Article Association of Acromegaly and Multiple Myeloma: a Case Report(Galenos Yayincilik, 2013) Atmaca, Murat; Yildiz, Saliha; Kalan, Isilay; Ozbay, Mehmet Fatih; Seven, Ismet; Ozturk, MustafaMalignancy is an important cause of mortality in acromegaly. Hematological malignancies are very rare in acromegaly. Here, we report an 80-yearold patient with acromegaly and multiple myeloma. Patient died within a month of diagnosis. Previous studies have shown that growth hormone and somatomedin-C activate B lymphocyte and somatomedin-C receptors are found in multiple myeloma cells. Possible effects of growth hormone and somatomedin-C on multiple myeloma progression are discussed in the light of the relevant literature.Article Contribution of Diffusion Weighted Mri To Diagnosis and Staging in Gastric Tumors and Comparison With Multi-Detector Computed Tomography(Assoc Radiology & oncology, 2017) Arslan, Harun; Ozbay, Mehmet Fatih; Calli, Iskan; Dogan, Erkan; Celik, Sebahattin; Batur, Abdussamet; Kotan, Mehmet CetinBackground. Diagnostic performance of Diffusion-Weighted magnetic resonance Imaging (DWI) and Multi-Detector Computed Tomography (MDCT) for TNM (Tumor, Lymph node, Metastasis) staging of gastric cancer was compared. Patients and methods. We used axial T2-weighted images and DWI (b-0,400 and b-800 s/mm(2)) protocol on 51 pre-operative patients who had been diagnosed with gastric cancer. We also conducted MDCT examinations on them. We looked for a signal increase in the series of DWI images. The depth of tumor invasion in the stomach wall (tumor (T) staging), the involvement of lymph nodes (nodal (N) staging), and the presence or absence of metastases (metastatic staging) in DWI and CT images according to the TNM staging system were evaluated. In each diagnosis of the tumors, sensitivity, specificity, positive and negative accuracy rates of DWI and MDCT examinations were found through a comparison with the results of the surgical pathology, which is the gold standard method. In addition to the compatibilities of each examination with surgical pathology, kappa statistics were used. Results. Sensitivity and specificity of DWI and MDCT in lymph node staging were as follows: N1: DWI: 75.0%, 84.6%; MDCT: 66.7%, 82%;N2: DWI: 79.3%, 77.3%; MDCT: 69.0%, 68.2%; N3: DWI: 60.0%, 97.6%; MDCT: 50.0%, 90.2%. The diagnostic tool DWI seemed more compatible with the gold standard method (surgical pathology), especially in the staging of lymph node, when compared to MDCT. On the other hand, in T staging, the results of DWI and MDCT were better than the gold standard when the T stage increased. However, DWI did not demonstrate superiority to MDCT. The sensitivity and specificity of both imaging techniques for detecting distant metastasis were 100%. Conclusions. The diagnostic accuracy of DWI for TNM staging in gastric cancer before surgery is at a comparable level with MDCT and adding DWI to routine protocol of evaluating lymph nodes metastasis might increase diagnostic accuracy.Article Ocular Findings in Sheehan's Syndrome(Springer, 2015) Atmaca, Murat; Kizildag, Esra; Candan, Zehra; Ozbay, Mehmet Fatih; Seven, IsmetSheehan's syndrome (SS) is one of the most common causes of hypopituitarism. The primary effect of SS is a deficiency in production of growth hormone (GH). A number of studies have supported the association between congenital GH deficiency and ocular anomalies. However, ocular findings such as central corneal thickness (CCT), intraocular pressure (IOP), and retinal nerve fiber layer thickness (RNFLT) have not been evaluated in patients with adult GH deficiency. The objective of this study was to evaluate ocular anomalies in SS with GH deficiency under a cross-sectional design. Thirty three SS patients with GH deficiency and 28 controls with no history of thyroid, adrenal, or pituitary gland diseases or surgery underwent complete hormonal and ophthalmological evaluation, including an assessment of CCTs, IOPs, and RNFLT. The mean CCTs were significantly lower in the SS group compared with the control group (p < 0.001). There was no significant difference between patients and controls in terms of mean IOP, mean corrected IOP, and mean RNFLT (p = 0.517, p = 0.186, p = 0.965, respectively). The mean CCT was positively correlated with insulin-like growth factor 1 (IGF-1; p < 0.01) and adrenocorticotropic hormone (ACTH; p < 0.01) and negatively correlated with the corrected mean IOP (p < 0.05). In covariance analysis, IGF-1 was found to be a potential predictor of the mean CCT (p = 0.023). This study is the first investigation of ocular findings in SS and adult GH deficiency. Adult GH deficiency is characterized by lower CCT values.Article Oxidative Stress in Patients With Thyroidectomy and Thyroparathyroidectomy Under Replacement Therapy(Springer, 2015) Kacmaz, Murat; Atmaca, Murat; Arslan, Ayse; Demir, Halit; Ozbay, Mehmet FatihSeveral studies have demonstrated an imbalance between free radicals and the antioxidative system in individuals with thyroid dysfunction. However, oxidative stress has not been evaluated in patients with thyroidectomy and thyroparathyroidectomy, who are under replacement therapy. The objective of this study was to evaluate the oxidative stress using malondialdehyde, nitric oxide, and catalase levels in patients with thyroidectomy and thyroparathyroidectomy. Nineteen patients with thyroidectomy, 20 patients with thyroparathyroidectomy, and 20 controls with no history of thyroid or parathyroid disease or surgery were included in the study. Serum malondialdehyde, nitric oxide, and catalase levels were examined. Levels of nitric oxide and malondialdehyde were elevated, and catalase levels decreased in patients with thyroidectomy and thyroparathyroidectomy compared with controls (p value for all the parameters: p < 0.001). Free tetraiodothyronine was a potential predictor of malondialdehyde in the patient groups (p: 0.002). Catalase was negatively correlated with nitric oxide (p < 0.01) and malondialdehyde (p < 0.01). The results of the current study demonstrated that oxidative stress increased in patients with thyroidectomy and thyroparathyroidectomy despite the application of replacement therapies.Article A Rare Case: Gastric Cancer; Involving Primery Thoracal Vertebral Metastases(derman Medical Publ, 2013) Arslan, Harun; Partin, Mehmet Kadir; Ozbay, Mehmet Fatih; Arlan, AysePrimery bone metastases rarely occur in gastric cancer. This clinic condition is often encountered in Asian populations and male gender while thyrotoxicosis is frequently seen in women. The escape of potassium into cell is the mechanism responsible for this disease and its etiology is not completely known. Thyroid hormones, carbohydrate rich diet, alcohol consumption and excessive exercise are regarded as the precipitating factors. This clinical picture is generally difficult to define at first attack. Here, we report a forty-two-year-old male patient with BasedowGraves disease who was diagnosed as having thyrotoxic hypokalemic periodic paralysis during the first paralytic attack. His symptoms improved after potassium replacement and treatment with beta-blocker and antithyroid drugs. The permanent cure was achieved with radioactive iodine ablation therapy.Article Toxicity of Lidocaine Improved With Lipid Emulsion Treatment: Case Report(derman Medical Publ, 2015) Gonullu, Hayriye; Gonullu, Edip; Soyoral, Lokman; Ozbay, Mehmet Fatih; Kaya, YukselDuring general anesthesia and intensive care applications that require control of the airway respiratory, cardiovascular reflex responses occur thus myocardial oxygen delivery and consumption can be negatively affected. To prevent these effects, lidocaine, opioids, magnesium, calcium channel blockers, beta- blockers have been used. Local anesthetic toxicity; is usually known to occur when used over the range of safe dose of local anesthetics. It has been shown in a variety of animal and clinical studies that a lipid emulsion used for parenteral nutrition improves resistance to fatal cardiac effects caused by local anesthetics. Herein, we presented a case who had developed sudden bradycardia, asystole by iv. lidocaine which is used for tracheal intubation and improved dramatically after treatment with lipid emulsion in the light of the literatures. According to our knowledge it is the first case which is succesfully resuciatated with clinoleic in local anesthetic toxicity in literature.
