Article Evaluation of the Fractal Dimension in the Apical Region of Primary Teeth With Unilateral and Bilateral Infraocclusion(Cumhuriyet University Faculty of Dentistry, 2024) Kaya, S.; Avcı, B.Objective: This study aims to employ fractal analysis (FA) to determine potential variations in the mandibular trabecular bone structure in the apical region among individuals with unilateral and bilateral infraocclusion of primary teeth compared to a control group. Materials and Methods: Forty-three primary teeth identified from panoramic radiographs taken for diagnostic purposes between January 2018 and December 2023 in the Department of Oral, Dental, and Maxillofacial Radiology of our institution were included in this investigation. Categorical variables were analyzed using the chi-square test, while continuous variables were assessed through Student paired t-tests and ANOVA tests. The threshold for statistical significance was set at p<0.05. Results: The analysis revealed that the mean fractal dimension (FD) did not significantly differ between unilateral and bilateral infraocclusion groups based on gender (p>0.05). Although the FDs in the apical regions of unilaterally infraoccluded teeth and their contralateral counterparts were observed to be lower, the disparity based on occlusion position did not reach statistical significance (p 0.11). Moreover, no significant distinctions were observed in the FD or resorption levels between teeth exhibiting bilateral infraocclusion on the right and left sides (p: 0.877, p:0.938). It was noted that the presence of an opposing tooth positively influenced the FD; however, this effect did not achieve statistical significance. Conclusions: While infraocclusion is prevalent in mandibular primary molars, this study did not observe any discernible alterations in the mandibular trabecular bone structure attributable to infraocclusion. Unilateral infraocclusion, in particular, was found to be a common and relatively benign condition. Additionally, numerical anomalies such as hypodontia and supernumerary teeth might exhibit associations with infraocclusion. © (2024), (Cumhuriyet University Faculty of Dentistry). All rights reserved.Article Geological and Geophysical Observations on the Tectonic Features of Western Part of the Afyon-Akşehir Graben: a Contribution To the Arguments on the Two-Stage Extension Model(Hacettepe Universitesi Yerbilmleri, 2014) Kaya, S.; Esat, K.; Ecevitoǧlu, B.; Işik, V.; Kaypak, B.; Aldaş, G.U.; Seyitoǧlu, G.The NW-SE trending tectonic contact between the metamorphic basement and the Neogene sedimentary rocks, located at the western side of the Afyon-Akşehir Graben, is interpreted as a reverse fault and is named as the Sultandaǧi{dotless} fault by some researchers. On the contrary, other workers claim that the Sultandaǧi{dotless} fault is a normal fault. Some researchers define the Yakasenek reverse fault which is cut by the Sultandaǧi{dotless} normal fault. In this study, the geological and geophysical studies have been performed on this tectonic contact. The tectonic contact that corresponds to the Yakasenek reverse fault was determined as a NE-dipping plane using with the 'Seismic Beam Steering' method. The identification of the NE-dipping plane instead of SW-dipping shows that the Yakasenek reverse fault, which is interpreted as a product of the Miocene-Pliocene contraction supporting two-stage graben model, does not exist. There is also no field evidence of reverse faulting in the area.Article Is a Single Dose of Antibiotic Prophylaxis Sufficient To Prevent Infections in Total Joint Arthroplasty(Yuzuncu Yil Universitesi Tip Fakultesi, 2024) Kaya, S.; Güven, N.; Ünal, Y.C.; Dündar, A.Introduction: Prosthetic infection is a serious complication that can develop after knee and hip arthroplasty and remains a common reason for revision surgery. Guidelines recommend various measures to prevent infection, howewer some professional associat ions argue that there is insufficient evidence for single-dose antibiotic prophylaxis. Our study compares the outcomes of patients receiving short-and long-term antibiotic prophylaxis in arthroplasty surgery. Materials and Methods: In this retrospective study of 424 patients undergoing knee or hip arthroplasty, two prophylaxis protocols were compared. Group 1 (190 patients, 44.8%) received cefazolin pre-and postoperatively on day 1, while group 2 (234 patients, 55.2%) received extended cefazolin (5 days postoperatively) and oral amoxicillin clavulanic acid (5 days). Early postoperative infection rates wer e evaluated. Results: In this study, 83.4% of 424 patients who underwent total knee and hip arthroplasty were female. Knee and hip operations were performed in 86.8% and 13.2% of the patients, respectively. The mean age did not show a statistically significant difference. There was no significant difference between the groups in terms of periprosthetic infection rates (p=0.828). Posto perative wound complications were seen in 34 (18%) patients in group 1 and 44 (19%) patients in group 2, but this difference was not statistically significant (p=0.704). No significant difference in wound complications and prosthesis infection rates was fo und between group 1 and group 2 patients with risk factors (p>0.05). Conclusion: In primary joint arthroplasty, extended oral antibiotic prophylaxis did not provide additional protection against single-day antibiotic prophylaxis. The results were similar in both risk and non-risk groups. Considering antibiotic resistance, side effects and costs, it is concluded that extended prophylaxis is unnecessary. All rights reserved.Article Is There an Increased Risk of Complications in Patients Diagnosed With Covid-19 Six Months Before Total Knee Arthroplasty(Verduci Publisher, 2023) Kaya, S.; Dundar, A.- OBJECTIVE: We aimed to compare the risk of developing complications between patients who had long-term COVID-19 and those who did not. PATIENTS AND METHODS: This is a retrospective study involving a total of 265 patients who had undergone total knee arthroplasty surgery. A cohort of patients (Group 1) who had been diagnosed with COVID-19 six months prior to total knee arthroplasty was established and matched to a cohort of patients who had never been diagnosed with COVID-19 (Group 2). Demographic information such as age and gender, underlying diseases, postoperative complications (surgical site infection, wound problem, periprosthetic fracture, deep vein thrombosis, pulmonary thromboembolism, hematoma, pneumonia, urinary tract infection) and exitus were recorded. RESULTS: Of the 265 patients, 217 (81.9%) were females and 48 (18.1%) were males. 41 of these patients (15.5%) previously had COVID-19 (Group 1). The number of people who did not have COVID-19 was 224 (84.5%) (Group 2). There was no difference between group 1 and group 2 in terms of complications. CONCLUSIONS: This study found that the rate of complications, including deep vein thrombosis, in patients who had COVID-19 six months before total knee arthroplasty was not higher than those who did not.Article Liver Involvement in Patients With Brucellosis: Results of the Marmara Study(Springer, 2014) Ozturk-Engin, D.; Erdem, H.; Gencer, S.; Kaya, S.; Baran, A. I.; Batirel, A.; Ormeci, N.Brucellosis is a zoonotic disease that primarily affects the reticuloendothelial system. But, the extent of liver damage in due course of the disease is unclear. This study included 325 brucellosis patients with significant hepatobiliary involvement identified with microbiological analyses from 30 centers between 2000 and 2013. The patients with a parts per thousand yen5 times of the upper limit of normal for aminotransferases, total bilirubin level a parts per thousand yen2 mg/dl or local liver lesions were enrolled. Clinical hepatitis was detected in 284 patients (87.3 %) and cholestasis was detected in 215 (66.1 %) patients. Fatigue (91 %), fever (86 %), sweating (83 %), arthralgia (79 %), and lack of appetite (79 %) were the major symptoms. Laboratory tests showed anemia in 169 (52 %), thrombocytopenia in 117 (36 %), leukopenia in 81 (25 %), pancytopenia in 42 (13 %), and leukocytosis in 20 (6 %) patients. The most commonly used antibiotic combinations were doxycycline plus an aminoglycoside (n = 73), doxycycline plus rifampicin (n = 71), doxycycline plus rifampicin and an aminoglycoside (n = 27). The duration of ALT normalization differed significantly in three treatment groups (p < 0.001). The use of doxycycline and an aminoglycoside in clinical hepatitis showed better results compared to doxycycline and rifampicin or rifampicin, aminoglycoside, doxycycline regimens (p < 0.05). However, the length of hospital stay did not differ significantly between these three combinations (p > 0.05). During the follow-up, treatment failure occurred in four patients (1 %) and relapse was seen in three patients (0.9 %). Mortality was not observed. Hepatobiliary involvement in brucellosis has a benign course with suitable antibiotics and the use of doxycycline and an aminoglycoside regimen seems a better strategy in select patients.Article Our Medium-Term Functional and Radiological Outcomes in Revision Total Knee Arthroplasty(Yuzuncu Yil Universitesi Tip Fakultesi, 2018) Ozdemir, H.; Kaya, S.; Adanas, C.; Tabak, A.Y.To investigate the medium-term outcomes of the patients who underwent revision total knee arthroplasty, and to come to a conclusion regarding the efficacy of the method by comparing these with literature data. 34 knees of 33 patients who underwent revision knee arthroplasty operation between January 2008 and January 2012 and were sufficiently followed-up were included into this study. Single-stage revision knee arthroplasty was performed for 25 knees for aseptic reasons, and two-stage revision knee arthroplasty was performed for 9 knees for septic reasons. For all patients, alpha, beta, gamma, sigma and total valgus angles were measured in pre-and post-operative anteroposterior (AP) X-rays and lateral X-rays based on American Knee Society radiological assessment form. For all patients, pre-and post-operative American Knee Society knee score and functional score was assessed. As laboratory work-up, all patients’ pre-and post-operative erythrocyte sedimentation rate, C-reactive protein, white blood cells values were compared. The patients’ mean knee score and functional score was detected to be 34.7 (4-57) and 33.5 (5-55), respectively, and to be 78.2 (62-90) and 73.8 (50-90) after revision. In pre-operative assessments performed based on Knee Society radiographic assessment form, mean alpha angle was found to be 95.3 (77-103), mean beta angle to be 84.8 (71-100), mean gamma angle to be 5.8 (0-30), mean sigma angle to be 83.3 (49-97), and mean total valgus angle to be 0.1 (-13-12). In post-operative assessments, mean alpha angle was found to be 94.7 (90-99), mean beta angle to be 88.4 (84-92), mean gamma angle to be 3.7 (0-10), mean sigma angle to be 88.1 (85-92), and mean total valgus angle to be 3.1 (-1-8). We think that when revision knee arthroplasty operation is performed for proper indication and according to the surgical technique, it is a highly effective and preferable method in decreasing the pain severity and improving the life quality of the patients. © 2018, Yuzuncu Yil Universitesi Tip Fakultesi.
