We aimed to compare the endogenous oxidative damage markers of coenzyme Q10, nicotinamide dinucleotide oxidase 4, malondialdehyde, and ischemia-modified albumin levels in patients with pneumonia caused by SARS-CoV-2 and in an healthy control group. We also aimed to compare these parameters between patients with severe and non-severe pulmonary involvement. Methods: The study included 58 adult patients with SARS-CoV-2 pneumonia and 30 healthy volunteers. CoQ10 and MDA levels were determined by high-pressure liquid chromatography. NOX4 and IMA levels were determined by ELISA assay and colorimetric method. Results: Higher levels of CoQ10, MDA, NOX4, and IMA and lower levels of COQ10H were observed inpatients with SARS-CoV-2 pneumonia than in the control group. MDA, IMA, NOX4, and CoQ10 levels were significantly higher in patients with severe pulmonary involvement than in patients with non-severe pulmonary involvement, but no significant difference was observed in CoQ10H levels. CoQ10 levels were significantly and positively correlated with both ferritin and CRP levels. Conclusion: SARS-CoV-2 pneumonia is significantly associated with increased endogenous oxidative damage. Oxidative damage seems to be associated with pulmonary involvement severity.Article Associations Between the Standardized Uptake Value of 18f-Fdg Pet/Ct and Demographic, Clinical, Pathological, Radiological Factors in Lung Cancer(E-century Publishing Corp, 2015) Sunnetcioglu, Aysel; Arisoy, Ahmet; Demir, Yusuf; Ekin, Selami; Dogan, ErkanObjectives: F-18-fluoro-2-deoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) is extensively used to diagnose and stage of lung cancer. The aim of the current study was to investigate the correlation of demographic, clinical, pathological and radiological factors with primer tumor FDG Uptake in patients with lung cancer. Materials and methods: This cross-sectional, clinical study was performed on a total of 57 lung cancer patients newly diagnosed that underwent FDG PET/CT. In addition to descriptive variables, histopathological diagnosis, tumor site and size, hemoglobin level, red cell distribution width, neutrophil to lymphocyte ratio were noted for each patient. The correlation of these variables to SUVmax values in FDG PET/CT was investigated. Results: A total of 57 patients (4 women, 53 men) with an average age of 60.8 +/- 9.4 (range: 33-89) participated in the study. Histopathological diagnoses were consistent with squamous cell carcinoma (28, 49.1%), adenocarcinoma (15, 26.3%) and small cell cancer (14, 24.6%). The SUVmax of primary tumor was positively correlated with tumor size (P<0.001). The tumor SUVmax of squamous cell carcinoma (SqCC) (17.49 +/- 8.37) was higher than that of adenocarcinoma (AC) (12.80 +/- 4.77) and small cell carcinoma (SCC) (12.40 +/- 5.80) (P=0.038). Conclusion: SUVmax value was significantly higher for squamous cell carcinoma and it SUVmax values in PET scans was found to be positively correlated with tumor size. This study suggests that, tumor size and histologic subtype had influences upon FDG uptake in lung cancer.Article Batın Cerrahi Sonrası Radyolojik Bulgu Gösteren Akciğer Komplikasyonlarının Değerlendirilmesi(2021) Ekin, Selami; Sunnetcioglu, Aysel; Mermi̇t, Buket; Yuzkat, NureddınGİRİŞ ve AMAÇ: Cerrahi girişimler ve uygulanan anestezi sonrasında meydana gelen patofizyolojik değişiklikler akciğer komplikasyonlarının gelişimini kolaylaştırmaktadır. Bu çalışmada batın cerahisi sonrası radyolojik bulgu gösteren akciğer komplikasyonlarının değerlendirilmesi amaçlandı. YÖNTEM ve GEREÇLER: Genel enestezi altında batın cerahisi yapılan 619 hastadan istenen göğüs hastalıkları konsültasyonları retrospektif olarak incelendi. Radyolojik bulgu saptanan hastalar çalışmaya alındı. BULGULAR: Postoperatif göğüs hastalıkları konsültasyonu istenen hastaların %20,7’sinde radyolojik bulgu tespit edildi. Hastaların yaş ortalaması 57.28±11.2 olarak saptandı. Değerlendirilen 128 hastanın, 81 (63.2%) üst batın, 47’ne (36.7%) alt batın cerahisi yapılmıştı. Üst batın cerahi yapılan hastaların 51’i (62.9%) 60 yaş üzerindeydi. En sık akciğer komplikasyonları plevral effüsion (32.8%), pnömoni (26.5%), atelektazi (17.9%) idi. Üst batın cerahisinde ateletazi(69.5%) ve plevral effüzyon(66%) daha fazla iken alt batın cerrahisinde pnömoni (76%) daha sıktı. TARTIŞMA ve SONUÇ: Plevral efüzyon, pnömoni ve atelektazi, batın cerahisi sonrası radyolojik bulgu veren en yaygın akciğer komplikasyonlarıydı. Özellikle üst batın cearahisi sonrası ateletazi ve plevral efüzyon, alt batın cerahisi sonrası ise pnömoni daha sık görülmesine rağmen bu komplikasyonların gelişiminde risk faktörleri arasında önemli bir fark yoktu.Article Blood Prestin Levels in Covid-19 Patients(Lippincott Williams & Wilkins, 2023) Turan, Mahfuz; Alp, Hamit Hakan; Yildiz, Hanifi; Baran, Ali Irfan; Ekin, Selami; Akin, Ramazan; Bozan, NazimBackground: Many studies have found that viral infections affect different tissues, including the inner ear. Coronavirus disease 2019 (COVID-19), a viral infection, is a significant health problem worldwide. Prestin is a motor protein with important functions both in the outer hair cells of the inner ear and in cardiac tissue. In addition, prestin is promising as an early biomarker in the detection of ototoxicity. To determine the severity of infection in COVID-19 patients and to determine whether other tissues are affected by the infection, lactate dehydrogenase (LDH), C-reactive protein (CRP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatine kinase MB (CK-MB), biochemical markers such as ferritin and D-dimer are used. This study aimed to compare prestin levels in patients with COVID-19 and healthy volunteers.Methods: In blood samples taken from 45 patients diagnosed with COVID-19 and 40 healthy volunteers, prestin levels were determined with the kit that used an enzyme-linked immunosorbent assay method and was commercially available. At the same time, LDH, CRP, ALT, AST, CK-MB, ferritin, and D-dimer levels were also detected in both patients and healthy control groups and correlations with prestin levels were examined.Results: The main result of our study is that serum prestin levels in COVID-19 patients are significantly higher than in healthy controls (p < 0.001). In addition, a statistically significant strong positive correlation was found between prestin-LDL (r = 0.537, p = 0.001), prestin-CRP (r = 0.654, p = 0.001), and prestin-D-dimer (r = 0.659, p = 0.001).Conclusion: The levels of prestin, a motor protein in inner ear outer hair cells and cardiac myocytes, were found to be higher in COVID-19 patients than in healthy volunteers. It also showed a positive correlation with CRP and D-dimer. She delivered her 7(th) child 4 weeks ago. Symptoms had started after delivery and increased during the last 3 days. PA chest roentgenogram showed diffuse and bilateral miliary nodules. Physical examination revealed temperature 39.2 degrees C, pulse 130/min, arterial tension 90/60 mmHg, respiratory rate 35/min, and lung sounds normal. Deep venous thrombosis was detected in the right leg. SaO(2) was 69% and PaO2/FiO(2) was 86 while on 6 L/min O-2 inhalation. Computed tomography of thorax showed filling defects as well as miliary nodules. The tuberculin skin reaction was positive. Bilateral tuberculosis foci were seen in the choroid of retina. Treatment consisted of antituberculous drugs, anticoagulants, fluid and nutritional support. PaO2/FiO(2) rapidly improved. Recurrent pneumothorax developed. A tube thoracostomy was inserted. This case was interesting regarding the coexistence of miliary tuberculosis, recurrent pnomothorax, and venous thromboembolism.specialization-in-medicine-thesis.listelement.badge Comparison of 25-Hydroxy Vitamin D Levels in Patients Diagnosed With Chronic Obstructive Pulmonary Disease in the Stable Period and During Acute Exacerbation(2023) Haylu, Mine; Ekin, SelamiKronik Obstrüktif Akciğer Hastalığı (KOAH), günümüzde hem gelişmiş, hem de gelişmekte olan ülkelerde önemli bir halk sağlığı sorunu, önemli bir morbidite ve mortalite nedenidir. Bu çalışmada Kronik Obstrüktif Akciğer Hastalığı tanısı alan stabil dönemdeki ve akut alevlenme sırasındaki hastalarda 25-Hidroksi Vitamin D seviyelerini karşılaştırmayı planladık. Yöntem: Çalışma için, Van Yüzüncü Yıl Üniversitesi Tıp Fakültesi Araştırma Hastanesi Göğüs Hastalıkları polikliniğine başvuran ya da servis, yoğun bakım ünitelerinde tedavi gören KOAH tanılı, çalışmaya katılmayı kabul eden olgular alınmıştır. KOAH tanılı 60 stabil dönem, 60 alevlenme dönemi ve 40 kişilik sağlıklı bir kontrol grubu oluşturularak prospektif olarak yapıldı. Tüm gönüllü hasta ve sağlıklı bireylerden 25-Hidroksi Vitamin D için 1 adet biyokimya tüpüne 5 ml kan örnekleri alındı. Alınan numuneler 3500 rpm de 2 dk santrifüj edilip serum örnekleri elde edildi ve çalışma gününe kadar -40 C0 de saklandı. 25 -Hidroksi Vitamin D seviyeleri, Van Yüzüncü Yıl Üniversitesine bağlı Bilimsel Araştırma Projesi Koordinasyon Birimi tarafından temin edilen ve enzim bağlı immuno sorbent ölçüm (ELISA) metodu ile çalışan kitlerle ölçülmüştür. Bulgular: KOAH alevlenme dönemi (n:60), KOAH stabil dönem (n:60) ve sağlıklı kontrol grubu (n:40) olmak üzere üç gruptan oluşan toplam 160 hasta çalışmaya dahil edildi. KOAH alevlenme grubundaki 25-hidroksi vitamin D seviyeleri hem sağlıklı kontrol grubundan (p=0.001) hem de KOAH stabil grubundaki 25-hidroksi vitamin D seviyelerinden yüksekti (p= 0.001). Bununla birlikte sağlıklı kontrol grubundaki 25-hidroksi vitamin D düzeyleri, KOAH stabil gruptan istatistiksel olarak anlamlı derecede farklı değildi (p= 0.338). Yaş açısından incelendiğinde KOAH alevlenme grubunun hem sağlıklı kontrol grubundan hem de KAOH stabil gruptan istatistiksel olarak anlamlı derecede yüksek olduğu saptandı. (sırası ile p=0.001 ve p=0.035). Sonuç: Elde edilen bulgular ışığında son GOLD raporuna ve diğer birçok çalışmanın aksine, stabil dönem devam tedavisi alan hastaların 25-Hidroksi D vitamin seviyesinin kontrol grubu ile istatistiksel olarak farklı olmaması ve akut atak nedeni ile başvuran KOAH hastalarından anlamlı şekilde düşük saptanması, 25-Hidroksi D vitaminin KOAH' a karşı koruyuculuğunun beklendiği gibi olmadığı ve daha önce yapılan çalışmalar ve öngürülmesi beklenen hipotezimizin aksine, atak geçirme riski ile hastaların daha sık karşılaştığı sonucunu düşündürmektedir.Article Could a New Measurement, a New Body Shape Index, Replace Bmi in Detecting Obesity and Predict the Presence of Obesity and Depression in Asthma Control(2022) Ekin, Selami; Mermi̇t, Buket; Gunbatar, Hulya; Yıldız, Hanifi; Sunnetcioglu, AyselObesity is common in asthma. Depression is thought to be one of the risk factors that increase obesity. It is known that depression has an effect on poor asthma control. Body Mass Index (BMI) is mostly used to define obesity. In recent years, however, the ‘A Body Shape Index’ (ABSI) based on waist circumference (WC) measurement has been developed and the higher ABSI corresponds to a more central body volume concentration. Our aim was to examine the effect of obesity and depression on asthma control in a way that questions the usability of ABSI, which is new in abdominal obesity measurement. A total of 99 asthmatic patients aged between 18-80 years who were followed up in Chest Diseases outpatient clinics were included in the study. Demographic and medical history of the patients were recorded. Waist circumference /BMI2/3 X Length(m) 2/3 was calculated for ABSI measurement. Beck Depression Scale (BDS) and Asthma Control Test (ACT) were applied. Pulmonary Function Test was applied to all patients by the same trained person. Obese and morbidly obese patients had poor asthma control. We found that the presence of depression caused poor asthma control in all BMI groups. The poorly controlled asthma group and the obese and morbidly obese patients had higher depression scores. BMI and waist circumference were higher in the poorly controlled asthma group (p=0.002, 0.033 respectively). However, there was no significant difference between the asthma groups in terms of ABSI (p=0.529). The findings of this study indicate that depressive symptoms, increased BMI and WC were significantly associated with poor asthma control. But ABSI is no superior to BMI in detecting asthma control and depressive mood. However, we should prompt our patients to treatment and exercise, especially for abdominal obesity. We should recommend that asthma patients with depression consult a psychiatrist. In this way, we can control our asthma patients more effectively by minimizing the existing risks.Article Delftia Acidovorans Pneumonia With Lung Cavities Formation(Corporacion Editora Medica Valle, 2019) Yildiz, Hanifi; Sunnetcioglu, Aysel; Ekin, Selami; Baran, Irfan; Ozgokce, Mesut; Asker, Selvi; Akyuz, SumeyyeCase Description: A 52-year-old female patient was admitted to our clinic with complaints of cough, sputum, fever and fatigue. The patient has been receiving immunosuppressive therapy for thrombocytopenic purpura for 5 years. Clinical Finding: Inspiratory crackles were heard on both hemithorax. Oxygen saturation measured with the pulse oximeter was 97%. Chest X-ray showed diffuse reticular opacities that were more prominent in the upper zones of both lungs. WBC counts were 17,600 mm(3) and Platelet counts were 29,000 mm3. Thorax CT showed that there were many thin-walled cavities and millimetric nodules accompanied by ground-glass infiltrates in the upper and middle lobes. Gram staining of bronchial fluid, taken by bronchoscopy, revealed Gram-negative bacilli and intense polymorphonuclear leukocytes. The bacteria were defined as Delftia acidovorans by BD Phoenix automated system. Treatment and outcomes: The patient was hospitalized with suspicion of opportunistic pulmonary infections and cavitary lung disease. After the empirical treatment of intravenous piperacillintazobactam and oral clarithromycin, her clinical and radiological findings significantly regressed, and she was discharged with outpatient follow-up. Clinical Relevance: This is the first example of cavitary pneumonia due to Delftia acidovorans in an immunocompromised patient. We would like to emphasize that Delftia pneumonia should be considered in the differential diagnosis of pulmonary cavitary involvement in such patients.Article Effect of Inhaled Steroids on Laryngeal Microflora(Taylor & Francis Ltd, 2016) Turan, Mahfuz; Ekin, Selami; Ucler, Rifki; Arisoy, Ahmet; Bayram, Yasemin; Yalinkilic, Abdulaziz; Cankaya, HakanConclusions As is known, this study is the first study to evaluate the effect of inhaled steroids on laryngeal microflora. The data support that ICS usage causes changes in the larynx microflora. Purpose The aim of this study was to determine the alteration in larynx microbial flora of the patients treated with ICS comparing the culture results of a control group. In addition, laryngeal microflora was compared to the smears obtained from the vallecula and pharynx. Materials and methods The study included 39 patients (mean age = 45.5 +/- 12.76 years) who had been using a corticosteroid inhaler and control group consisting of 27 persons (mean age = 43.07 +/- 13.23 years). Culture samples were obtained from the pharynx, larynx, and vallecula in the patient and control groups, and they were evaluated in the microbiology laboratory. Obtained culture results were named by the same microbiologist according to the basic microorganism classification method. ResultsCoagulase-negative staphylococci (CNS), Streptococcus viridians (VGS) and candida albicans were detected to grow significantly more in the patient group in all three anatomic localizations compared to the control group. Neisseria spp, basillus spp, and Non-viridans alpha-hemolytic streptococcus were detected to grow significantly more in the control group in all three anatomic localizations compared to the patient group.Article Endobronchial Aspergilloma: a Case Report(Elsevier Science Bv, 2016) Sunnetcioglu, Aysel; Ekin, Selami; Erten, Remzi; Parlak, Mehmet; Esen, RamazanA 28-year-old male with B-cell acute lymphoblastic leukemia presented to our pulmonary clinic with progressive dyspnea, cough, hemoptysis, and fever. Diagnostic bronchoscopy revealed white masses at the entrance of the right middle lobe bronchus and distal to the right main bronchus. Histopathological examination of the biopsy specimen showed those masses to be hyphae of Aspergillus. (C) 2016 The Authors. A 49-year-old woman with a history of multiple myeloma consulted to our pulmonary service with progressive dyspnoea and cough and abnormal chest X-ray. A lesion measuring 6 x 5 cm in size existed in anterior baseline of the right lung's lower lobe in thoracic computed tomography in addition to right bronchial narrowing and atelectasis distal in lesions. Diagnostic bronchoscopy was performed previously for the obstruction and biopsy was taken from the lesion in the right middle lobe bronchus. Endobronchial biopsies showed extensive tumour infiltration with plasmocytoid cells. Immunohistochemistry was positive CD138 and Lamda. Methods: The patient group was newly diagnosed as having OSA (apnea-hypopnea index [AHI]>5/h) using a polysomnographic examination. A group of subjects with simple snoring were included into the control group. Clinically relevant comorbidities were systematically assessed in face-to-face interviews. All patients were asked to complete the 15-item International Index of Erectile Function (IIEF-15) questionnaire for the evaluation of ED. The patients with OSA and ED were evaluated according to these comorbidities. Results: Of the 94 patients, 39 patients were excluded because of severe diseases. OSA was observed in 38 (69.1%) of the 55 patients. ED was seen in 24 (63.2%) patients with OSA, and in 8 (47.1%) patients without OSA (p>0.05). There were no statistical differences between the groups' ages, IIEF scores, and body mass index (BMI) scores. There were statistically significant differences between the groups' AHI scores (p<0.05). There was a significant correlation between the groups' AHI scores, BMI, and age (p<0.05). There was no statistically significant difference in patients with OSA, with and without comorbidity in terms of ED. Conclusion: The rate of ED was higher in patients with OSA who had no other comorbidities. Therefore, ED can be a sensitive marker of OSA.Article Evaluation Serum Levels of Ykl-40, Periostin, and Some Inflammatory Cytokines Together With Il-37, a New Anti-Inflammatory Cytokine, in Patients With Stable and Exacerbated Asthma(Mosby-elsevier, 2021) Yildiz, Hanifi; Alp, Hamit Hakan; Sunnetcioglu, Aysel; Ekin, Selami; Cilingir, Buket MermitIntroduction: As asthma has a wide range of inflammatory pathways, the researchers were focused on the markers that may be associated with exacerbation and stability in asthma. Objective: Our aim is to investigate the serum levels of some inflammatory markers and cytokines in stable and exacerbated asthmatic patients. Methods: The study included in 59 non-smoker asthma patient (Exacerbated=25, Stable=34) and 30 healthy volunteers. The serum level of periostin, YKL-40, IL-4, IL-5, IL-37, and TNF-alpha were detected by enzyme-linked immunosorbent assay. Results: Except for IL-37, the periostin, YKL-40, IL-4, IL-5, and TNF-alpha level in asthmatic patients were significantly higher than those of healthy control. In the exacerbated group, the periostin, YKL-40, IL-5, and TNF-alpha level were significantly higher than stable asthma and healthy control groups. The serum levels of IL-4 in exacerbated and stable asthma groups were significantly higher than healthy control group. There was a significant difference between IL4 levels, in stable asthma and healthy control groups. In exacerbated asthma group, IL-37 level was significantly lower than stable and healthy control groups. The highest area under the ROC curve (AUC) was found for IL-4. While there was a significant negative correlation between these parameters and FEV1, there was a positive correlation between IL-37 and FEV1, but not significant. Conclusions: This study showed that increased serum periostin, YKL-40, IL-5, IL-4, and TNF-alpha and decreased serum IL-37 were associated with exacerbation showing uncontrolled asthma. Primitive neuroectodermal tumors (PNET) and ESFT have a similar neural phenotype and can be considered in the same entity. Here, we will present 28 years old patient with Ewing Sarcoma. Patient was admited chest pain. Chest radiograph showed a suspicious lesion in the left paracardiac area. Computed tomography (CT) scan and positron emission tomography (PET)/CT result were compatible with malignant tumor. The patient was underwent surgical resection as the bronchoscopic result couldn't a malignant finding and pathological finding was detected as Ewing's sarcoma. Its main clinical features are loud snoring and breathing stoppage during sleep. Ischemia could be a consequence of noise-induced hearing loss because cochlear oxygen tension is reduced during and after noise exposure. In this study, we evaluated auditory function in patients affected by OSA and simple snoring. Material/Methods: A total of 66 participants (male to female ratio: 40: 26) were included in the study, of which 21 were in the control group, 18 were in the simple snoring group, and 27 were in the OSA patient group. Polysomnography and audiometric examination were performed in all participants. Results: The mean ages of the participants in the control, simple snoring, and OSA groups were 39.14 +/- 9.9, 37.28 +/- 8.2, and 41.56 +/- 8.99 years, respectively. There were no statistically significant differences among groups regarding age or sex; however, there were statistically significant differences among groups in body mass index, apnea-hypopnea index scores, mean saturation, and duration under 90% saturation. In addition, statistically significant differences were found between the patient group and the control and simple snoring groups concerning the mean saturation, duration under 90% saturation, and the extended high frequency of hearing. Conclusions: These data show that snoring may cause hearing loss at extended high frequencies.
