Article Chronotype Differences in Suicidal Behavior and Impulsivity Among Suicide Attempters(Taylor & Francis inc, 2011) Selvi, Yavuz; Aydin, Adem; Atli, Abdullah; Boysan, Murat; Selvi, Fatih; Besiroglu, LutfullahMorning- and evening-type individuals differ on a number of psychological and biological variables. There has been increasing interest in the relationship between chronotype and personality traits. The aim of this study was to investigate the relationship between impulsivity and chronotype in suicide attempters. Eighty-nine suicide attempters were included in the study, and systematic information on suicide attempts was recorded. The Morningness-Eveningness Questionnaire was applied to determine chronotype, and attempter impulsivity was measured by the total score of the Barratt Impulsiveness Scale. Significant differences between chronotype and impulsivity scores were found. Evening-type subjects reported significantly higher impulsivity scores than both neither- and morning-types. A significant association between chronotype and type of suicide attempt was detected. The largest proportion of violent suicide attempters were evening-type subjects. Violent suicide attempters also reported significantly higher impulsivity scores than nonviolent attempters. Previous studies have pointed out possible relations between eveningness and impulsivity. Current findings suggest that eveningness may be a risk factor for violent suicide attempts by increasing impulsivity. Though hallucinations, delusions and agitation are sometimes present, the symptoms of delirium are clinically distinct from those induced by other psychiatric disorders. Datura stramonium is a hallucinogenic plant which is a member of belladona alkaloids. The toxic effects of Datura stramonium most often include visual and auditory hallucinations, confusion and agitation. We report a patient who was poisoned with Datura stramonium and presented to the emergency department with psychiatric symptoms.Article Hepatic Encephalopathy in Connection With Budd-Chiari Syndrome Due To Infection With Echinococcus Multilocularis: a Case Report(Elmer Press inc, 2011) Dulger, Ahmet Cumhur; Kemik, Ozgur; Selvi, Fatih; Begenik, Huseyin; Emre, Habib; Erdur, Fatih MehmetBudd-Chiari syndrome (BCS) is a hepatic venous outflow block generally resulting from disorders affecting hepatic venous system. Elevated hepatic venous pressure results in portal hypertension. BCS may also cause hepatic encephalopathy. Echinococcus multilocularis is a tapeworm parasite and the natural course of the disease may affect liver parenchyma as well as hepatic venous tree. It is the most terrible parasitic disease of the liver and is easily confused with hepatic malignancies. Albendazole therapy may suppress disease progression. Alveolar echinococcosis of the liver rarely causes Budd-Chiari syndrome-related hepatic encephalopathy (HE). We report a rare case of alveolar echinococcosis-related BCS with HE, who was successfully managed by rifaximin and albendazole.Article Kafa Travmalı Hastalarda Epidemiyolojik Veriler ve Bölgesel Faktörler(2017) Bilvanisi, Sevdegül Karadaş; Selvi, Fatih; Gönüllü, HayriyeAmaç Bu çalışmada; Kafa travması nedeniyle acil servise başvuran hastaların demografik verilerini geniş bir olgu sayısıyla değerlendirmeyi, bölgesel faktörleri belirlemeyi ve ülkemizdeki eksik olan epidemiyolojik veri tabanının oluşturulmasına katkı sağlamayı amaçladık. Gereç ve Yöntemler: Ocak 2008-Aralık.2009 tarihleri arasında, 2602 hastanın kayıtları geriye dönük olarak araştırılarak kafa travmalı olgular belirlendi. Kafa travması olan 531 hastanın demografik, radyolojik ve klinik bulguları ile kafa travması etyolojisi, eşlik eden yaralanmaları ve Yaralanma Ciddiyet Skoru'na göre değerlendirilerek bölgesel epidemiyolojik veriler belirlendi. Hastalar Glaskow Koma Skoruna (GKS) göre gruplandırılarak morbitide ve mortaliteye etki eden faktörler analiz edildi. Kafa travması sıklığı % 20,4 (n=531) olarak tespit edildi. .Etyolojik olarak en sık araç içi trafik kazalarının ve yüksekten düşmenin (sırasıyla %29 ve %24,4) akut kafa travmasına sebep olduğu belirlendi. Akut kafa travmalı hastalar GKS skorlarına göre: Hafif (n= 296;%55,8), orta (n=170;%32) ve ağır (n=65;%12,2) olarak gruplandırıldı. Ortalama ISS puanı 12 olarak tespit edilmiştir. Düşük GKS skorlu hastalar mortalite daha fazlaydı. Bulgular:Sonuç:Doğu Anadolu Bölgesi'nde çok sayıda olguyla yapılan bu çalışmanın sonuçları; kafa travmalı olgularda bölgesel faktörlerin epidemiyolojik verilerde etkili olabileceğini göstermektedir. While it is difficult to diagnose pyogenic liver abscess under normal conditions, pregnancy makes the diagnostic process even more difficult. Nowadays, it is also sold for rifles as an injector form to capture the stray dogs. The introduction of the field of veterinary medicine has increased the availability of succinilcolin. In this study, a case was introduced who injected to his left arm a 3 ml (60 mg) pharmaceutical product whose active substance was succinylcholine and applied to the emergency service one hour later with non-specific complaints such as fatigue and abdominal pain. Ülkemizde kafa travmalarının epidemiyolojisi konusunda yapılmış az sayıda çalışma vardır. Bu çalışmanın amacı kafa travması geçiren vakaları değerlendirmek ve kafa travmaları konusunda ülkemizin epidemiyolojik verilerine katkıda bulunmaktır.Materyal ve Metod:Bu çalışmada, 01.01.2008 ile 31.12.2009 yılları arasında Yüzüncü Yıl Üniversitesi Tıp Fakültesi Hastanesi Acil Servisine kafa travması ile başvuran hastaların kayıtları geriye dönük olarak inceledik.Hastaların yaşı, cinsiyeti, travma özellikleri, travmanın ciddiyeti (Glasgow Koma ve Travma Ciddiyet Skorları kullanılarak), nöroradyolojik değerlendirme (Beyin Tomografisi bulguları), mortalite ve morbidite frekansları, yattığı servis, tedavi yöntemleri, hastanede kalma süresi, operasyon gereksinimi ve çıkış durumları kaydedildi. Tüm analizler, SPSS (Statistical Package for Social Science) İstatistik paketi 15,0 versiyonu kullanılarak ve p<0,05 istatistiksel olarak anlamlı kabul edilerek yapıldı.Bulgular:Acil servisimize iki yıl boyunca 2602 multitravma hastası başvurmuş ve bunların 531'ini (%20.4) akut kafa travmalı hastalar oluşturmaktaydı. Olguların çoğunluğu (%76) erkek ve yaş ortalaması 23.8 idi. Akut kafa travmaları 1?10 yaş arasındaki çocuklarda daha sık idi. Kafa travmalarına en sık sebebi trafik kazalarıydı (%38.2). Akut kafa travmaları daha çok Temmuz ve Ağustos aylarında görülmekte idi. Glasgow Koma Skoruna (GKS) göre % 33'ü hafif (GCS:13-15), %57'si orta (GCS:9-12) ve %10'u ağır kafa travmasıydı (GCS:3-8) .Akut kafa travmalı hastaların acil servise en sık bilinç kaybı yakınması ile başvurduğu ve tüm akut kafa travmalı olgulara Bilgisayarlı Beyin Tomografisi çekildiği görüldü.Sonuç:Erişkin yaş grubundaki kafa travmalarının etyolojisinde birinci neden trafik kazaları, çocukluk çağında ilk sırayı yüksekten düşmeler almaktadır. Kafa travmalı hastalarda geliş GKS prognoz açısından önemli bir göstergedir. In this study, spontaneous pneumothorax (SP) cases were divided into two groups and evaluated retrospectively according to age, sex, diagnostic methods, treatments, and results. Material and Methods Between June 2003 and May 2006, 107 patients with SP were enrolled into our study. There were 44 (41.13%) primary spontaneous pneumothorax (PSP) and 74 (58.87%) secondary spontaneous pneumothorax (SSP) patients. Age, gender, underlying lung disease, smoking history, symptoms, diagnosis, treatment type, surgical indication, morbidity, recurrence, mortality, and hospital stay of the patients were reviewed. Results 77 patients (71.96 %) were male and 30 patients (28.04 %) were female and their mean age was 45.7 +/- 19.1 years. Chronic obstructive lung disease (COLD) was the most common (39.68%) cause detected in the cases with secondary spontaneous pneumothorax. In twenty (18.695%) patients recurrence was observed and sixteen (14.95 %) of these patients underwent surgery. Whereas 49 (% 45.79) patients were managed by tube thoracostomy, 21(19.62%) patients were managed by tube thoracostomy+surgery and 19 (17.75%) patients were managed by tube thoracostomy+ pleurodesis. Operative indications were prolonged air leak and bullae + recurrence. Hospital mortality was 1.86%. The mean postoperative hospitalization time of the patients was 9.1 +/- 3.5 days. Conclusions Spontaneous pneumothorax is a pathology with low mortality and curable disease when diagnosed and treated in time. The primary treatment modality for spontaneus pneumothorax is conservative treatment or the thoracotomy depending on the degree of pneumothorax. Surgical procedures can be applied safely, with low recurrence rate, when the tube thoracostomy remains unsuccessful.Article Seasonality of Self-Destructive Behaviour: Seasonal Variations in Demographic and Suicidal Characteristics in Van, Turkey(informa Healthcare, 2013) Aydin, Adem; Gulec, Mustafa; Boysan, Murat; Selvi, Yavuz; Selvi, Fatih; Kadak, Muhammed Tayyib; Besiroglu, LutfullahObjective. Seasonality is one of the most interesting but still elusive issue in suicide research. Linkages of seasonality in suicides to possible contributors such as gender, type of method used, and climatic factors have received attention in different cultures. We aimed at evaluating seasonal trends in suicidal behaviour according to demographic characteristics, reasons for self-destructive behaviour and means preferred in suicide. Methods. Our aim was to assess the seasonal variation in self-destructive behaviour in terms of completed and attempted suicides in a 3-year time period from 2008 to 2010 in Van, Turkey. A total of 1448 cases were registered over a 3-year time period in the city. Seasonal deviations in demographic characteristics, reasons for suicide, and methods of suicide were evaluated. Results. Seasonal distribution of both completed and attempted suicides by gender did not significantly deviate. On the other hand, we found a significant decline in self-destructive behaviours among those who were single and student in the spring. We found a peak in self-destructive behaviours in the autumn among Individuals who suffer from psychological problems. Conclusions. Subjects with psychological difficulties were more prone to commit suicide in autumn. Seasonal differences in methods of suicide used by suicidal subjects were not significant.Article Spontaneous Pneumothorax and Pneumomediastinum in Pregnancy(Galenos Yayincilik, 2014) Karadas, Sevdegul; Okyay, Ayse Guler; Odabasi, Dolunay; Selvi, FatihSpontaneous acute pneumothorax may occur as a result of spontanous rupture of subpleural blebs or bullae and it is extremely rare during pregnancy. Spontaneous acute pneumothorax and pneumomediastinum were diagnosed in a young pregnant woman admitted with the complaints of swelling and pain on her neck and upper thorax following 2 days' dyspnea. Nasal oxygen, analgesics and antiemetics were used as needed during ten days' hospitalisation. Physical findings, chest X-ray and oxygen saturation improved at the end of this period. Since supportive treatment was sufficient, invasive treatments such as surgery or thorax tube were not required. Although rare, pneumothoax should be remembered in any pregnant woman with dyspnea and chest-pain and must be confirmed radiographically to distinguish it from other diseases and conditions. In this report, a case of spontaneous acute pneumothorax and pneumomediastinum in a 10 week primigravida is presented.Article Uvular Edema Due To Ecbalium Elaterium: a Case Report(Emergency Medicine Physicians Assoc Turkey, 2012) Aydin, Irfan; Karadas, Sevdegul; Gonullu, Hayriye; Selvi, FatihIn recent years, herbal medicine use has become frequent among the public as alternative medical treatment. People use many plants and herbs to treat the symptoms of diseases. However, many cases of life-threatening side effects of such drugs have been reported in the literature. In this study, a patient who administered drops of Ecbalium elaterium juice into his nose for the treatment of sinusitis and developed uvular edema is presented with a review of the literature.
