Article Does Smoking Affect the Onset Time of Sensory Blocks or the Duration of Motor Blocks in Parturient Women? a Randomized Controlled Trial(Wolters Kluwer Medknow Publications, 2022) Gunes, H. A.; Yuzkat, N.Background: In general, smoking or exposure to secondhand smoke is still common worldwide, and the rate of smoking in women of childbearing age is gradually increasing. Cesarean section rates have been increasing in recent years, and anesthesia guidelines recommend regional anesthesia for cesarean sections. Since nicotine and local anesthetics have different effects on ligand-gated ion channels, smoking may affect spinal anesthesia in pregnant women. Aim: The aim of this study was to investigate the effects of smoking on spinal anesthesia, which is applied for cesarean sections in pregnant women. Patients and Methods: After approval from the institution's ethics committee, 100 pregnant women were divided into two groups: current smokers (Group S, smoker) (n = 50) and lifelong nonsmokers (Group NS, nonsmoker) (n = 50). The dose of local anesthetic was adjusted according to the height of each patient. After free cerebrospinal fluid flow was observed, all patients were given 20 mu mu g of fentanyl in 0.05 mg/cm hyperbaric 0.5% bupivacaine within 10 seconds. The onset of sensory and motor block, the duration of sensory and motor block, and the visual analogue scale (VAS) score were monitored. Results: Data from 100 parturient women were investigated. Even though the median time required for the onset of sensory block to occur was significantly higher in Group S (P = 0.019), the duration of motor block was found to be shorter (P = 0.003); however, the duration of sensory block was similar in both groups (P = 0.771). VAS scores were significantly higher in Group S (P = 0.001). Conclusions: In conclusion, the pregnant women who smoked had longer motor block onset times, shorter motor block durations, higher VAS scores, and lower patient satisfaction levels.Article Effects of Increasing Positive End-Expiratory Pressure (Peep) Values on Intraabdominal Pressure and Hemodynamics: a Prospective Clinical Study(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Dumanlidağ, U.S.; Yuzkat, N.; Soyalp, C.; Gülhas, N.In the present study, the purpose was to compare the effects of Positive End-Expiratory Pressure (PEEP), which is applied to intensive care patients, on Intraabdominal Pressure (IAP) and hemodynamic parameters. The patients were selected from among the patients who received mechanical ventilator support and PEEP at various levels in Intensive Care Unit, who were between 18-80 years of age, who did not have abdominal surgery. In the present study, a total of 64 patients were divided into 3 groups. Those who had PEEP value at 4 cmH2O were included as Group 4, those with PEEP value between 5-8 cmH2O were included in Group 8, and those with PEEP value between 9-12 cmH2O were included in Group 12. The intraabdominal pressures, central venous and arterial blood pressures, heart rates, peripheral oxygen saturation values, body temperatures, fluid balances and urine volumes were measured at 0, 6, 12, 18 and 24th hours. The Intra-Bladder Pressure Measurement Method was employed to measure the intra-abdominal pressure. The lowest IAP values were measured in Group 4, and the highest values were measured in Group 12. The IAP values that were measured in Group 12 were higher than the other groups at a significant level. The intra and inter-group blood pressures, body temperatures, urine outputs, central venous pressures, and fluid balance values were similar in all groups. In the present study, it was concluded that IAP was low in low PEEP values, and the IAP was high in higher PEEP levels; and this increase caused mild intrabdominal hypertension; however, did not affect hemodynamics. © 2019, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Effects of Sedation Doses of Propofol and Midazolam on Levels of Ngal, Cystatin-C, Kim-1 in Rats(Yuzuncu Yil Universitesi Tip Fakultesi, 2022) Soyalp, C.; Kömüroğlu, A.U.; Yuzkat, N.; Basbugan, Y.; Tunçdemir, Y.E.Limited data are available regarding the impact of their sedation doses midazolam and propofol on early biomarkers of acute kidney injury (AKI). This study aimed to investigate the effects of sedation doses propofol and midazolam on early biomarkers such as neutrophil gelatinase-associated lipocalin (NGAL), cystatin C (CyC) and kidney injury molecule 1 (KIM-1) of AKI. A total of 24 Wistar albino rats were separated into three groups (n = 8 per group): a control group (intraperitoneal injecti on [IP] saline injection once daily for 7 days), a propofol group (IP injection of 2.5 mg/kg propofol once daily for 7 days), and a midazolam group (IP injection of 5 mg/kg midazolam once daily for 7 days). For each group, urinalysis (for urea, creatinine, total protein, NGAL, CyC, and KIM-1) was performed on Day 0 and Day 7; serum analysis (for urea, creatinine, total protein, albumin globulin, ALT, AST, NGAL, CyC, and KIM-1) was performed on day 7. No significant difference was noted between control, propofol and midazolam groups in terms of Day 7 serum KIM-1, CyC, and NGAL levels and Day 0 and Day 7 urinalysis findings (KIM-1, CyC, NGAL, urea, and creatinine levels). DISCUSSION AND CONCLUSION: The findings revealed a similar safety profile for seven-day propofol and midazolam administration in rats in terms of the traditional (creatinine, urea) and early biomarkers (NGAL, CyC, KIM-1) of AKI. © 2022, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article The Efficiency of Hemodialysis in Transfusion-Related Acute Lung Injury (Trali): a Case Report(Yuzuncu Yil Universitesi Tip Fakultesi, 2016) Goktas, U.; Yuzkat, N.; Soyoral, L.; Cegin, M.B.; Kahraman, A.Transfusion-Related Acute Lung Injury (TRALI) is an important life-threatening complication that is related with blood transfusion. The frequency is reported as 1/5.000. It is generally characterized with hypoxia that appears at the 2-6th hours after the blood transfusion, bilateral infiltration in the chest radiography, and non-cardiogenic pulmonary edema. Acute respiratory distress, hypotension, tachycardia and fever accompany the clinical picture. Past surgery, blood transfusion, and sepsis are among the factors that trigger the disease. In this study, the efficiency of the hemodialysis applied in the right time in the treatment of a heavy TRALI case developed after a blood transfusion has been presented. © 2017 Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Knowns and Unknowns About Regional Anesthesia Techniques and Local Anesthetics(2013) Isik, Y.; Goktas, U.; Bilal Cegin, M.; Yuzkat, N.; Kati, I.In this study, a questionnaire was used to obtain information about the ideas and experiences of doctors working as operating staff from several specialties in regards to regional anesthesia and local anesthetic drugs to determine the extent to which their knowledge corroborates these types of application and to obtain data for studies to fulfill the future needs of this field. The questionnaire consisted of 18 questions about the use of regional anesthesia and local anesthetic drugs at the time of an operation at nearby hospitals. In total, 109 doctors from 12 branches were included in this study. Although the preference of doctors regarding the selection of regional anesthesia as the first priority varied by field, this choice was not affected by variables such as age, being a research assistant or specialist and working in a state or private hospital. Doctors who were 25-30 years old had more information about local anesthetic drugs than doctors who were 40 years old and older, and this difference was significant. Because doctors aged 40 years and older had less information about local anesthetic drugs among all the doctors studied, training doctors via postgraduate education about this subject would be a better decision.Article Platelet Dysfunction and Gross Bleeding Associated With Uremia(Yuzuncu Yil Universitesi Tip Fakultesi, 2015) Goktas, U.; Yuzkat, N.; Isik, Y.; Çegin, M.; Palabiyik, O.; Kati, I.Patients with chronic kidney disease (CKD) are prone to bleeding due to the platelet dysfunction caused by uraemia. Therefore, the mortality and morbidity in patients with chronic kidney disease increase. The 81-year-old female patient, in dialysis dependent CKD, had an emergency cholecystitis operation with uremia-induced platelet dysfunction has evolved. Patient with gross bleeding intraoperatively had a cardiac arrest during operation. After cardiopulmonary resusitation cardiac impulses were started and patient was taken into intensive care unit. After the administration of estrogen and Factor VIII Inhibitor Bypass Activity treatment for the bleeding diathesis, bleeding has reduced and after 6 days from the operation patient was externed to his service. The chronic kidney disease may lead to hemorrhagic diathesis and unexpected abundant bleeding by creating a platelet dysfunction. All rights reserved.Article Pregabalin Administered as an Anxiolytic Agent in Ultrasound-Guided Infraclavicular Block: a Controlled, Double-Blind, Dose-Ranging Trial(verduci Publisher, 2016) Cegin, M. B.; Soyoral, L.; Yuzkat, N.; Baydi, V.; Goktas, U.OBJECTIVE: Adjuvant agents, given with local anesthetics or via venous, oral, or rectal routes for peripheral nerve blocking, have been in use for a long time. Literature studies about pregabalin usage in peripheral nerve blocking are limited in number. In this study, we aimed to reveal the blocking quality of pregabalin administered orally in various doses as an anxiolytic agent and its effective dose range. PATIENTS AND METHODS: Eighty patients who underwent upper extremity bone surgery were included in the study. The cases were divided into 4 randomized groups of 20 patients. The group that did not receive any medication before the surgery was named the Control Group (Group C), the group that received 75 mg pregabalin per os was named Group P75, the group that received 150 mg pregabalin per os was named Group P150, and the group that received 300 mg pregabalin per os was named Group P300. The study had a controlled and double-blind design. Before, during and after routine peripheral nerve blocking, vital signs, Ramsey Sedation Scale, Patient Satisfaction, Visual Analog Scale, and termination durations of sensorial and motor blocks were recorded. RESULTS: Motor block initiation durations of all groups given pregabalin were significantly shorter than those of Group C. Sensorial block termination durations were similar in Group P150 and Group P300, and both were significantly longer than those in Group C and Group P75. First analgesic requirement time for Group P150 and Group P300 were significantly longer than that of Group P75. Although there was no significant difference between postoperative patient satisfaction and VAS values, first analgesic requirement times of the pregabalin administered groups were longer than those of the control group. CONCLUSIONS: The patients, who are about to undergo surgery, generally develop anxiety about death, not waking up from anesthesia, disability, pain and loss of ability to work. Pre-gabalin is an anti-epileptic, analgesic and anxiolytic agent. With these characteristics, it can be used to reduce pre-operative anxiety, for prophylaxis against convulsions and post-operative analgesia. One hundred fifty mg of pregabalin provides sufficient and effective analgesia, and this dose positively affects the quality of the block.Article Safe Inhalational Anesthetics in Patients With Coronary Artery Disease: Effects of Sevoflurane and Desflurane on Ischemia-Modified Albumin Levels(Yuzuncu Yil Universitesi Tip Fakultesi, 2024) Soyalp, C.; Turk, O.; Oksüz, E.; Yuzkat, N.; Demir, C.; Demir, H.Introduction: Sevoflurane and desflurane are commonly used anesthetic agents with known cardiac side effects. Ischemia-modified albumin (IMA) is a useful biomarker that increases in cardiac conditions. This study investiga tes the effects of desflurane and sevoflurane on cardiac parameters and IMA levels in patients with coronary artery disease (CAD). Materials and Methods: Sixty CAD patients aged over 18 years scheduled for elective surgery were enrolled. Patients were rand omly divided into two groups: group I, desflurane (D) (n=30) and group II, sevoflurane (S) (n=30). Pre-and post-operative cardiac parameters were recorded for each patient. IMA measurement in serum was determined spectrophotometrically. Results: Postoperative IMA levels were significantly higher than preoperative IMA levels in both groups. The rate of change between post-and pre-operative IMA levels was significantly lower in group S than in group D. Postoperative heart rate (HR) and mean arterial pressure (MAP) levels were significantly lower than preoperative HR and MAP levels in both groups, whereas no significant differenc es were found between the two groups with regard to pre-and post-operative HR and MAP levels. Conclusions: Sevoflurane is safer and has fewer cardiac side effects in CAD patients than desflurane. One of them, propofol, is used frequently because of its advantages like fast effect and short duration. Involuntary tonic-clonic movements similar to epilepsy are rarely seen when using propofol. A 41-year old male patient weighing 80 kg and admitted with acute appendicitis presented hemiballismus like severe extrapyramidal movements in all his extremities when propofol was administered.
