Article The Activities of Serum Paraoxonase and Arylesterase and Lipid Profile in Acute Myeloid Leukemia: Preliminary Results(verduci Publisher, 2015) Cebi, A.; Akgun, E.; Esen, R.; Demir, H.; Cifci, A.OBJECTIVE: To investigate the activities of serum paraoxonase-1 (PON1) and arylesterase (ARE), and the lipid profile in newly diagnosed acute myeloid leukemia (AML) patients. PATIENTS AND METHODS: Thirty-two persons (16 of AML and 16 of healthy control) were included to the study. PON1 and ARE activities were measured as spectrophotometrically in serum samples. High density lipoprotein (HDL), low density lipoprotein (LDL), total cholesterol (TC), triglyceride (TG) were analyzed in autoanalyzer. RESULTS: PON1 activities were respectively 16.04 U/L and 18.6 U/L in AML and healthy controls. There was no statistical significance between groups (p > 0.05). The mean ARE activities were respectively 0.21 U/L and 0.36 U/L in AML and healthy controls. Serum ARE activity significantly decreased in AML group (p < 0.001). Serum HDL values were significantly decreased (181.8 +/- 76.2 mg/dl; p = 0.002) in AML. There was no difference in total cholesterol, LDL and triglyceride values (respectively; 181.8 +/- 76.2 mg/dl, 120.6 +/- 64.6 mg/dl, 157.3 +/- 87.2 mg/dl; p > 0.05) between AML and controls. CONCLUSIONS: This is the first documented study about serum PON1 activity in AML patients. Although serum PON1 activities were not changed in both groups, our data suggest that the decreased serum ARE activity and HDL levels may be related the pathogenesis of AML.Article Disseminated Intravascular Coagulation: a Single Center Experience(Yuzuncu Yil Universitesi Tip Fakultesi, 2018) Ekinci, O.; Candar, O.; Dogan, A.; Esen, R.; Demir, C.We aimed to evaluate the clinical and demographic data of adult patients diagnosed with acute disseminated intravascular coagulation (DIC) who had been referred to the hematology unit by other clinics, as well as to evaluate the effect of clinical and laboratory parameters on patient survival. Materials and Methods: The records of 87 adult patients admitted to the Department of Hematology of Yuzuncu Yil University Faculty of Medicine between January 2013 and May 2017 and diagnosed with acute DIC were retrospectively examined. Of the patients participating in the study, 64 (73.6%) were female and 23 (26.4%) were male. Regarding the distribution of etiology in patients with acute DIC, the most common etiology, at 49.3%, was obstectic pathologies. Acute DIC was more frequent in women and younger adults (p = 0.001). Patients with trauma had the highest mortality rate (p = 0.001). Patients diagnosed with acute DIC who died as a result were older, and mortality was higher in males (p < 0.001 for both). Comparing ISTH scores, patients with acute DIC had significantly higher mortality (p = 0.028). In this study, a majority of the patients diagnosed with acute DIC were young and female. De spite their high incidence of acute DIC however, the mortality rate for young female patients was low. The mortality rate was higher in elderly and male patients, although intensive treatment was administered following diagnosis of acute DIC. We believe that early diagnosis and appropriate treatment are of critical importance for patients thought to have acute DIC, regardless of underlying condition. © 2018, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Evaluation of Serum Levels of Trace Elements in Myeloproliferative Neoplasms: a Case-Control Study(Yuzuncu Yil Universitesi Tip Fakultesi, 2021) Yetişgin, F.; Bilici, M.; Esen, R.The lack or excess of some of trace elements or heavy metals could be associated with the r isk of hematologic malignancy. This study aimed to compare the patients with myeloproliferative neoplasm with the serum concentrations of Zinc (Zn), copper (Cu), manganese (Mn), nickel (Ni), lead (Pb), iron (Fe), cadmium (Cd) and cobalt (Co) in healthy ind ividuals. Healthy individuals in the control group and newly diagnosed patients with myeloproliferative neoplasm were involved in the research. The patient and control groups were similar in terms of socioeconomic status and eating habits. The serum levels of the trace elements were determined via Atomic Absorption Spectrophotometry. The myeloproliferative neoplasm group consisted of 60 patients while the control group consisted of 20 volunteers. In the patient group, serum Zn and Fe concentrations were significantly lower than the control group (p=0.001 and p=0.001). Serum Cu, Pb, Cd, Co, Ni and Mn concentrations were higher in the patient group (p=0.001, p=0.001, p=0.001, p=0.001, p=0.001, p=0.027, respectively). When a comparison was made between Mn serum level and the control group, Mn serum level was determined to be low in PMF (p>5%). Serum Cd and Ni levels were higher in PMF, ET, CML and PV (p=0.005). Pb serum level was higher in ET, CML and PV (p=0.001, p=0.001, p=0.001, respectively). Serum Fe and Zn levels were lower in the PMF, ET, CML and PV groups (p=0.001 and p >0.5, respectively, for Fe). Serum Zn and Fe levels were lower and Cu, Pb, Cd, Co, Ni and Mn levels were higher in patients with myeloproliferative neoplasm. In particular, Zn is a very strong antioxidant element, and the fact that Zn is low in patients suggests that it could be an important marker in PV, ET, CML and PMF cases. © 2021, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Multiple Primary Synchronous Gastric, Esophageal, and Rectal Cancer and Isolated Esophageal Metastasis From Rectal Cancer: Case Report(Turkiye Klinikleri, 2021) Sönmez, G.M.; Aytekin, A.; Erten, R.; Aldemir, M.N.; Sakin, A.; Esen, R.Synchronous tumors are defined as two or more neoplasms either identified simultaneously or within 6 months after the initial diagnosis in the same patient. Herein, we report a 63-year-old man with synchronous gastric, rectal carcinoma, and in situ esophageal carcinoma who also had isolated esophageal metastasis from rectal carcinoma. The patient man was diagnosed with rectal carcinoma. 18F-fluorodeoxyglucose (FDG) positron emission tomography-computed tomography was performed for staging and demonstrated increased 18F-FDG uptake in the rectum lesion as well as in the gastric cardia and corpus. Esophagogastroduodenoscopy was performed. After performing multiple biopsies, histopathological examination of the esophageal specimen demonstrated metastatic adenocarcinoma, in situ esophageal squamous cell carcinoma, and gastric adenocarcinoma. Isolated esophageal metastasis from rectal cancer is a highly rare entity. Gastric and esophageal cancers can be observed simultaneously with colorectal cancers; therefore, we recommend performing routine esophagogastroduodenoscopy for patients diagnosed with colorectal cancer. © 2021 by Turkish Society of Medical Oncology.Article A Patient With Acute Myeloid Leukemia Who Presented With Peritonitis While on Continuous Ambulatory Peritoneal Dialysis(2012) Soyoral, Y.U.; Begenik, H.; Esen, R.; Dilek, I.; Dulger, A.C.; Demirtas, L.; Erkoc, R.Peritonitis is a common complication in patients on continuous ambulatory peritoneal dialysis. In some cases peritoneal fluid is unremarkable. In these instances, one must evaluate for other causes of gastrointestinal pathology or a myriad of other causes. We report an unusual case of acute myeloid leukemia who presented with peritonitis in a continuous ambulatory peritoneal dialysis patient.Article Prevalence of Helicobacter Pylori in Patients With Brucellosis(BMJ Publishing Group, 2012) Esen, R.; Dulger, A.C.; Begenik, H.; Demirtas, L.; Ebinc, S.; Aytemiz, E.; Aslan, M.Background: Helicobacter pylori infection affects approximately 40 million individuals in Turkey. Our prefecture is an endemic area for H. pylori infection and brucellosis. However, there are no data on the H. pylori prevalence among patients with brucellosis. Objectives: The primary aim of the current study was to investigate the link between H. pylori infection and brucellosis. The secondary aim was to determine the seroprevalence of H. pylori infection in healthy individuals. Methods: Between May 2010 and May 2011, serum was collected from patients with Brucella infection (n = 111) and healthy individuals (n = 120) and analyzed using an enzyme-linked immunosorbent assay kit. Brucella infection was defined as a positive Wright test or blood culture positivity for Brucella in conjunction with disease symptoms and findings. Results: Among the 111 patients with brucellosis, 69% (n = 77) had a seropositivity result for H. pylori IgG. The healthy control subjects had an 87% (n = 104) seropositivity for H. pylori IgG. The serum H. pylori IgG antibody seropositivity was significantly lower in the patients with Brucella infection compared with the control subjects (P G 0.001). Conclusion: Our results show that the serum prevalence of H. pylori among the patients infected with brucellosis is significantly lower than that in the control population. A negative correlation between H. pylori infection and brucellosis infection might be hypothesized. Copyright © 2012 by The American Federation for Medical Research.Article Seropositivity of Toxoplasma Gondii Among Blood Donors and Patients With Hematologic Malignity(Yuzuncu Yil Universitesi Tip Fakultesi, 2023) Yıldız, S.; Esen, R.; Karaman, K.; Beyhan, Y.E.Toxoplasma gondii (T. gondii) is an opportunistic parasite. Transmission from T. gondii-infected donors to recipients is possible through blood and organ transplantation. In this study, it was aimed to investigate the presen ce of T. gondii antibodies in healthy donors and in blood recipients with immunosuppressed hematological malignancies. A total of 92 patients included in the present study (46 donors and 46 patients). Sixteen of the hematological patients had lymphoblastic leukemia and 14 had acute myeloblastic leukemia. The presence of IgG and IgM Toxoplasma antibodies and IgG avidity values were investigated by ELISA. Twenty-two (47.8%) of patients and 11 (23.9%) of the donors were found to be positive for T.gondii IgG antibodies. IgG positivity was detected in all patient groups except those with aplastic anemia. The highest IgG positivity was found in acute myeloblastic leukemia (8) and lymphoblastic leukemia (5). IgM seropositivity was detected totally three cases (3.3%), two of 46 patients (4.3%) and one 46 of donors (2.2%). Although the number was low, all patients with chronic myeloid leukemia were detected both IgG and IgM positive (100%). Low avidity was found in all three of the IgM positive cases. Considering that toxoplasmosis is an infection that causes severe clinical symptoms and death, routine diagnostic methods that detect the presence of parasites in donor and patient groups will be beneficial for the prevention of T.gondii transmission. © 2023, Yuzuncu Yil Universitesi Tip Fakultesi.
