Article Assessing Effectiveness of Anaerobic Threshold and Respiratory Compensation Point on Fat and Carbohydrate Oxidations During Exercise in Sedentary Males(Mattioli 1885, 2020) Ugras, S.; Algul, S.; Ozcelik, O.Study Objectives: Anaerobic threshold (AT) and respiratory compensation point (RCP) are two important metabolic set points. We aimed to determine the effects of exercise intensity at AT and RCP on the balance of substrate oxidation rates. Methods: Eleven male participants performed an incremental exercise test to exhaustion on a cycle ergometer to estimate AT and RCP. Subsequently, we conducted three 30 minute constant load exercise tests at AT (WAT), RCP (WRCP) and 25% below AT (WArticle Comparative Evaluation of Mda Levels During Aerobic Exercise in Young Trained and Sedentary Male Subjects(Yuzuncu Yil Universitesi Tip Fakultesi, 2018) Algul, S.; Ugras, S.; Kara, M.The impacts of acute aerobic exercise on oxidative stress as determined altered MDA levels have been examined in young trained and sedentary male subjects. Total of 20 (10 trained and 10 sedentary) male subjects performed an aerobic cycling exercise at the anaerobic threshold for about 30 min. The venous blood samples (pre and post exercise) and analysed for MDA using HPLC methods. Wilcoxon and Mann Whitney U-tests were included in the statistical methods for data analysis. Acute exercise caused a systematic increase in MDA levels in trained (36.2%) and sedentary (55.4%) subjects during exercise (p<0.001). The increase in MDA levels were higher in sedentary subjects (0.79±0.08 µmol/L vs 1.02±0.05 µmol/L) compared to trained subjects (0.73±0.05 µmol/L vs 0.97±0.07 µmol/L) (p<0.001). Acute exercise may cause higher levels of increase in oxidative stress as determined alteration of MDA levels in sedentary subjects. Thus the sedentary subjects should pay more attention to the physical activities. © 2018, Yuzuncu Yil Universitesi Tip Fakultesi. Some of them require myringoplasty. In this study, we used hyaluronic acid, a polysaccharide with high-molecular-weight which may be obtained commercially and affects modulations of cell function. An experimental myringoplasty was performed on rats in which a chronic dry tympanic membrane perforation was formed. While physiological saline was embedded in Gelfoam® which was supported under and over the graft, in group 1, hyaluronic acid embedded gelfoam was used in group 2. Better epithelization, less calcification and scar tissue were established in group 2 myringoplasties. This study shows that hyaluronic acid may be used in myringoplasty especially in myringosclerotic tympanic membrane perforations.Article Elastosis Perforans Serpiginosa(1997) Dilek, F.H.; Ugras, S.; Akpolat, N.A 21-year-old male with brown keratotic papules on the nape of his neck was admitted to our clinic. Lesions were in characteristic arcuate and serpiginous patterns. The diagnosis of elastosis perforans serpiginosa (EPS) was confirmed by skin biopsy. The pathogenesis of EPS is still unclear. There is evidence for the transepidermal elimination of morphologically and biochemically abnormal elastic fibers. EPS was described in idiopathic, penicillamine induced and connective tissue disorder associated forms. In this article, we described and discussed the clinical and morphologic data in view of the literature.Article Enzymatic Dissection for Palliative Treatment of Esophageal Carcinoma: an Experimental Study(2005) Isik, A.F.; Ozturk, G.; Ugras, S.; Karaayvaz, M.Patients with esophageal carcinoma generally lose their chance of curable surgical treatment when symptoms become evident. At this stage chemoradiotherapy and palliative treatment methods are the only options. We investigated the potential use of enzymatic treatment in tissue loosening to improve the esophageal passage especially in patients with advanced esophageal carcinoma. Specimens were prepared from resection materials (one adenocarcinoma, two squamous cell carcinomas) which were removed from three consecutive patients. They were treated with pancreatin at 2.5%, 5% and 10% of concentrations. Macroscopic and histopathologic examinations were performed to evaluate the effect. At 2.5% enzyme concentration no macroscopically change was observed in adenocarcinoma while mild softening occurred in the tissue samples of squamous cell carcinoma. Histopathologic examination revealed tissue and cell necrosis destruction at 5 and 10% concentrations in the latter samples. We observed clear destruction of squamous cell carcinoma tissue in both macroscopically and microscopically with in vitro enzyme application. This procedure, not only helps to open esophageal passage, but may also delay the growth of tumour and support the chemoradiotherapy. Enzymatic tumoral dissection (tumourolysis) is a promising method to alleviate the symptoms of esophageal carcinoma. © 2005 Published by European Association for Cardio-Thoracic Surgery. Physical examination revealed icteric sclera, generalized lymphadenopathy, diffuse abdominal tenderness and marked hepatosplenomegaly. Laboratory findings showed bicytopenia, coagulopathy, hypofibrinogenemia and hypoproteinemia in addition to elevated ESR, C-reactive protein, serum bilirubin, transaminases, alkaline phosphates, lactic dehydrogenase, gamma-glutamyl transpeptidase, and triglyceride levels. Hepatitis-B surface antigen and hepatitis B core IgM antibody were positive, while lymph node biopsy demonstrated infiltration with histiocytic series displaying marked erythrophagocytosis. The patient was diagnosed with hepatitis B virus infection-associated hemophagocytic syndrome. The condition deteriorated rapidly and he died 12 days after admission. This report suggests that in patients with hepatitis B infection presenting unexplained fever, localized bleeding due to thrombocytopenia and showing a rapid clinical deterioration, early bone marrow or lymph node biopsy for diagnosis and intensive therapy should be performed.Article Fibrosarcomatous Change in Dermatofibrosarcoma Protuberans(1996) Ugras, S.; Kutluhan, A.; Bekerecioglu, M.; Karakok, M.Dermatofibrosarcoma protuberans which included fibrosarcomatous areas is rarely seen. In one case, we estimated the proportion of area of dermatofibrosarcoma protuberans and fibrosarcomatous area. We also estimated mitotic rate, cellularity, and cytologic anaplasia in these areas. Histologically, the fibrosarcoma area showed a cellularity, mitotic figures, and cytologic anaplasia exceed those seen in the dermatofibrosarcoma protuberans area. The proportion of the fibrosarcoma area was 89%. In both dermatofibrosarcoma protuberans and fibrosarcoma areas, giant cells, necrosis and haemorrhagea was absent. In conclusion, we believe that the dermatofibrosarcoma protuberans which included fibrosarcomatous areas may be an intermediate type between dermatofibrosarcoma protuberans and fibrosarcoma.Article Head and Neck Granulocytic Sarcoma With Acute Myeloid Leukemia: Three Rare Cases(Medquest Communications LLC, 2001) Çankaya, H.; Ugras, S.; Dilek, I.We conducted a retrospective review of pathology files and hospital records and identified three unusual presentations of granulocytic sarcoma associated with acute myeloid leukemia (AML) of the head and neck. At least one mass was observed on the skin of all three patients. A 17-year-old boy had masses in each temporal region that were accompanied by bilateral facial paralysis. He was administered chemotherapy and radiotherapy, but he died of infection secondary to a second relapse 29 months after the initial diagnosis. A 17-year-old girl had a tumor in the right parotid area. She received chemotherapy, but she died of infection and bleeding 2 months after the initial diagnosis, A 33-year-old man had numerous tumors widely disseminated over his skin. He received chemotherapy and was in remission 12 months after the initial diagnosis, but he eventually relapsed and died. Granulocytic sarcoma can be localized in unexpected regions, including the head and neck. This tumor is very often misdiagnosed as a malignant lymphoma, which leads to delayed treatment and a poor outcome. Therefore, clinical and histopathologic findings should be evaluated before any diagnosis of malignant tymphoma is pronounced. Immunohistochemical stains should also be performed on patients with suspected granulocytic sarcoma, and aggressive chemotherapy or immunotherapy should be administered. We believe that high-dose chemotherapy can improve survival rates in granulocytic sarcoma associated with AML.Article Histopathological Effects of Desflurane on the Liver and the Kidney Using Light Microscopy(2010) Kati, I.; Tekin, M.; Tas, A.; Ragbetli, C.; Ugras, S.The aim of this study was to histopathologically assess the toxic effect of desflurane on the liver and the kidney. The rats (n = 150) were grouped into five main and three subgroups according to the exposure time to the anesthetic agent. The first group was planned as 1 MAC Desflurane+N2O+O2, the second group as 1 MAC Desflurane+O2, the third group as 2 MAC Desfiurane+N2O+O2, the fourth group as 2 MAC Desflurane+O2 and the 5th group as the control group. Each group was given the anesthetic agent for 1, 3 or 6 h. The rats that were exposed to the anesthetic agent with an adequate dose and for an adequate period of time were sacrificed properly. Livers and kidneys were resected and examined by light microscopes following the routine tissue follow-up procedures. Significant histopathological changes were observed in all groups compared to the control group. However, the pathological findings of the group 2 MAC Desflurane+N2O+O2 at the 6th h with the light microscope was significantly more than those of other groups at all times. It was concluded that although, desflurane caused cellular changes in liver and kidney tissue, it did not cause any clinical changes. Nevertheless, further studies are required to support this opinion. © Medwell Journals, 2010.Article Investigation of Irisin Level in Brucella Patients(Yuzuncu Yil Universitesi Tip Fakultesi, 2021) Algul, S.; Senol, F.F.; Ugras, S.; Esmer, F.; Kara, M.; Ekici, P.; Ozcelik, O.This study aimed to determine whether the changes in the metabolism of Brucella patients affect the irisin hormone. Also, it is aimed to investigate the relationships between other laboratory findings used in the diagnosis of brucellosis patients and the irisin hormone. 160 persons (80 brucellosis, 80 controls) were included in this study. Blood was drawn from the people participating in the study following their overnight fast. Irisin level was measured by ELISA method, CRP and sedimentation levels were measured using autoanalysers and commercial kits. It was determined that the irisin level of Brucella patients was statistically significantly higher than the control group (p <0.05). It was determined that there is a negative correlation between irisin level and CRP and sedimentation level in Brucella patients. In Brucella patients, the higher irisin level than the control group showed a negative correlation with CRP and sedimentation levels, which are infection diagnosis criteria; It suggests that the level of irisin can be considered as a supportive diagnostic marker in these patients. Further studies are needed to evaluate irisin hormone as a supportive diagnostic criterion. © 2021, Yuzuncu Yil Universitesi Tip Fakultesi. Methods: 380 patients who underwent tonsillectomy due to chronic tonsillitis were included in the study. The patients were divided into two groups according to peroperative bleedings. Group 1, included the patients whose bleedings stopped spontaneously or with gauze tampon and Group 2, the patients whose bleedings stopped with suturation-ligation or cauterisation. Results: Although there was no difference between the groups in age, sex, acute tonsillitis attack number, tonsil volume, lymphoide hyperplasia, lymphocyte infiltration to surface epithelium, epithelial defect, plasma cell and fibrosis; a statistical difference was present in view of polymorphonuclear leucocyte occurence against Group 1. Conclusion: We consider that an acute infection limited to the tonsils, exists in some cases with chronic tonsillitis and therefore this state may become a minor factor causing to bleeding in tonsillectomy.
