Article Comparison of Classification Systems for Congenital Melanocytic Nevi(Wiley-blackwell, 2010) Turkmen, Arif; Isik, Daghan; Bekerecioglu, MehmetBACKGROUND Congenital melanocytic nevi (CMNs) are found in approximately 1% of newborn infants, but these represent only a small proportion of the total population of nevi. They vary widely in size, from a small spot to a large area. Later in childhood, these lesions become thickened, verrucous, and hairy. Giant CMNs predispose to malignant melanoma, with a reported incidence of 2% to 31%. OBJECTIVE To compare three different classification methods of the CMNs to determine which is most accurate. PARTICIPANTS AND METHODS Sixty patients were included in the study (34 male, 26 female), with an average age of 17.4 (range 3-32). The nevi were evaluated using three different classification methods: total area of the nevus (in cm2), greatest nevus dimension, and percentage of nevus surface area to total patient body surface area. An appropriate treatment procedure for each case was applied, and participants were followed from 1997 to 2007. RESULTS Malignant transformation was noted in 15.4% of participants with congenital nevi, which was confirmed histopathologically after excision. CONCLUSION We recommend the calculation of total nevus area as the most useful method for assessment of the risk of developing melanoma in a CMN. Peripheral nerve damage that requires surgical repair does not result in complete recovery because of collagen scar formation, ischemia, free oxygen radical damage, and other factors. To date, the best treatment method has not yet been determined. In this study, we designed an experimental peripheral nerve injury model, and researched the possible effects of melatonin hormone, based on evidence of its strong antioxidant and cell-protective effects via mimicking the effects of calcium channel blockers. Materials and Methods. We randomized 24 healthy female albino rats into three groups: the pinealectomy group, melatonin group, and control group. In the pinealectomy group, craniotomy, pinealectomy, sciatic nerve transection, and coaptation were performed, and 0.9% NaCl was injected intraperitoneally. In the melatonin group, craniotomy (without pinealectomy), sciatic nerve dissection, and coaptation were performed, and melatonin was injected intraperitoneally, instead of NaCl. In the control group, craniotomy (without pinealectomy), sciatic nerve dissection and coaptation, and intraperitoneal NaCl injection were performed. In each group, nerve recovery was evaluated histologically, functionally, and electrophysiologically. Functional and electrophysiologic evaluations were conducted before surgery and at 4 and 12 wk. Results. At 4 wk, no significant difference was observed between the groups. However, at 12 wk, significant electrophysiologic and functional improvement was observed only in the melatonin group. Conclusions. Melatonin seems to have a beneficial effect on nerve recovery. However, this effect is not effective at physiologic doses. Future comparative studies with melatonin versus other nerve-regenerating agents are necessary to determine the clinical utility of melatonin hormone. (C) 2011 Elsevier Inc. All rights reserved.Article Evaluation of Dynamic Magnetic Resonance Imaging in Assessing Velopharyngeal Insufficiency During Phonation(Lippincott Williams & Wilkins, 2008) Atik, Bekir; Bekerecioglu, Mehmet; Tan, Onder; Eflik, Omer; Davran, Ramazan; Arslan, HalilBackground: Velopharyngeal insufficiency (VPI) expresses the structural and neuromuscular disorder of soft palate and pharyngeal walls inhibiting the normal functions of velopharyngeal (VP) sphincter mechanism. In this study, efficacy of dynamic magnetic resonance imaging in the diagnosis of VPI is investigated. Materials and Methods: A total of 32 cases, 16 controls and 16 cleft palates, were included in this study. T1 fast spin echo-weighted imaging during rest, dynamic investigations with True-fast imaging with steady-state precession sequence during /sss/ and /mmm/ phonations were performed. Results: During /sss/ phonation, complete closure was observed in the control group, whereas mean VP opening was 4.11 cm(2) preoperatively and 0.21 cm(2) postoperatively in the cleft palate group. In the postoperative period, only 3 patients did not have complete closure. In the second operations, performed 6 months later, only muscle repair was done. All 3 had complete closure. Conclusions: In cleft palate cases with delayed diagnosis, appropriate application of muscle repair will be sufficient for anatomic repair of VPI without any extra procedures. In addition, dynamic magnetic resonance imaging is an objective, noninvasive, reliable, and effective modality that may be used in the diagnosis and treatment of VPI without any extra investigations.Other Maksilla ve Mandibula Yerleşimli Odontojenik Mikzoma(1999) Bekerecioglu, Mehmet; Kutluhan, Ahmet; Egeli, Erol; Uğraş, Serdar; Çankaya, Hakan; Kiris, MuzafferÇene kemiklerinin nadir bir tümörü olan odontojenik mikzoma histolojik olarak benign, nüksleri sık infiltratif bir tümördür. Tedavisinde daha çok agresif cerrahi girişimler tercih edilmektedir. Yirmi dört ve 31 yaşlarında iki kadın hastada, biri mandibulada, diğeri maksillada odontojenik mikzoma saptandı, ilk hastaya parsiyel maksillektomi ve kortikal kemikle ön duvar rekonstrüksiyonu yapıldı. İkinci hastaya marjinal mandibula rezeksiyonu, kitle ekstirpasyonu ve kostal kemik ve plakla onarım yapıldı, iki hastanın da sırasıyla 12 ay ve altı aylık takiplerinde nüks saptanmadı.Article Prolabial Lengthening by Turkish Tulip Method in Bilateral Cleft Lip Repair(Lippincott Williams & Wilkins, 2006) Atik, Bekir; Tan, Oender; Bekerecioglu, Mehmet; Kiroglu, Ahmet F.; Tekes, LuetfiObjective. In bilateral cleft lip, there is a characteristic deformity called cleft lip nose characterized by short columella and prolabium with a pressed nose. Although lots of surgical techniques were described for columella lengthening and correction of the nose deformity, no technical method was suggested for prolabium lengthening. Study Design. In this paper we propose a simultaneous bilateral cleft lip repair and lengthening of the prolabium., and describe a new technique called "Turkish tulip" for this aim. Patients and Method, Eleven patients (6 males and 5 females) with bilateral cleft lip were treated using this method. Patients' ages ranged from 3 months to 17 years at the time of operation. Five patients had incomplete and six had complete bilateral cleft lips. The patients were evaluated in terms of functional and aesthetic results in postoperative period. Results. The average follow-up time was 8 months (ranged from 4 months to 15 months). There were no postoperative complications. The prolabium was lengthened adequately in all patients. No notch and whistle deformity was seen in our series. The patient or parent satisfaction was good or perfect in all cases. Conclusions. To avoid the disadvantage of the long time course required to correct the nose deformity and to lengthen the prolabium, we propose the "Turkish tulip" technique with the primary repair of bilateral clef lips simultaneously. With this technique it is possible to lengthen the columelloprolabial complex with cleft lip repair in the same session without any intervention to any part of the nose including the columella. As a preliminary study, according to the early results, this new technique seems to have good cosmetic outcomes.Article Reconstruction of Lower Eyelid Defects Using a Cross Upper Eyelid Flap Composited With Ear Cartilage(Blackwell Publishing, 2007) Atik, Bekir; Tan, Onder; Bekerecioglu, Mehmet; Cinal, Adnan; Tekes, LutfiBackgrounds Basal cell carcinomas (BCC) most frequently involve the lower eyelid and are treated with total excision. Various techniques have been proposed for reconstruction of the excised eyelid. Objective Because most flaps used in such techniques are bulky, thinner, and aesthetically more favorable, flaps have been sought recently. Methods Defects of the lower lid have been closed with a combination of cross-flaps and choncal cartilage prepared from the upper eyelid. Results Eleven lower lids from 10 patients operated for BCC were reconstructed. All flaps survived. The duration of follow-up was 10 months, and no complications such as relapse, ectropion, or lagophthalmus occurred. Conclusions The upper eyelid flap was found to be an appropriate cover for both the skin and the conjunctiva due to its hairless and smooth structure. The outcome in patients followed up for a mean of 10 months was successful, cosmetically and functionally.Article Use of Rotation Flap in Repair of Cleft Palate and Velopharyngeal Insufficiency(Lippincott Williams & Wilkins, 2011) Isik, Daghan; Durucu, Cengiz; Isik, Yasemin; Atik, Bekir; Kocak, Omer Faruk; Karatas, Erkan; Bekerecioglu, MehmetAlthough cleft palate anomaly is frequent, the criterion standards in surgical treatment have not been determined yet. There are a few techniques described for cleft palate repair owing to the limited tissue in the palatal mucosa, the rigid structure of the palatal mucosa, and the limited vascularity of the hard palate. In this study, a novel cleft palate repair technique based on separating the soft palate from the hard palate as a musculomucosal flap and using it as a rotation flap has been described. The operation is evaluated individually for each anomaly because variations occur in the surgical technique according to the extension of the cleft toward the teeth in the palate. This operation was performed on a total of 28 patients (17 girls and 11 boys) aged between 1.5 and 16 years and presented to our clinic. Patients were assessed for speech analysis outcomes, tympanogram values, hearing functions, magnitude of palatal lengthening during the operation, and rate of fistulae. Statistically significant differences in values of the speech analysis and the audiometric assessment were determined between before and 6 months after surgery. Complete recovery of otitis was observed 1 month after surgery without another treatment in 9 (42.8%) of 21 patients who were detected to have serous otitis media preoperatively. Tension-free closure, lower risk of fistula, good restoration of velopharyngeal functions, ability to be performed on all types of cleft palate, ability to provide a good intraoperative exposure, and being a single stage seem to be the most important advantages of this technique.Other Van'da Kseroderma Pigmentozum(2000) Bekerecioglu, Mehmet; Uğraş, Serdar; Metin, Ahmet; Delice, İbrahimKseroderma pigmentozum yaklaşık 1/250.000 sıklığında görülen ultraviole ve diğer karsinojenlere karşı kalıtsal hassasiyetle karakterize bir DNA tamir bozukluğu hastalığıdır. Kliniğimize başvuran hastalar arasında sık görülmesi nedeniyle planlanan bu çalışma ile hastalığın özellikleri ve sık görülme sebeplerinin saptanması amaçlandı. Çalışmada hastalıktan değişik derecede etkilenmiş yaşları 3 ile 27 arasında değişen 9' u erkek 7' si bayan 16 olgu ele alındı. Hastaların tümünün akraba evliliğinden doğma olgulardı ve 13' ünün (%81.25) 4 farklı aileye ait kardeş olgulardan oluşuyordu. Deri belirtileri başlama yaşı 3.07±1,71 olarak saptanan hastaların %75' inde göz bulgularına; %81,25' inde SCC,BCC, malin melanom, atipik fibroksantom ve anjiyomdan oluşan değişik deri neoplazi Plasitk ve Rekonstruktif Cerrahi Anabilim Dalınca tedavi edildi. Tedavisi yapılan ve malinite saptanamayan olgulara koruyucu öneriler ve medikal tedavi verilerek takip altına alındı. Van ilinde Kseroderma Pigmentozum sıklığının literatürde bildirilen orandan daha yüksek olması bölgede, akrabalar arası evliliğin sıklığına, çok çocuk yapmaya ve bölgenin coğrafi koşullarına bağlandı.Other Xeroderma Pigmentozumda Göz Bulguları(1997) Şimşek, Şaban; Metin, Ahmet; Yaşar, Tekin; Demirok, Ahmet; Çinal, Adnan; Bekerecioglu, MehmetKseroderma pigmentosum (KP) otozomal reseslf yada daha az cinse bağlı resesif olarak geçen nadir görülen bir genodermatozdur. Çalışmada 8 KP olgusunun göz bulguları incelenmiştir. Ol¬guların 5'i (%62.5) erkek, 3'ü (%37.5) kadın, semptomların ilk başladığında ortalama yaş olguların %50'sinde 4 yaş ve altında idi. Olguların anne-babalarında akraba evliliği %60 ve aynı ailede baş¬ka bir hastanın varlığı %75 olarak tespit edilmiştir. Oküler bulgular olarak hastaların %25'inde görme azlığı, %87.5'inde fotofobi, %62.5'inde madarozis, %100'iinde kapaklarda pigmentasyon, %12.5'inde kapakta bazal hücreli karsinom, %100'ünde kronik konjonktival hiperemi, %50'sinde pterjium, %25'inde korneal opasifikasyon ve vaskülarizasyon saptanmıştır. Hastalardan elde edilen bu veriler literatür ile karşılaştırılarak tartı¬şılmıştır.
