The Efficiancy of Inhaler Salbutamol Transient in Tachypnea of Newborn ?
Abstract
Yenidoğanın geçici takipnesi (TTN) term ve terme yakın yenidoğanlarda en sık rastlanan solunum sıkıntısı nedenidir. TTN fetal akciğer sıvısının emilimindeki gecikme sonucu gelişen akciğer ödemi ile karakterize olup, kendi kendini sınırlayan yenidoğana özgü parankimal bir akciğer hastalığıdır. Sıvı kısıtlaması ve oksijen desteği dışında özgün bir tedavisi yoktur. Oral kullanıldığında diüretiklerin yararlı olmadığı bilinmektedir. İntravenöz kullanımıda klinik seyri değiştirmemiştir. TTN tedavisinde ß2AA kullanılmaktadır. İnhale epinefrinin böyle bir etkisi olmamasına rağmen, Armangil ve arkadaşları salbutamolün TTN tedavisinde etkili olduğunu bildirmişlerdir.Çalışmamızda TTN tanısı alan bebeklere tedavide uygulanan 0.15 mg/kg ve 0.5 mg/kg'dan inhale salbutamolün hastaların klinik, fizik muayene ve labratuvar bulguları üzerine olan etkilerinin araştırılması amacıyla gerçekleştirildi. Çalışma randomize, çift kör ve prospektif olarak yürütüldü. Bu çalışma Yüzüncü Yıl Üniversitesi Tıp Fakültesi Araştırma Hastanesi ve Diyarbakır Kadın Doğum ve Çocuk Hastalıkları Hastanesi YYBÜ'lerinde Ağustos 2011 ile Haziran 2012 ayları arasında yürütüldü. 34 ile 39. gebelik haftaları arasında doğan toplam 60 hasta çalışmaya alındı. Hastalar TTN tanısı aldıktan sonra randomize, çift kör ve prospektif olarak üç ayrı tedavi protokolüne alındı. Birinci gruba tek doz 0.15 mg/kg'dan inhale salbutalmol, ikinci gruba tek doz 0.5 mg/kg'dan inhale salbutamol ve konrol grubuna tek doz serum fizyolojik verildi. Çalışmaya alınan 60 hastanın 21 (%35) birinci grup, 20'si (%33.3) ikinci grup ve 19 (%31.6) hastada kontrol grubunu oluşturuyordu. Yatış süresi kontrol grubuna göre ikinci grupta anlamlı olarak düşük bulundu (p<0.05). Tedavi sonrası bakılan solunum sayısında birinci ve ikinci grupta kontrol grubuna göre anlamlı olarak düşme görüldü (p<0.05). Solunum destek süreleri birinci ve ikinci grupta kontrol grubuna göre istatistiksel olarak anlamlı düşük olduğu görüldü (p<0.05). Tedaviye başlamadan önce belirlenen FiO2 ihtiyacında, birinci grupta kontrol grubuna göre zaman içinde anlamlı düşme olduğu belirlendi (p<0.05). Birinci ve ikinci grupta kontrol grubuna göre yaş akciğer klinik skorlamasında anlamlı azalma olduğu görüldü (p<0.05). Birinci ve ikinci grupta kontrol grubuna göre solunum destek derecesinde anlamlı azalma olduğu görüldü(p<0.05)Inhale salbutamolün yenidoğanın geçici takipnesinde klinik ve fizik muayene üzerine özellikle 0.5 mg/kg dozunda verildiğinde , etkin bir tedavi olabileceği gözlemledik.Anahtar kelimeler: yenidoğanın geçici takipnesi, pulmoner sıvı dengesi, inhale salbutamol.
Transient tachypnea of the newborn (TTN) is most common respiratory distress reason at term and near?term newborns. TTN is a self-limiting parenchymal lung disease which is specific to neonatal period and characterized with pulmonary edema which develops as a result of delay at fetal pulmonary fluid absorption. There is no original treatment other than fluid restriction and oxygen support. It is known that; diuretics are not useful when used orally. Also intravenous usage did not change clinical course. ß2AA has been using at TTN treatment. Armangil et al has reported that salbutamol is effective at TTN treatment, despite epinephrine inhalation has not such effect.Our study was performed for to investigate the effects of 0.15 mg/kg and 0.5 mg/kg salbutamol inhalation administered to newborns who were diagnosed TTN at the treatment on the patients' clinical, physical examination and laboratory findings. Study was conducted as randomized, double-blind and prospectively. This study was conducted at Yüzüncü Yıl University Medical Faculty Research Hospital and Diyarbakır Maternity and Children Hospital?s Neonatal Intensive Care Unit (NICU) between August 2011 and June 2012. A total of 60 patients, born between 34 and 39 gestational weeks, were studied.after patients were diagnosed TTN; they were taken three different therapy protocol with double-blind and prospective methods. Single dose 0.15 mg/kg salbutamol inhalation to first group, single dose 0.5 mg/kg salbutamol inhalation to second group and single dose serumphysiologic to control group given. Of 60 patients in the study; they were constituted the groups as; 21 (%35) at first group, 20 (%33.3) at the second group and 19 (%31.6) at the control group. Hospitalization time was found significantly lower at the second group than the control group (p<0.05). There was significantly decrement at respiration rate after treatment of the first and second groups than the control group (p<0.05). It was seen that; respiratory support times were statistically significantly lower at first and second groups than the control group (p<0.05). Singnificant decrement was determined at the first group than the control group about FiO2 need which was determined before the treatment (p<0.05). Significant decrement was seen at wet lung clinical scoring at the first and second group than the control group (p<0.05).Significant decrement was seen at respiratory support degree at the first and second groups than the control groups (p<0.05).We were observed that salbutamol inhalation can be an effective treatment at transient tachypnea of the newborn about clinical status and physical examination especially when it was given at 0.5 mg/kg dose.Key Words: Transient tachypnea of the newborn, pulmonary fluid balance, inhaled salbutamol.
