Conventional Comparison of Effectiveness and Utilization of Af Genital System in Vaginal Infectious Pathogens

Abstract

Amaç: Mikrobiyolojik hızlı tanı ve antibiyotik duyarlılığı tespit kitlerinden olan A.F. Genital Sistem®'in vajinit, pelvik inflamatuvar hastalık, preterm eylem ya da erken membran rüptürü ile ilişkili olabilecek enfeksiyöz patojenleri saptamadaki özgüllüğü ve duyarlılığının saptanması. Materyal-Metod: Çalışmamıza 197 hasta alınmıştır. Hastalar; Grup I'de vajinit tanısı konmuş olan 50 hasta, Grup II'de pelvik inflamatuvar hastalık tanısı konmuş 48 hasta, Grup III'te preterm eylem tanısı konmuş 49 hasta ve Grup IV'te erken membran rüptürü tanısı konmuş 50 hasta olmak üzere 4 gruba ayrılmıştır. Hastalardan alınan vajinal sürüntü örnekleri hem A.F. Genital Sistem® hem de konvansiyonel metodlar ile değerlendirilerek elde edilen bulgular karşılaştırılmıştır. İstatistik hesaplamalar için anlamlılık düzeyi %5 olarak alınmıştır. Hesaplamalar için SPSS (ver:13) istatistik paket programından yararlanılmıştır. Bulgular: Kültür sonuçlarına göre A.F. Genital Sistem® için en yüksek duyarlılık E. Faecalis için saptanmış olup %85.7 olarak bulunmuştur. En yüksek özgüllük ise E .coli için %98,2 olarak bulunmuştur. Testin doğruluk oranı Candida ve S. Aureus için %93,4 olarak hesap edilmiştir. Tartışma: A.F. Genital Sistem® testi E. facelis ve Candida için istatistiksel olarak anlamlı olup iyi bir test olarak kullanılabilir. Fakat diğer mikrooganizmalar için istatistiksel olarak anlamlı olmakla beraber çok kullanışlı bir test olmayabilir. AF Genital Sistem®, jinekoloji ve obstetrik pratiğinde en sık görülen patojenleri ve bunların antibiyotik duyarlılıklarını tespit için uygun bir yöntem olabilir. Vajinal enfeksiyöz patojenlerin tanısında AF Genital Sistem®'in doğruluğu ve kullanılabilirliğinin konvansiyonel metodlarla karşılaştırılması amacıyla farklı gruplarda ve daha büyük olgu serilerinde AF Genital Sistem® ile ilgili çalışmalara ihtiyaç vardır. Aim: The aim of this study is to investigate the specificity and sensitivity of a microbiologic rapid diagnostic and antibiogram sensitivity test, A.F. Genital System®, on infectious pathogens associated with vaginitis, pelvic inflammatory disease, preterm labor and early rupture of the membranes. Patients and Method: One hundred ninety seven patients were divided into 4 groups; Group I: 50 patients diagnosed with vaginitis, Group II: 48 patients diagnosed with pelvic inflammatory disease, Group III: 49 patients diagnosed with preterm labor, Group IV: 50 patients diagnosed with early rupture of the membranes. Vaginal swabs were examined with both A.F. Genital System® and conventional methods. Statistical analyses were performed by SPSS for Windows version 13 and a threshold of 5% was accepted for significance. Results: Culture results revealed that E. Faecalis had the highest sensitivity with 85.7% and E. Coli had the highest specificity with 98.2% for A.F. Genital System®. Accuracy of the test was 93.4% for Candida and S. Aureus. Conclusion: A.F. Genital Sistem® can be useful in E. facelis and Candida with statistically significant results. Besides, it is not considered as an effective method for other microorganisms in our study. A.F. Genital System® is a feasible method to investigate the most common pathogens in gynecology and obstetrics practice and their antibiotic sensitivity. More studies on larger series are needed to investigate the efficiency of A.F. Genital System® and comparison with conventional methods.
