Evaluation of the Treatment of Patients With Insufficient Keratinized Tissue and Peri-Implant Mucositis by Free Gingival Graft With the Help of Clinic and Immunological Parameters

Abstract

Bu çalışmanın amacı, klinik ve radyolojik değerlendirmeler ışığında peri-implant mukozitis tanısı konmuş yetersiz keratinize dişeti bulunan implantların serbest dişeti grefti (SDG) ile tedavi etkinliğinin belirlenmesidir. Çalışmamıza overdenture ile tedavi edilmiş 22 hasta dahil edilmiştir. Peri-implant bölgeden sondalamada kanama (SK), cep derinliği (CD), gingival indeks (Gİ), plak indeksi (Pİ), keratinize dişeti yüksekliği (KDY) PİOS hacmi ve TNF-α kayıtları alınmıştır. Başlangıç periodontal tedavi uygulandıktan sonra rastgele seçilen çenelerin bir tarafına SDG uygulanırken (Test grubu), diğer tarafa idame prosedürleri (kontrol grubu) uygulanmıştır. 6. ayda klinik ve immünolojik parametreler tekrar kaydedimiştir. Ameliyat öncesi ve sonrası, gruplar arası ve grup içi parametreler karşılaştırmalı olarak incelenmiştir. Test grubundaki implantlarda klinik ve immünololojik parametrelerde istatiksel olarak anlamlı düşüş gözlenmiştir. Kontrol grubunda ise, SK, Pİ ve PİOS hacmi parametrelerinde anlamlı düşüş gözlenirken; CD, Gİ, KDY, PİOS hacmi ve TNF- α değerlerinde istatiksel farlılık bulunmamıştır. Uygulanan tedavi protokolleri gruplar arası değerlendirildiğinde, test grubunda kontrol grubuna göre anlamlı oranda anlamlı düşüş gözlenmiştir. Yaptığımız çalışma sonucunda yetersiz keratinize dişeti bulunan peri-implant bölgeye SDG uygulamasının klinik ve immünolojik parametrelere olumlu sonuçları gözlenmiştir. The purpose of this study is to determine the efficiency of the treatment of the implants with insufficient keratinized tissue which is diagnosed as peri-implant mucositis by free gingival graft (FGG) via clinic and radiologic evaluation. It has been observed 22 patients who are treated by overdenture. The bleeding on proping (BOP), pocket depth (PD), gingival index (GI), dental plaque index (PI), the width of keratinized tissue (KTW), peri-implant crevicular fluid volume (PICF) and TNF-􏰅 on the peri-implant area have been recorded. While FGG is applied on the randomly chosen sides of jaws (test group), standard procedures are complied on the other sides of jaws (control group) after initial periodontal treatment is applied. In the 6th month, the clinic and immunological parameters recorded again. The parameters are examined comparatively with respect to pre-operation and post operation, among groups and inside group. It has been observed statistically decrease in the clinic and immunological parameters in the implants of the test group. In the control group BOP, PI and PICF volume parameters were significantly decreased whereas no statistically significant difference was found in PD, GI, KT width, PICF volume and TNF-􏰅 values. When the treatment protocols were evaluated between the groups, a significant decrease was observed in the test group compared to the control group. As a result of this study, improved clinical and immunological parameters were observed after FGG application to peri-implant region with inadequate keratinized tissue.
