Article Antioxidant Status and Oxidative Stress in Primary Open Angle Glaucoma and Pseudoexfoliative Glaucoma(Taylor & Francis inc, 2011) Erdurmus, Mesut; Yagci, Ramazan; Atis, Omer; Karadag, Remzi; Akbas, Ali; Hepsen, Ibrahim F.Purpose: The aim of this study was to establish the antioxidant status and oxidative stress in patients with primary open-angle glaucoma (POAG) and pseudoexfoliative glaucoma (PEG). Methods: Serum levels of total antioxidant capacity (TAC) and superoxide dismutase (SOD) as indicators of antioxidant status; and total oxidant status (TOS), nitric oxide (NO), protein carbonyl (PC), and malondialdehyde (MDA) as indicators of oxidative stress were measured from the blood samples of patients with POAG (n = 23), PEG (n = 24) and healthy control subjects (n = 19) by spectrophotometry. Results: Mean TAC level was 0.6 +/- 0.1 mmol/L in the POAG group; 0.5 +/- 0.1 mmol/L in the PEG group and 1.2 +/- 0.3 mmol/L in the control group (p = 0.001). Mean SOD level was 13 +/- 0.5 mg/L in the POAG group, 11.6 +/- 0.2 mg/L in the PEG group and 9.4 +/- 0.6 mg/L in the control group (p = 0.001). Mean TOS level was 19.6 +/- 2.6 mu mol/L in the POAG group, 21.2 +/- 4.2 mu mol/L in the PEG group and 15.1 +/- 7 mu mol/L in the control group (p = 0.001). Mean NO level was 74.3 +/- 14.4 mu mol/L in the POAG group, 66.1 +/- 8.1 mu mol/L in the PEG group and 62.3 +/- 13.5 mu mol/L in the control group (p = 0.005). Mean PC level was 641.5 +/- 102.5 nmol/mg in the POAG group, 988.3 +/- 214.7 nmol/mg in the PEG group and 654.4 +/- 150.7 nmol/mg in the control group (p = 0.001). Mean MDA level was 1.9 +/- 0.2 mu mol/L in the POAG group, 1.7 +/- 0.4 mu mol/L in the PEG group and 1.1 +/- 0.2 mu mol/L in the control group (p = 0.001). Conclusion: The findings of the present study are potentially of significance and add to the growing body of evidence for oxidative stress in POAG and PEG. Decreased antioxidant defense and increased oxidative stress system may play an important role in the pathogenesis of POAG and PEG.Article Bilateral Lower Eyelid Basosquamous Cell Carcinoma: a Rare Case(Pakistan Medical Assoc, 2014) Esmer, Oktay; Karadag, Remzi; Bayramlar, Huseyin; Soylu, Erkan; Kosem, MustafaBasosquamous cell carcinoma is a subtype of basal cell cancer. It is known to be more aggressive than basal cell cancer. A 70-year-old male patient was admitted to our clinic for evaluation of cosmetic problems caused by masses on both lower eyelids for at least two years. The mass excision from and reconstruction of lower eyelids were performed. Histopathological examination of the resected masses was consistent with basosquamous cell carcinoma. We present a case of basosquamous cell carcinoma involving bilateral lower eyelids with a different clinical appearance. This report describes a girl presenting with a Spider bite over her right upper eyelid.Article Effect of Brow Lifting Using Botulinum a Toxin on Upper Eyelid Height in Patients With Ptosis Undergoing the Frontal Sling Technique(Lippincott Williams & Wilkins, 2013) Isik, Daghan; Tekin, Hakan; Karadag, Remzi; Bilgili, Serap Gunes; Atik, BekirIn this study, brow lifting with botulinum A toxin was performed on patients whose ptosis was corrected using the frontal sling technique, and the effects of this application on ptosis were investigated. Seven patients ( with 12 eyelids) on whom the frontal sling procedure was performed using a tensor fascia lata graft were enrolled in the study. The patients underwent brow lifting using botulinum A toxin. Digital photographs of the patients were obtained before and 21 days after botulinum A injection. In digital imaging analysis, although a statistically significant elevation was detected in the eyelids and brows of the patients following botulinum A injection, it did not to lead to a significant difference in the degree of lagophthalmos. In the current study, this procedure was found to reduce the degree of ptosis without increasing the degree of lagophthalmos in patients who had previ-ously undergone ptosis correction using the frontal sling.Article Effect of Dialysis Type on Orbital Vascular Flow in Patients With End-Stage Renal Disease(informa Healthcare, 2012) Yakut, Zeynep Ilerisoy; Karadag, Remzi; Akcay, Ali; Bavbek, Nuket; Akay, Hatice; Koktener, AsliPurpose: To evaluate ocular blood flow dynamics by color Doppler ultrasonography (CDU) in patients with end-stage renal disease (ESRD). Additionally, to investigate the effect of dialysis type on ocular blood flow by comparing the findings of peritoneal dialysis (PD) subjects, hemodialysis (HD) subjects, and healthy controls. Material and methods: Forty patients (21 HD and 19 PD) and 40 controls were included in the study. CDU and spectral analysis of temporal posterior ciliary artery (TPCA) and central retinal artery (CRA) were performed to evaluate peak systolic flow velocity (PSV), end diastolic flow velocity (EDV), and resistive indices (RIs). Ocular blood flows were examined before and after HD. Post-HD findings were compared with those in PD subjects and healthy controls. Results: PSV and EDV values in CRA and TPCA after HD sessions were found to be significantly decreased when compared with pre-dialysis values. There was no statistically significant difference between the pre-dialysis and post-dialysis RI values of both arteries. Systolic and diastolic blood flows in CRA and TPCA were higher and RI values were lower in PD subjects than in HD and controls. No significant difference was seen between HD subjects and controls. Conclusion: After a single HD session, ocular blood flows of patients with ESRD were normalized. But PD subjects show higher systolic and diastolic ocular blood flows than healthy controls, suggesting that HD is more effective than PD for achieving normal ocular blood flow.Article Effects of Different Doses of Systemic Isotretinoin on Eyes: a Histopathological and Immunohistochemical Study in Rats(Lippincott Williams & Wilkins, 2020) Karadag, Remzi; Karadag, Ayse Serap; Ozlu, Emin; Oguztuzun, Serpil; Simsek, Gulcin Guler; Esmer, Oktay; Bilgili, Serap GunesPurpose: To evaluate ocular side effects associated with systemic isotretinoin histopathologically. Methods: In this multicenter study, a total of 15 male and 15 female rats were randomly divided into 3 equal groups according to the oral dose of isotretinoin they were administered: 0 mg/kg/d (group A), 7.5 mg/kg/d (group B), and 15 mg/kg/d (group C). Biopsy specimens were taken from the globe conjunctiva, cornea, and eyelid conjunctiva. Expression levels of human beta-defensin-1, human beta-defensin-2, toll-like receptor (TLR)-2, and TLR-4 were evaluated by immunohistochemical methods. Results: The number of goblet cells in eyelid conjunctiva was significantly lower in group B than that in group A and group C (P = 0.002). The sizes of meibomian gland acini were significantly smaller in group B and group C than those in group A (P < 0.001). Fibrosis of eyelid conjunctiva was significantly higher in group C and group B than that in group A (P = 0.002). The levels of staining of TLR-4 in the cornea with group B were significantly lower compared with group C (P = 0.035). Conclusions: Our study suggests that isotretinoin in the early period affects eyelid conjunctiva and meibomian glands without affecting the globe conjunctiva and cornea. Occurrence of the initial symptoms of isotretinoin on the eyelids, especially on the meibomian glands, suggests that the symptoms of patients occur because of evaporative dry eye.Article Evaluation of Ocular Findings in Patients With Vitiligo(Wiley, 2016) Karadag, Remzi; Esmer, Oktay; Karadag, Ayse S.; Bilgili, Serap Gunes; Cakici, Ozgur; Demircan, Yuhanize Tas; Bayramlar, HuseyinIn this study, we aimed to investigate ocular manifestations in patients with vitiligo. Sixty-one patients with vitiligo were included in the study. From the patients who referred for examination to the dermatology and ophthalmology clinic, 57 patients without any systemic disease were taken as the control group. In both groups, otorefractometry, keratometry, visual acuity test, intraocular pressure measurement, anterior segment, and fundus examinations of the eye with slit lamp, Schirmer test, and perimetry were carried out. The mean age was 24.54 +/- 11.90years and 23.03 +/- 8.72years in the patients and control group, respectively. The mean Schirmer test results were as follows: 16.74 +/- 9.11mm and 17.64 +/- 9.41mm for the right and left eyes of the patients, and 21.96 +/- 12.51mm and 23.42 +/- 12.51mm for the right and left eyes of controls, respectively. Of the patients, 36 eyes showed lenticular findings. However, only 12 eyes of the controls have some lenticular findings. Twenty-nine eyes in the vitiligo group and four in the controls showed some fundus findings. When the two groups were compared with each other, there was a statistically significant difference between them in terms of Schirmer test results, lens, and fundus findings (P<0.05 for all). However, there was no significant difference in terms of age, gender, visual acuity, refraction, keratometry, intraocular pressure, perimetry, and corneal findings (P>0.05 for all). Patients with vitiligo may have more lenticular and retinal findings than normal. It is self-limited in immunocompetent hosts. It can present as numerous, treatment-resistant, atypical lesions in immunosuppressive patients and resistant to treatment. Atypical forms can rarely be seen in immunocompetent patients. A case of a two-year-old child with umbilicated papules on the eyelid is presented here. Laboratory investigations were unremarkable for systemic diseases, and surgical excision was successfully performed.Article Nasolacrimal System Obstruction, Ptosis and Esotropia Due To Chemotherapy in Acute Lymphoblastic Leukemia(informa Healthcare, 2013) Esmer, Oktay; Karadag, Remzi; Soylu, Erkan; Oner, Ahmet Faik; Burakgazi-Dalkilic, EvrenEight-year-old girl was admitted to our clinic with the complaint of constant epiphora in the right eye. It was reported that this complaint began after the start of chemotherapy with a diagnosis of Acute Lymphoblastic Leukemia (ALL) about 5 years ago. In addition, eyelid ptosis associated with esotropia also occurred during that period. We found that nasolacrimal duct obstruction did not improve with medical treatment but ptosis and esotropia improved with pyridoxine and pyridostigmine treatment during that period. Examination of the eye on admission revealed nasolacrimal duct obstruction on the right side. Other ocular findings were normal. Nasolacrimal system obstruction, ptosis and esotropia combination has not been reported previously in patients using systemic drugs or receiving chemotherapy due to ALL.Article Ocular Findings in Patients With Alopecia Areata(Wiley, 2016) Esmer, Oktay; Karadag, Remzi; Cakici, Ozgur; Bilgili, Serap Gunes; Demircan, Yuhanize Tas; Bayramlar, Huseyin; Karadag, Ayse S.Objectives This study investigated ocular findings in patients with alopecia. Methods A total of 42 patients with alopecia (31 male, 11 female; 84 eyes) and 45 healthy individuals (28 male, 17 female; 90 eyes) were enrolled in the study. Of the patients with alopecia, 34 had alopecia areata, seven had alopecia universalis, and one had ophiasis alopecia. Seven patients had eyebrow involvement and seven had eyelash involvement. Autorefractometry, keratometry, visual acuity, central corneal thickness and intraocular pressure (IOP) measurements, bilateral anterior and posterior segment examinations, Schirmer's tests, and visual field examinations were performed in both groups. Results The mean +/- standard deviation age of the subjects was 25.21 +/- 10.88 years in the alopecia group and 28.24 +/- 9.31 years in the control group. Lens abnormalities were observed in 35 eyes in the alopecia group and in 11 eyes in the control group (P < 0.05). Posterior segment abnormalities were seen in 29 eyes in the alopecia group and four eyes in the control group (P < 0.05). There were no statistically significant differences in age, sex, visual acuity, refractive error, keratometric findings, IOP, central corneal thickness, perimetry, or Schirmer's test results between the alopecia and control groups (P > 0.05). Conclusions Patients with alopecia may have more lenticular and retinal findings than normal individuals, but those findings do not interfere with visual acuity. Palpebral anthrax is a rare form of cutaneous anthrax. Untreated cutaneous anthrax can be lethal. Patients with palpebral anthrax can develop complications including cicatrisation and ectropion. Thus, anthrax should be considered in differential diagnosis for patients presenting with preseptal cellulitis in high-risk regions.
