Article Altered Serum Levels of Elements in Acute Leukemia Cases in Turkey(Asian Pacific Organization Cancer Prevention, 2011) Demir, Cengiz; Demir, Halit; Esen, Ramazan; Sehitogullari, Abidin; Atmaca, Murat; Alay, MuratObjective: The purpose of the study was to compare serum concentrations of some elements [zinc (Zn), copper (Cu), manganese (Mn), magnesium (Mg), lead (Pb), iron (Fe), cadmium (Cd) and cobalt (Co)] in acute leukemia patients with those of healthy subjects. Methods: The study group consisted of newly diagnosed acute leukemia patients and the controls were matched for socioeconomic stauts and eating habits. The elements levels in the patient group were measured before treatment with an atomic absorption spectrophotometer. The selection criteria for the patients and controls were the lack of recent blood transfusion history and taking any medication with mineral supplement. Results: The acute leukemia group composed of 42 patients and there were 40 persons in the control group. There was no difference between the age of the two groups (p=0.239). Serum levels of Zn, Mg and Mn were significantly lower with acute leukemia than in controls (p<0.001, p=0.011, p<0.001, respectively), while Cu, Pb and Cd were significantly elevated (p=0.003, p<0.001, p<0.001, respectively). There were no significant differences regarding Co and Fe (p=0.323 and p=0.508, respectively) Conclusion: In this study, we found levels of Zn, Mg and Mn to be lowered and of Cu, Pb and Cd to be elevated in patients with leukemia. Further studies are needed to clarify the role of these elements in pathogenesis of acute leukemia.Article Aspirin Resistance in Patients With Type Ii Diabetes Mellitus(Taylor & Francis Ltd, 2014) Tasdemir, Eyyup; Toptas, Tayfur; Demir, Cengiz; Esen, Ramazan; Atmaca, MuratBackground. Diabetic patients exhibit platelet hyperreactivity, which renders them resistant to antithrombotic treatments. We aimed to investigate the prevalence and predictors of aspirin resistance in diabetic patients. Material and methods. A total of 93 diabetic and 37 non-diabetic participants were included into the study. Aspirin resistance was measured with a whole-blood desktop platelet function analyzer (PFA-100) with an epinephrine agonist. Results. Altogether 41.9% patients with DM were aspirin non-responders. Aspirin resistance was observed in 43.2% of nondiabetic patients (p = 0.89). Presence of diabetes mellitus had no effect on aspirin response (RR 0.95 (95% CI 0.44-2.05), p = 0.89) in the whole study population. Hypercholesterolemia was the only predictor of aspirin resistance in multivariate analysis in diabetic patients (RR 3.09 (95% CI 1.17-8.16), p = 0.023). Conclusion. The prevalence of aspirin resistance is comparable in diabetic and non-diabetic patients. Hypercholesterolemia is the only independent predictor of aspirin resistance in diabetic patients.Article Association of Acromegaly and Multiple Myeloma: a Case Report(Galenos Yayincilik, 2013) Atmaca, Murat; Yildiz, Saliha; Kalan, Isilay; Ozbay, Mehmet Fatih; Seven, Ismet; Ozturk, MustafaMalignancy is an important cause of mortality in acromegaly. Hematological malignancies are very rare in acromegaly. Here, we report an 80-yearold patient with acromegaly and multiple myeloma. Patient died within a month of diagnosis. Previous studies have shown that growth hormone and somatomedin-C activate B lymphocyte and somatomedin-C receptors are found in multiple myeloma cells. Possible effects of growth hormone and somatomedin-C on multiple myeloma progression are discussed in the light of the relevant literature.Article Association of Multiple Myeloma and Gastric Adenocarcinoma(2011) Atmaca, Murat; Demir, Cengiz; Efe, Servet; Taşdemir, EyüpMultipl miyeloma ile solid tümör birlikteliği gösterilmiştir. Recent studies have shown that, apart from aging, various other factors also play a role in auditory changes. Studies on the association of hearing loss (HL) with obesity are limited in advanced age cases and present contradictions. In this study, the association between obesity and hearing thresholds in women aged 18-40 years has been assessed. Forty women diagnosed with obesity (mean age, 31.8 years) and 40 healthy non-obese female controls (mean age, 30.5 years) were included in this prospective study. Each subject was tested with low (250, 500, 1000 and 2000 Hz) and high (4000, 6000 and 8000 Hz) frequency audiometry. In the case and control groups, the average hearing thresholds at low frequencies were 16.03 +/- A 4.72 and 16.15 +/- A 2.72 (p = 0.885) for the right ear, respectively, and 16.15 +/- A 5.92 and 14.71 +/- A 3.18 (p = 0.180) for the left ear, respectively. The average hearing threshold levels at high frequencies were 20.70 +/- A 10.23 and 15.33 +/- A 3.87 (p = 0.003), respectively, for the right ear, and 22.91 +/- A 15.54 and 15.87 +/- A 4.35 (p = 0.007), respectively, for the left ear with statistical significance. This is the first report on the association of obesity with hearing threshold in women aged 18-40 years. We have demonstrated that obesity may affect hearing function, particularly that related to high frequencies. Hearing loss can be prevented by avoidance or control of obesity and its risk factors. Moreover, an auditory screening of obese cases at an early stage may provide early diagnosis of HL and may also contribute to their awareness in the fight against obesity.Letter Author's Reply: Predictors of Aspirin Resistance(informa Healthcare, 2014) Toptas, Tayfur; Tasdemir, Eyyup; Demir, Cengiz; Esen, Ramazan; Atmaca, MuratArticle Bone Marrow Biopsy Findings in Brucellosis Patients With Hematologic Abnormalities(Lippincott Williams & Wilkins, 2012) Demir, Cengiz; Karahocagil, Mustafa Kasim; Esen, Ramazan; Atmaca, Murat; Gonullu, Hayriye; Akdeniz, HayrettinBackground Brucellosis can mimic various multisytem diseases, showing wide clinical polymorphism that frequently leads to misdiagnosis and treatment delay, further increasing the complication rates. In this study, we aimed to examine bone marrow biopsy findings in brucellosis cases presenting with hematologic abnormalities. Methods Forty-eight brucellosis cases were prospectively investigated. Complaints and physical examination findings of patients were recorded. Patients' complete blood count, routine biochemical tests, erythrocyte sedimentation rate, C-reactive protein and serological screenings were performed. Bone marrow biopsy and aspiration was performed in patients with cytopenia, for bone marrow examination and brucella culture, in accordance with the standard procedures from spina iliaca posterior superior region of pelvic bone. Results Of the 48 patients, 35 (73%) were female and 13 (27%) were male. Mean age was (34.8 15.4) years (age range: 15-70 years). Anemia, leukopenia, thrombocytopenia and pancytopenia were found in 39 (81%), 28 (58%), 22 (46%) and 10 patients (21%), respectively. In the examination of bone marrow, hypercellularity was found in 35 (73%) patients. Increased megacariocytic, erythroid and granulocytic series were found in 28 (58%), 15 (31%) and 5 (10%) patients, respectively. In addition, hemophagocytosis was observed in 15 (31%) patients, granuloma observed in 12 (25%) and increased eosinophil and plasma cells observed in 9 (19%) patients. Conclusion According to the results of our series, hemophagocytosis, microgranuloma formation and hypersplenism may be responsible for hematologic complications of brucellosis. Chin Med J 2012;125(11):1871-1876Article Development of Agranulocytosis After Discontinuation of Methimazole: an Unusual Case(Hindawi Ltd, 2015) Ucler, Rifki; Atmaca, Murat; Candar, Omer; Alay, Murat; Goy, Burhan; Kara, Erdal; Demir, YusufAgranulocytosis is a rare and critical adverse effect of antithyroid drugs (ATD). The occurrence of agranulocytosis in continuous ATD treatment patients is well known; however, a case of ATD agranulocytosis occurring following the discontinuation of methimazole (MMI) treatment is not a usual situation. We herein describe a case of a 41-year-old woman who was previously administered methimazole (MMI) for ten days and developed ATD-induced agranulocytosis and symptoms of an upper respiratory tract infection after three weeks following discontinuation of MMI treatment. A thorough hematologic and serological evaluation did not disclose an alternative cause for the agranulocytosis. After receiving empirical antibiotic treatment, she responded successfully with clinical improvement of her symptoms and resolved neutropenia on the seventh day. This case is atypical because agranulocytosis developed after discontinuation of MMI, which strengthens the importance of remaining alert for signs of agranulocytosis even after discontinuation of ATD treatment.Article The Effect of Disease Control on Mean Platelet Volume and Red Blood Cell Distribution in Patients With Acromegaly(E-century Publishing Corp, 2015) Ucler, Rifki; Aslan, Mehmet; Atmaca, Murat; Alay, Murat; Ademoglu, Esra Nur; Candan, Zehra; Gulsen, IsmailObjectives: Several studies have shown increased atherogenic risk factors and biomarkers of inflammation and atherosclerosis in association with growth hormone excess. Mean platelet volume (MPV) and red blood cell distribution (RDW) are currently gaining interest as new independent cardiovascular risk factors. The aim of this study was to evaluate the effect of disease control on MPV and RDW in acromegaly patients. Materials and methods: We retrospectively enrolled 36 acromegaly patients (23 males, 13 females; mean age 41.94 +/- 11.55). Patients were divided into two groups: disease controlled by surgical treatment alone (group A) or by somatostatin analog (SSA) therapy (group B). MPV and RDW measurements were evaluated during active and inactive disease periods in the two groups. Results: There were statistically significant increases in MPV and RDW in patients receiving SSA therapy (P = 0.012 and P = 0.020, respectively). The differences in MPV and RDW changes in patients receiving surgical treatment alone were not statistically significant (P=0.364 and P=0.339, respectively). Conclusions: This is the first report on the evaluated the effect of disease control on MPV and RDW in acromegaly patients. Our study results showed that MPV and RDW measurements are significantly increased in acromegaly patients with disease controlled by SSA therapy. Therefore, acromegalic patients treated with SSAs may have increased cardiovascular risk based on an increase in MPV and RDW.specialization-in-medicine.listelement.badge Effects of Parity History on Serum Oxidative Stress and Dheas Levels in Postmenopausal Women(2014) Göy, Burhan; Atmaca, MuratGiriş: Menapoz yaşlanma sürecindeki doğal bir adımdır. MDR; MDA'nın DHEAS'a oranı ile hesaplandı. Hyperglycemia can be considered a determining factor in the development of diabetic neuropathy as well as neuropathic pain. There is a relationship between the excessive production of reactive oxygen species (ROS) and the pathogenesis of diabetic neuropathic pain. Taxifolin, on the other hand, is a flavonoid that has been documented to inhibit ROS production. Objectives. To investigate the effects of taxifolin, which has antioxidant and neuroprotective effects, on alloxan-induced hyperglycemia-induced neuropathy and neuropathic pain, biochemically and histopathologically. Materials and methods. The albino Wistar male rats were divided into 3 groups: healthy group (HG), only alloxan group (AXG) and alloxan+taxifolin group (ATG). Hyperglycemia in animals was caused through intraperitoneal injection of alloxan at a dose of 120 mg/kg. Paw pain thresholds of animals were measured using Basile algesimeter. Sciatic nerve tissues were examined biochemically and histopathologically in order to evaluate neuropathy. Results. Our experimental results revealed that taxifolin significantly prevented the increase of plasma glucose concentration level with alloxan administration, the decrease of the paw pain threshold related to hyperglycemia, the change of oxidant-antioxidant balance in the sciatic nerve tissue in favor of oxidants, and the deterioration of tissue morphology in animals. Conclusions. Our experimental results indicate that taxifolin alleviates alloxan-induced hyperglycemiarelated neuropathy and neuropathic pain.Article Endojen Eritropoietinin Parathormon Salgısı Üzerine Etkileri(2021) Kılıç, Esat; Özbay, Mehmet Fatih; Özkan, Sezai; Gönüllü, Edip; Adanaş, Cihan; Atmaca, MuratAmaç: Hipotezimiz, doku hipoksisinin parathormon (PTH) kadar eritropoietin salgısını da artıracağıydı. Paratiroid\rhormonu ile eritropoietin düzeyleri arasında pozitif korelasyon vardı (p<0,05).\rSonuç: Çalışmamız doku hipoksisine yanıt olarak eritropoietin düzeylerindeki artışın PTH salınımını\rartırdığını gösteren ilk çalışma olması ile ayırt edilmektedir.Article Erythrocyte Catalase and Carbonic Anhydrase Activities in Acute Leukemias(Asian Pacific Organization Cancer Prevention, 2010) Demir, Cengiz; Demir, Halit; Esen, Ramazan; Atmaca, Murat; Tasdemir, EyuepObjective: To determine activity of catalase (CAT) as a antioxidant and carbonic anhydrase (CA) in erythrocytes from acute leukemia cases. Methods: Subjects were recruited from patients attending the outpatient clinics or hospitalised in the ward of the Hematology Department of Yuzuncu Yil University Hospital. Venous blood samples were taken from a total of 67 individuals (31 with acute leukemia and 36 healthy) included in the study. CAT enzyme activity was determined in erythrocytes using Aebi's method and CA by hydration of CO2. Results: CAT activity was found to be significantly decreased (P<0.001) on average in acute leukemia cases as compared to the control group while erythrocyte CA activity was significantly increased (P<0.001). Conclusions: Our findings point to malfunction of the antioxidant system in acute leukemia patients. Hence we need to investigation the cause and also its possible contribution to prognosis. Furthermore, clarification of the relationship between the antoxidant system and CA inhibitors in the pathogenesis of acute leukemia appears warranted.Article An Interesting Cause of Hyperandrogenemic Hirsutism(Hindawi Ltd, 2014) Atmaca, Murat; Seven, Ismet; Ucler, Rjfki; Alay, Murat; Barut, Veysi; Dirik, Yaren; Sezgin, YasinMild clinical signs of hyperandrogenism such as hirsutism may appear during the menopausal transition as part of the normal aging process, but the development of frank virilization suggests a specific source of androgen excess. We report a case of a 68-yearold woman with signs of virilization that had started 6 months before. Clinical analyses revealed high levels of serum testosterone for a postmenopausal woman. Pelvic MRI and abdomen CT showed no evidence of ovarian and adrenal tumor. Postmenopausal hyperandrogenism can be the result of numerous etiologies ranging from normal physiologic changes to ovarian or rarely adrenal tumors. Our patient was found to have iatrogenic hyperandrogenism. This condition is rarely reported cause of virilization.Article Mesterolone Treatment of Aging Male Syndrome Improves Lower Urinary Tract Symptoms(Pakistan Medical Assoc, 2014) Dugeroglu, Harun; Ozturk, Mustafa; Atmaca, Murat; Seven, IsmetObjective: To investigate the effects of mesterolone on prostate in patients treated for aging male syndrome. Methods: The cross-sectional study was conducted from June to September, 2009, at endocrinology and metabolism department of Yuzuncu Yil University, Van, Turkey, and comprised patients with symptoms of aging male syndrome and/or low testosterone. They were given mesterolone 50mg/day per oral for two months. Aging Male Symptoms and International Prostate Symptom Score questionaires and prostate-related quality of life scores were completed and prostate ultrasonography (USG) was performed before and after the treatment. Total testosterone, free testosterone, gonadotropins, estradiol, prolactin, sex-hormone binding globulin, as well as total and free prostate-specific antigen were also studied. Results: Of the 34 patients in the study, 22(64.70%) had their prostate volume increased, while 12(35.29%) had it decreased. The change, however, was not statistically significant (p<0.098). Mesterolone significantly improved Aging Male Symptoms, International Prostate Symptom and prostate-related quality of life scores (p<0.001). These improvements though significant were independent of the changes in prostate volume. Total testosterone, sex-hormone binding globulin andestradiol decreased, while free testosterone showed no change (p<0.002, p<0.001, p<0.024, p<0.337). The fraction of free testosterone increased (p<0.001), while total and free prostate-specific antigen did not change (p<0.368 and p<0.841) Conclusion: Mesterolone proved to be a safe alternative in the treatment of Aging Male Syndrome. It also improved lower urinary tract symptoms and prostate-related quality of life.Article Ocular Findings in Sheehan's Syndrome(Springer, 2015) Atmaca, Murat; Kizildag, Esra; Candan, Zehra; Ozbay, Mehmet Fatih; Seven, IsmetSheehan's syndrome (SS) is one of the most common causes of hypopituitarism. The primary effect of SS is a deficiency in production of growth hormone (GH). A number of studies have supported the association between congenital GH deficiency and ocular anomalies. However, ocular findings such as central corneal thickness (CCT), intraocular pressure (IOP), and retinal nerve fiber layer thickness (RNFLT) have not been evaluated in patients with adult GH deficiency. The objective of this study was to evaluate ocular anomalies in SS with GH deficiency under a cross-sectional design. Thirty three SS patients with GH deficiency and 28 controls with no history of thyroid, adrenal, or pituitary gland diseases or surgery underwent complete hormonal and ophthalmological evaluation, including an assessment of CCTs, IOPs, and RNFLT. The mean CCTs were significantly lower in the SS group compared with the control group (p < 0.001). There was no significant difference between patients and controls in terms of mean IOP, mean corrected IOP, and mean RNFLT (p = 0.517, p = 0.186, p = 0.965, respectively). The mean CCT was positively correlated with insulin-like growth factor 1 (IGF-1; p < 0.01) and adrenocorticotropic hormone (ACTH; p < 0.01) and negatively correlated with the corrected mean IOP (p < 0.05). In covariance analysis, IGF-1 was found to be a potential predictor of the mean CCT (p = 0.023). This study is the first investigation of ocular findings in SS and adult GH deficiency. Adult GH deficiency is characterized by lower CCT values.Article Oxidative Stress in Patients With Thyroidectomy and Thyroparathyroidectomy Under Replacement Therapy(Springer, 2015) Kacmaz, Murat; Atmaca, Murat; Arslan, Ayse; Demir, Halit; Ozbay, Mehmet FatihSeveral studies have demonstrated an imbalance between free radicals and the antioxidative system in individuals with thyroid dysfunction. However, oxidative stress has not been evaluated in patients with thyroidectomy and thyroparathyroidectomy, who are under replacement therapy. The objective of this study was to evaluate the oxidative stress using malondialdehyde, nitric oxide, and catalase levels in patients with thyroidectomy and thyroparathyroidectomy. Nineteen patients with thyroidectomy, 20 patients with thyroparathyroidectomy, and 20 controls with no history of thyroid or parathyroid disease or surgery were included in the study. Serum malondialdehyde, nitric oxide, and catalase levels were examined. Levels of nitric oxide and malondialdehyde were elevated, and catalase levels decreased in patients with thyroidectomy and thyroparathyroidectomy compared with controls (p value for all the parameters: p < 0.001). Free tetraiodothyronine was a potential predictor of malondialdehyde in the patient groups (p: 0.002). Catalase was negatively correlated with nitric oxide (p < 0.01) and malondialdehyde (p < 0.01). The results of the current study demonstrated that oxidative stress increased in patients with thyroidectomy and thyroparathyroidectomy despite the application of replacement therapies. Article Prevalence of Autoantibodies Related To Some Autoimmune Disorders in Patients With Chronic Idiopathic Thrombocytopenic Purpura(Sage Publications inc, 2011) Demir, Cengiz; Esen, Ramazan; Atmaca, Murat; Efe, ServetWe investigated the prevalence of antinuclear antibody (ANA), thyroid antimicrosomal (AMA) and antithyroglobulin (ATA), antigliadin (AGA) immunoglobulin G (IgG)-A, anti-endomisium (EMA) IgG-A, and tissue transglutaminase (tTG) IgG-A in 87 patients with chronic idiopathic thrombocytopenic purpura (cITP) and in 95 healthy controls. Antinuclear antibody positivity was found in 13 of 87 patients and 3 of 95 controls (P = .007). Antithyroglobulin positivity was found in 27 of patients and in 7 of the controls (P < .001). AMA positivity was found in 20 of patients and 8 of the controls (P = 0.008). Antigliadin IgG was positive in 17 patients and 1 controls (P < .001) whereas Antigliadin IgA was positive in 9 of patients and in 1 of the controls (P = .007). Anti-endomisium (IgG and IgA were not different between both groups. Tissue transglutaminase IgG was detected in 7 of patients and in 1 of the controls (P = .029). Tissue transglutaminase IgA was detected in 5 of patients and in none of the controls (P = .023). We believe that larger studies are needed to determine the long-term impact and clinical importance of these autoantibodies.Article Primary Testicular Lymphoma: a Case Report(2010) Gunes, Mustafa; Taşdemir, Eyüp; Atmaca, Murat; Demir, Cengiz; Bulut, GülayPrimer testis lenfomaları nadir görülen malignitelerdir.
