Article Calcaneal Brown Tumor With Primary Hyperparathyroidism Caused by Parathyroid Carcinoma: an Atypical Localization(2004) Dogan, A.; Algün, E.; Kisli, E.; Harman, M.; Kösem, M.; Tosun, N.Brown tumors are one of the characteristics of primary hyperparathyroidism, although, in some cases, they are noted with secondary hyperparathyroidism as well. The authors present a case of a 50-year-old woman with primary hyperparathyroidism caused by parathyroid carcinoma with an unusual location of a brown tumor in the calcaneus. She first presented with pain and swelling over the heel and ankle, and the diagnosis was suspected by radiographs. Biopsy of the calcaneal lesion confirmed a brown tumor. After the parathyroid lesion was removed surgically, her symptoms were relived. The calcaneal lesion was treated with immobilization of the foot.Article Effects of Intra-Articular Injections of Sodium Hyaluronate (Orthovisc) and Betamethasone on Osteoarthritis of the Knee(1998) Tekeoglu, I.; Adak, B.; Goksoy, T.; Tosun, N.The purpose of this study was to compare the efficacy and tolerability of intra-articular injections of 20 mg of Sodium Hyaluronate and 3 mg- betamethasone in an open randomized trial. 40 patients with gonarthritis were randomly allocated in 2 different groups of 20 people. All subjects included in the groups were female with mean age of 57.84 57.84±5.66 and 58.26±6.05 years. Intra-articular injections were administered once a week for three weeks. All patients suffering from inflammatory knee osteoarthritis were examined prior to and after the study. Efficacy rates of the two treatment methods during subsequent examinations in the 3(rd) and 15(th) weeks were noted. WOMAC assessment, Visual Analogue Scale, clinical severity and radiological severity according to Kellgren Lawrance index were also assessed in both groups. Both medications were well tolerated with no complications. Results in the 3(rd) week were in favour of betamethasone group. However there were clinically significant difference in favour of Sodium Hyaluronate (Orhovisc) treatment group on the 15(th) week. According to results of 15 week follow-up study comparing the efficacy and tolerability rates between Sodium Hyaluronate and Betamethasone, Hyaluronate injection could be more effective in osteoarthritis on the long term basis.Article The Effects of Nicotine on Fracture Healing: an Experimental Study(2002) Agca, M.; Tuncay, I.; Tosun, N.; Akpinar, F.This study is aimed to evaluate treatment research the effects of nicotine on fracture healing. Thirty rats were used. Nicotine was injected subcutaneously at a dosage of 2 mg/kg/day to group A and 4 mg/kg/day to group B. Control group (group C) received 0.5 ml/kg/day saline solution. Three weeks later, transverse fractures were performed on to the left ulnar and radial bones for each animal. Animals were designed to one of two follow-up groups which were euthanized either 2 or 3 weeks. On the 14th day of fracture, randomly chosen five rats from each group of A, B and C were sacrificed. At the 21st day of fracture, same procedure performed for all remaining rats. Both had surgical and histopathological evaluation were performed. Transverse fractures performed on the 21st day of study were confirmed by direct radiograms. One rat died spontaneously before completing the study was therefore excluded from the final analysis. The results indicate that healing process proceeds in each group. There was no significant difference in fracture healing between groups A and B two weeks after the fracture. However, fracture healing was significantly delayed in nicotine treated group comparing to control group. We continue to support the notion that nicotine has a negative effect on fracture healing.Article Rice Bodies in Knee Joint: a Case Report(2002) Tuncay, I.; Tosun, N.; Uǧraş, S.; Ince, Ö.Rice bodies with synovial origin resemble cartilage and they are usually sourced of fibrin. They are supposed as synovial chondromatosis wrongly. Tuberculosis, rheumatoid arthritis or other seronegative arthropathies may exist in the etiology. Twenty-one years old patient with complainted of occasional swelling and pain, underwent diagnostic arthroscopy. Massive rice bodies with synovitis in the joint were observed in the knee arthroscopy under local anesthesia. Afterward, open surgical extirpation with subtotal synovectomy was performed. Fibrinous structure was observed histopathologically. No recurrence was found during follow up period, also no underlying tuberculosis or rheumatologic pathology was detected.Article Tibial Osteomyelitis Following Intraosseous Infusion: a Case Report(2004) Dogan, A.; Irmak, H.; Harman, M.; Ceylan, A.; Akpinar, F.; Tosun, N.Fluids, medications, and blood products can be rapidly administered via intraosseous infusion under emergency conditions, particularly to pediatric patients aged from 0 to 2 years. A five-month-old infant who had been hospitalized with a diagnosis of sepsis developed swelling and hyperemia at the infusion site 10 days after an intraosseous infusion in the right proximal tibia. Physical examination showed a serous discharge from a fistula on the anteromedial side of the right proximal cruris. Plain radiographs demonstrated periosteal reaction in the right tibia and osteolytic areas in the proximal metaphysis. With a diagnosis of acute osteomyelitis, drainage and medullary irrigation were performed and parenteral antibiotic treatment was initiated. Cultures from the surgical site yielded Candida albicans, upon which fluconazole (8 mg/kg) treatment was administered for four weeks. A complete clinical and radiographic improvement was observed at the end of a 12-month follow-up.Article Variations of Abductor Pollicis Longus and Extensor Pollicis Brevis Muscles: Surgical Significance(1998) Aydinlioǧlu, A.; Tosun, N.; Keles, P.; Akpinar, F.; Diyarbakirli, S.To investigate variations of the abductor pollicis longus and the extensor pollicis brevis muscles, an anatomic study was performed on 15 cadaver forearms. Some variations were observed as reported in previous studies. An unusual insertion of the abductor pollicis longus muscle and the extensor pollicis brevis muscle also was encountered during the dissection. Both tendons of these muscles were found to have inserted into the inferior side of the base of the first metcarpal bone together, instead of dorsal side. Our study suggested that the extensor pollicis brevis muscle and the abductor pollicis longus muscle differentiate from a common muscle.
