Prevalence of Dientamoeba Fragilis in Ulcerative Colitis, Irritable Bowel Syndrome and Crohn's Disease

Abstract

Dientemoeba fragilis insanlarda patojen etkiler gösterip klinik olgulara yol açması ile ilgili tartışmalı bir parazittir. Birçok gastrointestinal şikâyetlere neden olabilen bu parazitin uygun tanı yöntemleri kullanıldığında dünyanın birçok farklı bölgesinde yaygın görülebildiği bildirilmektedir. Bu çalışmada amaç ülseratif kolit (ÜK), İrritabl bağırsak sendromu (İBS) ve Crohn hastalığında (CH) D. fragilis yaygınlığını tespit etmektir. Bu tanıları almış SBÜ Van Eğitim ve Araştırma Hastanesi ve Yüzüncü Yıl Üniversitesi, Dursun Odabaşı Tıp Merkezine başvuran toplam 60 hastanın gaita örneği incelendi. Çalışmaya konrol grubu olarak sağlıklı 20 kişi dahil edildi. Hasta grupları üç Crohn hastası, 16 ÜK ve 41 İBS olarak belirlendi. Örnekler direkt mikroskobik bakı (MB), trikrom boyama (TB) ve gerçek zamanlı polimeraz zincir reaksiyonu (qPCR) yöntemiyle incelendi. Çalışmada ÜK ve Crohn hastalığında TB yöntemiyle D. fragilis'e rastlanmazken 41 İBS hastasının üçünde D.fragilis görüldü. Boyama sonrası yapılan qPCR aşamasında hasta grubunda 12 kişide ( 9-İBS/3-ÜK) D. fragilis tespit edildi. Kontrol grubunda ise qPCR aşamasında bu parazite bir kişide rastlandı. Bu çalışma, Van ilinde D. fragilis üzerine yapılan ilk ve tek çalışma olması niteliğiyle paraziter epidemiyoloji ve bilimsel literatüre katkıda bulunacaktır.

Dientemoeba fragilis is a controversial parasite related to its pathogenic effects and clinical manifestations in humans. It is reported that this parasite, which can cause many gastrointestinal complaints, can be seen widely in many different parts of the world when appropriate diagnostic methods are used. The aim of this study is to determine the prevalence of D. fragilis in ulcerative colitis (UC), irritable bowel syndrome (IBS) and Crohn's disease (CH). Stool samples of 60 patients with these diagnoses who applied to SBU Van Training and Research Hospital and Yüzüncü Yıl University Dursun Odabaşı Medical Center were analyzed. Twenty healthy people were included in the study as a control group. The patient groups were determined as three Crohn's patients, 16 UC and 41 IBS. The samples were analyzed by direct microscopic examination (MB), trichrome staining (TB) and real-time polymerase chain reaction (qPCR). In the study, D. fragilis was not found in UC and Crohn's disease with the TB method, while D. fragilis was found in three of 41 IBS patients. D. fragilis was detected in 12 patients (9-IBS/3-UC) in the qPCR stage after staining. In the control group, this parasite was found in one person during the qPCR stage. This study will contribute to the parasitic epidemiology and scientific literature as it is the first and only study on D. fragilis in Van.
