The Efficiency of Using Daily Single Dose of Low Molecular Weight Heparin Threatment in Out Patient at the Deep Vein Thrombosis

Abstract

AMAÇ; Derin venöz trombozu (DVT) bulunan ve kliniğimizde tedavisi yapılan hastalara ayaktan tek doz enoksaparin tedavisinin etkinliğinin ve güvenirliliğinin belirlenmesi, kısa ve orta dönem sonuçlarının literatür ile karşılaştırılarak değerlendirilmesi amaçlanmıştır.MATERYAL VE METOD; Aralık 2004 ve Nisan 2008 Tarihleri arasında kliniğimizde DVT'li 30 hastaya ayaktan tek doz enoksaparin tedavisi uygulandı. Hastaların 17 (%56 )'si erkek 13 (%44 )`ü bayandı. Ortalama yaş 46.2 ± 27 (19?74) idi. Bütün hastalara kg başsına 1.5 kg/mg hesabıyla tek doz enoksaparin uygulandı. Hastalar ilk görüldükleri ve 10.gün, 1.ay, 3.ay, 6.ay, 12.ay ve 18. ayda tuberositas tibia ve uyluk ortası çap farkları ile bilateral alt extremite venöz doppler ve biyokimya monitarizasyonu ile takip edildi.BULGULAR; Hastaların hiçbirinde pulmoner emboli bulgusu ve şikayeti olmadı.Vakaların ikisi bilateral, 19'u sol alt ekstremit de ve 9'u sağ alt ekstremitede gözlendi. Tek bir hastada derin ven trombozu nüksü gerçekleşti. Bir hastamızda enoksaparine bağlı olduğu düşünülen yaygın cilt döküntüleri gerçekleşti ve ilaç kesildikten sonra hastanın şikayetleri düzeldi. Bu yan etki rapor edildi. İlaçla ilgili başka bir yan etki rapor edilmedi. Çalışma süresi sonunda hastaların 11'in de doopler USG sonucu tam iyileşme sağlandığı ispatlandı. Hastaların 10'unda sadece venöz reflü ve 9 hastada parsiyel tromboz mevcuttu.SONUÇ: DVT de ayaktan tek doz Enoksaparin tedavisi güvenilir ve etkili bir yöntemdir

BACKGROUND: The aim of these study is to evaluate the efficiency and the confidence of the single dose outpatient enoksaparin treatment of deep vein thrombosis in our clinic, with short and long term results comparison with literatureMATERIALS AND METHODS: Between december 2004 and April 2008; 30 patients with DVT treated with single dose enoksaparin at YYU Hospital, Department of Cardiovascular surgery. 17 (56%) of the patients were male, while remaining 13 (44%) of the patients were female. The avarage of the patients age were 46,2 ± 27 (19-74). All patients were treated single dose enoksaparin 1.5 mg per kilogram. All patients were fallowed initially, 10th day, 1st month, 3th month, 6th month, 12th month and 18th months later with the results of tuberositas tibia and middle thigh diameter diffrence, bilateral low extremity venous doppler and biochemistry results.RESULTS: There were no pulmonary embolism complication complaints and findings. There were two patients with bilaterally involvement, 19 patients with left, 9 with right involvement. There only one recurrence. There were only one patients who had widespread skin eruptions related to enoksaparin which abated with the cessation of the therapy. This side effect is reported. There were no other such side effects. After the study 11 patients were completely treated with enoksaparin proved with doppler USG study, there were venous reflux in 10 patients and partial obstruction in 9 patients.CONCLUSION: Enoksaparin is efficient confident single dose outpatient treatment deep vein thrombosis.
