Clinical, laboratory and postmortem findings are described. Angora goats were examined clinically and biochemical analysis of blood, electrocardiogram and histopathological examinations were carried out. All rights reserved.Article Anti-Fibrogenic Effects of Captopril and Candesartan Cilexetil on the Hepatic Fibrosis Development in Rat - the Effect of At1-R Blocker on the Hepatic Fibrosis(Elsevier Gmbh, 2003) Tuncer, I; Ozbek, H; Ugras, S; Bayram, IBackground/Aim: Angiotensin converting enzyme (ACE) and angiotensin II (AT-II) have been suggested to play an important role in liver fibrogenesis. There is a significant relationship between inheritance of hightened expression of transforming growth factor beta1 (TGF-beta1) and AT-II and the development of progressive hepatic fibrosis. The purpose of this study was to investigate the effects of captopril. an ACE inhibitor and candesartan cilexetil, an AT-II type I receptor (AT1-R) blocker, on liver fibrosis induced in rats by carbon tetrachloride (CCl4) administration. Methods: rats were divided into 4 experimental groups: The first group was given CCl4 alone; the second was given both CCl4 and captopril (100 mg(.)kg(-1.)day(-1)); the third was given both CCl4 and candesartan cilexetil (8 mg(.)kg(-1.)day(-1)); fourth group was given 0.9% NaCl only. Seven weeks after initiating the treatment. indices of fibrosis were assessed. Results: Candesartan cilexetil treatment significantly reduced the fibrosis development. These inhibitory effects were not observed in the captopril-treated group. The mean fibrosis score was significantly lower in the CCl4/candesartan group compared with the group applied to CCl4 alone and the group applied to CCl4/captopril. Similarly, the number of alpha-smooth muscle actin positive cells was markedly suppressed by candesartan treatment. Conclusions: The results suggest that AT-II plays a pivotal role in hepatic fibrogenesis and candesartan significantly attenuates the progression of liver fibrosis. This drug may provide an effective new strategy for prevention of liver fibrosis. Its effectiveness should be investigated in chronic liver disease associated with progressive fibrosis.Article Assessment of Testicular Cytology by Fine-Needle Aspiration and the Imprint Technique: Are They Reliable Diagnostic Modalities(Blackwell Science Ltd, 1997) Odabas, O; Ugras, S; Aydin, S; Yilmaz, Y; Atilla, MKObjective To investigate whether testicular cytology may be considered diagnostic in the evaluation of infertile men. Patients and methods Specimens of testicular tissue obtained either surgically (imprint smear) or through fine-needle aspiration (FNA) were used as a source of cytological smears; 58 testes from 24 men with azoospermia or severe oligospermia and from five men with advanced prostate cancer were evaluated cytologically and compared with the histological diagnosis. Results FNA caused no apparent trauma, The results from FNA smears generally agreed with the histological findings but four patients with no spermatozoa in the FNA smears were diagnosed histologically as hypospermatogenic and two others judged histologically as having Sertoli-cell-only (SCO) syndrome and spermatogenic arrest had detectable spermatozoa in their FNA smears. There was complete agreement between the results of imprint smears and histological findings in those patients with SCO syndrome and spermatogenic arrest, There were no evident differences in sperm counts between hypospermatogenesis and normal spermatogenesis on the imprint slides, but FNA smears detected this difference. Conclusion FNA of the testis is a relatively non-invasive and reproducible technique for evaluating qualitative and quantitative cytology. However, it is insufficient for diagnosing some testicular pathologies. Atypical fibroxanthoma (AFX) is a pleomorphic tumour that occurs infrequently on the limbs and trunk in children, We report a child with XP who presented with AFX of the facial skin and the lower lip, The diagnosis of AFX was confirmed using histological and immunohistochemical techniques. We discuss the possibility that ultraviolet-induced damage might be implicated in the pathogenesis of AFX.Article Bcg Vaccination To Prevent Implantation of Endometriosis(Elsevier Sci Ireland Ltd, 2001) Gül, A; Yasar, T; Ugras, STo investigate whether BCG vaccination can prevent endometrial implantation in a rat model. Forty sexually mature virgin Wistar-Albino rats weighing 185-215 g were randomly assigned (double-blind) to two groups. The first group (n = 20) were injected with BCG 3 weeks before endometrial implantation to the eye. The second group (n = 20) with BCG not injected before endometrial implantation was the control. Photobiomicroscopy observation was done weekly and 6 weeks post endometrial implantation all eyes were investigated histologically. Five implants grew in the anterior surface of the iris of the first (vaccinated) group and 17 in the second (control) group. The difference was statistically significant. Systemic prophylaxis with BCG can exert an inhibitory effect on endometrial transplantation. (C) 2001 Elsevier Science Ireland Ltd. All rights reserved.Article Blood and Tissue Findings in the Diagnosis of Mesenteric Ischaemia: an Experimental Study(Walter de Gruyter Gmbh, 1998) Aydin, M; Guler, O; Ugras, S; Bakir, B; Sekeroglu, RIn this experimental study, after inducing mesenteric ischaemia in dogs, we investigated routine blood biochemistry, serum malondialdehyde (MDA) levels, histopathological changes and tissue MDA levels in the liver and intestines. The study group included 10 dogs. Six dogs which underwent sham operation served as controls. In the study group, the MDA level in liver tissue increased after superior mesenteric artery (SMA) ligation. Various degrees of necrosis were seen in the ileal biopsies 10 hours after the ligation. Hyperaemia and focal necrosis in the liver accompanied the intestinal necrosis if it was limited within the mucosa. However, massive or centrilobular necrosis was observed in the liver of those dogs which had intestinal necrosis that extended into muscular layers. MDA levels in intestinal tissue measured after SMA ligation were significantly higher than the preligation levels. On the other hand, the difference between serum and tissue MDA levels was not significant. The alterations in MDA were not significant in the sham group. She had been hospitalized with the diagnosis of brucellosis in another hospital. At presentation, she had paraplegia, sphincter dysfunction, and bilateral sensory loss below the T6 level, and was initially diagnosed with transverse myelitis caused by brucellosis. On the third day of hospitalization, however, agglutination test for brucella was negative, but it was positive for Salmonella. Therefore, transverse myelitis was considered to be due to salmonellosis. Thoracic spine magnetic resonance imaging showed an extradural, paraspinal mass at the level of T6-T7. The mass was totally extracted, and histopathological examination revealed Ewing's sarcoma. During follow-up, no improvement in paraplegia was noted and an enlarged presacral decubital ulcer developed. Aside from supportive care, local radiotherapy was applied. Unfortunately, the patient died from probable infection 9 months after the diagnosis. We describe the case of a 57-year-old man who presented with high fever, weight loss, palpitations and a tender goitre. To achieve this ultrasonography was performed preoperatively and ruler assessment postoperatively. These measurements were then compared and evaluated using the Wilcoxon test. The results were not statistically significant (p=0.6592, z=0.4410). Plain films and CT showed extensive, full thickness, and skull destruction at the vertex. Gadolinium-enhanced MRI revealed neoplastic invasion of the meninges and both cerebral hemisphers down to the lateral ventricle. This case represents an example of (SCC) imitating a primary brain tumor by exhibiting intracranial extension.Article Desmoplastic Medulloblastoma in a 48-Year Male(Tohoku Univ Medical Press, 2004) Bayram, I; Ibiloglu, I; Ugras, S; Yilmaz, N; Harman, MMedulloblastoma is a malignant invasive embryonal tumor of the cerebellum with preferential manifestation in children. The peak of occurrence is seven years of age Seventy percent of medulloblastomas occur in individuals Younger than 16. In adulthood, 80% of medulloblastomas arise in the 21-40 years age group. A 48-year-old male patient was admitted to the hospital with complains of headache, ataxia, morning vomitting and difficulty in speech was operated with the diagnosis of presence of mass of 4 x 7 cm size retaining a diffuse homogenous contrast in the posterior fossa. The diagnosis of desmoplastic medulloblastoma was given after histopathological examination. Immunohistochemical examination revealed that neoplastic cells showed staining with neuron-specific enolase and synaptophysin but not with glial fibrillary acidic protein. This lesion showed nodular, reticulin free-zones (pale islands) surrounded by densely packed, highly, proliferative cells. The pale regions within the tumor did not contain retictifin fibers. Desmoplastic medulloblastoma is encountered especially in adulthood. This type of tumor rarely occurs beyond the fifth decade of life. We present a case of desmoplastic medulloblastoma in a 48-year-old male. - desmoplastic medulloblastoma; adulthood; posterior fossa (C) 2004 Tohoku University Medical Press.Article The Diagnosis by Fine Needle Aspiration Biopsy of Hydatid Cyst of the Pancreas(Assoc Soc Scientifique Med Belges, 1997) Ugras, S; Sakarya, ME; Arslan, H; Bozkurt, M; AKdeniz, HWe describe a case of pancreatic hydatid cyst in which the definitive diagnosis was made at fine needle aspiration cytology. The most common site of hydatid cyst is the liver (65%), followed by the lungs (25%). The hydatid cyst of the pancreas is rare since it accounts for less than 1% of the various sites of hydatid disease. The diagnosis may be difficult when the presentation is that of an unexplained epigastric mass or cyst, despite suggestive radiological and ultrasonographic features. Modern serology tests are positive in up to 80% of the abdominal hydatid cysts. It is mandatory to obtain a fine needle aspiration biopsy for definite diagnosis and for appropriate treatment planning. Surgical removal of the hydatid cyst still remains the most effective treatment.Article Effect of Intra-Articular Administration of Hyaluronan and Cortisone on Secondary Osteoarthritis of the Infected Rabbit's Knee(Tohoku Univ Medical Press, 2001) Karakok, M; Ugras, S; Tosun, N; Fuat, A; Aydin, AHyaluronan is known to play an important role in the healing of degenerative osteoarthritis. But there is no clear consensus how it effects on osteoarthritis compare to cortisone. The purpose of the research was to determine the comparative effects of hyaluronan and cortisone on the healing of degenerative osteoarthritis. A rabbit model used in which & degenerative osteoarthritis were created in the articular cartilage by the inoculation of Staphylococcus Aureus (S. Aureus). The rabbits divided into two groups namely injected with hyaluronan (group A) and cortisone (group B) at days 20, 23 and 26. After hyaluronan and cortisone injections for treatment of osteoarthritis at day of 25 and 35 six rabbits (each of three injected with hyaluronan and cortisone) and at day 50 (six of injected with hyaluronan and five of cortisone) were sacrified. In biopsies obtained from these knees both articular cartilage degeneration and inflammation around the joint were examined by light microscopy. It was seen that the histopathologic findings were consistent with active chronic inflammation at day 10, followed by chronic inflammation. At day 5 of treatment the degeneration decreased (60%) in group A, and clinical findings disappeared after day 9 of treatment. At day 15 of hyaluronan treatment the degeneration degree regressed and at day 30 absolutely healed (all except :)ne). In group B, at day 5 of treatment, the clinical findings disappeared and the healing rate was 72%. At day 15 of cortisone treatment, the degeneration slightly increased and at day 30 the degeneration degree increased more than that day of 15. The results showed that cortisone is effective in the treatment of cartilage degeneration and inflammation early in the course of the septic arthritis whereas the therapeutic effect of hyaluronan is higher late in the course of the disease. Further studies are required in order to understand the effectiveness of the combined or consecutive use of these drugs in degenerative osteoarthritis.Article The Effect of Lymphatic Blockage on the Amount of Endotoxin in Portal Circulation, Nitric Oxide Synthesis, and the Liver in Dogs With Peritonitis(Springer verlag, 1999) Güler, O; Ugras, S; Aydin, M; Dilek, FH; Dilek, ON; Karaayvaz, MThis study was performed to investigate the effect of lymphatic blockage on the amount of endotoxin in portal venous blood, nitric oxide synthesis, the release of aspartate aminotransferase (AST) from the liver, hepatic damage, and survival in an experimental model of dogs with peritonitis, The dogs were divided into a control group (group 1), an unligated thoracic duct peritonitis group (group 2), and a ligated thoracic duct peritonitis group (group 3), Peritoneal fluid and blood from the portal vein and femoral artery were taken for peritoneal culture, endotoxin, and AST assay, respectively, and liver biopsies were performed to assess for hepatic damage and for nitric oxide assay, There was a higher bacteria count in the peritoneal fluid from group 3 than in that from group 2 (P < 0.0001), Bacteria grew in all of the blood cultures from the group 2 animals, but growth was seen only in blood cultures from four of the group 3 animals. The levels of endotoxin, nitrite, and AST levels in group 3 were significantly increased in comparison with those in group 2 (P < 0.0001), Extensive hepatocellular necrosis,vith hemorrhage was observed in the livers of the group 3 animals, and all of them died,within 48 h, The results of this study suggest that the blockage of lymph flow has a negative effect on liver and survival in dogs with peritonitis, and that hepatic damage is directly related to the amount of endotoxin to which the liver is exposed,Article The Effect of the Suture-Induced Delay Phenomenon on Skin Flap Survival and Lipid Peroxidation in Rats(Lippincott Williams & Wilkins, 1998) Bekerecioglu, M; Kutluhan, A; Ugras, S; Akpolat, NSkin flap ischemia has been associated with the presence of free radicals. In this study we designed a suture-induced delay model in 30 rats using two different suture techniques. A total of 13 rats that underwent McFarlane's acute random-flap model served as controls. We found a statistically significant difference between the flaps of the experimental and control groups regarding the viability of flaps. Malondialdehyde levels in tissue samples taken from the proximal and distal ends of each flap were estimated at 6 and 24 hours postoperatively. Malondialdehyde levels in the experimental groups were found to be quite lower than those of the controls. In conclusion, the suture-induced delay model seems to be effective in improving flap viability. Also, we found lower malondialdehyde levels in experimental groups compared with controls.Article The Effect on Survival of Thoracic Duct Ligation in Experimental Peritonitis(H G E Update Medical Publ Ltd., 1999) Aydin, M; Guler, O; Yigit, MF; Ugras, S; Bozkurt, HBACKGROUND/AIMS: It has been shown that systemic bacteremia and endotoxemia in peritonitis is mainly related to lymphatic transport via the thoracic duct. This study was performed to investigate the effect on mortality of thoracic duct ligation in experimental peritonitis. METHODOLOGY: Thirty dogs were divided into three groups. Groups I, II, and LII were control, unligated, and ligated thoracic duct peritonitis groups, respectively. Liver biopsy, blood and peritoneal fluid cultures were taken and survival time was established. RESULTS: Bacteria were determined in peritoneal fluid in all animals in groups II and III. Growing bacteria numbers in group III were two times higher than in group II. While bacterium was grown on blood cultures in all group II animals, growing was determined on blood cultures in only 2 animals in group III. Diffuse necrosis was determined in the liver of 2 animals who died within 72 hours in group II. Another 8 animals had minimal focal necrosis in their livers. Diffuse and progressive necrosis was determined in the liver of all animals in group III. The difference between liver necrosis in group II and group III was found to be statistically significant (p=0.002). CONCLUSIONS: This experimental study demonstrates that thoracic duct ligation decreases bacteremia rates clearly but that mortality increases significantly.
