Article A Case of Brucellosis Lymphadenitis Mimicking Scrofuloderma(TIP ARASTIRMALARI DERNEGI, 2007) Karsen, H.; Karahocagil, M.K.; Akdeniz, H.; Sünnetçioǧlu, M.; Evirgen, Ö.Brucellosis is a zoonotic disease, which may be manifested by multisystemic involvement. While the complications related to musculoskeletal and gastrointestinal system are frequently seen, those related to the other systems are rarely encountered. In this paper, we present a brucellosis lymphadenitis case in the right side of the neck in a 20 years old female patient, presenting with a mass formation in the soft tissue mimicking scrofuloderma, with dimensions of 10x6x4 cm. Especially in endemic areas, brucellosis lymphadenitis should also be remembered in addition to scrofuloderma.Article Case Report: Two Plasmodium Vivax Malaria Cases in the Van Province(2009) Karahocagil, M.K.; Baran, A.I.; Yaman, G.; Ciçek, M.; Bilici, A.; Binici, I.; Akdeniz, H.Malaria is an important parasitic infection which is endemic in the Eastern Mediterranean and Southeastern Anatolia regions and sporadic in other regions of Turkey, while Plasmodium vivax is the most common cause. Two patients who were admitted to our hospital in October with complaints of high fever, chills, nausea-vomiting, generalized body pain and fatigue and diagnosed as P. Vivax malaria are presented, because they were two brothers with no history of travel outside of Van city.Article Complete Recovery From Clinical Rabies: Case Report(Turkiye Klinikleri, 2013) Karahocagil, M.K.; Akdeniz, H.; Aylan, O.; Sünnetçioǧlu, M.; Ün, H.; Yapici, K.; Baran, A.I.A 17-year-old male had been bitten by his dog. He was given one dose of rabies vaccine after being bitten. Twenty days after vaccination, he developed complaints of itching and paresthesia at the bite area on his forearm. He was transferred to our department with the preliminary diagnosis of rabies due to his complaints of fever, abdominal pain, dysphagia and difficulty of breathing after 3-5 days. Rabies viral nucleic acid was positive by reverse transcriptase polymeraze chain reaction in the cerebrospinal fluid and saliva samples, and corneal smear was found positive with fluorescence antibody method. Patient's complaints markedly subsided after hospitalization and he was discharged on 66th day with complete recovery. In the literature, complete recovery in patients with clinical rabies is limited to a few cases. In this paper, we present the first case recovering from human clinical rabies, whose diagnosis was confirmed by direct diagnostic methods. © 2013 by Türkiye Klinikleri.Article Evaluation of Peritonitis Attacks in Patients Applying Peritoneal Dialysis(Yuzuncu Yil Universitesi Tip Fakultesi, 2023) Baran, A.I.; Karahocagil, M.K.; Soyoral, Y.U.; Mülayim, G.Peritoneal dialysis is one of the renal replacement therapies used to treat patients with end-stage renal disease. Peritonitis is a common complication of peritoneal dialysis. Although the incidence of peritonitis has decreased, it is still a problem and the most important determinant of hospitalization, mortality, and morbidity. The aim of this study was to evaluate the incidence of peritonitis in peritoneal dialysis patients, compare culture methods, and determine the causative microorganisms and antibiotic susceptibility. Patients who were on the peritoneal dialysis treatment program and developed peritonitis were included in the study. Demographic, clinical, and laboratory data of the cases were recorded. The growth rates of pathogens in the peritoneal fluid, causative microorganisms, and antibiotic susceptibility results were evaluated in the peritonitis cases. During the study, 47 episodes of peritonitis occurred in 28 patients. The mean incidence of peritonitis was 0.57 attacks/patient-year. Growth rates in the blood culture system and solid media were 51% and 46.1%, respectively, and there was no significant difference between them. According to the culture results, 75% were Gram-positive microorganism and the most common pathogens were coagulase-negative staphylococci. In our study, no significant difference was found between inoculation of peritoneal fluid into blood culture bottles and solid media. The most common pathogens were coagulase-negative staphylococci. As methicillin-resistant staphylococci are common, treatment with vancomycin seems appropriate. It was thought that ceftazidime, which is used for gram-negative bacteria, may not be sufficient due to resistance, and studies with more gram-negative cases are needed to evaluate this. © 2023, Yuzuncu Yil Universitesi Tip Fakultesi. Two cases were left with advanced motor mental sequel. In this paper, clinical and laboratory findings along with diagnosis and treatment of Herpes simplex virus encephalitis were discussed.Article Salmonella Ovarian Abscess in a Patient With Systemic Lupus Erythematosus Presented as Acute Abdomen: a Case Report and Review of the Literature(Yuzuncu Yil Universitesi Tip Fakultesi, 2020) Baran, A.I.; Sunnetcioglu, M.; Yakan, U.; Batur, A.; Karagoz, E.; Karahocagil, M.K.A Salmonella ovarian abscess is a rare entity, and is similarly uncommon in cases with Systemic Lupus Erythematosus. To the best of our knowledge, thirty four cases of Salmonella ovarian abscess have been published in the literature, and only four of them were related to Systemic Lupus Eritematosus. All rights reserved.Article Thrombocytopenia Due To the Use of Rifampicin in Brucellosis(2012) Demir, C.; Sayin, R.; Soyoral, Y.; Karahocagil, M.K.Thrombocytopenia may develop after drug therapy and is a life-threatening condition. Thrombocytopenia may also occur after high-dose use of rifampicin which is an antibiotic. Here, we report five cases of thrombocytopenia developing after the use of rifampicin for the treatment of brucellosis. Four cases had experienced diffuse petechiae and ecchymosis with severe thrombocytopenia. The other patient had a complicated thrombocytopenia with intracerebral hemorrhage. As a result, the usage of rifampicin in the treatment of brucellosis could lead to systemic or central complications.Article The Value of Diffusion-Weighted Imaging in the Diagnosis of Active Sacroiliitis(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Akdeniz, H.; Avcu, S.; Ünal, Ö.; Bora, A.; Karahocagil, M.K.The aim of this study was to evaluate the effectiveness of diffusion-weighted imaging (DWI) in the diagnosis of active sacroiliitis. In this prospective study, 66 patients with clinical prediagnosis of active sacroiliitis were evaluated. Four patients who were not suitable for MRI examination were excluded. Twenty subjects with no complaint of low back pain constituted the control group. All of the patients underwent sacroiliac MRI examination performed with a 1.5 Tesla unit using phase array body coil. Before DWI, oblique axial and coronal T1 and T2-weighted TSE followed by STIR sequences were obtained. DWI examinations were obtained on SS-SE EPI sequence through chemical shifting selective fat suppression technique. ‘b’ value was chosen as 50, 400 and 800 mm2/sec and total scanning time was 114 seconds. ADC calculations were made from the ADC maps by placing ROI on the active inflammatory regions in case group and on sacrum and iliac bones in control group. Of the 62 cases, 42 had a radiologic diagnosis of active sacroiliitis, and 20 were regarded as normal. Mean ADC values in 42 patients with active sacroiliitis were significantly higher than control group. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy rate of DWI in the diagnosis of active sacroiliitis were found to be 100% for all. The cut-off ADC value was 0.94 x 10-3 mm2/sec for right sacroiliitis and 0.89 x 10-3 mm2/sec for left sacroiliitis. We believe that DWI and ADC quantification can be used successfully for the early diagnosis and follow-up of active sacroiliitis. © 2019, Yuzuncu Yil Universitesi Tip Fakultesi. All Rights Reserved.
