    Accommodation Spasm Following Tetanus Diphtheria (Td) Vaccination

    Accommodation spasm is a rare condition that presents with pseudomyopia usually in children and young adults. Most of the cases are associated with emotional conditions that causes functional spasm. Sometimes, an underlying organic cause can be detected. Tetanus-diphtheria vaccine (Td) is a combined vaccine in toxoid form and performs to prevent infectious diseases caused by Clostiridium tetani and Corynebacterium diphtheriae, respectively. In this case report, we present the clinic features of a 13-year-old girl who applied with decreased vision after Td vaccination and diagnosed with isolated accommodation spasm. © 2020, Yuzuncu Yil Universitesi Tip Fakultesi.     Congenital Hypothyroidism in Axenfeld-Rieger Syndrome

    Axenfeld-Rieger syndrome (ARS) manifests varying degrees of anterior segment dysgenesis and carries a risk of resistant glaucoma. It can affect ocular and extraocular structures derived from the neural crest. A small part of the thyroid gland originates from the neural crest, and defects in thyroid origination can result in thyroid agenesis. Thyroid agenesis is a cause of congenital hypothyroidism.     Conjunctival Lymphangiectasia: a Case Report

    In this article, a 51-year-old man with conjunctival lymphangiectasia who applied to the hospital with conjunctival swelling was presented. Conjunctival resection of lesion was performed and no recurrence was found in the follow-up examination at 9 months. This is an uncommon clinical condition in which conjunctival swelling occurs as a consequence of enlarged lymphatics of the bulbar conjunctiva. © 2018, Yuzuncu Yil Universitesi Tip Fakultesi.     Effect on Intraocular Pressure of Intravitreal Dexamethasone Implant

    Introduction: To investigate the effect of intravitreally administered 0.7 mg dexamethasone implant on intraocular pressure. Materials and Methods: The study included patients with macular edema due to retinal vein occlusion or diabetic retinopathy who underwent intravitreal dexamethasone implant administration. Patients with glaucoma, ocular hypertension, and neovascularization at the angle were excluded from the study. The intraocular pressure was measured using applanation tonometry before and one week, two weeks, one month, three months, and six months after the procedure. Topical anti-glaucomatous is initiated in the participants with intraocular pressure equal to or higher than 22 mmHg. Results: One hundred and five eyes of 101 patients comprising 55 (54.45%) male and 46 (45.54%) females were included in the study. The mean intraocular pressure was 14.43±2.53 mmHg before injection, and the mean postprocedure intraocular pressures were 15.31±3.66 mmHg (p=0.132) at the first day, 14.87±3.11 mmHg (p=0.371) at the first week, 16.78±3.82 mmHg (p=0.001) at the second week, 17.38±4.30 mmHg (p=0.001) at the first month, 16.92±4.55 mmHg (p=0.001) at the third month and, 15.59±2.78 mmHg (p=0.005) at the sixth month, respectively. Five patients (%4.8) experienced intraocular pressure rise >21mmHg after the procedure in which IOP was controlled with anti-glaucomatous medication in all of them. Conclusion: Intravitreal dexamethasone implantation may be interpreted as a safe treatment modality in patients with retinal vein occlusion or diabetic retinopathy in terms of secondary IOP increase.     The Effects of Corneal Edema on the Intraocular Pressure Measurements

    We aimed to evaluate the possible effects of corneal edema on intraocular pressure (IOP) measurements obtained with three different tonometers on the first day after cataract surgery. Sixty-four eyes of the 64 patients underwent phacoemulsification and IOP measurements using Goldmann applanation tonometer (GAT), Tono-Pen applanation tonometer (TAT), and Pascal digital contour tonometer (DCT) the first day after surgery. Central corneal thickness (CCT) measurements were used to quantify the postoperative corneal edema. Intraclass correlation coefficient and Bland-Altman plot were used to assess the compatibility of IOP measurements among three method pairs. TAT and DCT IOP measurements were found to be compatible with each other (p=0.141) in the corneal edema group. The GAT and TAT IOP measurements in the eyes with corneal edema were inversely proportional to the CCT values (p=0.011 and p=0.033, respectively), but the DCT relationship was not statistically significant (p=0.44). The lack of a significant relationship between DCT and CCT in edematous corneas may suggest that DCT, as in normal corneas, can perform CCT-independent measurements.     A Rare Complication in Intravitreal Dexamethasone Implantation: Migration of the Implant To the Anterior Chamber

    Intravitreal dexamethasone implant (Ozurdex®; AllerganInc., Irvine, CA); It was approved by FDA (Food and Drug Administration) for central retinal vein occlusion, retinal vein occlusion and macular edema secondary to uveitis secondary. One of the rare complications of intravitreal dexamethasone implantation is the migration to the anterior chamber of the implant.     Relationship Between Ocular Pseudoexfoliation and Sensorineural Hearing Loss

    The purpose of this study is to evaluate the air and bone conductance rates in audiogram between healthy individuals and the patients having pseudoexfoliation (PEX). Patients with evidence of ocular PEX and controls were included in the study. Air conduction hearing thresholds (minimal perceptible sounds) were determined at frequencies of 250, 500, 1000, 2000, 4000, 6000, 8000 Hz and bone conduction hearing thresholds were determined at frequencies of 500, 1000, 2000, 4000 Hz for each ear. At these frequencies, air and bone conductance rates in audiogram were measured for patients with PEX and control group to analyze the relation between conductance rates and PEX cases. 86 participants were enrolled in this case-control study, in which 40 of them having PEX. The acquired results showed that the PEX group displayed significantly higher hearing tresholds in air conductance compared with control subjects a t frequencies of 250, 500, 1000, 2000, 4000 Hz, but not at frequencies of 6000, 8000 Hz. Similar results were obtained in bone conduction thresholds as well, in terms of the tendency of decreasing significance level between the groups in higher frequencies. In our study, we have shown that there is a statistically significant sensorineural hearing loss in PEX patients comparing age control subjects. These findings are more significant in lower frequencies, and the significance level is decreased toward the higher frequencies. Based on these findings, we speculate that endolymphatic pressure increase could have a role in the pathogenesis of PEX related sensorineural hearing loss. © 2021, Yuzuncu Yil Universitesi Tip Fakultesi.     A Year With Intraorbital Tree Branch

    Intraorbital foreign bodies are relatively rare cases among eye emergencies. A complete ophthalmologic examination, supported by detailed anamnesis and diagnostic imaging, is impor-tant for each patient with ocular trauma. The first choice imaging method for suspected orbital foreign body is computed tomography (CT). Diagnosis is relatively easy in these cases, as CT can even de-tect metallic objects approximately 2 mm in size. Wooden orbital foreign bodies are seen low-density and even may sometimes not be seen in CT. Also, the change in their structure over time may cause more difficulty in diagnosis. In this article, we present a patient who was di-agnosed with a wooden structure intraorbital foreign body by oph-thalmological examination and magnetic resonance imaging, one year after ocular injury with the tree branch. © 2020 by Türkiye Klinikleri.
