Article The Effect of Growth Hormone Deficiency on Inflammatory Markers in Sheehan's Syndrome(2020) Alay, Murat; Yıldız, Saliha; Sakin, Ayşegül; Sönmez, Gülçin MiyaseAim: Mean platelet volume (MPV), lymphocyte count (LC), neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (PLR)are currently gaining interest as new markers of inflammation and independent risk factors for cardiovascular diseases. The aim ofthis study was to evaluate the effect of growth hormone (GH) deficiency on MPV, NLR, and PLR values in patients with Sheehan’ssyndrome.Materials and Methods: 23 women with Sheehan’s syndrome (SS), and 30 healthy women as control group were included in thestudy. The study was performed retrospectively. Demographic and clinical characteristics, hormone and haematological parametersof subjects were evaluated. MPV, NLR, PLR and LC values were compared between two groups.Result: The mean age of SS and control groups were 53.1 ± 11.1 and 55.1 ± 10.5 years, respectively. MPV, PLR, NLR were lower andLC was higher in SS group than the control group. (p = 0.010, 0.033, 0.017, 0.010 respectively). A significant positive correlation wasfound between insulin-like growth factor1 (IGF1) and MPV, PLR in the SS group (p = 0.003 r: 0.62, p: 0.033 r: 0.46).Conclusion: MPV, NLR, PLR were lower and LC was higher in SS group compared to control group. There was a significant positivecorrelation between IGF1 and MPV, PLR in the SS group. These results showed that growth hormone deficiency led to a decrease insome inflammatory markers such as MPV, NLR, PLR. Increased LC in these patients may indicate inflammation.Article Factors Affecting Glycemic Control in Patients With Type 2 Diabetes Mellitus Using Oral Antidiabetics: a Single-Center Experience(2021) Alay, Murat; Sakin, AyşegülIn this retrospective study, we aimed to investigate the factors affecting glycemic control in patients with type 2 diabetes mellitus (DM) treated with oral antidiabeticdrugs (OADs). From 2019 to 2020, type 2 DM patients treated with OADs were included in this study. Patients less than 18 years old and those treated with insulin wereexcluded from the study. The patients were divided into two groups according to HbA1C levels as follows: good glycemic control (GGC) (<7%) or poor glycemic control(PGC) (≥7). Of the 505 type 2 DM patients, 270 (53.5%) were female. There were 194 (38.4%) patients in the GGC group and 311 (61.6%) in the PGC group. The rateof marriage and employment were higher in the GGC group than those in the PGC group. Likewise, education level and, and high-density lipoprotein were higher in theGGC group than those in the PGC group. In the multivariate analysis, being married (odds ratio [OR], 0.39), duration of DM (OR, 1.12), having a high-school graduate(OR, 0.43), triglyceride level (OR, 1.001), and high-density lipoprotein level (OR, 0.979) were the factors affecting glycemic control. In this study, we found that beingmarried and having a higher education level and high-density lipoprotein level were associated with GGC in type 2 DM patients treated with OADs.Article Metastatik Küçük Hücreli Akciğer Kanserinde Platin Seçiminin Sağkalim Üzerine Etkisi ve Sağkalimi Etkileyen Faktörler: Tek Merkez Deneyimi(2021) Aldemir, Mehmet Naci; Sakin, Ayşegül; Alay, MuratAmaç: Bu retrospektif çalışmada, metastatik KHAK hastalarında platin (karboplatin veya sisplatin) seçiminin sağkalım üzerine etkisini ve sağkalımı etkileyen faktörlerin belirlenmesini amaçladık. Gereç ve Yöntem: ilkseri tedavi de platin bazlı kemotrapi ile tedavi edilen, 18 yaş ve üzeri, dataları mevcut toplamda 87 metastatik KHAK tanılı hasta alındı. Hastalar aldığı platin bazlı tedavi tipine göre sisplatin+etoposit(SE) ve karboplatin + etoposit(KE) olarak gruplandırıldı. Bulgular: Çalışmaya alınan 87 hastanın 16 (%18,4) sı kadındı. Ortanca yaşı 58 idi. SE ve KE ile tedavi edilen hasta sayıları sırası ile 65 ve 22 idi. SE ve KE ile tedavi edilen hastaların ortanca progresyonsuz sağkalımı(PSK), sırasıyla 6 ay ve 5 aydı (p=0.171), karşılık gelen ortanca genel sağkalım (GSK) süreleri 9 ay ve 8 aydı (p=0.173). Performans skoru, torakal radyoterapi uygulanması ve ilk seri tedavi yanıtı sağkalımı etkileyen bağımsız faktörler olarak bulundu. Sonuç: Çalışmamızda, ilk seri tedavide SE ‘ye kıyasla KE tedavisi metastatic KHAK’inde etkilemedi. Ancak tanı anında performans skoru kötü olanların sağlamının performans skoru iyi olanlardan daha düşük olduğu gözlendi. Bunun yanında, torakal radyotertapi verilmesi ve ilk seri kemoterapiye daha iyi bir yanıt sağkalımı iyileştirdi.Article Red Blood Cell Distribution Width as a Possible Predictor of Diagnosis and Survival in Gastric Cancer(2020) Sakin, Ayşegül; Kotan, Çetin; Alay, Murat; Sahin, Suleyman; Sakin, Abdullah; Aytekin, Aydın; Esen, RamazanObjectives: To compare the preoperative Red Cell Distribution Width (RDW) value in curatively-operated gastric cancer(GC) patients without receiving neoadjuvant chemotherapy (NACT) vs. control group, with the aim of investigating itspreoperative prognostic effect.Methods: Receiver Operator Characteristics (ROC) curve of RDW value was plotted for DFS. The area under curve (AUC)of the RDW was 0.714 with 73.5% sensitivity and >5.5 with 71.1% specificity. Patients were divided into 2 groups as RDW≤15.5 and RDW >15.5.Results: The study included 330 GC patients (37.7% female and 62.3% male) and 330 healthy controls (63.9% maleand 36.1% female). ROC curves were used to confirm the predictive role of preoperative RDW value in predicting thepresence of GC. For GC, the AUC of RDW was 0.665 with 61.3% sensitivity and 14.1 with 64% specificity. There was apositive correlation between disease stage and RDW in GC patients (Rho=0.338, p<0.001). Five-year DFS was 81.1% inthe low-RDW group and 61.9% in the high-RDW group (p=0.001). Similarly, Corresponding 5-year overall survival (OS)rates were 74.4% and 57.7 (p=0.001). In multivariate analysis, male gender, stage III disease, high CEA, and RDW ≥15.5were the factors associated with worse DFS, whereas adjuvant therapy (p=0.036) prolonged DFS significantly.Conclusion: In our study, preoperative RDW was found to be both predictive and prognostic for GC.
