Article Age-Dependent Changes in the Hyoid Bone Morphology in Children(Springer France, 2024) Beger, Burhan; Yalinkilic, Abdulaziz; Erdem, Mehmet Zeki; Akdemir, Zulkuf; Beger, OrhanPurposeThis radiologic work aimed to display the alteration in the hyoid bone (HB) morphology in the pediatric population with advancing age. MethodsThis pediatric examination consisted of computed tomography images of 129 subjects (49 males / 80 females) aged 1-17 years. ResultsThe anterior-posterior length of HB, the lengths of right and left greater horns, the width and height of HB's body, and the distance between the midpoints of the posterior ends of the greater horns increased with advancing age (p < 0.001), but the angle of the right and left greater horns (p = 0.022) decreased. Four configurations regarding HB shape were observed: Type A (U-shaped HB) in 8.5% (11 HBs) out of 129 children, Type B (B-shaped HB) in 33.3% (43 HBs), Type C (D-shaped HB) in 45% (58 HBs), and Type D (V-shaped HB) in 13.2% (17 HBs). HB shape types correlated with the pediatric age (p < 0.001), but not gender (p = 0.153). ConclusionsMost of the parameters increased until the postpubescent period, but the angle of the right and left greater horns decreased after the late childhood. Our linear functions representing the growth pattern of HB in children may be useful to estimate HB size.Article Anterior Cerebral Artery and Anterior Communicating Artery Variations: Assessment With Magnetic Resonance Angiography(Elsevier Science inc, 2021) Yokus, Adem; Toprak, Nursen; Gunduz, Ali Mahir; Akdeniz, Huseyin; Akdemir, Zulkuf; Dundar, Ilyas; Arslan, HarunBACKGROUND: Although anterior cerebral artery (ACA) and anterior communicating artery (AcomA) variations have been frequently investigated by contrast enhanced angiography, the number of studies performed with magnetic resonance angiography is quite low. Our aim in this study is to investigate ACA and AcomA variations with time of flight magnetic resonance angiography (TOF MRA). METHODS: Source and maximum intensity projection images of 581 adult patients (273 males, 308 females) who were scanned for intracranial arteries with the TOF MRA method were retrospectively analyzed. RESULTS: We found the typical configuration rate of the ACA and AcomA complex to be 40.6%. The most common structural anatomic variations out of typical configuration were AcomA aplasia (18.9%) and AcomA hypoplasia (17.9%), respectively. The least frequent variation that we found was duplicated AcomA (0.9%). When we examined gender comparatively, unilateral A1 segment aplasia was found to be higher in males and azygos ACA was found to be higher in females (P = 0.001 and P = 0.029, respectively). CONCLUSIONS: In this study, in which we used the TOF MRA technique, we detected high rates of anatomic variations of ACA and AcomA. This method, which is noninvasive, does not require contrast material and does not contain ionizing radiation, can be preferred especially in the evaluation of patients with kidney disease and contrast allergy.Article Assessment of Liver Volume With Computed Tomography and Comparison of Findings With Ultrasonography(Springer, 2014) Bora, Aydin; Alptekin, Cem; Yavuz, Alpaslan; Batur, Abdussamet; Akdemir, Zulkuf; Berkoz, MehmetIn this study, we aimed to investigate the impact of non-alcoholic hepatic steatosis on the liver volume. As investigating hepatic steatosis, we utilized computed tomography (CT) to determine the degree of steatosis and we utilized hepatobiliary ultrasonography (USG) for densitometry and correlation. As hepatosteatosis group, 35 patients over 18 years of age and whose abdominal CT scans were requested by several clinics and performed routinely were included in this study, and as control group, 40 healthy subjects without hepatosteatosis (clinically and radiologically) and correlated with hepatosteatosis group in terms of age and gender were included in this study. CT densitometry and liver attenuation index (LAI) of all individuals who participated in our study were calculated, and contrast images of patients were transferred to CT-Volume Software (Siemens Syngo Multimodality Workplace; Version VE52A). In this study, interactive and automated volume measurement techniques were used together. The volumes were measured separately in patient and control group. In this study for each stage in USG, there was found a direct correlation in terms of LAI and volume, and this correlation was statistically significant (p < 0.01). Furthermore, statistical significance between size and USG stage draws attention (p < 0.05). A significance relationship between USG stage and age could not be determined. As a result, we have reached the conclusion that CT densitometry can be used as an assistive technique along with USG to determine the degree of steatosis in the non-alcoholic fatty liver disease, and there is a positive linear correlation between the liver size and volume, and liver volume increases in the non-alcoholic fatty liver disease.Article A Case of Metastatic Esophageal Carcinoma in a Pregnant Woman With Radiologic Findings(Premchand Shantidevi Research Foundation, 2016) Akdemir, Zulkuf; Karaman, Erbil; Arslan, Harun; Calli, Iskan; Akdeniz, HuseyinThe incidence of all malignant tumours during pregnancy is extremely rare and esophageal carcinoma ranges from 0.07 to 0.1% of all malignant neoplasms. The physiological changes during pregnancy frequently mask the complaints and symptoms related to the disease. The physical and physiological clinical conditions limit the diagnostic approaches. Therefore, the stage of cancer at the time of diagnosis is usually advanced. The management with cancer surgery and chemotherapy regimens must be modified in pregnant women in order to minimize fetal and maternal risks. Here, we report a very rare case of metastatic esophagus cancer in a 39-year-old woman with 28 weeks of pregnancy and aim to show the ultrasound (USG) and magnetic resonance imaging (MRI) findings with treatment and follow up management.Article Efficacy of Strain Elastography in Diagnosis and Staging of Acute Appendicitis in Pediatric Patients(int Scientific Literature, inc, 2018) Arslan, Harun; Akdemir, Zulkuf; Yavuz, Alpaslan; Gokcal, Fahri; Parlakgumus, Cemal; Islamoglu, Necat; Akdeniz, HuseyinBackground: In the present study, the role and efficiency of strain elastography (SE) were evaluated in diagnosis and staging of acute appendicitis in pediatric patients. Material/Methods: We enrolled 225 pediatric patients with suspected clinical and laboratory findings of acute appendicitis. Gray-scale sonographic findings were recorded and staging was made by the colorization method of SE imaging. Appendectomy was performed in all patients and the results of the surgical pathology were compared with the imaging findings. The sensitivity, specificity, and accuracy of SE imaging were determined in terms of evaluating the "acute appendicitis". Results: Sonographic evaluation revealed acute appendicitis in 100 patients. Regarding the SE analysis, cases with appendicitis were classified into 3 groups as: mild (n=17), moderate (n=39), and severe (n=44). The pathological evaluation revealed 95 different stages of appendicitis and normal appendix in 5 cases: acute focal (n=10), acute suppurative (n=46), phlegmonous (n=27), and perforated (n=12), regarding the results of surgical pathology. Five patients with pathologically proven "normal" appendix were noted as " mild stage appendicitis" based on gray scale and SE analysis. In total, when gray-scale and SE results were compared with pathology results regardless of the stage of appendicitis, sensitivity, specificity, positive predictive value, negative predictive value, and accuracy rates were 96%, 96%, 95%, 96.8%, and 96%, respectively. No statistically significant difference was detected between other groups (P<0.05). Conclusions: In acute appendicitis, the use of SE imaging as a supportive method for the clinical approach can be useful in diagnosis, and its results are closely correlated with the histopathologic stage of appendix inflammation.Article Giant Thyroid Abscess Related To Postpartum Brucella Infection(Hindawi Ltd, 2015) Akdemir, Zulkuf; Karaman, Erbil; Akdeniz, Hseyin; Alptekin, Cem; Arslan, HarunThyroid gland infection, although rare, may be a life threatening disease. Thyroid abscess, arising from acute suppurative thyroiditis (AST), is a rare clinic condition depending on widespread use of antibiotics. Infection may involve one or both lobes and abscess formation may not be apparent until late stage of the progress of illness. Thyroid left lobe is more often affected than the right one. Brucellosis, especially obvious in endemic areas, is a widely seen zoonosis around the world. Although brucella infection can affect many organs through various complications, thyroid gland infection is rare. We aimed to present ultrasonography (USG) and magnetic resonance images (MRI) of a case with an acute thyroiditis which rapidly developed and grew fast on the left half of the neck during the first postpartum month. As far as we know from literature reviewing, our case is the first case report of a thyroid abscess arising from brucella infection which is developed in first postpartum period with images of ultrasonography and MRI.Article Herlyn-Werner Syndrome: Merits of Sonographic and Magnetic Resonance Imaging for Accurate Diagnosis and Patient Management in 13 Cases(Elsevier Science inc, 2015) Yavuz, Alpaslan; Bora, Aydin; Kurdoglu, Mertihan; Goya, Cemil; Kurdoglu, Zehra; Beyazal, Mehmet; Akdemir, ZulkufStudy Objective: To review the main sonographic and magnetic resonance imaging manifestations of Herlyn-Werner-Wunderlich syndrome, characterized by uterus didelphys, obstructed hemivagina, and ipsilateral renal agenesis, and to discuss the value of techniques for its diagnosis and treatment. Participants: Thirteen patients with Herlyn-Werner-Wunderlich syndrome referred to our department with acute abdomen were identified for the period from 2009-2012. Design: Retrospective chart review. Setting: Two tertiary academic centers. Interventions: None. Main Outcome Measures: The evaluation of the clinical features, ultrasonography and magnetic resonance imaging findings, and the treatment protocols. Results: Ultrasonographic evaluation of didelphic or double uterus in 10 and bicomuate-bicollis uterus in 3 patients with hematocolpos or hematometrocolpos associated with agenesis of the ipsilateral kidney revealed the diagnosis. In 10 patients, magnetic resonance imaging provided extra information. In 1 patient, a 25-week pregnancy was additionally detected and follow-up was suggested. Seven patients underwent surgical excision of the septum and drainage of the obstructed vagina. Total hysterectomy and hemicolpectomy were performed in 1 patient with 2 children suffering from severe chronic pelvic pain due to pelvic inflammatory disease. Conclusions: Despite its rarity, accurate diagnosis and morphologic description of Herlyn-Werner-Wunderlich syndrome with radiologic modalities are of importance in relieving the clinical complaints by enabling the suitable surgical treatment to be identified.Article The Success of Deep Learning Modalities in Evaluating Modic Changes(Elsevier Science inc, 2024) Yuksek, Mehmet; Yokus, Adem; Arslan, Harun; Canayaz, Murat; Akdemir, ZulkufBACKGROUND: Modic changes are pathologies that are common in the population and cause low back pain. The aim of the study is to analyze the modic changes detected in magnetic resonance imaging (MRI) using deep learning modalities. METHODS: The sagittal T1, sagittal and axial T2 - weighted lumbar MRI images of 307 patients, of which 125 were female and 182 were male, aged 19 - 86 years, who underwent MRI examination between 2016 - 2021 were analyzed. Modic changes (MC) were categorized and marked according to signal changes. Our study consists of 2 independent stages: classification and segmentation. The categorized data were first classified using convolutional neural network (CNN) architectures such as DenseNet-121, DenseNet-169, and VGG-19. In the next stage, masks were removed by segmentation using U -Net, which is the CNN architecture, with image processing programs on the marked images. RESULTS: During the classification stage, the success rates for modic type 1, type 2, and type 3 changes were 98%, 96%, 100% in DenseNet-121, 100%, 94%, 100% in DenseNet-169, and 98%, 92%, 97% in VGG-19, respectively. At the segmentation phase, the success rate was 71% with the U -Net architecture. CONCLUSIONS: Evaluation of MRI findings of MC in the etiology of lower back pain with deep learning architec- tures can significantly reduce the workload of the radiol- ogist by providing ease of diagnosis.Article Use of Virtual Touch Tissue Quantification Elastography Technique in Fetal Lung Maturation(Lippincott Williams & Wilkins, 2023) Arslan, Harun; Kucukbas, Gokce Naz; Turkoglu, Saim; Akdemir, Zulkuf; Yokus, Adem; Gunduz, Ali Mahir; Sahin, Hanim GulerThis study is an analysis of fetal lung stiffness by virtual touch tissue quantification (VTTQ) elastography to predict fetal lung maturation. Evaluation of fetal lungs was first performed in B mode, and fetal lungs were analyzed at 3 different periods at third trimester in each pregnant woman, at 28 to 31, 32 to 36, and 37 to 41 weeks. Fetal lung elastography was performed at regions with the least acoustic shadow and far from ribs and heart. Each fetal lung assessment were done by taking mean lung stiffness obtained by measuring stiffness of both left and right fetal lungs. T test analysis showed no significant difference in fetal lung stiffness between male and female fetuses among 3 gestational periods. Analysis of variance was performed to evaluate fetal lung stiffness of the fetuses at 3 different gestational periods (28-31, 32-36, and 37-41 weeks). This analysis showed significant difference (P < 0.01). Duncan multiple comparison analysis did not show significant difference in fetal lung stiffness between 28 and 31 weeks and 32 and 36 weeks, whereas fetal lung stiffness of fetuses at 37 to 41 weeks were significantly greater (P < 0.01). This study is first step to analyze fetal lung maturation noninvasively using VTTQ elastography technique by measuring fetal lung stiffness.
