnosed with SCH, aiming to identify potential cardiovascular risks. An observational cross-sectional study was conducted at Van Yüzüncü Yıl University from January 2024 to January 2025. Fifty-four patients with SCH (TSH levels 5.0-10 µIU/mL, normal fT4) were compared to fifty-four healthy controls. Echocardiographic measurements, including EAT thickness and TACT, were obtained. No significant differences were found between groups regarding age, gender, and various laboratory parameters. TSH levels were significantly higher in the patient group 7.39±1.29 µIU/mL compared to control s 2.33±0.93 µIU/mL, p=0.001; EAT 6.74±0.96 mm vs. 4.73±0.41 mm, p=0.001 and TACT 122±8.70 ms vs. 87.8±5.33 ms, p=0.001 were also significantly elevated in SCH patients. A strong positive correlation was observed between EAT and TACT (r=0.895, p=0.001) as w ell as between TSH and both EAT and TACT (r=0.701, p=0.001; r=0.822, p=0.001). SCH patients exhibit significantly increased EAT and TACT levels compared to healthy controls, indicating a heightened risk for adverse cardiovascular outcomes. The strong correlation between TSH, EAT, and TACT underscores the importance of monitoring TSH levels and suggests that proactive management of SCH may mitigate cardiovascular risks. © 2025, Yuzuncu Yil Universitesi Tip Fakultesi.
