Article Activities of Cholinesterases, Adenosine Deaminase and Myeloperoxidase in Patients With Familial Mediterrenean Fever(Baycinar Medical Publ-baycinar Tibbi Yayincilik, 2012) Ozkol, Halil; Ediz, Levent; Sekeroglu, Ramazan; Tekeoglu, Ibrahim; Keskin, SiddikObjective: The aim of this study was to determine Acetylcholinesterase (AChE), Butyrilcholinesterase (BChE), Adenosine Deaminase (ADA) and Myeloperoxidase (MPO) activities in patients with Familial Mediterranean Fever (FMF) during attacks and attack-free periods. Although an inflammatory process is the main pathology in FMF, the activities of AChE, BChE and MPO which might be affected by inflammation has not yet been assessed in FMF. Materials and Methods: The subjects were divided into 3 groups: Group 1. FMF patients in acute attack period (FMF-AP); Group 2. FMF patients in attack-free period (FMF-AFP); Group 3. Healthy Control subjects (HC). The first two groups consisted of 41 and 35 patients, respectively. Thirty individuals without a history of other potential health problems constituted the healthy control (HC) group. We measured the activities of MPO, ADA, AChE and BChE in whole blood and serum of the study groups. Acute phase reactants (AFP) were also evaluated. Results: In the current study, while ADA and MPO activities increased, AChE decreased significantly in both whole blood and serum in FMF-AP group, BChE decreased in only whole blood in this group compared with FMF-AFP and HC groups (p<0.05). In FMF-AP group, both whole blood and serum ADA (r=0.313, r=0.267), and MPO (r=0.341, r=0.253) activities were correlated with C-reactive protein, respectively (all p values <0.001). Conclusion: Our study demonstrated that there are significant differences between FMF-AP and other groups in terms of AChE, BChE, ADA and MPO activities. Thus, we suggest that elevated ADA, MPO and decreased AChE and BChE activities may be considered as supportive markers to distinguish FMF attacks from attack-free periods. However, further larger-scale studies are needed to validate these results. Patients with PsA who met CASPAR classification criteria enrolled by Turkish League Against Rheumatism (TLAR)-Network. Patients were categorized into three groups based on disease duration: early stage (< 5 years), middle stage (& GE; 5, < 10 years), and late stage (& GE; 10 years). All patients underwent clinical and laboratory assessment using standardized protocol and case report forms. The associations between psychological variables and clinical parameters were assessed by a multivariate analysis. Of the 1113 patients with PsA (63.9% female), 564 (%50.7) had high risk for depression and 263 (%23.6) for anxiety. The risk of psychological burden was similar across all PsA groups, and patients with a higher risk of depression and anxiety also experienced greater disease activity, poorer quality of life, and physical disability. Multivariate logistic regression revealed that female gender (OR = 1.52), PsAQoL (OR = 1.13), HAQ (OR = 1.99), FiRST score (OR = 1.14), unemployment/retired (OR = 1.48) and PASI head score (OR = 1.41) were factors that influenced the risk of depression, whereas the current or past enthesitis (OR = 1.45), PsAQoL (OR = 1.19), and FiRST score (OR = 1.26) were factors that influenced the risk of anxiety. PsA patients can experience a comparable level of psychological burden throughout the course of their disease. Several socio-demographic and disease-related factors may contribute to mental disorders in PsA. Data were obtained from the Turkish League Against Rheumatism (TLAR)-Network, involving patients who met the CASPAR criteria. Diagnostic delay was defined as time interval from symptom onset to PsA diagnosis, categorized as <= 2 years and > 2 years. Temporal trends were assessed by grouping patients based on the year of diagnosis. Various factors including demographics, clinical characteristics, disease activity, quality of life, physical function, disability, fatigue, and well-being were examined. Logistic regression models were used to identify factors associated with diagnostic delay. Among 1,134 PsA patients, mean diagnostic delay was 35.1 months (median: 12). Approximately 39.15% were diagnosed within 3 months, and 67.02% were diagnosed within 24 months. Patients experiencing longer delays had higher scores in Psoriatic Arthritis Quality of Life Questionnaire (PsAQoL), Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-Fatigue), Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), patient's global assessment (PtGA) and physician's global assessment (PhGA). Diagnostic delay has decreased over time, with median delay falling from 60 to 24 months throughout pre-2010 and 2015-2019 terms. Several factors were identified as significant contributors to delayed diagnosis, including lower levels of education (OR = 2.63), arthritis symptoms preceding skin manifestations (OR = 1.72), low back pain at first visit (OR = 1.60), symptom onset age (OR = 0.96), and psoriasis subtype (OR = 0.25). Timely diagnosis of PsA is crucial for effective management and improved outcomes. Despite recent improvements, about one-third of PsA patients still experience delays exceeding 2 years. By identifying influential factors such as education level, arthritis symptoms preceding skin manifestations, initial visit symptoms, age of symptom onset, and psoriasis subtype, healthcare practitioners may create specific techniques to help in early detection and intervention.Article The Effect of Gender on Disease Activity and Clinical Characteristics in Patients With Axial Psoriatic Arthritis(Taylor & Francis Ltd, 2021) Nas, Kemal; Kilic, Erkan; Tekeoglu, Ibrahim; Keskin, Yasar; cevik, Remzi; Sargin, Betul; Tuncer, TirajeObjectives In this study, we aimed to evaluate the effect of gender on clinical findings, disease activity, functional status and quality of life in patients with axial involvement in Turkey. Methods Patients with PsA who met the CASPAR classification criteria were enrolled consequently in this cohort. Turkish League Against Rheumatism (TLAR)-Network was formed with the participation of 25 centres. The demographic variables, fatigue, diagnostic delay, the beginning of peripheral arthritis, enthesitis, dactylitis and spine involvement, inflammatory low back pain, BASFI, HAQ, HAQ-s, visual analogue scale-pain (VAS-pain), anxiety, depression and disease activity parameters (ESR, DAS28, BASDAI) were recorded. Axial involvement was assessed according to clinical and radiological data according to modified New York (MNYC) or Assessment of SpondyloArthritis international Society (ASAS) criteria. Results A total of 1018 patients with PsA were included in this study. Of the 373 patients with axial involvement, 150 were male (40.2%) and 223 (59.8%) were female. Spondylitis was detected in 14,7% of men and 21,9% of women in all patients. Pain score (VAS) (p < .002), fatigue (p < .001), ESR (p < .001), DAS28 (p < .001), BASDAI score (p < .001), PsAQoL (p < .001), HAQ score (p < ,01), HAQ-S score (p < .001), anxiety (p < .001), depression (p < .024), FACIT (p < .001) and FiRST (p < .001) scores were statistically significantly worse in women than males with axial PsA. However, quality of life was better (p < .001) and PASI score (p < .005) were statistically worse in male patients than in female patients with axial involvement. Conclusion This study has shown that the burden of disease in axial PsA has significant difference between genders. Disease activity, physical disability, functional limitation, depression and anxiety scores were higher in female patients, while quality of life were better and PASI score were higher in male patients. There are no studies on the sexual function of male patients with Behcet's disease (BD), but it is probable that male sexual dysfunction may be seen in this chronic condition. Aim. The aim of this study was to assess the effect of BD on male sexual function and psychiatric status, and to examine the relationship between sexual function and depression in this population. Methods. Patients with a diagnosis of BD for at least one year were included in the study. The patients' age, educational level, and duration of disease were recorded. A healthy control group was selected with highly similar characteristics to the patient group. The sexual functions of the patient and the control groups were assessed using the International Index of Erectile Functions (IIEF), and their emotional status was evaluated using the Beck Depression Inventory (BDI). Main Outcome Measures. The results of the questionnaires in patient and control groups were compared. The relationship between the clinical findings and questionnaire scores was assessed in the patient group. Results. Forty-two patients with BD and 42 healthy individuals were included in the study. The mean subscale scores of the IIEF for erectile function were significantly lower in the patient group compared to the control group (P<0.001). The IIEF score was not related to active skin findings, active oral ulcers, active genital ulcers, eye involvement, or medication for BD, but it was related with history of arthritis. The BDI and IIEF scores were negatively correlated in the patient group. Conclusion. BD has a negative impact on men's psychological state and sexual function. The objective of this study was to explore the andinflammatory effects of thymoquinone on arthritis in rat models. Rats with arthritis induced by Freund's incomplete adjuvant were assigned to five groups: group 1: controls 0.9% NaCl (n = 7); group 2: 2.5 mg/kg thymoquinone (n = 7); group 3: 5 mg/kg thymoquinone (n = 7); group 4: Bacilli Chalmette Guerin (BCG) 6 x 105 CFU (n = 7); group 5: methotrexate 0.3 mg/kg (n = 7). Signs of inflammation on the claw and radiological signs were searched for and TNF-alpha and IL-1beta were measured. The results of the control and other groups were compared. As a result, thymoquinone, confirmed clinically and radiologically, suppressed adjuvant-induced arthritis in rats. Patients and methods: Between March 2008 and January 2009, 97 patients with BD and 95 age- and sex-matched healthy controls were included in the study. The patients with BD were divided into two groups according to the presence or absence of FM. All participants were evaluated by the Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Pittsburgh Sleep Quality Index (PSQI), and Medical Outcomes Study Short Form-36 (SF-36). Results: Compared to the controls, FM prevalence was higher (19.6% vs 3.2%) with increased BDI and BAI scores, while the subscores of SF-36 including physical function, physical role, pain, general health, social function, and emotional role decreased in BD patients (for all p<0.001). All BD patients with FM were females (n=19), while only 46% of BD patients without FM were females (n=36) (p<0.001). In BD patients with FM, compared to female BD patients without FM, the mean age (p=0.017), disease duration (p=0.028), and BDI scores (p=0.017) were significantly higher, while the PSQI scores (p=0.001) and SF-36 subscores, except general health (p=0.114), were significantly lower (for all p<0.05). Conclusion: Our study results suggest that patients with BD have an increased severity of depression and anxiety, higher frequency of FM, and worsened QoL than healthy individuals. PsA patients have been recruited by the Turkish League Against Rheumatism-Network from various centers in Turkey in this cross-sectional study. Patients with a body mass index (BMI) >= of 30 kg/m(2) were considered obese. Differences among patients with regard to obesity status were assessed with health-related quality of life measures (PsA Quality of Life Questionnaire [PsAQoL]), psychological status (Hospital Anxiety and Depression Scale [HADS]), and disease activity parameters (the Disease Activity index for PSoriatic Arthritis [DAPSA], Disease Activity Score 28-C-reactive protein [DAS28-CRP], Bath Ankylosing Spondylitis Disease Activity Index [BASDAI], Psoriasis Area and Severity Index [PASI]), physical functions (Ankylosing Spondylitis Functional Index [BASFI], Health Assessment Questionnaire [HAQ], and Health Assessment Questionnaire for the spondyloarthropathies [HAQ-S]). Pain was assessed using visual analog scale of pain (VAS-P), and fatigue was evaluated using visual analog scale of fatigue (VAS-F) and Functional Assessment of Chronic Illness Therapy (FACIT). A total of 1033 patients with PsA, 650 (62.9%) non-obese and 383 (37.1%) obese were included in the study. The PsAQoL, HADS-Anxiety, HADS-Depression, DAPSA, DAS28-CRP, BASDAI, BASFI, HAQ and HAQ-S scores of the obese group were higher than the non-obese group (p < 0.05). VAS-P and PASI scores were similar between group of patients with and without obesity. Obese patients had higher median scores of VAS-F and FACIT than non-obese patients (p < 0.05). Linear regression analysis showed that BMI affects the quality of life, depression, and disease activity. Consequently, obesity has significant associations with higher disease activity, lower QoL, risk of anxiety, depression, and fatigue. Steroid treatment is considered the major risk factor for the development of ON in SLE. ON involving at least tree anatomical localizations is called multifocal osteonecrosis (MFON). We presented a case with MFON diagnosed with SLE seven years ago. The patient has received corticosteroids at various doses and had obvious destruction in the shoulders and hips apparent in plain X-rays. Although the knees seemed normal in X-rays, scintigraphy and magnetic resonance imaging (MRI) revealed ON of the knees. SLE patients with ON in one joint should be screened with MRG and/or scintigraphy in the early stages of the disease even when the plain X-rays seem normaldue to the increased risk of multifocal ON involving other joints.Article Overlap of Multiple Sclerosis and Antiphospholipid Syndrome(Turkish League Against Rheumatism, 2009) Tekeoglu, Ibrahim; Tombul, Temel; Hiz, Ozcan; Gulcu, Elif; Toprak, MuratAntiphospholipid syndrome (APS) and multiple sclerosis (MS) are two diseases that may show similar clinical and radiological findings and sometimes cause confusion. In the presence of thrombosis, repetitious abortion history, and lupus anticoagulant antibodies or anticardiolipin antibodies, APS should be considered. However, in previous studies, autoantibody levels were detected as being high in MS patients when compared to the normal population. Laboratory study of a patient with fever, fatigue, joint pain and cyanosis in the ankles since 1995 and history of two abortions at 6 weeks gestation revealed: C-reactive protein: (++++), erythrocyte sedimentation rate: 70 mm/hour, antinuclear antibodies: 1/1000 homogeneous, and LE cell positivity. Brain magnetic resonance imaging, visual evoked potantial, somatosensorial evoked potential, and oligoclonal band investigations of the patient, who applied to the Neurology Clinic of the hospital in 1999 with complaints of disruption in walking, imbalance and fatigue, were found compatible with MS. The patient had suffered from recurrent attacks and had been directed to our clinic with probable diagnosis of systemic lupus erythematosus when pain in knees and macular style skin lesion on the right ankle finally developed in 2007. Investigations conducted at six-month time intervals revealed anticardiolipin IgG: positive, anticardiolipin IgM: negative and antinuclear antibody (ANA): positive. When ANA and anticardiolipin IgG positivity and abortion history of the patient were evaluated together, APS accompanying MS was diagnosed and 100 mg aspirin was added to the treatment. In conclusion, MS and APS can not only appear together but may also imitate each other. Thus, cases diagnosed as MS should be carefully evaluated in terms of clinical and laboratory findings of APS. Here, we report the first case in the literature of a patient with paraplegia due to primary extradural hydatid cyst who developed a syrinx after surgery. Material and Methods This trial was performed using the data of consultations made by our PM& R department in patients hospitalized in other clinical departments between December 2007 and December 2008. The department which requested the consultation, the reason for the consultation request, the diagnosis made and the treatment recommended by our PM& R Department were all recorded. The results were given in the number of patients and percentages. Results A total of 373 consultations were requested by 17 different departments (186 females and 187 males) throughout the study. The most common diagnoses made by our department were neurological and rheumatological diseases. Conclusions The results of our study suggest that our department is primarily regarded as a rehabilitation unit, while it is also known as a therapeutic clinic, particularly for rheumatological diseases, which we mainly attribute to the presence of a rheumatology division in our department. In the light of our findings, we concluded that diagnosis and therapy of rheumatological diseases should be particularly included in the educational programs of PM& R residents.Article Pleuropericarditis Associated With Sulphasalazine in a Case of Rheumatoid Arthritis: a Drug-Induced Lupus-Like Syndrome(Turkish League Against Rheumatism, 2011) Hiz, Ozcan; Gulcu, Elif; Ozkan, Yasemin; Tekeoglu, Ibrahim; Aladag, NesimPericarditis, a life-threatening condition, can be seen in rheumatic diseases. It may occur secondary to rheumatic diseases or drugs used in the treatment. In this article, we present a case of 47-year-old female, who had joint complaints for 15 years and had treatment of methotrexate and corticosteroid for six years. In the last three months, after the addition of sulphasalazine to her treatment, she developed chest pain, palpitations, rash, fever, and pleuropericarditis and was subsequently diagnosed with systemic lupus erythematosus induced by sulfasalazine. Patients and methods: A multicenter cross-sectional study involving 1,134 PsA patients from 25 referral centers across seven regions was conducted. Demographic and clinical characteristics, comorbidities, joint involvement, extra-articular manifestations, and disease activity measures were evaluated across regions. Results: A total of 1134 PsA patients from seven different geographic regions in T & uuml;rkiye participated in this study. The highest number of participants was from the Marmara region (n=409), with subsequent representation from Central Anatolia (n=370), Aegean (n=139), Mediterranean (n=60), Black Sea (n=60), Eastern Anatolia (n=60), and Southeastern Anatolia (n=36) regions. There were significant variations in demographic profile, including age, body mass index, age of disease onset, educational status, comorbidities, and family history of both psoriasis and PsA. Clinical features, such as enthesitis, dactylitis, uveitis, and joint involvement, demonstrated significant variation across regions. Additionally, disease activity measures, including pain, patient and physician global assessments, acute phase reactants, disease activity indices, quality of life, and functional status, displayed considerable regional differences. Conclusion: This nationwide study revealed substantial regional diversity in demographic data, clinical characteristics, disease activity, and quality of life among PsA patients in T & uuml;rkiye. These findings stress the need to customize treatment approaches to address regional needs and to conduct further research to uncover reasons for disparities. Materials and Methods: A total of 104 male patients with chronic low back pain were enrolled in the study. All subjects completed the Oswestry Disability Index (ODD. Additionally, the intervertebral disc degeneration (DD) was graded according to the Thompson's classification. After bone mineral density was measured the patients were divided into two groups: normal (n=50) and low (n=54) bone density groups. In addition, for comparing the data of patient sub-groups, bone mineral density was measured in 30 healthy subjects. Results: T and Z-scores of the L2 - L4 and the femoral neck were found to be significantly lower in the patient group compared to the control group. In the patient group, there was a positive correlation between the ODI score and the DD score. There were negative correlations between both the ODI score and the DD score and L2-L4 and femoral neck T scores and Z-score. The DD and ODI scores were found to be significantly higher in a patient with low bone density compared to a patient with normal bone density. Conclusion: We think that the low bone mineral density in young and middle-aged male patients with chronic lower back pain is related to disability and physical inactivity. The objective of this study was to explore the anflinflammatory effects of thymoquinone on arthritis in rat models. Rats with arthritis induced by Freund's incomplete adjuvant were assigned into five groups: group 1: controls 0.9% NaCl (n = 7); group 2: 2.5 mg/kg thymoquinone (n = 7); group 3: 5 mg/kg thymoquinone (n = 7); group 4: Bacilli Chalmette Guerin (BCG) 6 x 10(5) CFU (n = 7); group 5: methotrexate 0.3 mg/kg (n = 7). Signs of inflammation on the claw and radiological signs were searched for and TNF-alpha and IL-1beta were measured. The results of control and other groups were compared. As a result, thymoquinone, confirmed clinically and radiologically, suppressed adjuvant-induced arthritis in rats. Material and Method: Total 74 cases with primary knee osteoarthritis (between 40-75 years old, 52 female, 22 male) included the study. Patients assigned randomly into two groups to receive acetaminophen 3000 mg/day or acetaminophen 3000 mg/day plus colchicine 1.5 gr/day for six months. 5 female and 4 male patients withdrew the study because of lost to follow up after six month treatment. No patient discontinued the study because of drug adverse effects. Result: When compared with paired t test, before therapy (bt) WOMAC scores and VAS scores versus visit 1 (v1-1,5. month after initiation of therapy), visit 2 (v23. month after initiation of therapy), visit 3 (v3-4,5. month after initiation of therapy), and visit 4 (v4-6. month after initiation of therapy) WOMAC scores and VAS scores, there was a statistically significant decrease of scores (p< 0.001). This mean both acetaminophen and acetaminophen plus colchicine is effective in the treatment of knee osteoarthritis. When two groups compared each other with impaired student t test, WOMAC pain, morning stiffness and functional scores and VAS scores showed significant decrease in the colchicine group versus acetaminophen group (p< 0.05). This decrease was more apparent in v2 and v3 WOMAC and VAS scores (p< 0.001). Both colchicine and acetaminophen was well tolerated. No serious laboratory abnormality was noted in either group. No patient withdrew because of any drug related adverse events. Discussion: Both acetaminophen 3000 mg/day and 3000 mg acetaminophen plus 1,5 gr/day colchicine is effective in the treatment of patients with knee osteoarthritis. But the addition of colchicine to acetaminophen produced significantly greater symptomatic benefit at v1, v2, v3, v4 than acetaminophen alone. This may show probable disease modifying effect of colchicine in osteoarthritis which require further long period laboratory and radiologic investigations.
