Article Application of Ureteroscopic Pneumatic Lithortripsy in Patients With Ureter Calculi: Our Five Years Experience(Professional Medical Publications, 2012) Gecit, Ilhan; Pirincci, Necip; Taken, Kerem; Benli, Erdal; Ceylan, KadirObjectives: To evaluate the efficacy and reliability of ureteroscopic pneumatic lithoripsy (URS-PL) in patients with ureter calculi. Methodology: A total of 612 patients who were treated with URS-PL in Medical school of Yuzuncu Yil University, Van State Hospital and Bingol State Hospital in the period between April 2005-May 2010 were studied. Diagnoses of ureter stones were made with intravenous pyelography (IVP) or computerize tomography (CT). Ureteral balloon dilation was not performed on any patient. On the post-operative 1st, 7th and 21st days, control direct urinary system graphic (DUSG) was taken. The rates of stone free and complication were analyzed with the ki-kare test. P < 0.05 was accepted as significant. Results: The mean age of the patients was 39 +/- 10 (18-75). There were 417 males (68.13%) and 195 females (31.86%). The mean volume of calculi in patients was 10.1 +/- 2.3 mm. Of these patients, 121 had proximal ureter stone, 169 medial ureter stones and 322 had distal ureter stones. In the first session of URS-PL, the calculi of 96 patients in proximal ureter (80%), those of 143 patients in medial ureter (85%) and those of 305 patients (95%) in distal ureter were cracked successfully. The mean stone free rate was 92.4% (79.4% in upper ureter, 94.1% in middle ureter and 93.7% in distal ureter (P < 0.05). Overall complication rate was 7.6% (7.7% in distal ureter, 3.9% in middle ureter and 11.8% in proximal ureter) (P>0.05). Most common peroperative complications of the procedure were ureteral perforation 4,2% (n=26), infection 2.2% (n=14), and mucosal laceration 1.9% (n=12). The most common late complication was ureteral stricture in 1.3% (n=8) of patients. Conclusion: Although URS-PL is a commonly used application for the treatment of patients with proximal and middle ureter stones, its common usage could also be extended to patients with distal ureter stones in the first stage of treatment, and it could be considered as an efficient and reliable procedure.Article Beneficial Influence of Topical Extra Virgin Olive Oil Application on an Experimental Model of Penile Fracture in Rats(Sage Publications inc, 2015) Gunes, Mustafa; Ozkol, Halil; Pirincci, Necip; Gecit, Ilhan; Bilici, Salim; Yildirim, SerkanPenile fracture (PF) is known as a traumatic rupture of the tunica albuginea of corpus cavernosum. In this study, we aimed to investigate the healing influence of topical extra virgin olive oil (EVOO) on PF through evaluating levels of some oxidative stress biomarkers for the first time. Histopathological evaluation was also realized. A total of 18 male Sprague-Dawley albino rats were divided into three groups of six rats each as control group, in PF (alone) group, and PF + EVOO group. Experimental PF was formed via incising from the proximal dorsal side of the penis in the rats of all groups except control. While in PF (alone) group, fracture was formed and the incision was primarily closed, in PF + EVOO group in addition to foregoing processes, EVOO was also administrated topically twice a day for 3 weeks. At the end of the experiment, all rats were killed and penectomy was carried out. While malondialdehyde, myeloperoxidase, lipid hyroperoxide, and total oxidant status significantly (p < 0.05) increased, reduced glutathione and total free sulfhydryl groups markedly (p < 0.05) decreased in PF (alone) group when compared with PF + EVOO group. Levels of these parameters were reversed to nearly normal values by topical EVOO application. Protection by EVOO is further substantiated via the improved histological findings in PF + EVOO group as against degenerative changes in the rats of PF (alone) group. Our data revealed that EVOO has protective effect in penile cavernosal tissue through probably its antioxidant, free radical defusing, anti-inflammatory, and antimicrobial effects.Article Comparison of Cystoscopy With Diffusion-Weighted Magnetic Resonance Images Used in the Diagnosis and Follow-Up of Patients With Bladder Tumors(Asian Pacific Organization Cancer Prevention, 2010) Ceylan, Kadir; Taken, Kerem; Gecit, Ilhan; Pirincci, Necip; Gunes, Mustafa; Tanik, Serhat; Karaman, IhsanPurpose: To compare diffusion-weighted magnetic resonance imaging (DW-MRI) with cystoscopy in the diagnosis and follow-up of patients with bladder tumor and to investigate any histopathological correlation. Materials and Methods: Totally 59 patients, between 31-85 years (mean age 60 +/- 13) referred to our clinic due to a hematuria complaint were enrolled and evaluated by upper urinary system pathology and then DW-MRI (average 7 days) and cystoscopy. Apparent diffusion coefficients (ADCs) of images were calculated. Results: While a mass in bladder was determined with cystoscopy in 43 out of 59 patients, the mass was not deternined in 16 of the patients(n=34 malign, n=9 benign). While a mass was determined in 40 out of 59 patients with DW-MRI, the mass was not determined in 19 of the patients(n=40 malign, n=19 benign). Regarding ADC values, mean ADC values of 34 patients who were diagnosed with a bladder tumor (1.05 +/- 0.22 x 10(-3)mm(2)/s), were significantly lower than the mean ADC values obtained from the normal bladder wall (1.830 +/- 0.18 x 10(-3)mm(2)/s) whereas a statistically significant difference was found (p<0.001). ADC values in 9 patients with a benign lesion (1,73 +/- 0.12 x 10(-3)mm(2)/s), were not found statistically different from the mean ADC values obtained from the normal bladder wall (1.78 +/- 0.2x10(-3)mm(2)/s) (p>0.05). A significant difference was determined between ADC values of benign lesions and the ADC values of malign lesions (p<0.001). Conclusions: According to cystoscopy, values of DW-MRI's related with sensitivity, specificity and accuracy were found, respectively 90%, 93% and 91%. As the DW-MRI is a non-invasive and a rapid technique, and does not contain ionized radiation and because it is accepted as an important marker of tumor cellularity, it may be used as an alternative in future diagnosis and follow-up of bladder tumors.Article Comparison of the Techniques of the Microscopic Varicocelectomy and Inguinal Varicocelectomy(derman Medical Publ, 2012) Pirincci, Necip; Gecit, Man; Curies, Mustafa; Taken, Kerem; Canbeyt, Ozcan; Ceylanl, KadirAim: The aim is to compare the results of two different varicocelectomy techniques. Material and Method: Between July 2005 and February 2011; 68 patients who are of 84 varicocele whose ages were ranged from 15 to 39 (26.4 +/- 6) and whom inguinal varicocelectomy or subinguinal microscopic varicocelectomy were performed due to the clinical varicocele were retrospectively evaluated. The complication rates; preoperative and postoperative sperm parameters and pregnancy rates of both groups were evaluated and compared. Results: The duration of the operation in the subinguinal microscopic group was significantly long compared to the inguinal group. Hydrocelectomy and the recurrence of varicocele were not observed in the subinguinal microscopic group; hydrocele in 3 cases and the recurrence of varicocele in 5 cases were detected in the inguinal group. The difference in between was significant. There was a recovery of 75% in the number of sperm and / or movement in the subinguinal microscopic group and a recovery of 64% in the inguinal group; these values were statistically significant comparable level in both group. The rates of the pregnancy in the first year was 29% in the subinguinal microscopic group and was 26% in the inguinal group; there was no significant difference among the pregnancy rates between both groups. Discussion: Subinguinal microscopic varicocelectomy has yielded better results with low complication and higher success rates compared to the inguinal varicocelectomy.Article Comparison of the Use of Prilocaine and Lidocaine Gel During the Cystoscopy(derman Medical Publ, 2012) Cecit, Ilhan; Benifi, Erdal; Keles, Ibrahim; Kayak, Servet; Pirincci, Necip; Guiles, Mustafa; Ceylan, KadirAim: In this study, the difference of the activity between the prilocaine and lidocaine gel instillation into the urethra was investigated in order to reduce the patient's pain and to increase the comfort of the procedure during the cystoscopy which was commonly used in the practice of urology. Material and Method: 134 patients who were decided to be performed the cystoscopy for various reasons between the years of 2008 and 2011 were randomly divided into two groups2% of lidocaine gel to the first group and 20 cc of 2% of prilocaine to the second group were given from the urethral route 5 minutes before the cystoscopy. Visual Analog Scale (VAS) was used in the assessment of the pain levels of the patients during the cystoscopy. The discomforts of the patients during the process were assessed with the question like "would you have the process done again if necessary". In patients in both groups, the levels of the pain and the hemodynamic changes were recorded at the beginning of the procedure, during the procedure and after the procedure. Results: There was no significant difference in terms of the hemodynamic changes between the two groups. If it is necessary to repeat the cystoscopy, while the average score of VAS for prilocaine was 3.8 and the rate of the performance was 80%, these values for lidocaine were about 5 and 70% respectively. Cystoscopy was viewed as more comfortable both for the doctor and for the patient in the group used prilocaine. It was understood that the majority of the patients would be able to perform the cystoscopy again if necessary. Discussion: Reducing the sense of the pain in the urethra, urethral instillation of the prilocaine has provided less pain during the rigid cystoscopy especially for the outpatients and has prevented the formation of the bad memories about the process. Prilocaine has drawn its score of VAS down 1.2 points compared to lidocaine and has increased the rate of doing cystoscopy again of patient by 10% when required.Article Complete Duplication of the Bladder and Urethra in the Coronal Plane: Case Report With Review of the Literature(Karger, 2013) Pirincci, Necip; Gecit, Ilhan; Gunes, Mustafa; Tanik, Serhat; Ceylan, KadirComplete duplication of the bladder and urethra without other congenital anomalies is a very rare congenital anomaly. Materials and Methods: A total of 31 pediatric patients with renal trauma (male = 25, female = 6) in whom we used a conservative approach in our clinic between the years 2002 and 2010 were included in the study. Patients were assessed according to the type of trauma, type of treatment, findings of radiological imaging, concomitant other organ injuries and complications occurring during follow-up. Results: Surgery was required in just 4 of 31 pediatric patients who were treated with the conservative approach. Of the patients who underwent surgery, only one child needed nephrectomy. No complications were detected in the clinical and radiological follow-up of the patients who received the conservative approach. Conclusions: The main objective in the treatment of renal trauma occurring in childhood is to protect the kidneys. If the hemodynannic findings are stable, a conservative approach should be the first preferred method of treatment in every grade of renal trauma that can occur in childhood. Copyright (C) 2013 S. Secondary tumors may develop months and years after the primary tumor. We aimed to report a case of cutaneous Ewing's sarcoma at the site of surgery 3 years after BEP chemotherapy. PRESENTATION OF CASE: 21 year old male underwent radical orchiectomy in 2008. After one year surgical site complaints brought him to same hospital. A limited surgical resection was made. As his complaints continued he applied to our clinic. We resected the lesion with a 5 cm safety margin with the light of previous medical history. Pathology revealed cutaneous Ewing's sarcoma, and patient received VACD-IE chemotherapy. He is free of recurrence till now. DISCUSSION: Chemotherapy may cause secondary cancer especially in long term. In this case secondary tumor is diagnosed three years after surgery. Patient underwent therapeutic surgery and received chemotherapy (VACD-IE) for secondary Ewing's sarcoma. Early diagnosis and definitive treatment provide recurrence free survival in the patient. CONCLUSION: Secondary tumors can emerge months or years after primary tumor therapies, and are not related with the primary tumors. Any lesion or sign should be investigated carefully. The aim of this study was to assess DKK-1 and its correlation with clinic-pathological features in patients with bladder cancer. Materials and Methods: DKK-1 levels were determined in serum samples from 90 patients with bladder cancer before transurethral tumor resection. The concentrations of DKK-1 were determined by using enzyme linked immune-sorbent assay (ELISA). Results: Elevated preoperative DKK-1 levels were associated with tumor stage (p<0.001), grade (p<0.001) and histological grade (p<0.001). Conclusions: The results of our study demonstrated that the level of serum DKK-1 is correlated with both disease progression and increase in the tumor grade. Preoperative serum DKK-1 elevation may thus represent a novel marker for the determination of bladder cancer and the detection of patients with a likely poor clinical outcome.Article Effects of Shock Waves on Oxidative Stress and Some Trace Element Levels of Rat Liver and Diaphragm Muscles(Wiley, 2012) Gecit, Ilhan; Kavak, Servet; Meral, Ismail; Gunes, Mustafa; Pirincci, Necip; Sayir, Fuat; Ceylan, KadirThis study was designed to investigate whether the short-term extracorporeal shockwave lithotripsy (ESWL) exposure to kidney produces an oxidative stress and a change in some trace element levels in liver and diaphragm muscles of rats. Twelve male Wistar albino rats were divided randomly into two groups, each consisting of six rats. The animals in the first group did not receive any treatment and served as control group. The right-side kidneys of animals in group 2 were treated with two-thousand 18?kV shock waves while anesthetized with 50?mg kg-1 ketamine. The localization of the right kidney was achieved after contrast medium injection through a tail vein under fluoroscopy control. The animals were killed 72 h after the ESWL treatment, and liver and diaphragm muscles were harvested for the determination of tissue oxidative stress and trace element levels. Although the malondialdehyde level increased, superoxide dismutase and glutathione peroxidase enzyme activities decreased in the livers and diaphragm muscles of ESWL-treated rats. Although glutathione level increased in liver, it decreased in diaphragm muscles of ESWL-treated animals. Fe, Mg and Mn levels decreased, and Cu and Pb levels increased in the livers of ESWL-treated animals. Fe and Cu levels increased, and Mg, Pb, Mn and Zn levels decreased in the diaphragm muscles of ESWL-treated animals. It also causes a decrease or increase in many mineral levels in liver and diaphragm muscles, which is an undesirable condition for the normal physiological function of tissues. Twelve male Wistar albino rats were divided equally into two groups. First group was used as control. The right-side kidneys of animals in second group were treated with 2,000 18-kV shock waves under anesthesia. Localization of the right kidney was achieved following contrast medium injection through a tail vein under flouroscopy control. The animals were sacrificed 72 h after the ESWL treatment, and the kidneys were taken. Malondialdehyde level was higher in the ESWL group than in the control. Reduced glutathione levels, superoxide dismutase, and glutathione peroxidase activities were lower in the ESWL group than those of the control. Fe, Cu, Pb, Mn, Cd, and Ni levels were lower in the ESWL group than in the control, although Mg level was higher in the ESWL group than in the control. In conclusion, the result of the present study indicated that ESWL treatment produced oxidative stress in the kidney and caused impairments on the antioxidant and trace element levels in the kidneys of rats.Article Evaluation of Prolidase Activity, Oxidative Stress, and Antioxidant Enzyme Levels in Testicular and Penile Tissues After Human Chorionic Gonadotropin Treatment in Rats by Predicting Infertility and Erectile Dysfunction(Karger, 2018) Pirincci, Necip; Yildirim, Serkan; Tas, Abuzer; Kuscu, Yagmur; Ozan, Tunc; Firdolas, Fatih; Karakeci, AhmetObjectives: Prolidase plays a vital role in collagen turnover, matrix remodeling, and cell growth. We aimed to evaluate the association between treatment with chorionic gonadotropin and infertility and erectile dysfunction by investigating tissue prolidase activity, oxidative stress, and levels of antioxidant enzymes. Materials and Methods: The 16 male Wistar albino rats used in this study were randomly divided into 2 groups: rats treated with human chorionic gonadotropin (hCG) and control rats (n = 8 in each group). The rats in the hCG group were subcutaneously injected with 50 IU hCG daily for 15 days, while the rats in the control group were subcutaneously injected isotonic saline. All of the rats were sacrificed by a lethal overdose of sodium pentobarbital at the first month after hCG administration. Prolidase activity and levels of malonyl aldehyde, glutathione reductase, superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), and catalase (CAT) were estimated in the testicular and penile tissue. The testicles and penis were transversely dissected and placed in formalin. Results: Levels of prolidase and malonyl aldehyde in the testicular and penile tissues were significantly higher in the hCG group than in the control group (p < 0.001), while levels of glutathione reductase, SOD, GSH-Px, and CAT were significantly lower in the hCG group than in the control group (p < 0.001). Conclusions: In this study, we observed that treatment with hCG increased prolidase activity and oxidative stress and decreased the antioxidant capacity of penile and testicular tissues; therefore, this may affect fertility and erectile function. FarukAlthough many techniques have been described, new techniques with a wide range of therapeutic options are needed. The Kutlay technique is a novel technique that is based on the reconstruction of the neourethra with two horizontal meatal-based skin flaps. In the present study, the data of 31 patients who underwent surgery with the Kutlay technique are presented. Thirty-one patients with hypospadias with an average age of 5.6 years who did not have previous hypospadias repair were operated on with the Kutlay technique. Ten patients had chordee. Among those patients, three patients were circumcised. The patients were followed up for 4-13 months (average, 9.3 months). During the follow-up period, a fistula was observed in only one patient. None of the patients developed neourethral dehiscence, meatal stenosis, urethral stricture, wound infection, penile torsion, hematoma, or persistent or recurrent chordee. On uroflowmetry studies, the maximum flow rate of the patients was approximately 10.5 ml/s (range, 6-17 ml/s). The patients were observed to void with a single straight urinary stream in a forward direction. The Kutlay technique is a technique that is easily applied in patients with chordee and that provides a low risk of fistulas and acceptable functional and esthetic results. The factors that reduce the risk of fistula are the lack of the superposition of the suture lines of the skin and the urethra, the reconstruction of the urethra with well-vascularized flaps, and the replacement of the neourethra in its appropriate location through the tunnel created in the glans.Article Levels of Serum Trace Elements in Renal Cell Carcinoma Cases(Asian Pacific Organization Cancer Prevention, 2013) Pirincci, Necip; Gecit, Ilhan; Gunes, Mustafa; Kaba, Mehmet; Tanik, Serhat; Yuksel, Mehmet Bilgehan; Demir, HalitTrace elements which are essential components of biological structures may also be toxic when present at levels above the amounts required for biological function. In our study, trace element levels were measured with furnace atomic absorption spectrophotometry in 33 newly diagnosed renal cell carcinoma cases (preoperative) and 32 healthy controls. When compared with the control group, it was found that the levels of cadmium (Cd), lead (Pb) were higher and the levels of zinc (Zn), iron (Fe) and manganese (Mn) were lower in the patient group. These changes may be important in the formation of renal cell carcinoma, a question which should be explored with postoperative comparative studies.Article Plasma Human Neutrophil Proteins-1, And-3 Levels in Patients With Bladder Cancer(Springer, 2013) Gunes, Mustafa; Gecit, Ilhan; Pirincci, Necip; Kemik, Ahu Sarbay; Purisa, Sevim; Ceylan, Kadir; Aslan, MehmetHuman neutrophil proteins-1, -2, and -3 (HNPs -1, -2, and -3) are expressed in several tumor types. However, the role of HNPs 1-3 in human bladder cancer has not yet been determined. We investigated the association between the plasma levels of HNPs 1-3 and clinicopathological parameters in bladder cancer patients. The plasma levels of HNPs 1-3 were measured in 60 patients with bladder cancer and in 58 healthy controls. The plasma levels of HNPs 1-3 were determined by a solid-phase enzyme-linked immunosorbent assay (ELISA). Plasma samples were obtained before surgery. Plasma samples were permitted to clot and were then stored at -80 A degrees C until use. The levels of the HNPs increased from grade 1 to 4 tumors and this difference was statistically significant (p < 0.001). Additionally, plasma HNP levels were significantly higher in patients with metastatic bladder cancer and in patients with lymphovascular involvement, metastasis of the lymph nodes, and increased tumor burden (p < 0.001). The preoperative plasma levels of HNPs 1-3 paralleled the progression and pathological stages of the malignancies. This study suggests that HNPs 1-3 promote tumor invasion and are potential indicators of disease progression in patients with bladder cancer.Article Pregnancy and Renal Cyst Hydatid Disease: Case Report(Academic Journals, 2012) Gecit, Ilhan; Pirincci, Necip; Taken, Kerem; Eryilmaz, Recep; Ozveren, Huseyin; Deveci, SerkanHydatid cyst disease is an important health problem in many countries, including Turkey. While it can impact any organ, it is most often found in the lung and liver. Renal involvement is relatively rare. A diagnosis of renal cystic hydatidosis was established with ultrasound and magnetic resonance in a 22 year old patient who was 25 weeks pregnant after she presented with severe right flank pain. Albendazol was administered at a dose of 10 mg/kg/day. She was closely followed during her pregnancy. At 38 weeks, she gave birth to a healthy baby via vaginal delivery. Following her delivery, the cyst was resected. Post-operatively, she completed an 8 week course of albendazol at a dose of 10 mg/kg/day. 1 year later, the patient has done well with no complications. Preoperative serum MMP-7 and MMP-9 levels were significantly higher in cancer patients than control groups (p<0.001). In contast, serum TIMP-1 levels were lower (p<0.001). Alteration in MMP-7, and MMP-9, and TIMP-1 production may contribute to tumor angiogenesis and be associated with clinic-pathological features.Article The Prolidase Activity, Oxidative Stress, and Nitric Oxide Levels of Bladder Tissues With or Without Tumor in Patients With Bladder Cancer(Springer, 2017) Gecit, Ilhan; Eryilmaz, Recep; Kavak, Servet; Meral, Ismail; Demir, Halit; Pirincci, Necip; Taken, KeremThis study was designed to evaluate the malondialdehyde (MDA), glutathione (GSH) and nitric oxide (NO) levels, and also prolidase, glutathione peroxidase (GSH-Px), superoxide dismutase (SOD) enzyme activities in malignant and benign cancers of bladder tissue. A total of 59 patients admitted to our clinic due to microscopic or macroscopic haematuria, were prospectively included in the study. Because of some reasons (no request to participate in the study, the inability to reach, other malignancies, alcohol consumption, metabolic disease), eight patients were excluded from study. Of the 51 patients, 25 were bladder tumor patients, and 26 were patients without cancers. The bladder tissue samples were obtained from all patients under anesthesia (spinal, epidural or general) for the measurement of MDA, GSH and NO levels, and prolidase, GSH-Px and SOD enzyme activities. Among the patients with bladder cancers, 7 patients were females and 18 patients were males, with an average age of 68.4 +/- 2.49. Among patients without tumors, 6 patients were females and 20 patients were males, with an average age of 58 +/- 2.05. In patients with bladder tumors, the oxidants (MDA, NO, prolidase) were higher, and the antioxidants (SOD, GSH, GSH-Px) were lower than those in patients without tumors. It was concluded that the oxygen free radicals play a role in the etiology of bladder cancers similar to many other tumors and inflammatory conditions. Therefore, we assume that antioxidants may provide benefits in the prevention and treatment of bladder cancer.
