Evaluation of Subepithelial Connective Tissue Graft and De-Epithelialized Free Gingival Graft Practices in the Treatment of Gingival Recessions in Terms of Clinical Efficacy

Abstract
Çalışmamızın amacı çoklu dişeti çekilmelerinin tedavisinde tünel tekniği ile kombine olarak uygulanan subepitelyal bağ dokusu grefti(SBDG-Grubu), bistüri ile de-epitelize edilmiş serbest dişeti grefti(Bistüri-Grubu) ve Erbiyum, Krom: İtriyum-Skandiyum-Galyum-Garnet lazer ile de-epitelize edilmiş serbest dişeti grefti(Lazer-Grubu) tekniklerinin klinik etkinlik açısından değerlendirilmesidir. Araştırmamıza yaş ortalaması 36,02 olan 60 birey dahil edilmiştir. Bireylere ait demografik bilgiler toplanmıştır. Operasyonlardan önce ve 6 ay sonra dişeti çekilmesi görülen alanlarda plak indeksi(Pİ), gingival indeks(Gİ), sondlama derinliği(SD), çekilme derinliği(ÇD), klinik ataşman seviyesi(KAS), kök yüzeyi kapanma(KYK) oranı, yapışık dişeti miktarı(YDM), dişeti kalınlığı, dişeti fenotipi, provakatif hassasiyet(PH), yaşam kalitesi(OHİP-14) verileri toplanmış; ameliyat sırasında ise operasyon süresi kayıt altına alınmıştır. Operasyonu takiben ilk 3 gün palatinal bölgeye ait ağrı(Vizüel Analog Skala-VAS) değerlendirilmiştir. Katılımcıların tüm ağıza ait Gİ, Pİ ve SD skorları hem operasyon öncesinde hem de operasyonu takiben 6.ayda değerlendirilmiştir. Grupların tümünde tedavi sonrasında tüm ağız Gİ, Pİ ve SD skorları ve çekilme bölgesine ait PI, GI, SD, KAS, ÇD, ve PH değerleri azalmıştır(p<0,05). Tüm gruplarda tedavi sonrası DK değeri artmıştır(p<0,05). KYK oranı Lazer grubunda(%63,56±21,01); SBDG(%83,27±13,3) ve Bistüri(%80,63±15,4) gruplarına göre düşüktür(p<0,05). YDM, SBDG ve Bistüri grubunda tedavi sonrası artmıştır (p<0,05). Lazer grubunda ise tedavi öncesine göre benzer seyretmiştir(p>0,05). Grupların her birinde tedavi sonrası OHİP-14 değeri azalmış(p<0,05) ve dişeti fenotipi kalınlaşmıştır(p<0,05). Operasyon süresi lazer grubunda daha uzundur(p<0,05). Bu çalışmanın sınırları dahilinde tüm gruplarda klinik iyileşmeler gözlense de, SBDG ve bistüri gruplarında elde edilen başarı benzer olup, lazer grubundan daha yüksektir.
The aim of the study is to evaluate the clinical efficacy of subepithelial connective tissue graft(SCTG-Group), de-epithelialized free gingival graft(FGG) with scalpel(Scalpel-group), and laser de-epithelized FGG(Laser-Group) techniques applied in combination with the tunnel technique in the treatment of multiple gingival recessions. 60 individuals with a mean age of 36.02 were included in the study. Demographic information of individuals was collected. Plaque index(PI), gingival index(GI), probing depth(PD), recession depth(RD), clinical attachment level(CAL), root surface closure(RSC) rate, adherence in areas with gingival recession before and 6 months after the operations attached gingival height(AGH), gingival thickness(GT), gingival phenotype(GP), provocative sensitivity(PS), Oral health impacted profile-14(OHIP-14) data were collected; during the operation, time was recorded. Pain in the palatal region(VisualAnalogScale-VAS) was evaluated for the first 3 days following the operation. GI,PI and PD of the participants were evaluated both before the operation and at 6th month after the operation. Whole mouth GI,PI,PD and PI, GI,PD,CAL,RD, and PS values of the recession area decreased after treatment in all groups(p<0.05).In all groups, GT values increased after treatment(p<0.05). RSC rate in the Laser-Group(63.56±21.01%);it was lower than SBDG(83.27±13.3%) and Scalpel(80.63±15.4%) groups(p<0.05). AGH increased after treatment in SBDG and Scalpel-Groups(p<0.05). In the Laser-Group, it was similar to the pre-treatment(p>0.05). In each of the groups, the OHIP-14 value decreased and GP was thickened after treatment(p<0.05).The operation time was longer in the Laser-group(p<0.05). Although clinical improvements were observed in all groups within the limits of this study, the success achieved in the SBDG and Scalpel-Groups was similar and higher than the Laser-group.
