Investigation of Blastocystis Sp. Frequency in Patients With Ulcerative Colitis, Crohn's and Diarrhea

Abstract

Blastocystis memeliler, sürüngenler, kuşlar ile beraber artropodlar gibi çok geniş konak popülasyonuna sahiptir. Blastocystis türleri hazımsızlık, ishal, karın ağrısı, şişkinlik, bulantı ve kusma gibi çeşitli gastrointestinal sistem şikayetlerine neden olabilmektedir. Bu çalışmanın başlıca amacı Van Yüzüncü Yıl Üniversitesi Dursun Odabaşı Tıp Merkezi Parazitoloji Laboratuvarı'na gastroentoloji polikliniğinden gelen ülserataf kolit, Crohn ve ishal tanısı almış hastalarda Blastocystis sp. dağılımını belirlemektir. Ayrıca Blastocystis'in laboratuvar tanısında direkt mikroskobik inceleme (DM), kültür ve gerçek zamanlı polimeraz zincir reaksiyonu (qPZR) yöntemlerinin karşılaştırılması amaçlanmıştır. Çalışmaya 47 erkek ve 53 kadın olmak üzere toplamda 100 hasta dahil edildi. Olguların 61'ishalli, 35'i ülseratif kolit (ÜK) ve dördü Crohn hastası idi. Dışkı örnekleri DM, trikrom, kültür ve real-time PZR yöntemleriyle incelendi. DM ve trikrom boyamada 29 (%29), kültür 28 (%28) ve qPZR ile de 41 (%41), pozitiflik görüldü. Toplamda 42 hastada parazit tespit edildi, erkeklerin %40,4 (20/47)'si, kadınların %37,7 (22/53)'ü pozitif bulundu. Crohn hastaların %75'inde, ishallilerin %42,6'sında ve ÜK hastaların %37,1'inde parazite rastlandı. MB ve trikrom boyama %69 hassaslık gösterirken, PZR testinin yaklaşık %98 ile en hassas tanı yöntemi olduğu görüldü. Klinik vakalarda ve bölgemizde yüksek oranlarda Blastocystis yaygınlığına rastlanması parazitin önemini ortaya koymaktadır. Bu anlamda hasta gruplarında parazit varlığı ve genotipleri üzerine daha fazla çalışması yapılmasına ihtiyaç bulunmaktadır. Anahtar Kelimeler: Blastocystis sp., ülseratif kolit, Crohn, ishal

Blastocystis has a very large host population such as mammals, reptiles, birds as well as arthropods. Blastocyst species can use various create a gastrointestinal systems complaints such as indigestion, abdominal pain, blotaing nausea and vomiting. The purpose of this product is Blastocystis sp. teaches education. In addition, the laboratory of Blastocystis aims to evaluate direct microscopic (DM), culture and real-time polymerase chain reaction (qPCR) target. A totoal 100 patients with ulcerative colits, Crohn's and diarrhea were included in the study. Of the cases, 61 had diarrhea, 35 had ulserative colitis (UK) and 4 had Crohn's disease. 47 of the patients were male and 53 were female. Stool samples were examined by DM, trichrome culture and real-time PCR methods. 29 (29) positivy in DM and trichrome staining, culture in 28 (%28) in qPCR 41 (%41). In total, parasites were detected in 39 patients, 40.4% (20/47) of men and 37.7% (22/53) of women were positive. Parasites were found in 75% of Crohn's patients, 42.6% of diarrheal patients and 37.1% of UC patients. While MB and trichrome staining showed a sensitivity of 69%, PCR test was found to be the most sensitive diagnostic method with approximately 98%. The high prevalence of Blastocystis in clinical cases and in our region reveals the importance of the parasite. In this sense, there is a need for more studies on the presence and genotypes of parasites in patient groups. Keywords: Blastocystis sp., ulcerative colitis, Crohn, diarrhea
