Article The Impact of Anesthesia Method on Postoperative Cognitive Functions in Urological Surgeries: A Prospective Randomized Single-Blind Study(Yuzuncu Yil Universitesi Tip Fakultesi, 2023) Kurt, N.Introduction: Postoperative cognitive dysfunction contributes to cognitive impairments after surgery. Despite the potential of anesthesia methods causing cognitive dysfunction, conclusive evidence remains elusive. This study aims to compare the effects of general and spinal anesthesia on postoperative cognitive functions in patients undergoing urological surgeries. Materials and Methods: A total of 108 patients aged from 18 to 75 years, who underwent elective surgery with ASA I-II-III (American Society of Anesthesiology) risk classification, were included in this study. Regardless of the anesthesia method chosen for surgery, the Mini-Mental State Test (MMST) was administered by a clinician at 24 hours preoperatively to assess cognitive functions in a blinded manner. At postoperative 24 hours, the same independent clinician repeated the MMST assessment. Preoperative and postoperative MMST scores were recorded. Patients were categorized into two groups: Group G received general anesthesia, and Group S underwent spinal anesthesia. Demographic data, surgery durations, and MMST values of the cases were compared between groups. Results: Of the 108 included patients, 80 were male (74.1%) and 28 were female (25.9%). Forty-eight patients received general anesthesia (Group G), while 60 patients received spinal anesthesia (Group S). No significant differences were observed between the groups in terms of age, ASA classification, and comorbidities. The female-to-male ratio was statistically significantly higher in Group G, while the male-to-female ratio was higher in Group S. The average surgery duration in Group G (87.56 ± 28.15 minutes) was significantly longer than that in Group S (61 ± 18.91 minutes). When comparing preoperative and postoperative 24-hour MMST scores in both groups, no statistically significant differences were observed. Conclusion: This study showed that either general or spinal anesthesia showed no affect on postoperative cognitive dysfunction among patiens underwent urological surgery. © 2023, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Opinions of Faculty Members on the Assistant Physicians’ Use of Leave of Absence After Night Shift: a Survey Study(Yuzuncu Yil Universitesi Tip Fakultesi, 2023) Kurt, N.The opinions of the faculty members on the assistant physicians’ use of leave of absence after night shift matter, given that they play an essential role in their training. In this context, this study was carried out to review the opinions of faculty members on the consequences of assistant physicians’ use of leave of absence after night shift. This single-center, cross-sectional survey study, based on voluntary participation, was conducted with the faculty members of Van Yuzuncu Yil University Faculty of Medicine between February and March 2023. The study population consisted of faculty members in surgical and internal medical science departments. Faculty members in basic medical science departments were not included in the study since assistant physicians in basic medical science departments do not work the night shift. The survey consisted of nine items with checkboxes on participants' demographic data and the consequences of assistant physicians’ use of a leave of absence after night shift on the clinic's functioning, medical specialty training, patient turnovers, and relationships inside and outside the clinic. Of the 137 faculty members, 98 (71.53%) faculty members, 46 in surgical medical science departments and 52 in internal medical science departments, participated in the survey between February and March 2023 in an electronic environment. Of the respondents, 76 (77.5%) were male, and 22 (22.5%) were female. The mean age of the participants was 42.72 ± 4.9 (min. 34, max. 57) years. Of the 98 faculty members who participated in the survey, 84.0% and 79.6% stated that assistant physicians’ use of leave of absence after night shift adversely affected the medical specialty training and the clinic's functioning, respectively. In general, faculty members expressed the opinion that assistant physicians’ use of leave of absence after night shift negatively affected the medical specialty training, the functioning of the clinic, and the work carried out in collaboration with other clinics. All rights reserved.Article The Relationship Between the Presence of Gastrointestinal Symptoms and Anemia, Thyroid Functions and Bone Mineral Density in Celiac Patients(Yuzuncu Yil Universitesi Tip Fakultesi, 2019) Dertli, R.; Kayar, Y.; Surmeli, N.; Bilgili, M.A.; Karakarcayildiz, A.; Oflas, N.D.; Kurt, N.The prevalence of typical celiac patients decreases gradually with the investigation of patients presenting with non-gastrointestinal symptoms (non-GIS) like anemia, thyroid dysfunction, liver function test anomalies and dermatitis herpetiformis. The diagnosis of atypical and subclinical celiac has also increased due to the development of immunological and genetic tests. The aim of our study is to determine the relationship between the presence of GIS and anemia, thyroid functions, and bone mineral density in celiac patients. The study included 230 celiac patients who were diagnosed and followed-up in our clinics between 2015-2019. The demographic and anthropometric characteristics, initial presentation complaints, disease durations, dietary compliance of the patients were documented. Presence of anemia, thyroid functions, bone mineral density were documented and its relationship with presence of GIS was analyzed. The age range of the patients was 18-65 years, and the mean age was 33,4±10,6 years. A total of 170(73.9%) patients were female and the duration of disease follow-up was 4,8±3,5 years. Osteoporosis/osteopenia was found in 146(63,5%) patients, anemia was found 65(28.3%) and thyroid dysfunction was found 58(25.2%). Patients without GIS were significantly more anemic than patients with GIS (39.1%, 24.1% respectively) (p<0.05). In terms of thyroid function tests, the rates were similar in both groups and there wasn't significant difference (p>0.05). Osteoporosis was significantly higher in patients with GIS (p<0.05). The number of patients with atypical celiac disease is increasing. Celiac disease should be considered in patients presenting with unexplained anemia, a history of thyroid disease, and abnormal bone density. © 2019, Yuzuncu Yil Universitesi Tip Fakultesi. All rights reserved.Article Retrospective Evaluation of Anesthesia Methods in Pediatric Orthopedic Surgery: A Single-Center Study of 1468 Patients(Yuzuncu Yil Universitesi Tip Fakultesi, 2025) Keleş, M.Ş.; Kurt, N.; Yüzkat, N.; Soyalp, C.; Keskin, M.E.; Güneş, H.Y.Introduction: This study aimed to evaluate the distribution of anesthesia methods and complication rates in pediatric patients aged 0–18 years who underwent orthopedic surgery at the Department of Anesthesiology and Reanimation, Van Yuzuncu Yil University Facult y of Medicine, between 2019 and 2021. Materials and Methods: The study was designed as a retrospective, cross-sectional, and descriptive study. A total of 1468 pediatric patients aged 0–18 years classified as ASA I–II who underwent orthopedic surgery were included. Patient data including age, sex, type of surgery, anesthesia method applied, and anesthesia-related complications were recorded and analyzed. Results: Of the 1468 patients included in the study, general anesthesia was administered to 70.8%, peripheral nerve blocks to 16%, and spinal anesthesia to 12.6%. A significant increase was observed in the rates of general and spinal anesthesia in 2021, while the rate of peripheral nerve blocks decreased. The most common complication was tachycardia, observed at a rate of 3.5% in 2019. Complication rates decreased significantly in subsequent years. Conclusion: General anesthesia remains the most preferred method in pediatric orthopedic surgeries. However, regional anesthesia techniques are increasingly used and demonstrate safety. These findings suggest that regional techniq ues can be effectively and widely applied in pediatric populations with appropriate patient selection. © 2025, Yuzuncu Yil Universitesi Tip Fakultesi.
